Applying the Integrated Behavior Change Model to Understanding Physical Activity Among Older Adults: A Qualitative Study

We explored older adults’ experiences of physical activity (PA) and related decision-making processes underlying PA. Twenty Australians (Mage = 73.8 years) participated in semistructured interviews. Data were analyzed using thematic analysis, and identified themes were matched deductively within motivational, volitional, and implicit processes of the integrated behavior change model for PA. Motivational influences such as participants’ time orientation toward health and perceptions of what PA should be like were frequently featured in participants’ narratives. Volitional processes were also identified, with participants reporting different ways of coping with competing priorities. Physical surroundings and habitual PA were the identified themes within implicit processes. Together, these findings contribute to a better understanding of subjective experiences of older adults regarding PA. They also add to a more contextual understanding of multiple decision-making processes underpinning older adults’ PA engagement. Identified concepts may be used in future research and PA interventions targeting older adults.

This chapter focuses on the role of motivation for understanding adult age differences in decision making. It begins by reviewing research on cognitive aging and decision making. It then reviews research that highlights older adults’ motivation to selectively allocate cognitive resources when making decisions and their motivation to draw upon life experience and emotion regulation strategies. Next, because age-related shifts in motivation accentuate a focus on meaningful social relationships, the chapter considers social aspects of aging and decision making. It discusses age-related differences in selection of social network members and considers factors that may motivate older adults to involve others in the decision-making process. It suggests how research on motivation and aging and decision making could be used to inform decision-making interventions. After suggesting directions for future research, it concludes by highlighting the implications of research on aging and decision making for understanding contextual and motivational influences on cognitive aging.

Objectives: To explore the effects of membership in a fitness program for older adults on social isolation, loneliness, and health. Method: Using survey responses from SilverSneakers members and matched nonmembers, regression path analysis was used to examine the influence of SilverSneakers membership on physical activity, social isolation, loneliness, and health, and the interrelationships among these concepts. Results: SilverSneakers membership directly increased physical activity and self-rated health, directly decreased social isolation, and indirectly decreased loneliness. Decreased social isolation and loneliness were associated with better self-rated health: social isolation and loneliness had independent direct effects on health, while social isolation also had an indirect effect on health mediated through loneliness. Discussion: Members of SilverSneakers experienced better health through increased physical activity, reduced social isolation, and reduced loneliness. Future research should explore independent effects of social isolation and loneliness on health and the mechanisms by which membership reduces social isolation and loneliness.

Abstract Shared decision making (SDM) in older patients is more complex when multiple chronic conditions (MCC) have to be taken into account. The aim of this research is to explore the effect of the evidence based implementation intervention SDMMCC on (1) the preferred and perceived participation (2) decisional conflict and (3) actual SDM during consultations. 216 outpatients participated in a video observational study. The intervention existed of a SDM training for geriatricians and a preparatory tool for patients. Consultations were videotaped and coded with the OPTIONMCC. Pre- and post-consultation questionnaires were completed. Participation was measured by the Patients’ perceived Involvement in Care Scale (PICS). Decisional conflict was measured by the Decisional Conflict Scale (DCS). The patients mean age was 77 years, 56% was female. The preparatory tool was completed by 56 older adults (52%), of which 64% rated the tool as positive. The preparatory tool was used in 12% of the consultations. The mean overall OPTIONMCC score showed no significant changes on the level of SDM(39.3 vs 39.3 P0.98), however there were significant improvements on discussing goals and options on sub-items of the scale. There were no significant differences found in the match on preferred and perceived participation (86.5% vs 85.0% P 0.595) or in decisional conflict (22.7 vs 22.9 P0.630). The limited use of the preparatory tool could have biased the effect of the intervention. In future research more attention must be paid towards the implementation of preparatory tools, not only among patients but also among geriatricians.

BACKGROUND Smartphone apps promoting physical activity (PA) are abundant, but few produce substantial and sustained behavior change. Although many PA apps purport to induce users to compare themselves with others (by invoking social comparison processes), improvements in PA and other health behaviors are inconsistent. Existing literature suggests that social comparison may motivate PA for some people under some circumstances. However, 2 aspects of work that apply social comparison theory to PA apps remain unclear: (1) how comparison processes have been operationalized or harnessed in existing PA apps and (2) whether incorporating sources of variability in response to comparison have been used to tailor comparison features of apps, which could improve their effectiveness for promoting PA. OBJECTIVE The aim of this meta-review was to summarize existing systematic, quantitative, and narrative reviews of behavior change techniques in PA apps, with an emphasis on social comparison features, to examine how social comparison is operationalized and implemented. METHODS We searched PubMed, Web of Science, and PsycINFO for reviews of PA smartphone apps. Of the 3743 initial articles returned, 26 reviews met the inclusion criteria. Two independent raters extracted the data from these reviews, including the definition of social comparison used to categorize app features, the percentage of apps categorized as inducing comparison, specific features intended to induce comparison, and any mention of tailoring comparison features. For reference, these data were also extracted for related processes (such as behavioral modeling, norm referencing, and social networking). RESULTS Of the included review articles, 31% (8/26) categorized app features as prompting social comparison. The majority of these employed Abraham and Michie’s earliest definition of comparison, which differs from versions in later iterations of the same taxonomy. Very few reviews specified what dimension users were expected to compare (eg, steps, physical fitness) or which features of the apps were used to induce comparison (eg, leaderboards, message boards). No review referenced tailoring of comparison features. In contrast, 54% (14/26) reviews categorized features for prompting behavioral modeling and 31% (8/26) referenced tailoring app features for users’ personal goals or preferences. CONCLUSIONS The heterogeneity across reviews of PA apps and the absence of relevant information (eg, about dimensions or features relevant for comparison) create confusion about how to best harness social comparison to increase PA and its effectiveness in future research. No evidence was found that important findings from the broader social comparison literature (eg, that people have differing preferences for and responses to social comparison information) have been incorporated in the design of existing PA apps. Greater integration of the mobile health (mHealth) and social comparison literatures may improve the effectiveness of PA apps, thereby increasing the public health impact of these mHealth tools. In developing a senior lecture for the 2014 national meeting of the North American Society for the Psychology of Sport and Physical Activity, I had the opportunity to reflect upon a career of research and to focus on three interesting questions that my colleagues and I have attempted to address. These questions have led to several studies that all revolve around identifying ways to increase the effects of exercise on cognitive performance. In particular, the questions examine the possibility of increasing effects by focusing on particular populations (e.g., older adults, children) and by increasing our understanding of dose-response relationships between exercise parameters (e.g., intensity, duration) and cognitive outcomes. I present empirical evidence relative to each of these questions and provide directions for future research on physical activity and cognitive functioning.

This review examines program features that influence attendance and adherence to group-based physical activity (PA) by older adults. Medline, PubMed, CINAHL plus, PsycINFO, and the Cochrane Library were searched for studies published from 1995–2016. Quantitative and qualitative studies investigating factors related to PA group attendance or adherence by persons aged 55 years and over were included. Searching yielded eight quantitative and 13 qualitative studies, from 2,044 titles. Quantitative findings identified social factors, instructor characteristics, PA types, class duration and frequency, and perceived PA outcomes as important for attendance and adherence, whilst qualitative studies identified settings, leadership, PA types, observable benefits, and social support factors. Studies were predominantly low- to moderate-quality. This review identified design and delivery considerations for group-based PA programs to inform best-practice frameworks and industry capacity building. Future research should use longitudinal and mixed-methods designs to strengthen evidence about facilitators of program reach and engagement.

Introduction While research has investigated the factors influencing acquisition and use of technologies/assistive devices by older adults, few studies have addressed the decision-making processes regarding technology adoption of older adults with age-related vision loss. Method This critical ethnography engaged 10 older adults with age-related vision loss in narrative interviews, participant observation sessions, and semi-structured in-depth interviews to understand their decision-making processes related to the acquisition and use of low vision assistive devices to support occupational engagement. Findings Study findings focused on the benefits and barriers to technology acquisition and use. Benefits of technology acquisition included: enhanced occupational engagement; independence; safety; insurance; and validation of the disability, while the barriers to technology acquisition included: cost; training; usability; lack of awareness of low vision rehabilitation services; fear of being taken advantage of; and desire to preserve a preferred self-image. Conclusion Considering the low uptake of vision rehabilitation services, the study findings are important to occupational therapy. A better understanding of the perceived benefits and barriers to technology adoption from the perspective of older adults will help occupational therapists maximize treatment planning designed to enhance the occupational engagement of older adults aging with vision loss.
