Context:If ankle joint cryotherapy impairs the ability of the ankle musculature to counteract potentially injurious forces, the ankle is left vulnerable to injury.Objective:To compare peroneal reaction to sudden inversion following ankle joint cryotherapy.Design:Repeated measures design with independent variables, treatment (cryotherapy and control), and time (baseline, immediately post treatment, 15 minutes post treatment, and 30 minutes post treatment).Setting:University research laboratory.Patients or Other Participants:Twenty-seven healthy volunteers.Intervention(s):An ice bag was secured to the lateral ankle joint for 20 minutes.Main Outcome Measures:The onset and average root mean square amplitude of EMG activity in the peroneal muscles was calculated following the release of a trap door mechanism causing inversion.Results:There was no statistically significant change from baseline for peroneal reaction time or average peroneal muscle activity at any post treatment time.Conclusions:Cryotherapy does not affect peroneal muscle reaction following sudden inversion perturbation.

Context:Coactivation of abdominal and pelvic-floor muscles (PFM) is an issue considered by researchers recently. Electromyography (EMG) studies have shown that the abdominal-muscle activity is a normal response to PFM activity, and increase in EMG activity of the PFM concomitant with abdominal-muscle contraction was also reported.Objective:The purpose of this study was to compare the changes in EMG activity of the deep abdominal muscles during abdominal-muscle contraction (abdominal hollowing and bracing) with and without concomitant PFM contraction in healthy and low-back-pain (LBP) subjects.Design:A 2 × 2 repeated-measures design.Setting:Laboratory.Participants:30 subjects (15 with LBP, 15 without LBP).Main Outcome Measures:Peak rectified EMG of abdominal muscles.Results:No difference in EMG of abdominal muscles with and without concomitant PFM contraction in abdominal hollowing (P = .84) and abdominal bracing (P = .53). No difference in EMG signal of abdominal muscles with and without PFM contraction between LBP and healthy subjects in both abdominal hollowing (P = .88) and abdominal bracing (P = .98) maneuvers.Conclusion:Adding PFM contraction had no significant effect on abdominal-muscle contraction in subjects with and without LBP.

Our objective was to quantify changes in muscle EMG activity due to repetitive work. Using a repeated measures design, 13 females participated in 3 conditions, each lasting two eight-hour days: a control inactive condition, and two repetitive work conditions with ulnar deviation tasks at 20 and 25 repetitions per minute. EMG of the extensor carpi ulnaris muscle (ECU) was recorded during voluntary isometric contractions (20% and 60% MVC) eight times throughout the work and control days. The amplitude of the EMG signal was lower on workdays compared to the control days. Initial median frequency of the EMG signal showed no change between the control and workdays; however, the decline of the median frequency with respect to time over the course of each isometric contraction was steeper during workdays compared to control days. These changes suggest that the muscles are in an early stage of fatigue when working for an eight-hour workday.

Context:The push-up-plus (PP) exercise has been recommended for strengthening of the serratus anterior (SA). Previous studies have investigated the effect of different stability properties of the base of support to adjust the difficulty level of SA muscle-strengthening exercises in the PP position. However, the optimal humeral-elevation angle (HEA) for selective activation and maximum contraction of the SA during PP has not been investigated.Objectives:To assess the effects of HEA during PP on electromyographic (EMG) activity in the SA, upper trapezius (UT), and pectoralis major (PM) and on the UT:SA and PM:SA activity ratios.Design:Comparative, repeated-measures design.Setting:University research laboratory.Participants:29 healthy men.Main Outcome Measures:The subjects performed PP at 3 different HEAs (60°, 90°, and 120°); EMG activity in the SA, UT, and PM was measured, and the UT:SA and PM:SA activity ratios were calculated. Differences in muscle activity and ratios between the 60°, 90°, and 120° HEAs were assessed using 1-way repeated-measures analysis of variance; the Bonferroni correction was applied.Results:SA muscle activity was significantly increased, in order of magnitude, at the 120°, 90°, and 60° HEAs. UT:SA and PM:SA activity ratios were significantly greater during performance of the PP at an HEA of 60° than at HEAs of 120° and 90°.Conclusion:The results suggest that an HEA of 120° should be used during performance of the PP because it produces greater SA activation than HEAs of 60° and 90°.

Abstract Background In the 1950s, with monitored live voice testing, the vu meter time constant and the short durations and amplitude modulation characteristics of monosyllabic words necessitated the use of the carrier phrase amplitude to monitor (indirectly) the presentation level of the words. This practice continues with recorded materials. To relieve the carrier phrase of this function, first the influence that the carrier phrase has on word recognition performance needs clarification, which is the topic of this study. Purpose Recordings of Northwestern University Auditory Test No. 6 by two female speakers were used to compare word recognition performances with and without the carrier phrases when the carrier phrase and test word were (1) in the same utterance stream with the words excised digitally from the carrier (VA-1 speaker) and (2) independent of one another (VA-2 speaker). The 50-msec segment of the vowel in the target word with the largest root mean square amplitude was used to equate the target word amplitudes. Research Design A quasi-experimental, repeated measures design was used. Study Sample Twenty-four young normal-hearing adults (YNH; M = 23.5 years; pure-tone average [PTA] = 1.3-dB HL) and 48 older hearing loss listeners (OHL; M = 71.4 years; PTA = 21.8-dB HL) participated in two, one-hour sessions. Data Collection and Analyses Each listener had 16 listening conditions (2 speakers × 2 carrier phrase conditions × 4 presentation levels) with 100 randomized words, 50 different words by each speaker. Each word was presented 8 times (2 carrier phrase conditions × 4 presentation levels [YNH, 0- to 24-dB SL; OHL, 6- to 30-dB SL]). The 200 recorded words for each condition were randomized as 8, 25-word tracks. In both test sessions, one practice track was followed by 16 tracks alternated between speakers and randomized by blocks of the four conditions. Central tendency and repeated measures analyses of variance statistics were used. Results With the VA-1 speaker, the overall mean recognition performances were 6.0% (YNH) and 8.3% (OHL) significantly better with the carrier phrase than without the carrier phrase. These differences were in part attributed to the distortion of some words caused by the excision of the words from the carrier phrases. With the VA-2 speaker, recognition performances on the with and without carrier phrase conditions by both listener groups were not significantly different, except for one condition (YNH listeners at 8-dB SL). The slopes of the mean functions were steeper for the YNH listeners (3.9%/dB to 4.8%/dB) than for the OHL listeners (2.4%/dB to 3.4%/dB) and were <1%/dB steeper for the VA-1 speaker than for the VA-2 speaker. Although the mean results were clear, the variability in performance differences between the two carrier phrase conditions for the individual participants and for the individual words was striking and was considered in detail. Conclusion The current data indicate that word recognition performances with and without the carrier phrase (1) were different when the carrier phrase and target word were produced in the same utterance with poorer performances when the target words were excised from their respective carrier phrases (VA-1 speaker), and (2) were the same when the carrier phrase and target word were produced as independent utterances (VA-2 speaker).

Objectives This study evaluates the effects of a prison-based addiction intervention program. The evaluation is based on a multidimensional data collection that draws a portrait of the respondents’ substance use, and of their psychological/emotional, social, and judicial spheres. It measures the changes, or lack thereof, in substance use; the psychological/emotional, social, and judicial spheres; as well as the post-treatment services used. Method A quasi-experimental repeated measures design (0, 6 weeks, and 6 months) was used. Effects of the program were identified by comparing the results obtained by a group of inmates who had participated in the program ( n = 80; experimental group) with those of another group who had received no intervention ( n = 70; control group). Results The preliminary results suggested a certain treatment effect related to impulsivity and psychological distress. Conclusion Although the preliminary results were promising, the experimental and control groups did not differ significantly when more robust analyses were used.

Context:Inertial training of the shoulder.Objective:To determine the differences of functional and EMG measures between a control group and an exercise group of overhead throwers.Design:Repeated measures.Setting:Research laboratory.Participants:17 intercollegiate baseball players and 19 intercollegiate softball players divided into 2 equal-size groups, control and exercise.Main Outcome Measures:Preliminary data were recorded, including throwing velocity, throwing accuracy, and EMG activity of the biceps brachii, upper pectorals, and posterior deltoid. The exercise group completed a 4-week training regimen on the Impulse Inertial Trainer. All participants returned for follow-up measures.Results:No significant group-by-time interaction occurred relative to ball velocity, accuracy, or EMG activity.Conclusion:For experienced throwers, functional measures and muscle activity during throwing were not affected by inertial training of the upper extremity.

Context:Identifying the foot positions that are vulnerable to lateral ankle sprains is important for injury prevention. The effects of foot position in the transverse plane on ankle biomechanics during landing are unknown.Objective:To examine the effects of toe-in or toe-out positioning on ankle inversion motion and moment during single-leg landing.Design:Repeated measures.Setting:Motion analysis laboratory.Participants:18 healthy participants (9 men and 9 women).Interventions:Participants performed single-leg landing trials from a 30-cm high box under 3 conditions: natural landing, foot internally rotated (toe-in), and foot externally rotated (toe-out).Main Outcome Measures:4 toe-in or toe-out angles were calculated against 4 reference coordinates (laboratory, pelvis, thigh, and shank) in the transverse plane. Ankle inversion angle, angular velocity, and external moment in the 200 ms after initial foot-to-ground contact were compared between the 3 landing conditions.Results:All toe-in or toe-out angles other than those calculated against the shank were significantly different between each of the 3 landing conditions (P < .001). Ankle inversion angle, angular velocity, and moment were highest during toe-in landings (P < .01), while eversion angle and moment were highest during toe-out landings (P < .001). The effect sizes of these differences were large. Vertical ground reaction forces were not different between the 3 landing conditions (P = .290).Conclusions:Toe-in or toe-out positioning during single-leg landings impacts on ankle inversion and eversion motion and moment. Athletes could train not to land with the toe-in positioning to prevent lateral ankle sprains.

Context:The effects of custom-molded foot orthotics on neuromuscular processes are not clearly understood.Objective:To examine these effects on postural control in subjects with different foot types.Design:Between-groups, repeated-measures design.Setting:Athletic training laboratory.Subjects:30 healthy subjects assigned to groups by foot type: planus (n = 11), rectus (n = 12), or cavus (n = 7).Interventions:Custom-fit semirigid orthotics.Main Outcome Measures:Static postural control was measured on a force plate. Dynamic postural control was measured using the Star Excursion Balance Test. Both measurements were assessed with and without orthotics at baseline and 2 weeks later.Results:For static postural control, a significant condition-by-group interaction was found. Subjects with cavus feet had a decreased center-of-pressure velocity while wearing orthotics. For dynamic postural control, a significant condition-by-direction-by-group interaction was found. Subjects with cavus feet had increased reach distances in 3 of 8 directions while wearing orthotics.Conclusions:Custom orthotics were associated with some improvements in static and dynamic postural control in subjects with cavus feet.

Context:Functional knee braces (FKB) are used prophylactically and in rehabilitation to aide in the functional stability of the knee.Objective:To determine if alterations in select lower extremity moments persist throughout a one hour period in healthy individuals.Design:2X5 repeated measures design.Setting:Biomechanics Laboratory.Subjects:Twenty subjects (14 male and 6 female, mean age 26.5±7 yrs; height 172.4±13 cm; weight 78.6±9 kg), separated into braced (B) and no brace (NB) groups.Intervention:A one-hour exercise program divided into three 20 minute increments.Main Outcome Measures:Synchronized three-dimensional kinematic and kinetic data were collected at 20-minute increments to assess the effect of the FKB on select lower extremity moments and vertical ground reaction forces.Results:Increase in hip moment and a decrease in knee moment were noted immediately after brace application and appeared to persist throughout a one hour bout of exercise.Conclusions:The FKB and the exercise intervention caused decreases in knee joint moments and increases in hip joint moments.

Context:It has been theorized that aquatic balance training differs from land balance training.Objective:To compare the effects of balance training in aquatic and land environments.Design:Between-groups, repeated-measures design.Setting:Biomechanics laboratory and pool.Participants:24 healthy subjects randomly assigned to aquatic (n = 8), land (n = 10), or control (n = 6) groups.Intervention:Four weeks of balance training.Main Outcome Measures:Balance was measured (pre, mid, post, follow-up). COP variables: radial area, y range, x range in single leg (SL), tandem (T), single leg foam (SLF), and tandem form (TF) stance.Results:A significant condition × time interaction for x range was found, with improvements for SL, SLF, and TF. Radial area improved, with post-test 1.01 ± .23 cm2and follow-up 1.06 ± .18 cm2significantly lower than pretest 1.18 ± .23 cm2. Y range significantly improved, with posttest (4.69 ± 1.02 cm2) lower than pretest (5.89 ± 1.26 cm2). The foam conditions (SLF & TF) were significantly different from non-foam conditions (SL & T) for all variables.Conclusions:Results of this study show that balance training can effectively be performed in both land and aquatic environments.
