Phakic intraocular lens implantation in a case of Tourette syndrome exacerbated by wearing glasses and contact lenses

A 25-year-old longsighted woman presented with a history of spectacle and contact lens induced exacerbation of Tourette syndrome symptoms. Preoperatively, she had very poor unaided vision (6/60), achieving good (6/9) vision in both eyes with spectacle correction. The patient underwent bilateral simultaneous implantation of phakic implantable collamer lens (ICL) implants (STAAR, USA) which sit in front of the natural lens. Postoperatively, her uncorrected visual acuity was markedly improved as were her manifestations of Tourette syndrome. She achieved her full potential of unaided vision (6/9). This is the first documented case of Tourette syndrome triggered by glasses and contact lenses in which bilateral phakic toric lens implants were effective in improving vision and controlling motor symptoms. Phakic toric ICL implantation is a reversible technique for the correction of visually significant ametropia in selected patients and has improved this patient’s quality of life.

Purpose. To assess the efficacy, safety, and predictability of the Visian Implantable Collamer Lens (ICL) model having a central port in patients over 40 years of age. Methods. This study included 33 eyes from 21 patients who underwent V4c ICL implantation for the correction of myopia and myopic astigmatism. We assessed uncorrected (UDVA) and corrected (CDVA) distance visual acuity, refraction, intraocular pressure (IOP), endothelial cell density (ECD), vault, and adverse events occurring over a 1-year period. Results. Mean age of the patients at the time of implantation was 43.52 ± 4.49 years (range: 40 to 56 years). Efficacy and safety indexes were 1 and 1.09, respectively. Surgical outcomes for CDVA were as follows: no eye lost any lines, 19 eyes (57.58%) showed no CDVA changes, 7 eyes (21.21%) gained 1 line, 4 eyes (12.12%) gained 2 lines, and 3 eyes (9.09%) gained ≥3 lines. Mean postoperative spherical equivalent (SE) was −0.09 ± 0.47 D. A total of 29 eyes (87.8%) were within ±0.50 D and 31 eyes (93.9%) were within ±1.00 D of the desired SE. At 1-year, mean IOP was 15.27 ± 3.03 mmHg (range: 9 to 20 mmHg, p=0.12 pre vs. post) and mean ECD was 2516 ± 234 cells/mm2 (p=0.29 pre vs. post). Mean postoperative vault was 320 ± 136 μm, with 201–300 μm being the most prevalent vault range for 9 eyes (31.03%). None of the eyes showed a vault >701 μm. There were neither intraoperative nor postoperative complications; in fact, all ICL implantation procedures were uneventful. Conclusions. Our study’s findings support the use of this lens in patients over 40 years of age. A long follow-up period is advisable to monitor ICL position relative to the crystalline lens.

<p>Answering the question of why Muslims fall down suffering in many fields, including economics, is the work done by Chapra in this book. By using the model of the dynamics of socio-economic  analysis  of  Ibn Khaldun,  he  began  to  read  and  trace the history of Muslims.  Eventually he gained a few main factors causing the decline of Islam, namely:  the movement of sufism which is not running on rails of origin,  poor appreciation of the  role of women, and declining quality of education. Deterioration occurs because of political authority (G)  negligent of  its responsibilities,  especially  in  upholding justice  and  Shariah,   guarantee facilities to the people (N),   and realize their full potential. ABSTRACT Purpose To evaluate optical quality and visual function in keratoconus patients corrected with RGP contact lenses and a novel special design of silicone hydrogel contact lens. Materials and methods Twelve eyes of six patients with keratoconus were enrolled to experience a new soft contact lens (Soft-K) for keratoconus made of a silicone-hydrogel material and the outcomes were compared to the performance with gas permeable lenses and spectacles. The three situations were compared for monocular and binocular high (100%) and low contrast (10%) ETDRS LogMAR visual acuity and contrast sensitivity function (CSF). Results On average, there was an improvement of more than two lines in visual acuity over spectacle correction and this is statistically significant for both gas permeable (GP) and Soft-K lens (p < 0.001). Visual acuity was not significantly different between GP and Soft-K lens for high contrast acuity but was slightly higher with GP lens for low contrast under binocular conditions. Monocular CSF showed a marked improvement with the Soft-K lens and GP, compared to spectacles correction, particularly for medium and high frequencies; conversely. Conclusion Soft-K silicone hydrogel soft contact lens produces a clinical and statistically significant improvement in visual acuity and contrast sensitivity function over spectacle correction. Corneal topography is the main method for assessing the regularity of the surface of the cornea. Corneal irregularity leads to a deterioration in its refractive properties and a decrease in the quality of vision. Learning the basics of corneal topography will help determine the choice of a color map for a specific situation, as well as understand and analyze the data associated with these maps. This article describes the main types of topographic maps, various patterns of corneal shape in normal and pathological conditions and how to use the data obtained to design and fit contact lenses. Despite the fact that images may vary depending on topographers used, the information presented in this article is universal.
