Generational and occupational differences in human factors knowledge and awareness: a comparison study

IntroductionIncreasing healthcare sector litigation, accountability and governance has resulted in the identification of human factors (HF) as a common source of error. Both NHS and military doctors must have awareness of HF to enhance safety and productivity. There is limited published evidence examining knowledge of HF in these two healthcare professional groups.MethodsDoctors of all grades and specialties across the NHS and 3 military groups including the Defence Deanery within the UK were invited to complete a 10-item web-based survey. Questions focused on training undertaken, HF knowledge and potential future training needs.ResultsThe survey link was emailed to 250 military and 1400 NHS doctors, 191 military and 776 NHS responded (response rate: 76% and 55%, respectively). Military doctors above foundation trainees are more familiar with HF, have had more training and recognise a requirement for additional training. Military foundation trainees had similar responses to their NHS colleagues. Doctors who had not undertaken any HF training are less likely to appreciate its value, with almost 60% of senior NHS doctors reporting no training. Foundation trainees have more training in HF than their senior peers when military seniors are excluded and more frequently identified a need for further training. Junior doctors identified stress, fatigue, communication and leadership more frequently, with seniors identifying work environment and music in theatre correctly more often.ConclusionNon-training grade doctors are less likely to seek HF training. Military doctors are more familiar with HF and have undertaken more training. Given the role of HF in communication, human error, potential litigation, stress, conflict and gross negligence manslaughter convictions, further education is vital.

The most recent ST3 Anaesthetic recruitment for posts commencing in August 2021 saw larger numbers of applicants (n = 1,056) compared to previous years, with approximately 700 applicants failing to secure an ST3 post. We surveyed 536 anaesthetic junior doctors who applied for ST3 posts during this application round with the aim of investigating their experience of the recruitment process this year (response rate 536/1,056 = 51%). Approximately 61% were not offered ST3 posts (n = 326), a similar proportion to that previously reported. We asked all respondents what their potential career plans were for the next 12 to 24 months. The majority expressed intentions to take up either CT3 top-up posts or non-training fellow posts from August 2021 (79%). Other options considered by respondents included: pursuing work abroad (17%), embarking on a career break (16%), taking up an ST3 post in intensive care medicine instead of anaesthetics (15%) and permanently leaving the medical profession (9%). A number of respondents expressed a desire to pursue training in a different medical specialty (9%). Some respondents expressed an intention to pursue further education or research (10%). A large proportion of respondents (42%) expressed a lack of confidence in being able to achieve the necessary training requirements to later apply for ST4 in August 2023. The majority of respondents reported not feeling confident in achieving GMC Specialty Registration in Anaesthesia in the future without a training number (75%), and that their wider life plans have been disrupted due to the impending time out of training (78%). We received a total of 384 free-text responses to a question asking about general concerns regarding the ST3 applications process. Sentiment analysis of these free-text responses indicated that respondents felt generally negatively about the ST3 recruitment process. Some themes that were elicited from the responses included: respondents feeling the recruitment process lacked fairness, respondents suffering burnout and negative impacts on their wellbeing, difficulties in making plans for their personal lives, and feeling undervalued and abandoned despite having made personal sacrifices to support the health service during the COVID-19 pandemic. These results suggest that junior anaesthetic doctors in the UK currently have a negative perception towards postgraduate training structures, which has been exacerbated by the COVID-19 pandemic, changes to the postgraduate training curriculum and difficulties in securing higher training posts.

MAPS, the Managed Assessment Portfolio System (see http://www.maps-ict.com), is a Web-based ePortfolio system that was developed to help both teachers and learners, initially with a focus of helping to raise standards in the teaching and learning of information and communications technology in the UK. MAPS has since developed into a system covering all stages and subjects of school education, and is now being used in further education contexts. This chapter plots the progress of MAPS from an initial sketch idea to its present form: supporting over 57,000 student portfolios. The authors then draw out a number of lessons learned from such extensive use. The chapter finishes with a look at forthcoming ePortfolio issues and consideration of the requirements of lifelong learning ePortfolios.

A number of recent trends have created a growing interest in the description of library resources at collection level, and some recent programmes have provided significant funding in this area. Collection-level description is seen as a tool to improve disclosure of resources, to support collaborative management of resources, and to enable a more integrated approach to digital services. The efforts of Research Support Libraries Programme have stimulated interest in the approach and resulted in the development of a number of regional- and subject-based services based on collection-level description. It has also advanced the theoretical debate and encouraged a convergence on a common approach to collection-level description (or at least highlighted the need for such convergence to take place). Approaches to Web-based resource discovery are focusing on services which provide integrated access to distributed resources, and here, too, the description of aggregates of resources is becoming significant, and this is a consideration in the JISC's efforts to enhance access to resources for the UK Higher and Further Education communities. Collection-level description is not a substitute for item-level description but an effective complement to it.

The public service administration system at the government offices of the Kemiri village office in the Kemiri District currently does not have a computerized and integrated system. Because the system runs, residents who submit letters for administrative completeness, still use the Ms.Word / Ms.Excel application. of course has many weaknesses including human error, not neat in file storage, resulting in the lengthy process of searching and making a cover letter and required reports. The analytical method used in this study is to use PIECES (Performance, Informance, Economy, Control, Efficiency, Service) analysis, the design of the model uses UML (Unified Modeling Language). The results of this study are web-based letter information systems at the Office of the Village Chief of Kemiri that can accessed using a local computer browser. Thus the information letter needed by the Kemiri Village community and more effective and efficient in making the letter.

The article outlines some human factors affecting the operation and safety of passenger air transport given the massive increase in the use of the VLA. Decrease of the impact of the CO2 world emissions is one of the key goals for the new aircraft design. The main wave is going to reduce the burned fuel. Therefore, the eco-efficiency engines combined with reasonable economic operation of the aircraft are very important from an aviation perspective. The prediction for the year 2030 says that about 90% of people, which will use long-haul flights to fly between big cities. So, the A380 was designed exactly for this time period, with a focus on the right capacity, right operating cost and right fuel burn per seat. There is no aircraft today with better fuel burn combined with eco-efficiency per seat, than the A380. The very large aircrafts (VLAs) are the future of the commercial passenger aviation. Operating cost versus safety or CO2 emissions versus increasing automation inside the new generation aircraft. Almost 80% of the world aircraft accidents are caused by human error based on wrong action, reaction or final decision of pilots, the catastrophic failures of aircraft systems, or air traffic control errors are not so frequent. So, we are at the beginning of a new age in passenger aviation and the role of the human factor is more important than ever.

There is growing interest in the study and practice of applying data science (DS) and machine learning (ML) to automate decision making in safety-critical industries. As an alternative or augmentation to human review, there are opportunities to explore these methods for classifying aviation operational events by root cause. This study seeks to apply a thoughtful approach to design, compare, and combine rule-based and ML techniques to classify events caused by human error in aircraft/engine assembly, maintenance or operation. Event reports contain a combination of continuous parameters, unstructured text entries, and categorical selections. A Human Factors approach to classifier development prioritizes the evaluation of distinct data features and entry methods to improve modeling. Findings, including the performance of tested models, led to recommendations for the design of textual data collection systems and classification approaches.

Purpose – The University of York Library, part of its Information Directorate, has successfully run an annual user survey using LibQUAL+ since 2008. The tool has proven invaluable in understanding user needs and measuring improvements. The ability to benchmark performance has been well received by university senior managers. Following this positive experience, the Directorate piloted the TechQual+ survey to assess its technology services. TechQual+ is a total market-survey tool developed on the same principles as LibQUAL+. York was the first university in the UK to do this. The purpose of this paper is to reflect upon the experience and its relevance to information services. Design/methodology/approach – A team was established to co-ordinate the distribution of the survey, which was e-mailed to all university members. The survey was conducted over a period of three weeks in December 2011 using their web-based tool. The team worked with the survey providers to make amendments in order to comply with UK Data Protection legislation. Findings – The TechQual+ tool provided a rich set of data on the IT needs of University of York students and staff, including a wealth of comments. As the first use of the tool in the UK, a number of outcomes arose from the pilot: methods for running the survey in order to meet UK data protection requirements; feedback on the tool itself, with some questions not understood from a UK context; a rich set of results data, with some similarities (and several differences) to those available through LibQUAL+. Originality/value – The paper presents the first application of the TechQual+ survey in UK and discusses the issues faced when implementing it in a UK context. The case study will be of interest to libraries or converged services interested in assessing their IT provision.
