ObjectiveTo assess the contribution of a digital health real-time monitoring platform towards the achievement of coverage targets during a national immunisation campaign in Indonesia.InterventionsA digital health platform was introduced to facilitate real-time reporting and data visualisation. Health workers submitted reports of children immunised each day by geolocation using mobile phones. Automated reports were generated for programme managers at all levels to enable early responses to coverage gaps.MethodsRisk profiles were generated for each district to assess precampaign immunisation programme performance. Digital health platform use and progress towards targets were monitored continuously throughout the campaign. Study outcomes were total coverage and time to achieve full (100%) coverage. Kaplan-Meier, Cox and linear regression analyses were used to estimate the associations and outcomes after adjusting for district risk profiles. A complementary qualitative assessment explored user experiences and acceptance through interviews with vaccinators and programme managers in provinces and districts selected through multistage random sampling.ResultsBetween August and December 2018, 6462 health facilities registered to use the digital health platform across 28 provinces and 395 districts. After adjusting for precampaign district risk profile and intracampaign delays due to vaccine hesitancy, districts with greater platform utilisation demonstrated higher coverage overall (R2=0.28, p<0.0001) and a shorter interval to achieving full coverage (>75% reporting compliance; Risk Ratio 15.4, 95% CI 5.8 to 40.6). Stronger effects were observed among districts experiencing implementation delays due to vaccine hesitancy. Results from 106 key informant interviews conducted in 6 provinces and 18 districts suggest high degrees of acceptability, ease of use and satisfaction.ConclusionA digital health platform introduced for real-time monitoring of a national immunisation campaign in Indonesia was feasible, well liked and associated with improved problem solving and programme performance, particularly among districts affected by vaccine hesitancy.Trial registration numberISRCTN10850448.

Manufacturing industries based on Internet of Things (IoT) technologies play an important role in the economic development of intelligent agriculture and watering. Water availability has become a global problem that afflicts many countries, especially in remote and desert areas. An efficient irrigation system is needed for optimizing the amount of water consumption, agriculture monitoring, and reducing energy costs. This paper proposes a real-time monitoring and auto-watering system based on predicting mathematical models that efficiently control the water rate needed. It gives the plant the optimal amount of required water level, which helps to save water. It also ensures interoperability among heterogeneous sensing data streams to support large-scale agricultural analytics. The mathematical model is embedded in the Arduino Integrated Development Environment (IDE) for sensing the soil moisture level and checking whether it is less than the pre-defined threshold value, then plant watering is performed automatically. The proposed system enhances the watering system’s efficiency by reducing the water consumption by more than 70% and increasing production due to irrigation optimization. It also reduces the water and energy consumption amount and decreases the maintenance costs.

Social media allow anti-vaxxers to quickly spread misinformation and false statements. This situation may lead to an increase in vaccine hesitancy. We wanted to characterize what arguments against COVID-19 vaccines run on Facebook in Poland. We analyzed Facebook comments related to the five events of the introduction of COVID-19 vaccines—announcements of the efficacy of the Pfizer-BioNTech (09.11.2020), Moderna (16.11.2020), and AstraZeneca (23.11.2020) vaccines, registration of the Pfizer-BioNTech vaccine by the European Medicines Agency (21.12.2020), and the first vaccination in Poland (27.12.2020). We collected the comments from fanpages of the biggest Polish media and then established their main anti-vaccine themes. We found that the negative arguments about COVID-19 vaccines can be divided into 12 categories. Seven of them are universal and also apply to other vaccines but five are new and COVID-19’ specific. The frequency of arguments from a given category varied over time. We also noticed that, while the comments were mostly negative, the reactions were positive. Created codebook of anti-vaccine COVID-19 arguments can be used to monitor the attitude of society towards COVID-19 vaccines. Real-time monitoring of social media is important because the popularity of certain arguments on Facebook changes rapidly over time.

The development of dry, ultra-conformable and unperceivable temporary tattoo electrodes (TTEs), based on the ink-jet printing of poly(3,4-ethylenedioxythiophene) polystyrene sulfonate (PEDOT:PSS) on top of commercially available temporary tattoo paper, has gained increasing attention as a new and promising technology for electrophysiological recordings on skin. In this work, we present a TTEs epidermal sensor for real time monitoring of respiration through transthoracic impedance measurements, exploiting a new design, based on the application of soft screen printed Ag ink and magnetic interlink, that guarantees a repositionable, long-term stable and robust interconnection of TTEs with external “docking” devices. The efficiency of the TTE and the proposed interconnection strategy under stretching (up to 10%) and over time (up to 96 h) has been verified on a dedicated experimental setup and on humans, fulfilling the proposed specific application of transthoracic impedance measurements. The proposed approach makes this technology suitable for large-scale production and suitable not only for the specific use case presented, but also for real time monitoring of different bio-electric signals, as demonstrated through specific proof of concept demonstrators.

IntroductionIndia has become a lighthouse for large-scale digital innovation in the health sector, particularly for front-line health workers (FLHWs). However, among scaled digital health solutions, ensuring sustainability remains elusive. This study explores the factors underpinning scale-up of digital health solutions for FLHWs in India, and the potential implications of these factors for sustainability.MethodsWe assessed five FLHW digital tools scaled at the national and/or state level in India. We conducted in-depth interviews with implementers, technology and technical partners (n=11); senior government stakeholders (n=5); funders (n=1) and evaluators/academics (n=3). Emergent themes were grouped according to a broader framework that considered the (1) digital solution; (2) actors; (3) processes and (4) context.ResultsThe scale-up of digital solutions was facilitated by their perceived value, bounded adaptability, support from government champions, cultivation of networks, sustained leadership and formative research to support fit with the context and population. However, once scaled, embedding digital health solutions into the fabric of the health system was hampered by challenges related to transitioning management and ownership to government partners; overcoming government procurement hurdles; and establishing committed funding streams in government budgets. Strong data governance, continued engagement with FLHWs and building a robust evidence base, while identified in the literature as critical for sustainability, did not feature strongly among respondents. Sustainability may be less elusive once there is more consensus around the roles played between national and state government actors, implementing and technical partners and donors.ConclusionThe use of digital tools by FLHWs offers much promise for improving service delivery and health outcomes in India. However, the pathway to sustainability is bespoke to each programme and should be planned from the outset by investing in people, relationships and service delivery adjustments to navigate the challenges involved given the dynamic nature of digital tools in complex health systems.

BACKGROUND An mHealth app was developed for front line health workers, to strengthen maternal, neonatal and child health services through increased transparency, accountability, and improved governance by collecting and disseminating real-time data. Key features included tracking frontline health workers through Global Positioning System during outreach visits, registration of clients; and recording data. OBJECTIVE In this paper we report the end user experiences of the captured during the qualitative end line study of the Hayat app. This includes perceptions and experience of the using the digital app with a focus on usability, validity of data, and community response. This paper aims to identify barriers and enablers to integration of mhealth and inform how digital technology can be utilized for strengthening the district health systems in LMICs. METHODS Qualitative assessment was carried out in select rural districts of Khyber Pakhtunkhwa and Gilgit-Baltistan in Pakistan and select catchments of Bamyan and in Badakshan Province in Afghanistan. Methods used for assessment included 17 focus group discussions with LHWs and 28 key informant interviews with health workers and key stakeholders. Thematic content analysis was undertaken based on an adapted framework derived from the World Health Organization guide for “Monitoring and Evaluating Digital Health Interventions” and technology acceptance model. RESULTS The study revealed high usability of the app both by the stakeholders. Overall, the app improved quality and timeliness of data, improved immunization coverage and maternal care through supportive supervision and monitoring of frontline health workers. Data validity improved and health managers were confident of the reliability of data collected through the app. However, connectivity issues, difficulty in access to remote sites, security issues, lack of incentives and increased workload were some of the perceived barriers identified by the end users. CONCLUSIONS The use of smart phone-based application has high acceptability among the HCPs, and has benefitted the existing health system functioning but providing reliable data, and better monitoring. However, successful integration of app in the health system will require inter sectorial collaboration to address the challenges identified in implementation. CLINICALTRIAL N/A
