Effective communication between members of the multidisciplinary team is imperative for patient safety. Within the Medicine for the Elderly wards at Royal Victoria Hospital (RVH) in Dundee, we identified an inefficient process of information-sharing between the orthopaedics outpatient department (OPD) at the main teaching hospital and our hospital’s rehabilitation teams, and sought to improve this by introducing several changes to the work system. Our aim was for all patients who attended the OPD clinic to have a plan communicated to the RVH team within 24 hours.Before our intervention, clinic letters containing important instructions for ongoing rehabilitation were dictated by the OPD team, transcribed and uploaded to an electronic system before the RVH team could access them. We analysed clinic attendances over a 4-week period and found that it took 15 days on average for letters to be shared with the RVH teams. We worked with both teams to develop a clinical communication tool and new processes, aiming to expedite the sharing of key information. Patients attended the OPD with this form, the clinician completed it at the time of their appointment and the form returned with the patient to RVH on the same day.We completed multiple Plan–Do–Study–Act cycles; before our project was curtailed by the COVID-19 pandemic. During our study period, seven patients attended the OPD with a form, with all seven returning to RVH with a completed treatment plan documented by the OPD clinician. This allowed rehabilitation teams to have access to clinic instructions generated by orthopaedic surgeons almost immediately after a patient attended the clinic, essentially eliminating the delay in information-sharing.The introduction of a simple communication tool and processes to ensure reliable transfer of information can expedite information-sharing between secondary care teams and can potentially reduce delays in rehabilitation.

BACKGROUND Healthcare for older patients is a worldwide challenge for public health system. A new medical Internet system in healthcare which is a new model of telegeriatrics system has been established. The key innovation is the new telegeriatrics system was conducted jointly by general practitioners in the Community Health Service Center and specialists in university teaching hospital. Unlike the typical telemedicine that has been practiced in other countries, the new model provides a solution for the key issues in telemedicine where a doctor is unable to conduct a direct physical examination and the associated potential diagnostic error. OBJECTIVE This study is to introduce the operation mechanism of the new Telegeriatrics system and analyze healthcare demands of older patients in different age groups applying the new Telegeriatrics system. METHODS 472 older patients (aged≥60) were enrolled and divided into the young older group (aged 60 to 74), the old older group (aged 75 to 89) and the very old group (aged≥90) according to the age stratification of World Health Organization. Proportion of the top 10 diseases of older patients of different age groups was analyzed. RESULTS The process of older patients’ diagnosis and treatment made by specialist and general practitioners formed a closed loop. It ensures the timeliness and effectiveness of diagnosis and treatment of older patients. The treatment effect can be observed by general practitioners and specialist can adjust the treatment plan in time. In this study, it was found that older patients in different age groups have different healthcare demands. Coronary heart disease and type 2 diabetes mellitus were found to be the main diseases of the older patients and the young older patients as well as the old older patients applying Telegeriatrics. CONCLUSIONS The new telegeriatrics system can provide convenient and efficient healthcare services for older patients and overcome the disadvantage of currently used models of telegeriatrics. Older patients in different age groups have different medical care demands. Cardiovascular diseases and metabolic diseases have become the main diseases of the elderly applying the new Telegeriatrics system. Healthcare policy makers should invest more medical resources to the prevention of cardiovascular diseases and metabolic diseases in the elderly.

AimsClinicians struggle to provide information to each other that supports safe patient transitions, especially across acute and community care jurisdictions. They need flexible communication tools to improve care coordination. Island Health introduced a Secure Mobile and Clinical Communication Solution (SMaCCS) to address these challenges in 2018. In this study we evaluated the SMaCCS system to understand the (1) volume and flow of healthcare communication, (2) degree of adoption and accessibility of the system and (3) user experience.MethodsThis was a prospective, cross-sectional, observational study. Island Health Information Management/Information Technology (IMIT) selected Vocera Collaboration Suite as the secure messaging platform. We invited healthcare providers in various roles in the hospital and community to use SMaCCS for their daily communications and system and survey data were collected between February and August 2018. System data and survey data were used to determine outcomes.ResultsA Sankey diagram represents the volume and flow of communication. A total of 2542 messages were sent and 79% of conversations included more than a single message. Eighty-one per cent of participants agreed that using a secure communication tool made them feel more comfortable sharing patient information. Most users (65%) perceived that the application was a useful method for transmitting simple information.ConclusionHowever, our study showed that different occupational roles require different frequencies and volumes of communication and there are numerous barriers to adoption that must be addressed before secure messaging can be an effective, ubiquitous method of clinical communication.

Lumbar spondylolisthesis typically presents as low back and/or lower extremity pain. Spondylolisthesis is most commonly observed in female patients and the elderly. Lumbar spondylolisthesis may result from congenital, isthmic, trauma-related, degenerative, and iatrogenic causes. Diagnostic imaging (radiographs, magnetic resonance imaging) and physical examination are needed to differentiate the categories of lumbar spondylolisthesis, which will assist in selecting the appropriate treatment. A customized interdisciplinary treatment plan tailored to the patient’s presentation and goals will produce clinical improvement. Surgical remediation is typically reserved for recalcitrant cases but can be effective.

The article is devoted to the general study of the peculiarities of normative and legal regulation of pension provision in connection with the loss of the breadwinner in Ukraine and the determination of directions for improving the mechanism of pension provision of disabled persons in connection with the loss of the breadwinner on the basis of the analysis of the current legislation of Ukraine. The concept, content, conditions, size and essence of the pension in connection with the loss of the breadwinner, since state pension provision is one of the fundamental measures to ensure the rights and freedoms of a person and a citizen in Ukraine, is revealed. Statistical analysis of the number of persons receiving a pension in connection with the loss of breadwinner during 2018-2020 was conducted. A number of problematic issues related to the introduction of the electronic system - "Portal of electronic services of the Pension Fund of Ukraine", among them: ignorance of the elderly in modern technologies; «human factor» is connected with the inattention of employees of the Pension Fund of Ukraine on the transfer of information to electronic registers; the lack of modern serviceable technological resources in the departments of the Pension Fund of Ukraine; the risk of theft or distribution of personal data by third parties, hackers or other persons. It was established that the improvement of the mechanism of pension provision of disabled persons in connection with the loss of breadwinner on the basis of the analysis of the current legislation of Ukraine will create prospects for improving the pension provision of citizens in order to realize the constitutional rights of citizens to social protection, including the right to pension provision in case of loss of breadwinner. The directions of improvement of the pension provision mechanism in case of loss of breadwinner are proposed, such as: raising the size of the pension with the calculation of prices for the consumer basket; promoting public awareness in the functioning of the electronic system of the Pension Fund of Ukraine; technological updating of equipment and equipment to avoid problems in the process of providing services for the population; development and installation of special programs for the protection of personal information of citizens in the electronic system of the Pension Fund of Ukraine and others.

Shortening hospital stays increases communication needs between nurses in inpatient and outpatient wards. Smooth information sharing is required to reduce the workload of nurses and improve the quality of patient care. However, electronic medical records (EMR) system does not have sufficient functions to support information sharing between wards, because EMR has been developed mainly for recording. This study led to three improvements; unified communication tool, common patient list linked to EMR, and outpatient nursing diagnosis.

Currently, Japan is rapidly aging. Japanese government agencies report that the percentage of elderly people whose ages are at least 65 years will increase by up to about 30 percent in 2025. As one of the measures towards this situation, the community-based integrated healthcare system will be introduced in Japan. The system aims to provide elderly people living at home with appropriate health, medical, and welfare services. We focus on the burden of sharing information on the situation of the elderly at home among health, medical, welfare staffs, and neighbors. We have been developing a supporting system for sharing information on the situation of the elderly at home and conducted a field test around one year. We consider that various stakeholders involved in the community comprehensive health care system could recognize the importance of information sharing and collaboration with them through this kind of social implementation
