Possible drug-induced follicular mucinosis in a dog

A nine-year-old, entire male basset hound presented with one-year history of pruritus and patchy to diffuse alopecia accompanied by erythema, starting a few days after applying an implant of deslorelin for treatment of benign prostatic hyperplasia. Dermatopathological examination was diagnostic for follicular mucinosis, accompanied by a characteristic lymphocytic mural folliculitis at the level of the isthmus. Partial improvement with anti-inflammatory doses of prednisone was observed. Thereafter, oral ciclosporin was administered on a daily regimen until complete remission was achieved, after which it was gradually tapered until withdrawal. No other implants were applied thereafter, and two years later the patient remains free of clinical signs.

Benign prostatic hyperplasia (BPH) is arguably the most common benign disease among men. This disease is often associated with lower urinary tract symptoms (LUTS) in men and significantly decreases the quality of life. Polyphenol consumption reportedly plays an important role in the prevention of many diseases, including BPH. In recent years, in addition to disease prevention, many studies have reported the efficacy and safety of polyphenol treatment against various pathological conditions in vivo and in vitro. Furthermore, numerous studies have also revealed the molecular mechanisms of the antioxidant and anti-inflammatory effects of polyphenols. We believe that an improved understanding of the detailed pharmacological roles of polyphenol-induced activities at a molecular level is important for the prevention and treatment of BPH. Polyphenols are composed of many members, and their biological roles differ. In this review, we first provide information regarding the pathological roles of oxidative stress and inflammation in BPH. Next, the antioxidant and anti-inflammatory effects of polyphenols, including those of flavonoids and non-flavonoids, are discussed. Finally, we talk about the results and limitations of previous clinical trials that have used polyphenols in BPH, with particular focus on their molecular mechanisms of action.

AbstractA 12 year old male Fox Terrier (case 1) and a one year old female Poodle (case 2) were presented with a history of dysorexia and vomiting, and case 2 also with distress associated with eating. Both dogs were diagnosed with acute pancreatitis based on clinical signs, blood analysis and ultrasonography which revealed the presence of a vegetable foreign body in the proximity of the pancreatic right lobe. Laparatomy enabled the gramineae awns to be removed which led to full resolution of clinical signs. We believe that these are the first reported cases of acute pancreatitis due to grass awns.

Introduction: Androgenetic alopecia (AGA) is associated with increased risk of several systemic diseases and some environmental factors, however, controversies exist. Since AGA and Benign Prostatic Hyperplasia (BPH) share common pathogenesis and AGA manifests some decades before BPH onset, it may serve as an early marker of BPH.Objective: This study was conducted to know AGA and its association with BPH in men ≥20 years of age.Materials and Methods: Clinically diagnosed cases of AGA (n=176) and 117 age matched healthy controls were enrolled. All cases and controls were subjected for abdomino-pelvic ultrasonography, urinary flowmetry, fasting lipid profiles, glycemic index and body mass index. International Prostate Symptom Score (IPSS) was also assessed.Results: Among 176 patients, 120 (68.18%) had Hamilton-Norwood grade III AGA and 56 (31.82%) had grade IV-VII AGA. In both groups, 140 (79.55%) cases and 93 (79.49%) controls were aged <35 years respectively. Family history of AGA was present in 108 (61.36%) cases and 2 (1.71%) controls. This observation was statistically significant with OR= 89.61 (95%CI 23.67-339.29). Three (1.7%) cases and none of the controls had prostate volume >30ml. Seventeen(9.66%) cases and 4 (3.42%) controls were graded as moderately/severely symptomatic IPSS. Statistically significant association was seen between family history and early onset of hair loss (<35 years) in a male sibling or parent.Conclusion: Although positive family history was associated with early onset of AGA, no association between AGA and BPH could be elicited in our study.

Abstract: Erectile dysfunction (ED) is an inability to achieve an erection sufficient for intercourse with his partner which causes dissatisfaction for both of them. The etiology of ED is classified as psychogenic, organic, drug abuse, and also by post-surgery. Benign prostatic hyperplasia (BPH) is a disease caused by aging. BPH clinical signs usually appear in more than 50% of men aged ≥50 years. This was a survey-descriptive study with a cross sectional design. Samples were obtained by using purposive sampling technique. Respondents were patients at Efrata Adventist Clinic in Manado. The instrument in this study was modified IIEF-5 questionnaire. The results showed that based on the duration of BPH, respondents who suffered from BPH >3 years were as many as 75.0% and <1 year were 7.1%. Based on the ages, respondents of 61-70 years were 46.5 and of 41-50 years were 7.1%. The erectile dysfunction of respondents was classified as moderate 42.9%, mild-moderate 32.1%, severe 17.9%, and mild 7.1%. Conclusion: Most of the erectile dysfunction with BPH >3 years was classified as moderate.Keywords: erectile dysfunction, BPH Abstrak: Disfungsi ereksi (DE) yaitu suatu ketidakmampuan untuk mencapai ereksi yang cukup untuk melakukan senggama bersama pasangannya sehingga menimbulkan ketidakpuasan diantara keduanya. Etiologi DE diklasifikasikan menjadi psikogenik, organik, penyalahgunaan obat-obatan dan juga oleh pasca tindakan bedah. Benign prostatic hyperplasia (BPH) adalah penyakit yang disebabkan oleh penuaan. Tanda klinis BPH biasanya muncul pada lebih dari 50% laki-laki yang berusia 50 tahun ke atas. Jenis penelitian ialah survei deskriptif-observasional dengan desain potong lintang. Pengambilan sampel dilakukan dengan teknik purposive sampling pada seluruh pasien di Klinik Advent Efrata Tikala Manado. Variabel penelitian ialah pasien BPH di Klinik Advent Tikala Manado. Instrumen penelitian menggunakan kuesioner IIEF-5 yang telah dimodifikasi. Hasil penelitian mendapatkan berdasarkan lama menderita BPH, responden yang menderita BPH >3 tahun sebesar 75,0%; 1-2 tahun sebesar 17,9%; dan <1 tahun sebesar 7,1%. Berdasarkan usia responden berusia 61-70 tahun sebesar 46,5% dan 41-50 tahun sebesar 7,1%. DE pada BPH paling banyak termasuk klasifikasi sedang (42,9%), diikuti ringan-sedang (32,1%), berat (17,9%) dan ringan (7,1%). Simpulan: Sebagian besar pasien DE dengan BPH >3 tahun termasuk dalam klasifikasi sedang. Kata kunci: disfungsi ereksi, BPH

Despite its long history of untoward side effects of a systemic autoimmune disease, drug-induced lupus can be difficult to recognize because of the disconnect between chronic drug usage and onset of symptoms. In this case, the patient was treated with hydralazine for two years when symptoms were initially reported, but a diagnosis of hydralazine-induced lupus was not considered for another half year. Despite treatment with steroidal and nonsteroidal anti-inflammatory medications during this period, rheumatologic symptoms and signs continued to deteriorate, consistent with the diagnosis of systemic lupus erythematosus. Not until the patient voluntarily discontinued hydralazine did symptoms begin to improve, fully resolving over the subsequent 6–12 months largely in the absence of anti-inflammatory medication. This patient demonstrates that failure to recognize a drug-induced disease etiology can result in substantial worsening of rheumatologic symptoms over the subsequent six months, ultimately satisfying criteria for systemic lupus erythematosus. While symptoms and signs largely normalized, some laboratory abnormalities and occasional arthralgia remained two years after discontinuing hydralazine, suggesting smoldering inflammatory disease.
