Osteochondroma of the Radial Tuberosity in Child

Osteochondromas are common benign bone tumors. They may occur on any bone preformed in cartilage, but usually are found on the metaphysis of a long bone near the physis. In this article, we report a case of the subluxation of the radial head caused by osteochondroma on the radial tuberosity. A 9-year-old female patient presented with a hard palpable mass and pain in her posterior aspect of left elbow joint. Preoperative radiography and MRI demonstrated an expansile bone lesion arising in the left radial tuberosity of the proximal radius. Resection of the bone tumor was done without any complications. Biopsy confirmed the diagnosis of osteochondroma without malignant degeneration. A one year after surgery, the elbow had full range of motion without any discomfort and complication.

SummaryA caudolateral radial head luxation and concurrent distal humeroulnar subluxation were diagnosed in the left forelimb of a five-month-old Shih Tzu. The problems were treated successfully by proximal ulna osteotomy, reduction of the radial head luxation and placement of a transarticular external skeletal fixator (TESF). Three weeks postoperatively the reduction of the radial head luxation had been successfully maintained, the humero-ulnar luxation had corrected and the fixator was removed. There was a full range of motion in the left elbow joint and the radial head could not be reluxated.A caudolateral left radial head luxation in a five-month-old Shih Tzu was treated by ulna osteotomy, reduction of the luxation and placement of a transarticular external skeletal fixator. A concurrent humero-ulnar subluxation suggested that the problem was developmental, associated with premature closure of the distal ulnar growth plate in a chondrodys-trophic breed.

A 57 year old man patient presented with fever and frontal headache. He had a background history of sero-positive rheumatoid arthritis which was well controlled on immunomodulatory disease modifying anti-rheumatoid drugs (DMARDS) including methotrexate and leflunomide. Six months earlier he had returned from Massachussetts in the USA after a one year period of residence there. On examination his vital signs were within normal limits and he was afebrile with a temperature of 36.1o C. His left elbow joint was warm, tender and swollen; examination was otherwise normal.

Abstract Background Primary cutaneous mucinosis are a heterogeneous group of diseases characterized by the deposition of glycosaminoglycans in the dermis and the follicles. These diseases are rare in children therefore their diagnosis and management are still challenging. Joint involvement has been reported in patients with secondary cutaneous mucinosis and, rarely, in primary mucinosis. We describe a case of Cutaneous Mucinosis of Infancy with joint involvement. Case presentation An healthy 5-year-old boy showed acute arthritis of the left knee and left elbow confirmed by ultrasound. Laboratory tests were within normal range. Symptoms disappeared after a course of nonsteroid anti-inflammatory drugs. One year later, the knee swelling reappeared; juvenile idiopathic arthritis was diagnosed and intra-articular steroid injection was performed. Due to persistence of arthritis of the knee he was admitted to our hospital. On physical examination variable skin-colored lesions were observed, which had been in existence for over 2 years. We performed a skin biopsy that showed an interstitial mucine deposition in the reticular dermis. Cutaneous Mucinosis of Infancy was diagnosed. Discussion and conclusions Cutaneous Mucinosis of Infancy is a persistent dermatosis with benign prognosis and no treatment is generally required. Our case report is particularly interesting because it is the first in which joint involvement has been reported in CMI, a disorder that has so far been described as limited to skin involvement. Further studies will be necessary in order to clarify the pathogenesis of joint involvement in primary mucinosis.

Extrapulmonary tuberculosis (TB) is known to occur in the musculoskeletal system, including the elbow joints. These cases are rarely found because the signs and symptoms are not specific to extrapulmonary TB or other diseases. We report a case of a 24-year-old male, who complained about pain in his left elbow and noticed swelling. Initially, he complained about pain all over his left arm, after several reflexology massages to alleviate his toothache. However, instead of seeking medical treatment, he visited a traditional massage therapist every week without improvement in his left arm pain for almost one year. Examination showed skin perforation with discharge. He also had fever during the first few days when the elbow became swollen. Weight loss and a decreased appetite were also noticed by the patient. The patient went to the orthopedic department and underwent surgery. Radiological examination indicated bone erosion on the left humerus and radius, while posteroanterior chest X-ray did not show any abnormality. Histopathological examinations from biopsy and fluid aspiration showed granulomas and datia Langhans cells. Mycobacterium tuberculosis was found on acid-fast bacteria smear and culture. The patient was administered multidrug tuberculosis therapy, which consisted of two months of an intensive phase and seven months of a continuation phase, in accordance with the World Health Organization’s guidelines for extrapulmonary tuberculosis treatment. He has currently undergone the continuation phase of the treatment and his condition has improved. Early detection of tuberculosis of the elbow can prevent damage to joint structure and impairment of joint function.

The current study aims to analyze the relationship of selected kinematic variables to the technique of pull shot in cricket. For the purpose of the study the researcher purposively selected five male inter-university cricket players from lnipe, gwalior of 18 to 24 years., the technique of pull shot in batting performance of each selected subjects was recorded on the basis of certain criteria (point system). It was divided into 2 components; 1. placement of feet and 2. execution (contact) each consist of maximum 10 points. The angles at selected joints were recorded to the nearest degree. The c.g. was measured nearest to the 1/10th of the centimetre. On the basis of sequence photographs obtained from the videography, the scholar developed stick figures from which selected kinematic variables was calculated. The center of gravity of whole body was determined by kinovea software. To find the the relationship of selected kinematic variables to the technique of pull shot in cricket pearson product moment correlation coefficient was used by means of spss software. It was found that, in case of moment execution angle at left shoulder joint and angle at left elbow joint showed significant relationship with performance.

<p class="abstract"><strong>Background:</strong> Elastic stable intramedullary nailing for the treatment of paediatric femur and tibial diaphyseal fractures was introduced by Prevot and colleagues in 1979. It follows three-point fixation principle that provides internal support in presence of cortical contact and an intact soft-tissue envelope. This technique has many advantages, including better reduction, dynamic axial stabilization, shorter hospitalization with early rehabilitation and low complication rate.</p><p class="abstract"><strong>Methods:</strong> This is a prospective observational study done at Kauvery Medical Centre, Trichy between May 2017 to May 2018 consisting of 39 children between age 5 to 16 years with diaphyseal fractures of femur and tibia. The fractures were treated by closed reduction and internal fixation with titanium elastic intramedullary nailing. The patients were evaluated clinically and radiologically and followed for an average of 6 months. Outcome was assessed using transcutaneous electrical nerve stimulation (TENS) scoring system used by Flynn et al.<strong></strong></p><p class="abstract"><strong>Results:</strong> Our series consisted of 39 patients (22 cases with fracture shaft of femur and 17 cases with fracture shaft of tibia), 33 males and only 6 females. Average time for radiological union was 9.89 weeks. All patients had full range of hip and ankle motion and 2 (5.1%) patients had mild restriction in knee flexion at 12 weeks.</p><p class="abstract"><strong>Conclusions:</strong> Elastic stable intramedullary nailing is an ideal method for treatment of paediatric femoral and tibial diaphyseal fractures due to lower complication rate and good functional outcome in comparison to other methods of treatment.</p>

Abstract. The Technical Note presents a statistically correct approach to evaluating simultaneous measurements of several atmospheric components under the assumption of photochemical equilibrium. We consider simultaneous measurements of OH, HO2, and O3 at the altitudes of the mesosphere as a specific example and their daytime photochemical equilibrium as an evaluating relationship. A simplified algebraic equation relating local concentrations of these components in the 50–100 km altitude range has been derived. The parameters of the equation are air temperature, air concentration, local zenith angle, and the rates of 9 reactions. We have performed one-year simulation of the mesosphere and lower thermosphere using a 3D chemical-transport model. The simulation shows that the discrepancy between the calculated evolution of the components and the equilibrium value given by the equation does not exceed 3–4 % in the full range of altitudes independent of season or latitude. We have developed the technique of statistic Bayesian evaluation of simultaneous measurements of OH, HO2 and O3 based on the equilibrium equation taking into account the measurement error. The first results of application of the technique to MLS/Aura data are presented in this Technical Note. It has been found that the satellite data of HO2 distribution regularly demonstrates essentially lower altitudes of mesospheric maximum of this component. This has also been confirmed by offline retrieval of HO2 from the MLS primary data.

AbstractTumoral calcinosis (TC) is an uncommon disorder characterized by the deposition of calcium phosphate in periarticular tissues. TC has been a controversial clinico-pathological entity first described in 1943. The calcium deposits are usually present in the large joints such as the hips and shoulders. Patients often present with localized swelling and reduced mobility around the involved joints which interfere with activities of daily living. A 12-year-old boy presented at our clinic with a painful swelling around his left elbow joint with 6-month duration over which the swellings progressively increased. Radiological imaging identified a soft tissue calcified mass present around both elbow joints. Excision and biopsy of the left elbow demonstrated a tumor consisting of myxoid material with multiple areas of calcification with a well-defined capsule. Microscopy confirmed typical features of TC. We present our case report due to unusual clinical presentation noted in this case.

SummaryThis case report describes an acute traumatic fracture of the medial coronoid process (MCP) in an immature male Rottweiler. On routine radiographs of the left elbow joint, clear abnormalities could not be detected. The definitive diagnosis was based on craniolaterocaudomedial oblique radiographs and a CT scan. The fragment was removed arthroscopically via a medial approach. Histologically the fragment did not show any signs of degeneration nor demineralisation, as seen in classical fragments of the medial coronoid process. One week after treatment, complete limb function had been regained. Follow-up radiographs after nine month showed a minimal increase of osteoarthrosis.

Introduction. To investigate the clinical results of arthroscopic management for the unstable inferior leaf of the lateral meniscus anterior horn and associated cysts through an inframeniscal portal. Methods. From March 2005 to October 2014, 64 patients with an unstable inferior leaf of the lateral meniscus anterior horn and associated cysts underwent arthroscopic management with an inframeniscal portal. The mean age of the patients was 36.9 years (range, 18 to 49 years). The mean follow-up period was 28 months (range, 24 to 44 months). Clinical results were assessed using physical examination, the Lysholm knee score, and postoperative magnetic resonance scanning. Results. The median Lysholm score improved significantly at 1 year after surgery and at final follow-up. Magnetic resonance scanning at least one year after the operation revealed no recurrent meniscal tears or cysts. No reoperations were required after an average follow-up of 28 months. All patients reported significant symptomatic relief after the operation. They had full range of motion at three months and returned to normal activities and sports one year after surgery. Conclusion. The direct inframeniscal portal can provide an effective approach to manage lesions in the anterior horn of the lateral meniscus with predictable clinical outcomes.
