A 9-year-old girl was referred to a trauma centre with severe head injury. 3D CT scan revealed depressed fracture involving the frontal bone on the right side, right parietal bone, and right superior orbital margin, right lamina papyracea. The frontal table was managed conservatively and open reduction and internal fixation was done for the supraorbital blow in to correct the ocular dystopia. The clinical course, possible mechanism, and management of the patient are discussed.

The displaced molar tooth is an unexpected and uncommon intraoperative complication of tooth extraction. Its prompt and early identification and management is crucial, as this may reduce the patient morbidity. Orthopantomogram (OPG) has limitations because of extensive anatomical superimposition of various structures. A 36-year-old female patient was referred for Computed Tomography (CT) of maxillofacial region with 3D CT scan for severe pain and swelling in right cheek region with inability to open mouth for 4 days following the dental intervention in outside dental clinic. CT scan of maxillofacial region showed bony defect in the right 3rd maxillary alveolus with multiple small bony fragments. Small fragment of molar tooth was displaced into the right infratemporal fossa and was impinging on right lateral pterygoid muscle with surrounding ill-defined soft tissue infiltrates in right retroantral fat. CT Face with 3D reconstruction helps in accurate localisation and gives appropriate anatomical detail in case of displaced molar tooth into infratemporal fossa. Thus, a detailed radiological investigation and documenting the relations of the displaced tooth or fragment with adjacent structures provides crucial information for proper surgical planning and avoiding possible complications.

Right coronary artery aneurysms are rare and may result from severe coronary disease, with few cases described in the literature. Mortality is high, and therapy is still controversial. We report the case of a 72-year-old woman with arterial hypertension, and a family history of coronary artery disease, who evolved for 2 months with episodes of palpitations and dyspnea on moderate exertion. During the evaluation, a giant aneurysm was found in the proximal third of the right coronary artery. The patient underwent surgical treatment with grafting of the radial artery to the right coronary artery and ligation of the aneurysmal sac, with good clinical course.

Abstract Background This study aims at determining the biological effect of 75/25 w/w nano-hydroxyapatite/chitosan (nHAp/CS) scaffolds on bone regeneration, in terms of fraction of bone regeneration (FBR), total number of osteocytes (Ost), and osteocyte cell density (CD), as well as its biodegradability. Methods Two critical-size defects (CSDs) were bilaterally trephined in the parietal bone of 36 adult Sprague-Dawley rats (18 males and 18 females); the left remained empty (group A), while the right CSD was filled with nHAp/CS scaffold (group B). Two female rats died postoperatively. Twelve, 11, and 11 rats were euthanized at 2, 4, and 8 weeks post-surgery, respectively. Subsequently, 34 specimens were resected containing both CSDs. Histological and histomorphometric analyses were performed to determine the FBR, calculated as [the sum of areas of newly formed bone in lateral and central regions of interest (ROIs)]/area of the original defect, as well as the Ost and the CD (Ost/mm2) in each ROI of both groups (A and B). Moreover, biodegradability of the nHAp/CS scaffolds was estimated via the surface area of the biomaterial (BmA) in the 2nd, 4th, and 8th week post-surgery. Results The FBR of group B increased significantly from 2nd to 8th week compared to group A (P = 0.009). Both the mean CD and the mean Ost values of group B increased compared to group A (P = 0.004 and P < 0.05 respectively). Moreover, the mean value of BmA decreased from 2nd to 8th week (P = 0.001). Conclusions Based on histological and histomorphometric results, we support that 75/25 w/w nHAp/CS scaffolds provide an effective space for new bone formation.

Background Eagle’s syndrome (Elongated styloid process) is often misdiagnosed due to its vague symptomatology. The diagnosis relies on detail history taking, palpation of styloid process in tonsillar fossa and imaging modalities.Objective To assess the length and medial angulation of elongated styloid process with the help of three dimensional computed tomography (3D CT) scan and to describe our clinical and surgical experience with patients suffering from Eagle’s syndrome.Method Prospective, analytical study conducted from August 2011 to August 2012 among 39 patients with Eagle’s syndrome. Detailed history taking, clinical examination and 3D CT scan was performed. Length and medial angulation was calculated. Patients with styloid process length longer than 2.50 cm underwent surgical excision via intraoral approach. Medial angulation of styloid process on both sides was correlated with each other using rank correlation coefficient. Wilcoxon Signed Rank test was applied to test significant difference between pre-operative and post-operative symptoms scores.Result Significant positive correlation was found between the medial angulation of styloid process on right side and left side (? =0.81, p<0.001). Significant difference was also observed between pre and post-operative symptoms scores (z=-5.16, p<0.001) .Conclusion Possibility of Eagle’s syndrome should always be considered while examining patients with vague neck pain. 3D CT reconstruction is a gold standard investigation which helps in studying the relation of styloid process with surrounding structures along with accurate measurement of its length and medial angulation.Kathmandu Univ Med J 2013; 11(4): 305-309

Dr. Bronnikov reported a case of Jackson's epilepsy treated operatively. A patient after being wounded with a hammer of the parietal bone first suffered paralysis of the right arm and difficulty in speaking, and after 2 months he had epileptiform seizures every 6 days. In the 4th month, he underwent surgery to remove fragments of the internal lamina pressing on the dura mater, and the defect was closed with a piece of fascia lata with fatty tissue. The patient was demonstrated 3 months after the operation; he had no seizures, his speech was normal, and his hand function had been restored.

Summary Core measurements are used for rock classification and improved formation evaluation in both cored and noncored wells. However, the acquisition of such measurements is time-consuming, delaying rock classification efforts for weeks or months after core retrieval. On the other hand, well-log-based rock classification fails to account for rapid spatial variation of rock fabric encountered in heterogeneous and anisotropic formations due to the vertical resolution of conventional well logs. Interpretation of computed tomography (CT) scan data has been identified as an attractive and high-resolution alternative for enhancing rock texture detection, classification, and formation evaluation. Acquisition of CT scan data is accomplished shortly after core retrieval, providing high-resolution data for use in petrophysical workflows in relatively short periods of time. Typically, CT scan data are used as two-dimensional (2D) cross-sectional images, which is not suitable for quantification of three-dimensional (3D) rock fabric variation, which can increase the uncertainty in rock classification using image-based rock-fabric-related features. The methods documented in this paper aim to quantify rock-fabric-related features from whole-core 3D CT scan image stacks and slabbed whole-core photos using image analysis techniques. These quantitative features are integrated with conventional well logs and routine core analysis (RCA) data for fast and accurate detection of petrophysical rock classes. The detected rock classes are then used for improved formation evaluation. To achieve the objectives, we conducted a conventional formation evaluation. Then, we developed a workflow for preprocessing of whole-core 3D CT-scan image stacks and slabbed whole-core photos. Subsequently, we used image analysis techniques and tailor-made algorithms for the extraction of image-based rock-fabric-related features. Then, we used the image-based rock-fabric-related features for image-based rock classification. We used the detected rock classes for the development of class-based rock physics models to improve permeability estimates. Finally, we compared the detected image-based rock classes against other rock classification techniques and against image-based rock classes derived using 2D CT scan images. We applied the proposed workflow to a data set from a siliciclastic sequence with rapid spatial variations in rock fabric and pore structure. We compared the results against expert-derived lithofacies, conventional rock classification techniques, and rock classes derived using 2D CT scan images. The use of whole-core 3D CT scan image-stacks-based rock-fabric-related features accurately captured changes in the rock properties within the evaluated depth interval. Image-based rock classes derived by integration of whole-core 3D CT scan image-stacks-based and slabbed whole-core photos-based rock-fabric-related features agreed with expert-derived lithofacies. Furthermore, the use of the image-based rock classes in the formation evaluation of the evaluated depth intervals improved estimates of petrophysical properties such as permeability compared to conventional formation-based permeability estimates. A unique contribution of the proposed workflow compared to the previously documented rock classification methods is the derivation of quantitative features from whole-core 3D CT scan image stacks, which are conventionally used qualitatively. Furthermore, image-based rock-fabric-related features extracted from whole-core 3D CT scan image stacks can be used as a tool for quick assessment of recovered whole core for tasks such as locating best zones for extraction of core plugs for core analysis and flagging depth intervals showing abnormal well-log responses.

We herein report three cases of group A Streptococcus (GAS) infection in a family. Patient 1, a 50-year-old woman, was transferred to our hospital in shock with acute respiratory distress syndrome, swelling in the right neck and erythemata on both lower extremities. She required intubation because of laryngeal oedema. At the same time, patient 2, a 48-year-old man, was admitted because of septic shock, pneumonia and a pulmonary abscess. Five days later, patient 3, a 91-year-old woman, visited our clinic with bloody stool. All three patients were cured by antibiotics, and GAS was detected by specimen cultures. During these patients’ clinical course, an 84-year-old woman was found dead at home after having been diagnosed with type A influenza. All four patients lived in the same apartment. The GAS genotypes detected in the first three patients were identical. When treating patients with GAS, appropriate management of close contacts is mandatory.

On May 4th of the current year a number of small Pleuronectids were captured by the hand in a pool left by the ebb tide at Plymouth Breakwater, and brought to me alive. Two of them were very transparent, and, from their habit of lying on the right side when at rest, evidently sinistral forms. One of them was almost perfectly symmetrical; while in the other the torsion of the facial region and eyes had commenced. The pigmentation had the form of interrupted transverse bands, which were most conspicuous on the dorsal and ventral fins; on the dorsal fin seven bands were indicated. The terminal portion of the original trunk, containing the notochord, was seen at the upper edge of the caudal fin. The neurochord was covered with pigment, forming a very distinct band, situated, however, not in the skin, but in the connective tissue surrounding the neurochord or spinal cord. The mouth was large, and the snout upturned. The pectoral fin was large, the pelvic small. But the most important characteristic was the presence of two straight spines projecting laterally from the auditory region. These have been called otocystic spines by Prof. McIntosh, but I think they would be more appropriately described as periotic spines, as they are evidently projections of the periotic cartilage or bone; to which particular bones of the periotic region they belong has not been determined. Mr. Holt cut sections of the spines in situ, and found that they consisted of a knob of periotic cartilage passing into a mass of undifferentiated cells, the whole forming the core of a dermal spine consisting of hyaline ossified tissue. In my specimens I observed a third spine, much smaller, situated in the region of the frontal bone, behind and above the eye; it was visible in both the stages.
