Titanium hypersensitivity is rare, but it may exist among patients who need dental implant treatment. It could contribute to mild or severe peri-implant tissue hypersensitivity reactions and affect patients’ oral and general health according to some clinical reports. In extreme cases, it may lead to implant failure or extraction. In this case report, a 64-year-old Caucasian female patient received a titanium implant placement on #5. Right after the implant placement, she reported the symptoms of pain, eczema, and slight extraoral swelling, along with significant burning sensation occurring intra- and extraorally. The symptoms were not released after the systemic intervention of antibiotics for six days. On retrieving her medical history, the patient reported a previous allergic reaction to jewelry-like earrings in her childhood. The diagnosis of titanium hypersensitivity was made based on the rapid onset of symptoms and her metal allergy history. Therefore, the dental implant was removed after diagnosis, and a bone allograft was used to preserve the alveolar bone volume. Immediately after implant extraction, the swelling and burning sensation were receded. A complete recovery was achieved three weeks later. The dimension of the alveolar ridge bone was found well maintained in 10 weeks follow-up visit. Conclusion. Rapid onset of peri-implant gingival swelling may indicate a hypersensitivity to titanium implant in the clinic. For patients with a history of allergy to jewelry, the hypersensitivity tests to titanium alloy such as patch test or lymphocyte transformation test should be recommended as part of the dental implant treatment plan.

With the advancement in the dental implant treatment, many new innovative improvements have been developed in the designs and components of implants. These improvements helps to handle the compromising situations where sometimes the good treatment option can become more challenging. One of the such grueling condition is implant placement in the anterior maxilla where dentist has to fulll both aesthetic and functional demand. Many aesthetic failures of implant have been reported in the literature in the anterior maxilla. One of them is placing implant in sites with deciency of facial bone thickness which sometimes resulted in malposition of implant. This case report discusses about the prosthodontic correction of malpositioned implant in the anterior maxilla by using castable abutment. The use of castable abutment is concluded as successful treatment option for complex misaligned and malposed single dental implant.

Objective: There are several developed protocols for implant placement. When treating the esthetic zone different alternatives can be presented according to case variations and implications of the treatment planning and clinical challenges. Immediate implant placement protocols, favor both the patient and the clinician. This particular technique reduces time needed throughout the planning and performing. This document presents the clinical outcome of immediate implant placement in esthetic zone. Clinical Considerations: This case report describes the treatment of a 33 years old patient that requested the rehabilitation of missing teeth in the esthetic zone, previously lost due to facial trauma. All medical and clinical implications were taken into consideration to establish the treatment plan. Radiographic analysis and digital planning were carried through, accordingly. In the second surgical phase of implant treatment, the augmentation of keratinize tissue, in the treated area had no significant results, therefore a third surgery was performed “vestibuloplasty”. Conclusions: The proper planification of immediate implant placement should be ideally included, all the diagnostic tools available, granting the complete visualization of the patient’s conditions, permitting the development of the ideal treatment plan. The treatment with osseointegrated implants inserted immediately after the tooth extraction could be an implantological secure alternative, achieving a successful treatment. However, there’re still several issues that should be addressed. Further investigation is needed.

Currently dental implant treatment become popular. More clinicians are being presented with the opportunity to perform implant services in their own practice. The use of one-piece implant can be considered as treatment for the replacement of missing tooth in the areas of limited tooth-to-tooth spacing and would bring less trauma for the soft tissue. A 52-year-old female patient presented with difficulty in mastication due to the loss of teeth of 35 region with limitation mesio distal space. Radiology examination did not reveal any radiolucencies in 35 region. Radiograph revealed sufficient bone height from crestal to the floor of the mandible. A one-piece implant was placed in the 35 region. The present case used an endosteal implant, one stage surgery, and delayed loading. Restoration with porcelain fused to the metal crown had been delivered three months after the one-piece implant placement. The result of this treatment indicated that one-piece implant could be considered as an effective therapy choice for replacement of missing tooth in order to rehabilitate the masticatory function.

INTRODUCTION: Immediate dental implants have greatly reduced the treatment time and the number of surgical interventions. Recently it has been noted that this treatment modality can be used in aesthetically demanding cases especially the anterior maxilla. CASE REPORT: In the present case report a 24 year old male patient reported to our institution with fractured upper front teeth. After careful examination and treatment planning immediate implant treatment was initiated. The teeth were extracted atraumatically. We placed three implants into the extraction sockets. The prosthetic rehabilitation was done with metal ceramic crowns. CONCLUSION: It was found that the immediate implant therapy has several advantages such as reduced treatment length, preservation of soft and hard tissues surrounding the implant and reduced number of operations. Immediate implant treatment therefore has a great future in the treatment of aesthetic zones.

The guided bone regeneration (GBR) technique has been used to achieve optimal bone volume augmentation and allow dental implant placement in atrophic maxilla and mandible, with predictable results and high survival rates. The use of bone substitutes has reduced the necessity of autogenous bone grafts, reducing the morbidity at the donor areas and thus improving the patients’ satisfaction and comfort. This clinical case report shows a clinical and histological evaluation of the bone tissue behavior, in a case that required the horizontal augmentation of the alveolar ridge, with the use of xenograft biomaterial and further dental implant placement. After six months of healing time, six implants were placed, and a bone biopsy was done. The histological analysis depicted some fragments of the xenograft bone graft, integrated with the new-formed bone tissue.

This case report describes the Indirect Pulp Treatment (IPT) of deep carious lesion in a permanent molar with Ledemix. A 18 years old male patient reported discomfort associated with thermal stimulation on the mandibular right first permanent molar. Radiographically revealed a deep buccal carious lesion, very close to the pulp. periapical radiolucency and periodontal space thickening was absent. Pulp sensitivity was confirmed by thermal pulp vitality tests. Based on the main complaint and the clinical and radiographic examinations, the treatment plan was established to preserve pulp vitality. Clinical procedures consisted of removing the infected dentin and lining the caries-affected dentin with ledermix cement, the tooth was restored with composite cement. The achievement of the optimal implant position is a critical consideration in implant surgery, as it can facilitate the ideal prosthesis design and allow adequate oral hygiene maintenance. The switch from bone-driven to prosthetic-driven implant placement, through a comprehensive diagnosis and adequate treatment plan, is a prerequisite for long-term successful implant-based therapy. The aim of the present case report is to describe a step-by-step prosthetic retreatment of a patient with primary treatment failure due to incorrect dental implant placement. Although dental implants achieve high survival rates, the success of implant prosthetic therapy significantly relies on an appropriate implant position. Malpositioned implants can cause damage to vital structures, like nerves or vessels. Moreover, improper implant positioning can result in esthetic, biological, and technical complications and can, in extreme situations, render the desired prosthetic rehabilitation impossible to achieve.

The aim of this case report is to show that bone remineralization around dental implants with a history of peri-implantitis is possible after irritant factors are removed and only conservative treatment is performed. Patient came to the clinic after three years of dental implant placement complaining about swelling, sensitivity and gingiva color changes at the posterior part of the maxilla. During radiographic and intraoral examinations peri-implantitis of the #24 implant site was diagnosed. The surgical treatment method was rejected and performed conservative treatment instead. The outcome is promising; periapical radiographs three months later showed bone remineralization as well as stable bone after 10 years. A key clinical message: Bone remineralization around dental implants with a history of peri-implantitis is possible after irritant factors are removed and conservative treatment performed.
