Background. Blood eosinophils predict the response to therapy, risk of exacerbation, and readmission in COPD. This study investigates whether blood eosinophils predict pulmonary rehabilitation (PR) outcomes in COPD. Methods. We categorized patients into eosinophilic (blood eosinophils ≥300 cells/ml) or noneosinophilic (<300 cells/ml). In a retrospective design, we compared changes within and between the two groups on BODE index, 6-minute walk test (6MWT), FEV1, and mMRC dyspnea scale. Results. Of 206 patients enrolled, 176 were included for analysis; 90 were eosinophilic. BODE index improved in both groups: (MD −1.25; 95% CI (−0.45, −4.25), P ≤ 0.001 ) in the eosinophilic and (MD −1.33; 95% CI (−1.72, −0.94), P ≤ 0.001 ) in the noneosinophilic, but a higher BODE index remained in the eosinophilic (4.98); adjusted mean change (β): 0.7 (95% CI (0.15, 1.26), P = 0.01 ). 6MWT improved by 29.3 m in the eosinophilic (95% CI (14.2, 44.4), P ≤ 0.001 ) vs. 115.1 m in the noneosinophilic (95% CI (−30.4, 260.6), P = 0.12 ). FEV1 did not change in the eosinophilic (MD −0.6; 95% CI (−2.64, 1.48), P = 0.58 ), but improved by 2.5% in the noneosinophilic (MD 2.5; 95% CI (0.77, 4.17), P = 0.005 ). There were no significant between-group differences in 6MWT and FEV1; adjusted mean changes (β) were −9.69 m (95% CI (−39.51, 20.14), P = 0.52 ) and −2.31% (95% CI (−5.69, 1.08), P = 0.18 ), respectively. There were no significant within- or between-group changes in the mMRC scale. Conclusion. Although PR improves the BODE index in both eosinophilic and noneosinophilic COPD, a higher eosinophil count (≥300 cells/ml) is associated with a higher (worse) BODE index. Blood eosinophils may predict PR outcomes.

The aim of the study is to investigate the influence exerted by short-term outpatient pulmonary rehabilitation on functional indices and the course of the perioperative period for patients with lung cancer against the background of COPD Material and methods. According to the study design, lung cancer patients with planned surgical treatment were distributed between groups according to the COPD severity (stages II-IV). At the outpatient stage, evaluation of the pulmonary function and 6-minute walk test were performed. The results of the test and the functional indices were registered with further establishment of the pulmonary rehabilitation procedure complex for the patient. The exercises were performed at home and designed for the duration of two weeks. The complex included cycles of breathing, strength and aerobic exercises with individually adjusted intensity depending on the severity of clinical COPD manifestations and the baseline physical status. Upon completion of the training course, the patients underwent repeated spirometry and the 6-minute walk test. The results obtained were registered and analysed with subsequent surgical treatment of the patients. Results. Minimal changes in functional indices upon completion of the course were registered in patients with COPD stage II, which prevented achievement of statistical significance. Therewith, patients with COPD stages III and IV who had undergone the course of exercises were noted to show statistically reliable improvement of pulmonary function (FEV1) and improvement of the overall physical status, which is testified to by enhanced tolerance to externally imposed physical loads (p<0.01). Apart from that, decreased HR values (p<0.01)) growth of SpO2 values (p<0.01) and decreased shortness of breath (p<0.01) were registered in these groups after the 6-minute walk test. No statistically significant differences in relation to the number of postoperative complications or the term of in-hospital stay were revealed between the groups in the postoperative period. Conclusion. Performance of a pulmonary rehabilitation course during the preoperative period reliably improves functional indices in patients with COPD stage III and IV, which may be of crucial significance for determination of functional operability at the outpatient stage

Abstract: Chronic obstructive pulmonary disease (COPD) is one of the leading causes of morbidity worldwide. In patients with COPD, the level of physical activity is reduced, which is associated with an increased risk of adverse outcomes of this disease. At the same time, the problem of physical activity in patients with occupational COPD has been little studied. There are a number of factors affecting the physical activity of patients with COPD, one of which is exercise tolerance. To evaluate it, special tests are used, one of which is the 6-minute walk test. The aim of the study was to compare the characteristics of exercise tolerance in patients with COPD and occupational COPD. We analyzed the case histories of patients with an established diagnosis of COPD undergoing inpatient treatment and pulmonary rehabilitation. Patients (n - 35) were divided into two groups. The first group - patients with generalized COPD (n -17), men from 55.0 ± 20.0 years. The second group is patients with occupational COPD (n-18), men from 55.0 ± 20.0 years. All patients underwent a 6-minute walk test. The analysis of the results has been carried out. On average, patients with occupational COPD walked a shorter distance in 6 minutes, although these differences were not significant. In the group of occupational COPD, a larger percentage of patients had a worse prognosis in the prevention of cardiovascular complications. The 6-minute walk test can be used in combination with other assessment methods at the stages of pulmonary rehabilitation in order to determine the prognosis, determine the tolerance of physical activity.
