Effects of Fluid Therapy on Mesenteric Microcirculation Using New Probe-Based Confocal Laser Endomicroscopy (Cellvizio®) in a Porcine Model of Endotoxic Shock

<b><i>Background:</i></b> Microcirculatory alterations have been observed at the early phase of sepsis, although macrocirculation seems preserved. The aim of this study was to analyze the effect of crystalloid fluid therapy on mesenteric microcirculation, assessed by using the confocal laser endomicroscope Cellvizio®, in an endotoxic porcine model. <b><i>Methods:</i></b> It is a prospective endotoxic shock (lipopolysaccharide infusion) experimental trial. Piglets were divided into 3 groups: 6 in the sham group (no LPS injection, no fluid), 9 in the control group (LPS infusion, no fluid), and 6 in the crystalloids group (LPS infusion and fluid resuscitation with crystalloids). Fluid resuscitation consisted in a fluid bolus of 20 mL/kg 0.9% saline over 30 min followed by a 10 mL/kg/h fluid rate over 4 h. Mesenteric microcirculation was assessed using a confocal laser endomicroscope (Cellvizio®). Blood flow within capillaries was visually assessed according to the point of care microcirculation (POEM) score. <b><i>Results:</i></b> At baseline, the 3 groups were similar regarding hemodynamic, biological, and microcirculatory parameters. At T360, the POEM score significantly decreased in the control and crystalloids groups, whereas it remained unchanged in the sham group (respectively, 1.62 ± 1.06, 1.2 ± 0.45, and 5.0 ± 0, <i>p</i> = 0.011). There was no significant difference in cardiac output at T360 between the sham and crystalloids groups (3.1 ± 0.8 vs. 2.3 ± 0.6, <i>p</i> = 0.132) or between the control and crystalloids groups (2.0 ± 0.6 vs. 2.3 ± 0.6, <i>p</i> = 0.90). <b><i>Conclusion:</i></b> There was no significant improvement of microcirculatory alterations after crystalloids resuscitation despite improvement in macrocirculatory parameters in early experimental sepsis.

Anesthetics are considered to be one of the important inducing factors of postoperative cognitive dysfunction (POCD). The hippocampal region of the rat is one of the action sites of general anesthesia drugs. L 655,708, a reverse agonist of gamma aminobutyric acid (GABA) receptor, can significantly improve short-term memory dysfunction in mice after anesthetized with isoflurane. So the purpose of this study is to investigate the effects of L-655,708 on expression of GABA, glutamate (GLU), and beta-endorphin (β-EP) in the dentate gyrus region of the hippocampus and cognition of rats anesthetized with propofol. In all, 30 male Sprague–Dawley (SD) rats were randomly allocated into the control group, sham group, and L-655,708 group, with 10 in each group. The cognitive function of rats was measured by Morris water maze before and 1 h after administration. Then the rats were sacrificed for brain tissues. Immunohistochemistry was used to study the expression of GABA, GLU, and β-EP in the hippocampus of anesthetized rats in each group. Compared with the control group, the latency of the sham group and L-655,708 group were significantly prolonged after administration ( P < 0.05). However, L-655,708 could shorten the prolonged latency ( P < 0.05). There was no significant difference in times of accessing original platform area between the three groups before and after medication ( P > 0.05). The expression level of GABA in the dentate gyrus region of hippocampus of rats in the sham group was significantly higher than that in the control group ( P < 0.05), while the expression level in the L-655,708 group was significantly lower than that in the sham group ( P < 0.05). No significant difference was found in the expression of GLU in the dentate gyrus region of hippocampus of rats in each group ( P > 0.05). Compared with the control group, the expression of β-EP was significantly lower in the dentate gyrus region of the hippocampus of sham group rats ( P < 0.05). However, the expression of β-EP in the L-655,708 group was significantly higher than that in the sham group ( P < 0.05). Cognitive dysfunction in rats anesthetized with propofol may be related to high expression of GABA and low expression of β-EP in the hippocampus. The mechanism of L-655,708 in reducing the cognitive impairment in propofol anesthetized rats may be bound up with down-regulating the expression of GABA and increasing the expression of β-EP in the hippocampus.

Fertility in cattle includes the ability of the uterus to provide the appropriate environment for pregnancy success, including the transport of spermatozoa before fertilization. Confocal laser endomicroscopy technology (Cellvizio) has been previously used in small ruminants to visualise labelled spermatozoa invivo in the uterine horns of ewes (Druart et al. 2009 Reprod. 138, 15-53). Nevertheless, to the best of our knowledge, no invivo study has reported a live visual analysis of the behaviour of spermatozoa in the bovine uterus upon AI. The aim of this study was to develop an experimental procedure to label bull spermatozoa and to visualise their progression in the uterine horns of dairy cows, using Cellvizio and uterine cervix catheterization. Fresh ejaculated bull spermatozoa were double-labelled with octadecyl rhodamine B chloride and MitoTracker Green FM dyes before dilution and freezing processing. At each step of the process, semen quality was evaluated with a semen quality analyzer (SQA-V b, Medical Electronic System) and compared to nonlabelled bull reference semen. The specificity of the labelling was validated invitro. We also developed a specific device to insert the Cellvizio fibre probe into the genital tract, in order to image vagina, cervix, and uterine body, as well as the proximal, middle, and distal parts of the uterine horn, including the utero-tubal junction (UTJ), before and after AI with fresh and frozen labelled spermatozoa. Then, 10 heifers were oestrus synchronized. Video sequences were recorded at oestrus (n=5), just before AI, at the time of AI (7-12h after the onset of oestrus), 30 and 120min after AI, and 14 days after oestrus (n=5; luteal phase), to investigate the effect of the steroid-primed uterine environment on the distribution and progression of spermatozoa in the genital tract. Using our approach, labelled spermatozoa were detected (1) at the day of oestrus and 14 days after oestrus, (2) in the uterine body at the time of AI, (3) in various parts of the genital tract 30min after AI, and (4) in the UTJ, the cervix, and the vagina 120min after AI. In addition, labelling the spermatozoa did not alter the fertilizing capacity: 4 of the 5 oestrus females were pregnant at Day 35. Additionally, an IVF test showed a blastocyst rate of 45.8% for labelled semen vs. 39.4% for the control group (Carvalho et al. 2017 Reprod. 154, 695-710). In conclusion, our study provides a method to phenotype in situ the transport of spermatozoa in the genital tract of heifers. Further investigations are in progress to optimize video quality, to develop an algorithm for an automatic analysis (spermatozoa count, speed, and path) of the recorded sequences.

AbstractBackground and objectivesTonsillectomy is one of the most commonly used pediatric surgeries with high stress levels for children and their parents. The present study was designed and implemented regarding the fact that limited studies have ever been carried out about the effects of complementary therapies such as acupressure on the preoperative anxiety levels in children throughout the world.Materials and methodsPresent study was a randomized clinical trial that was done in three groups with 144 children aged 5–12 years undergoing tonsillectomy. In present study, the children were allocated in three groups: intervention, control and sham. In the intervention group, the acupressure was applied on the Yintang point and in the sham group, was applied at a sham acupoint. No intervention was performed in the control group. Before and after intervention, the pediatric anxiety levels were measured using instruments. The results were analyzed using descriptive and inferential statistics such as the Kruskal-Wallis and Wilcoxon tests, chi-square (χ2) and analysis of variance (ANOVA) test. The significance level was considered p < 0.05.ResultsThe total anxiety score in children was (70.39 ± 20.93) in the control group, (67.83 ± 16.78) in the intervention group and (71.40 ± 21.82) in the sham group, not significant difference (p > 0.05). The different in the overall anxiety score among children before intervention compare to after intervention was as following: Control group (–3 ± 17.46), intervention group (8.42 ± 32.98) and sham group (–4.32 ± 24.47).ConclusionLike other surgical procedures, the tonsillectomy has serious stress for children. Our study demonstrates that the acupressure has a significant effect on the level of preoperative anxiety in children undergoing surgery. Therefore, nurses can use this technique to reduce the pediatric anxiety.

Background. Both long-term proton pump inhibitor use and surgical fundoplication have potential drawbacks as treatments for chronic gastroesophageal reflux disease (GERD). Our aim was to investigate the potential efficacy of antireflux mucosectomy (ARMS) in porcine and determine the optimal circumference of resection in relation to gastroesophageal junction (GEJ). Methods. Nine pigs were allocated into the following 3 groups by computerized randomization: group A: control, group B: 1/3 circumference of the esophagus, and group C: 2/3 circumference of the esophagus. We performed mucosectomy with a crescentic mucosal resection at 3 cm above the GEJ and 1 cm below the GEJ. The animals were kept on a liquid diet for 24 h prior to endoscopy. At 6 weeks, animals underwent esophagoscopy, barium radiography, gastric yield pressure (GYP), and gastric yield volume (GYV) determination. Results. The weight of swines has no significant difference, and all pigs had maintained their weight after the procedure. We both found scar formation at the GEJ in group B and C. Compared with group A and B, group C produced significantly higher GYP (24.23±3.42 mmHg, p=0.004) and significantly smaller GYV (2200.0±238.96 mL, p=0.028) after 6 weeks. Barium radiography showed that the width of the cardia was narrower (13.73±1.19 mm, p=0.032) in group C after 6-week postprocedure. Conclusion. Our study demonstrated the potential antireflux effect of ARMS. We also recommend the 2/3 circumference resection of mucosa at 3 cm distance from the GEJ.

Objective The transplantation of mesenchymal stem cells (MSCs) represents a promising approach for treating the ischemic and the nonischemic diseased heart. The positive effects of transplanting these cells could be shown, but the exact mechanisms remain unknown. We evaluated whether the injection site affects the improvement in left ventricular (LV) ejection fraction (EF) and angiogenesis in doxorubicin (Dox)–induced failing hearts. Methods Heart failure was induced in New Zealand white rabbits by doxorubicin treatment, followed by right ventricular MSC transplantation (RV-MSC, n = 6), LV MSC transplantation (LV-MSC, n = 6), sham treatment (sham group, n = 6), or no therapy (Dox group, n = 5). Healthy rabbits were used as control group (n = 8). Cells were isolated after bone marrow aspiration and transplanted locally into the ventricular myocardium. After 4 weeks, cardiac function and capillary density (CD31 staining) were measured. Results The transplantation of MSCs increased the EF significantly (LV-MSC, 39.0% ± 1.4%, and RV-MSC, 39.2% ± 2.6%, vs sham group, 29.8% ± 3.7%; P < 0.001), without significance between the MSC groups ( P = 0.858). Neither the evidence of a transdifferentiation nor any signs of cell engraftment of transplanted cells could be found. The capillary density (capillaries/high-power field) increased in both MSC groups compared with the sham group (LV-MSC by 8.3% ± 3.4%; and RV-MSC, 8.1% ± 2.2%; P < 0.05), without significance between the two MSC groups ( P = 0.927). Conclusions Injection of autologous MSCs in doxorubicin-induced cardiomyopathic rabbit hearts improves EF and enhances angiogenesis. Despite local application, we observed global effects on heart function and capillary density without significant difference between right and LV injection. The paracrine mechanism might be one possible explanation for these findings.

Candida albicans (C. Albicans) is one of the bacteria that reside in the oral cavity, and the ones living in medical and commercial denture resins, commonly cause diseases. Therefore, this study was conducted to confirm the antibacterial activity of C. Albicans using a denture base resin containing peony extract with antibacterial properties. Contact angle measurements and color change measurements were performed to confirm the physical change of the material added with the antibacterial agent to the denture reline resin. As a result of the antibacterial test, the experimental group exhibited antibacterial activity against C. Albicans. Compared to the optical density results, the results of the experimental group showed a significant difference. As a result of Fluorescent images showing (confocal laser microscope), the control group showed a lot of live bacteria, and no bacteria appeared in the experimental group. All group  did not show any physical changes. As a result of the contact angle measurement, the surface of the experimental group was changed to hydrophilic. In addition, there was no change in the color of the denture reline resin containing peony extract. In conclusion, it was confirmed that the peony extract contained antibacterial activity of the denture resin, and further studies should be conducted on various bacteria for denture base resin disinfection.

Introdution: One of the major challengs in ovarian tissue transplantation is overcomeing ischemia/ reperfusion injuries. During ischemia–reperfusion processes, oxygen free radicals constitute the most important component that induces damage of the grafted tissues. The aim of this study was to investigate the effect of L-Carnitine (LC) as an antioxidant on heterotopic transplantation of mouse ovarian tissue. Methods: In this laboratory experimental study, 5- week old female NMRI mice were divided into four groups: control, transplanted without administration (autograft), sham group (autograft+ saline) and LC group (autograft+ L- carnitine). Left ovarian tissues were transplanted into the Gluteal muscle for 3 weeks. After this time, ovarian tissues from all groups were removed and fixed in formalin for histological studies. Furthermore, rate of Caspase- 3 was assessed by immunohistochemistry test. Lipid peroxidation was assessed by measuring  malondialdehyde (MDA). One-way ANOVA and Tudey test was used to analyze the data using the spss 16 software. Significance was defined as P≤0.0. Results: The study results indicated that total follicular count in transplantedwithout administration and sham groups was significantly lower than the control group (p<0.05), but there was no significant difference between the control and LC groups. In addition, the rate of caspase-3 was decreased in the LC group, but no significant difference existed between all groups (p<0.05). A significant reduction in the concentration of MDA was observed in the LC group than that in the other transplanted groups (p<0.05). Conclusion: In this study, LC could improve the ovarian reserve to some extent, but its effect was not significant.
