Decellularization Strategies to Engineer Fibrocartilage Bioscaffolds: A Review

Fibrocartilage or known as meniscus tissues located in between the tibia and femur always subjected to extreme forces that can lead to injury especially for the sportsperson. The meniscal injury mean incidence in the general population is 66 per 100,000. The principal methods for the surgical management of fibrocartilage injury have been improvised from meniscectomy to meniscal repair and meniscal reconstruction that portrays different advantages and disadvantages in the short and long-term results. The inability to treat meniscus injury without osteoarthritis development in long-term results also motivates to find new treatment strategies. In this current era, the development of the multidisciplinary fields of tissue engineering and regenerative medicine provides new alternatives for the treatment approaches. This field involves the regeneration of the required tissue using scaffolds such as synthetic, natural, and biological scaffolds to restore the damaged one. Biological scaffolds are preferable because it tremendously mimics the native anatomical structure and has similar ratios and concentration of the proteins and growth factors that influence tissue repair and remodeling. The development of biological scaffolds with low immunogenic levels involves the decellularization process that eliminates all the cellular components while preserved the extracellular matrix (ECM) integrity and mechanics. In this review, the pros and cons of the recent decellularization strategies to engineer fibrocartilage scaffolds have been discussed. We believed that the ideal decellularization methods still need to be explored to develop suitable biological scaffolds that structurally and functionally mimic native tissue as a replacement for new tissue regeneration.

Objective: Approximately one out of five patients with Graves' disease (GD) undergoes a thyroidectomy after a mean period of 18 months of medical treatment. This retrospective and non-randomized study from a teaching hospital compares short- and long-term results of total (TT) and subtotal thyroidectomies (ST) for this disease. Methods: From 1987 to 1997, 94 patients were operated for GD. Thirty-three patients underwent a TT (mostly since 1993) and 61 a ST (keeping 4 to 8 grams of thyroid tissue - mean 6 g). All patients had received propylthiouracil and/or neo-mercazole and were in a euthyroid state at the time of surgery; they also took potassium iodide (lugol) for ten days before surgery. Results: There were no deaths. Transient hypocalcemia (< 3 months) occurred in 32 patients (15 TT and 17 ST) and persistent hypocalcemia in 8 having had TT. Two patients developed transient recurrent laryngeal nerve palsy after ST (< 3 months). After a median follow-up period of seven years (1-15) with five patients lost to follow-up, 41 patients having had a ST are in a hypothyroid state (73%), thirteen are euthyroid (23%), and two suffered recurrent hyperthyroidism, requiring completion of thyroidectomy. All 33 patients having had TT - with follow-ups averaging two years (0.5-8) - are receiving thyroxin substitution. Conclusions: There were no instances of persistent recurrent laryngeal nerve palsy in either group, but persistent hypoparathyroidism occurred more frequently after TT. Long after ST, hypothyroidism developed in nearly three of four cases, whereas euthyroidy was maintained in only one-fourth; recurrent hyperthyroidy was rare.

<p><b>Introduction:</b> The goal of this study was to compare the short- and long-term outcomes after aortic valve (AV) surgery carried out via standard sternotomy/partial sternotomy versus transapical transcatheter AV implantation (taTAVI).</p><p><b>Patients and Methods:</b> All 336 patients who underwent taTAVI between 2006 and 2010 were compared with 4533 patients who underwent conventional AV replacement (AVR) operations between 2001 and 2010. Using propensity score matching, we identified and consecutively compared 2 very similar groups of 167 patients each. The focus was on periprocedural complications and long-term survival.</p><p><b>Results:</b> The 30-day mortality rate was 10.8% and 8.4% (<i>P</i> = .56) for the conventional AVR patients and the TAVI patients, respectively. The percentages of postoperative pacemaker implantations (15.0% versus 6.0%, <i>P</i> = .017) and cases of renal failure requiring dialysis (25.7% versus 12.6%, <i>P</i> = .004) were higher in the TAVI group. Kaplan-Meier curves diverged after half a year in favor of conventional surgery. The estimated 3-year survival rates were 53.5% � 5.7% (TAVI) and 66.7% � 0.2% (conventional AVR).</p><p><b>Conclusion:</b> Our study shows that even with all the latest successes in catheter-based AV implantation, the conventional surgical approach is still a very good treatment option with excellent long-term results, even for older, high-risk patients.</p>

Movement patterns were studied on a 1-ha isolated reef surrounding Arch Rock in southern Tasmania. Short-term movements were identified from diver observations, and interpretation of long-term movements involved multiple recaptures of tagged individuals. Visual observations indicated that the sex-changing labrids Notolabrus tetricus, Pictilabrus laticlavius and Pseudolabrus psittaculus were all site-attached, with females having overlapping home ranges and males being territorial. In the non-sex-changing labrid Notolabrus fucicola and in the monacanthids Penicipelta vittiger and Meuschenia australis, there was no evidence of territorial behaviour and 1-h movements were in excess of the scale of the study. The long-term results indicated that all species were permanent reef residents, with most individuals of all species except M. australis always being recaptured within a home range of 100 m × 25 m or less. Only 15% of individuals of M. australis were always recaptured within this range category. The natural habitat boundary of open sand between the Arch Rock reef and adjacent reefs appeared to be an effective deterrent to emigration. The use of natural boundaries should be an important consideration in the design of marine reserves where the aim is to minimize the loss of protected species to adjacent fished areas.

Summary At our hospital, the main treatment for resectable esophageal cancer (EC) has since 2013 been total minimally invasive esophagectomy (TMIE). The aim of this study was to present the short- and long-term results in patients operated with TMIE. This cross-sectional study includes all patients scheduled for TMIE from June 2013 to January 2016 at Oslo University Hospital. Data on morbidity, mortality, and survival were retrospectively collected from the patient administration system and the Norwegian Cause of Death Registry. Long-term postoperative health-related quality of life (HRQL) and level of dysphagia were assessed by patients completing the following questionaries: EORTC QLQ-OG25, QLQ-C30, and the Ogilvie grading scale. A total of 123 patients were included in this study with a median follow-up time of 58 months (1–88 months). 85% had adenocarcinoma, 15% squamous cell carcinoma. Seventeen patients (14%) had T1N0M0, 68 (55%) T2-T3N0M0, or T1-T2N1M0 and 38 (31%) had either T3N1M0 or T4anyNM0. Ninety-eight patients (80%) received neoadjuvant (radio)chemotherapy and 104 (85%) had R0 resection. Anastomotic leak rate and 90-days mortality were 14% and 2%, respectively. The 5-year overall survival was 53%. Patients with tumor free resection margins of &gt;1 mm (R0) had a 5-year survival of 57%. Median 60 months (range 49–80) postoperatively the main symptoms reducing HRQL were anxiety, chough, insomnia, and reflux. Median Ogilvie score was 0 (0–1). In this study, we report relatively low mortality and good overall survival after TMIE for EC. Moreover, key symptoms reducing long-term HRQL were identified.

Aim: To study the short-and long-term results of 2RT nanosecond laser treatment for CSC. To compare clinical results with those with AntiVEGF. Methods: Nanosecond laser (2RT, Ellex) was used. Patients were followed by BCVA, FAF, OCT, Angio-OCT. Results: 90% of the patient treated with 2RT had improvement of visual acuity and contrast sensitivity. 19 % [1] patients showed no improvement. 81% of the 2RT treated patients had total resorbtion of subretinal fluid against 60% treated with AntiVEGF. Functional results correlated with the atrophy of RPE in the macula. 46% (19 patients) had resolution on the first month, 23% [2]-on the third and 12% [3]-on the sixth month. The time of resolution shows no relation to the baseline pigment epithelial atrophy. Four patients presented with a recurrency of the disease in the follow-up period. 19% [1] were non responders. 60% of AntiVEGF treated eyes were responders and 40% were non responders. Those who responded to treatment needed between 3 and 10 injections. Conclusion: 2 RT is a non-invasive treatment modality with no adverse effects and high success rates.
