Translation of the Dizziness Handicap Inventory into Chinese, Validation of it, and Evaluation of the Quality of Life of Patients with Chronic Dizziness

Vestibular dysfunction can have a tremendous impact on an individual’s quality of life. The purpose of this paper is to determine if the level of handicap reported by individuals on the Dizziness Handicap Inventory (DHI), an inventory developed for use with individuals with complaints of dizziness symptoms, will be consistent with that reported on the Activities-specific Balance Confidence Scale (ABC), a tool developed for use with elderly individuals that attempts to assess a person’s confidence level in performing activities of daily living (ADL’s). A sample of convenience was used consisting of 71 subjects (15 males and 56 females) from a local Balance and Vestibular Clinic. The subjects ranged in age from 26 to 88 years of age. Both the DHI and the ABC were administered as part of an initial physical therapy evaluation to new patients at the clinic. A moderately strong negative correlation was found between the scores of the two inventories ( r s = − 0.6350). The results suggest that the ABC is a valid tool for use with individuals with complaints of dizziness.

ABSTRACT Purpose: to evaluate the quality of life in institutionalized elderly people with dizziness complaint and to relate the results to the characteristics of dizziness and functional capacity. Methods: in this cross-sectional study, one-hundred and nineteen elderly residents in three geriatric long-term care institutions in Natal city, Brazil, were evaluated. Those who had presented dizziness in the former year (30/25.2%) were included in this study. The quality of life was measured by the Dizziness Handicap Inventory. Functional capacity was measured by the Berg Balance Scale, the Functional Reach Test, the Unipedal Stance Test with eyes open and closed, and the Falls Efficacy Scale - International. Results: associations were found between physical, functional and emotional aspects and the duration of dizziness (p=0.002, p=0.041 and p=0.004, respectively); the functional aspects with age (p=0.031), the physical aspects with the presence of falls in the previous year (p=0.039); and the physical, functional and emotional aspects of the Dizziness Handicap Inventory with fear of falling (p=0.004, p<0.001 and p=0.016, respectively). Conclusion: institutionalized elderly with dizziness complaints had a low perception of quality of life, and the duration of dizziness, age, falls and fear of falling negatively influenced their quality of life.

Összefoglaló. Bevezetés: A szédülés gyakori panasz, amellyel a betegek felkeresik a sürgősségi osztályt. Emellett fontos tünet, hiszen kihívást jelent mind a diagnosztika, mind a terápia szempontjából, és nagy hatással lehet a betegek életminőségére. Célkitűzés: Kutatásunk célja annak vizsgálata, hogy mennyire befolyásolta a szédülés a betegek életminőségét a sürgősségi osztály elhagyását követően. Módszer: A vizsgálat időtartama alatt 879, szédülést panaszoló beteg jelent meg a Semmelweis Egyetem sürgősségi osztályán. Részükre kérdőív került kiküldésre, amely tartalmazta a ’Dizziness Handicap Inventory’ (DHI-) kérdőívet is. Megkeresésünkre 308 beteg (110 férfi, 198 nő; átlagéletkor: 61,8 ± 12,31 SD) válaszolt, az általuk visszaküldött kérdőíveket részletesen elemeztük. Eredmények: A leggyakoribb diagnózisok közé a benignus paroxysmalis positionalis vertigo, a centrális egyensúlyrendszeri eltérések és a szédülékenység tartoztak. Az elemzés alapján különbség volt látható a fizikális, a funkcionális és az emocionális pontszámok között. Kiemelendő, hogy a legmagasabb értékeket a fizikális csoportban regisztráltuk. A részletes otoneurológiai kivizsgáláson átesett betegek DHI-értékeit összevetettük azokéival, akik nem jártak ilyen vizsgálaton, a két csoport értékei között azonban nem volt szignifikáns különbség (p = 0,97). Emellett a DHI-érték emelkedése volt látható a végleges diagnózisig eltelt idő függvényében. Következtetés: A végleges diagnózisig eltelt idő, illetve a megfelelő kivizsgálás hiánya jelentős hatással van a szédülő betegek életminőségére. Lényeges a kivizsgálás, a mielőbbi diagnózis és a részletes egyensúlyrendszeri vizsgálat szerepe, ugyanakkor az utóbbi indokolt esetben kell, hogy történjen. Orv Hetil. 2021; 162(30): 1216–1221. Summary. Introduction: Dizziness and vertigo are among the most common complaints in the emergency department. This may require interdisciplinary cooperation due to their complex presentation in the department and the effects on the patients’ quality of life. Objective: Our study aimed to assess the effect of an acute vertigo episode on the quality of life after patients’ discharge from the emergency department. Method: 879 patients examined at the Semmelweis University emergency department with vertigo and dizziness were included in the study. A questionnaire, including the Dizziness Handicap Inventory (DHI), was addressed to this population. We received 308 answered questionnaires back (110 males, 198 females; mean age 61.8 years ± 12.31 SD), which were further analyzed. Results: The most frequent diagnoses were benign paroxysmal positional vertigo, central lesions and dizziness. According to the analysis of the DHI questionnaire, a difference between physical, functional and emotional scores was shown, whereas the highest scores were registered in the physical group. The DHI questionnaire scores of patients undergoing a neurotological examination and those who did not were further compared, whereas no significant statistical difference was indicated (p = 0.97). In addition, an increase in DHI scores was seen depending on the time elapse for the definitive diagnosis. Conclusion: The absence of adequate examination and a late diagnosis of the dizziness cause have a significant impact on the quality of life of patients. Therefore, substantial investigation, early diagnosis, and detailed vestibular examination are essential, but the latter should take place in justified cases. Orv Hetil. 2021; 162(30): 1216–1221.

Exploring emotional aspects commonly associated with the symptom of dizziness is essential for a better understanding and prognosis of vestibular disorder, once it is known that dizziness can be associate as a cause or consequence from psychological disorders. The aim of this study was to analyse the emotional impact caused by dizziness/vertigo symptom in vestibulopathic patients comparing two protocols: Dizziness Handicap Inventory (DHI) and Questionário de Impacto Emocional da Vertigem (CIEV). We concluded that both questionnaires can be used to evalute the impact of dizziness in patients' quality of life, but they explore different aspects being considered complementary during the diagnosis and for a better prognosis.

  Latar belakang: Keluhan gangguan keseimbangan berupa dizziness sulit untuk diukur, sementara itu individu dengan keluhan dizziness sering mengalami kesulitan untuk beraktivitas dan terganggu kualitas hidupnya. Salah satu perangkat yang paling sering digunakan untuk menilai kecacatan (handicap) pasien gangguan keseimbangan adalah kuesioner Dizziness Handicap Inventory (DHI). Sampai sekarang, kuesioner ini belum pernah diadaptasi ke bahasa Indonesia. DHI versi adaptasi yang valid dan handal (reliable) dapat digunakan untuk mengukur kualitas hidup, dan sebagai modalitas untuk menilai efektivitas terapi pasien gangguan keseimbangan. Tujuan: Mendapatkan instrumen DHI adaptasi bahasa Indonesia dengan validitas dan reabilitas yang teruji. Metode: Desain potong lintang digunakan pada studi yang dilaksanakan di poliklinik THT Divisi Neurotologi Departemen THT RS Dr. Cipto Mangunkusumo sejak Agustus 2012 sampai Februari 2013, pada pasien gangguan keseimbangan usia dewasa. Kuesioner DHI yang terdiri dari 25 pertanyaan diterjemahkan ke bahasa Indonesia, kemudian dilakukan terjemahan balik dengan proses validasi dan juga adaptasi kultural. Hasil: Uji validitas dilakukan dengan uji korelasi Spearman dan mendapatkan korelasi yang bermakna pada seluruh butir pertanyaan pada tingkat signifikansi p < 0,1 dengan nilai r sebesar 0,409- 0,783. Uji reliabilitas mendapatkan hasil nilai Cronbach-α total 0,9265 yang sangat baik untuk semua skala. Kesimpulan: Instrumen DHI adaptasi bahasa Indonesia valid dan handal berdasarkan kaidah validasi transkultural WHO sehingga dapat digunakan sebagai instrumen psikometrik untuk menilai kualitas hidup pasien gangguan keseimbangan.Kata kunci: Dizziness Handicap Inventory, gangguan keseimbangan, kecacatanABSTRACT Background: Balance disorder symptoms, such as dizziness is difficult to be assessed, it could caused difficulties in doing daily activities, and declining the quality of life. The most frequent tools used to measure handicap in patients with balance disorder is Dizziness Handicap Inventory (DHI). This questionnaire has never been adapted to Indonesian language. A valid and reliable version of the DHI adaptation can be used to measure the quality of life and also as a modality for assessing the effectiveness of therapeutic outcomes in balance disorders patients. Purpose: To obtain an Indonesian adaptation of DHI instrument which is valid and reliable. Methods: A cross-sectional research design in Outpatient Clinic of Neurotology Division, Department of Otorhinolaryngology, Cipto Mangunkusumo General Hospital, from August 2012 to February 2013. Respondents were all adult patients with balance disorders. DHI questionnaire contained 25 questions was translated into Indonesian language and re-translated with validation process and cultural adaptation. Results: The validity test was carried out with Spearman correlation test and obtained a significant correlation on all questions at a significance level of p<0.1 with r value of 0.409-0.783. The reliability test obtained a Cronbach-α value of 0.9265 which was very good for all scales. Conclusion: The Indonesian adaptation of DHI instrument was valid and reliable based on WHO transcultural validation rules, therefore it can be used as a psychometric instrument to assess the quality of life of patients with balance disorders.

<p class="abstract"><strong>Background:</strong> Dizziness is a commonly reported complaint among elderly.. Among the elderly in particular, factors such as Ageing, presbycusis, diabetes, chronic kidney disease, osteopenia, and osteoporosis increase the risk. Dizziness handicap inventory is used to assess the quality of life among the individuals with vestibular dysfunction. This is also used to assess the impact of interventions for vestibular dysfunction. This study aims to estimate the prevalence of latent vestibular dysfunction among the Indian population, and the impact of interventions among the same.</p><p class="abstract"><strong>Methods:</strong> A prospective observational study was conducted at a tertiary care hospital. As per sample size estimates, 200 participants aged &gt;60 years without previous diagnosis of vestibular dysfunction were screened using appropriate tests. The quality of life of the affected individuals were assessed using Dizziness Handicap Inventory Score (DHIS) before and after intervention.  </p><p class="abstract"><strong>Results:</strong> The prevalence of latent vestibular dysfunction was found to be 23.5%. Most of them reported having moderate handicap due to the condition. Following intervention, those with moderate handicap either became normal (47.8%) or had residual mild handicap (52.2%). The mean DHIS score significantly decreased from 40.91 points during pre-intervention to 16.12 points post-intervention.</p><p class="abstract"><strong>Conclusions:</strong> Around one-fourth of the Indian elderly is found to have latent vestibular dysfunction. It has a major impact on the individual’s quality of life. Yet, screening and intervention is found to make a considerable improvement among the affected individuals.</p>
