Airway obstruction in children has a wide differential diagnosis that includes laryngomalacia, infectious processes, paralysis, extrinsic compression, and other rare anatomic anomalies. Isolated laryngeal lymphatic malformations are rare developments that can manifest with clinically significant airway obstruction. To the best of our knowledge, there have been fewer than 20 reported cases. These laryngeal mucosal lesions are best managed with radiofrequency ablation or laser ablation. We present a case of a 2-year-old child who presented with airway obstruction, initially diagnosed with laryngomalacia, who was subsequently diagnosed and treated for an isolated epiglottic lymphatic malformation.

<p class="abstract">Lymphatic malformations of the head and neck region frequently involve the upper aero digestive tract. Patients with these lymphatic malformations may present in early infancy with sudden airway compromise. This necessitates early intervention with intubation or tracheostomy for airway stabilization. The etiology and pathogenesis of lymphatic malformations is still unclear, and a wide array of treatment modalities has been proposed. We present a case report of a neonate with a lymphatic malformation involving the upper airway, and discuss how the patient was initially stabilized with a tracheostomy, following which he underwent a series of staged procedures, including bipolar radiofrequency ablation, which eventually led to successful decannulation.</p>

Background To review the clinical experience and early outcomes of endothermal perforator ablation. Method Retrospective review of an endovenous practice from 2007-2019. Clinically significant incompetent perforators were treated by Endovenous Laser Ablation (EVLA), or segmental radiofrequency ablation (RFA). Result Complete data were available for 110 of the 116 symptomatic incompetent perforating veins treated. Radiofrequency ablation of 20 perforators produced a 55% perforator closure rate, while 90 EVLA perforator ablations resulted in a closure rate of 80%. Closure rates with EVLA varied by location and perforator length. Closure rates for truncal ablation were 95.5% for RFA and 97.2% for EVLA. Conclusion Early closure rates following endothermal ablation of incompetent lower limb perforating veins are lower than those obtained for truncal ablation. EVLA perforator closure appears to be more effective than segmental RFA in most situations but short treatment lengths and location at the ankle are associated with the poorest outcomes.

A 3-months baby comes to our emergency department for an abdominal mass of elastic consistency that suddenly appeared for a week and grown exponentially in the last few days. Blood examinations identify a septic state of the child, and the emergency CT recognizes the abdominal mass’s cystic structure. The surgical removal and the histological analysis will confirm the diagnosis of Infected Common Macrocystic Lymphatic Malformation. This case allows us to remember to pediatricians the main features of Cystic Lymphatic Malformations that should always be considered during the differential diagnosis of abdominal masses of sudden onset in an infant.

Acute upper airway obstruction from laryngeal polyps is uncommon. However, a large pedunculated laryngeal polyp, when unrecognized, may produce sudden airway obstruction. The importance of an early diagnosis and treatment is stressed. Primary care physicians, endoscopists, anesthesiologists, and otolaryngologists should be aware of this condition and add it to their differential diagnosis of sudden respiratory obstruction.

Wilms’ tumor is the most frequent renal malignancy in childhood with the highest incidence per year, approximately 7,8 cases per 1.000.000in children under 15 years-old and frequently occurred in 2-5 years of age (highest incidences in 3 years-old). There are many differential diagnosis of intra-abdominal tumors and the correct differential diagnosis are detrimental to the prescribed treatments for the patients.Medical imaging along with pathology reports is a precise way to determine the appropriate diagnosis and treatment. Imaging gives information about tumor extension and distant metastasis, especially useful for indicating pre-operative chemotherapy.

This article describes a patient presenting with a sensory polyneuropathy and multiple autoantibodies, leading to the diagnosis of hepatitis C virus (HCV) infection. His widely positive autoantibody profile in the absence of clinically significant rheumatic disease illustrates the importance of interpreting autoimmune serology in the appropriate clinical context and the concept of HCV being a non-specific activator of the immune system. In addition, it highlights the importance of considering untreated HCV infection in the differential diagnosis of rheumatic complaints, particularly if the workup reveals multiple autoantibodies, as HCV is a potentially severe and life-threatening disease, which can be appropriately managed with effective antiviral therapy.

We present a rare case of KRAS keratinocytic epidermal nevus syndrome with lymphatic malformation, responsive to treatment with sirolimus, an mTOR inhibitor. A brief review of the current literature regarding sirolimus use in vascular malformations, lymphatic malformations, regional overgrowth syndromes, and RASopathies is discussed.
