Objective To compare the therapeutic effects of one-time root canal treatment versus two-time treatment in patients with irreversible pulpitis. Methods We classified 130 patients into a one-time treatment group (group A; n = 68) and a two-time treatment group (group B; n = 62) according to the number of patient visits. Clinical efficacy at 6 months and 1 year follow-ups as well as serum levels of interleukin-6, tumor necrosis factor alpha, and high-sensitivity C-reactive protein before and 1 week after endodontic treatment were observed. Additionally, the level of treatment satisfaction between the two groups was compared, and the degree of pain was evaluated using the visual analogue scale. Result One week after treatment, serum interleukin-6, tumor necrosis factor alpha, and high-sensitivity C-reactive protein levels were significantly decreased in the two groups; however, levels in group B were significantly lower than those in group A. Additionally, visual analogue scale scores of patients in group A were significantly higher than those of patients in group B. Conclusion One-time root canal therapy can effectively improve postoperative pain and the expression of inflammatory factors in the serum of patients undergoing treatment.

Objective. To clarify whether time lapsing from advent of fever as a first sign of sepsis may be indicative of the potency of monocytes for the release of pro- and anti-inflammatory mediators.Methods. Monocytes were isolated from blood of 51 septic patients and 9 healthy donors. Monocytes were incubated in the absence and presence of patients' serum and concentrations of tumour necrosis factor-alpha (TNF), interleukin (IL)-6, IL-10, and malondialdehyde (MDA) were estimated in supernatants. Patients were divided into three groups: group A: 12 hours; group B: 12–24 hours, and group C: 24 hours between initiation of fever and blood sampling.Results. TNF of supernatants of groups B and C was higher than controls, as also were IL-6 of A and C, IL-10 of A and B, and MDA of A. IL-6 of group A was increased after addition of patients serum. A negative correlation was found between time from initiation of symptoms and IL-6 of monocyte supernatants incubated in the presence of patients serum. Median IL-6 of survivors was higher than nonsurvivors.Conclusion. Monocytes are potent for the release of pro- and anti-inflammatory mediators within the first 24 hours upon advent of fever related to sepsis; serum stimulates further release of IL-6 within the first 12 hours.

Systemic inflammation describes certain metabolic alterations which are mediated by inflammatory cytokines. Theses occur essentially as a defensive body response towards offending agents such as surplus nutrient staffs. Our aim is to find out the role of inulin as a protective agent against metabolic inflammation. Twenty eight type 2 diabetic females were subjected to the estimation of their serum high sensitivity C-reactive protein, lipopolysaccharides, tumor necrosis factor alpha, adiponectin and HOMA-IR test before and after three weeks of inulin ingestion. There was a significant drop in the level of serum high sensitivity C-reactive protein, lipopolysaccharides, tumor necrosis factor alpha, HOMA-IR and a non-significant rise in serum adiponectin after inulin ingestion. In summary inulin can act as a useful protective agent in systemic inflammation.
