Objective: The immediate and long-term effects of traumatic exposure and subsequent posttraumatic stress reactions in people in high-risk occupations are well-documented. What is less evident is the impact of this traumatic exposure and subsequent traumatic stress symptoms on workers' response to acute stress situations. This study aimed to examine the association between prior traumatic exposure related to policing, current posttraumatic stress symptoms and biological markers of stress, and subjective appraisal of stress before, during, and after exposure to acutely stressful stimuli. Method: A stressful policing situation was created through the use of a video simulator room. Participants' responses to the simulated emergency were evaluated by monitoring heart rate, collecting salivatory samples for cortisol analysis, and repeated administration of a subjective measure of anxiety. Results: Biological indicators of stress, as measured by cortisol level and heart rate, were not associated with previous trauma exposure or trauma symptoms; however, biological response was associated with subjective anxiety. Vulnerability to psychological stress responses during an acute stress situation was also associated with lower levels of social support, previous traumatic exposures, and preexisting symptoms of traumatic stress. The importance of these factors became more pronounced as time progressed after the event. Conclusion: Previous trauma exposure did not put individuals at increased risk of biological distress during an acute stress situation. However, previous trauma and reduced social supports were associated with continuing psychological distress, confirming previous research and raising concerns about the cumulative negative effects of traumatic exposure on psychological health in emergency responders.

This chapter reviews the literature on the association between traumatic stress exposure and rates of both posttraumatic stress disorder (PTSD) and acute stress disorder (ASD) among children and adults. It begins by reviewing current definitions of PTSD and ASD, which vary substantially across diagnostic systems. The chapter highlights research linking large-scale events, such as natural disasters and acts of terrorism, with the emergence of PTSD and ASD, as well as the literature on the impact of individual traumatic events, such as sexual assault, child sexual abuse, and medical trauma. The chapter concludes by noting several important directions for future research in the area of trauma exposure and traumatic stress.

Substantial individual differences exist in how acute stress affects large-scale neurocognitive networks, including Salience (SN), Default Mode (DMN) and Central Executive Networks (CEN). These network-level changes upon acute stress may predict vulnerability to long-term stress effects, which can only be tested in prospective longitudinal studies. Using a longitudinal design, we investigated whether the magnitude of acute-stress induced functional connectivity changes (delta-FC) predicts the development of posttraumatic stress disorder (PTSD) symptoms in a relatively resilient group of young police recruits that are known to be at high risk for trauma-exposure. Using resting-state fMRI, we measured acute-stress induced delta-FC in 190 police recruits before (baseline) and after trauma exposure during repeated emergency aid services (16-month follow-up). Delta-FC was then linked to the changes in perceived stress levels (PSS) and post-traumatic stress symptoms (PCL and CAPS). Weakened connectivity between the SN and DMN core regions upon acute stress induction at baseline predicted longitudinal increases in perceived stress level but not of post-traumatic stress symptoms, whereas increased coupling between the overall SN and anterior cerebellum was observed in participants with higher clinician-rated PTSD symptoms, particularly intrusion levels. All effects remained significant when controlling for trauma exposure-levels and cortisol stress-reactivity. Except these neural effects, neither hormonal nor subjective measures were relevant. The reconfiguration of large-scale neural networks upon acute stress induction is relevant for assessing and detecting risk and resilience factors for PTSD. This study highlights the SN connectivity-changes as a potential marker for trauma-related symptom-development, which is sensitive even in a relatively resilient sample.

AbstractAs the novel coronavirus (COVID-19) has spread globally, a significant portion of pregnant and delivering women were infected with COVID-19. While emerging studies examined birth outcomes in COVID-19 positive women, knowledge of the psychological experience of childbirth and maternal wellness remains lacking. This matched-control survey-based study included a sample of women recruited during the first wave of the pandemic in the US who gave birth in the previous six months. Women reporting confirmed/suspected COVID-19 (n = 68) during pregnancy or childbirth were matched on background factors with women reporting COVID-19 negativity (n = 2,276). We found nearly 50% of COVID positive women endorsed acute traumatic stress symptoms at a clinical level in response to childbirth. This group was more than twice as likely to endorse acute stress and to have no visitors during maternity hospitalization than COVID negative women; they were also less likely to room-in with newborns. The COVID positive group reported higher levels of pain in delivery, lower newborn weights, and more infant admission to neonatal intensive care units. Our findings suggest COVID-19 affected populations are at increased risk for traumatic childbirth and associated risk for psychiatric morbidity. Attention to delivering women’s wellbeing is warranted during the pandemic.

Background: The aim of the study was to determine the amount of trauma impact, post-traumatic stress symptoms and current psychopathological distress in a sample of former German children of World War II.Methods: 93 participants were recruited through the local press, and assessed using the modified Post-traumatic Diagnostic Scale (PDS) and the Symptom Checklist (SCL-90-R).Results: Subjects reported a high qualitative and quantitative degree of trauma exposure. 13.8% reported PTSD-related symptoms after the war, and 10.8% reported current symptoms. PTSD symptoms after World War II were significantly correlated with current psychopathological distress.Conclusions: In line with other studies, our data document a high degree of trauma exposure during warchildhood. In comparison with other studies on PTSD in warchildren, there is a persisting high prevalence of war-associated PTSD symptoms in this sample. Despite some methodological limitations, our data underline the urgent need for further studies on the ageing group of former children of World War II.

The prevalence of posttraumatic stress disorder (PTSD) as it relates to people’s experiences of the COVID-19 pandemic has yet to determined. This study was conducted to determine rates of COVID-19 related PTSD in the Irish general population, the level of comorbidity with depression and anxiety, and sociodemographic risk factors associated with COVID-19 related PTSD. A nationally representative sample of adults from the general population of the Republic of Ireland (N = 1,041) completed self-report measures of all study variables. The rate of COVID-19 related PTSD was 17.7% (95% CI = 15.35 - 19.99%: n=184), and comorbidity with generalized anxiety (49.5%) and depression (53.8%) was high. Meeting the diagnostic requirement for COVID-19 related PTSD was associated with younger age, male sex, living in a city, living with children, moderate and high perceived risk of COVID-19 infection, and screening positive for anxiety or depression. Traumatic stress problems related to the COVID-19 pandemic are common in the general population. Our results show that health professionals responsible for responding to the COVID-19 pandemic should expect to routinely encounter traumatic stress problems.
