A brief report of suicide statistics — completed suicides, attempts with lethal intent (a new category), and other attempts — is presented. The highest risk group in Halifax County is the male from late adolescence to early middle life. Statistics from Canada and Nova Scotia reveal a marked increase in the rate of completed suicides in the young adult male during the past twenty years. It is noteworthy that completed suicides and attempts with lethal intent frequently occur in persons with no clear evidence of significant depressive illness and that many patients with severe depressive illness are never seriously suicidal. Many persons and some families have a pattern of reacting to stress and/or psychiatric illness with suicidal behaviour. There is often little correlation between the seriousness of the stress and/ or illness and the seriousness, of the suicidal behaviour. Most suicide's were considered to be not preventable by ordinary techniques of management. Various patterns of behaviour relevant to preventability are described. A brief description of reactions of physicians and nurses to suicidal behaviour is given. There is a negative reaction of both groups to suicidal attempts which are not serious, particularly to repeaters; and this reaction is much stronger in nurses than in physicians. There is also a brief description of unusual complications of suicidal attempts and of the very high medical costs of caring for those who make attempts.

ABSTRACTSerrated adenoma is a newly described entity in the group of gastric adenomas. Until date only 20 cases of gastric serrated adenoma have been reported. It is an important entity to be diagnosed accurately as it has a very high-risk of malignant transformation, especially those located in the cardia of stomach. Serrated adenoma associated with adenocarcinoma is more frequent in the elderly, but pure serrated adenoma is common in the young, in whom follow-up is mandatory. Gastric serrated adenoma has distinct location, definite histomorphology and characteristic Ki-67 immunohistochemical staining. Ki-67 staining helps to differentiated pure serrated adenoma from those associated with adenocarcinoma. We present a young adult male, incidentally detected to have gastric serrated adenoma.

Driving to the symposium, the vague anxiety and discomfort I had heen experiencing for the past few days finally culminated in the realization that 7 years ago that day, I awoke from my first night as an inpatient at a psychiatric unit.Since then and because of that, I've learned a great deal about depressive and manic-depressive illness. I've learned about stigma and shame. I've learned about the devastation that the violence we benignly call “mood disorder” wreaks on families. I have been awed by the array of treatments we've found to ease the suffering, and humbled by the fact that we still have a long way to go. I have become painfully aware of how unenlightened the world still is about psychiatric illness, and saddened by the cruel irony that the only thing more stigmatizing than suffering from a psychiatric illness is seeking treatment for it.

BackgroundThe number of elderly prisoners has increased significantly in Western countries over the past decade. Little is known about the psychiatric morbidity of this population.AimsTo determine the prevalence of psychiatric morbidity in elderly sentenced prisoners.MethodA stratified sample of 203 male sentenced prisoners aged over 59 years, from 15 prisons in England and Wales, representing one in five men in this age group, was interviewed using semi-structured standardised instruments for psychiatric illness and personality disorder.ResultsMore than half of the elderly prisoners had a psychiatric diagnosis. The most common diagnoses were personality disorder and depressive illness.ConclusionsThe prevalence of depressive illness was five times greater than that found in other studies of younger adult prisoners and elderly people in the community. Underdetected, undertreated depressive illness in elderly prisoners is an increasing public health problem.

This paper examines two young adult novels, Run Like Jäger (2008) and Summer of Fire (2009), by Canadian writer Karen Bass, which centre on the experiences of so-called ordinary German teenagers in World War II. Although guilt and perpetration are themes addressed in these books, their focus is primarily on the ways in which Germans suffered at the hands of the Allied forces. These books thus participate in the increasingly widespread but still controversial subject of the suffering of the perpetrators. Bringing work in childhood studies to bear on contemporary representations of German wartime suffering in the public sphere, I explore how Bass's novels, through the liminal figure of the adolescent, participate in a culture of self-victimisation that downplays guilt rather than more ethically contextualises suffering within guilt. These historical narratives are framed by contemporary narratives which centre on troubled teen protagonists who need the stories of the past for their own individualisation in the present. In their evacuation of crucial historical contexts, both Run Like Jäger and Summer of Fire support optimistic and gendered narratives of individualism that ultimately refuse complicated understandings of adolescent agency in the past or present.

Since the treatment of juveniles as in-patients in a special unit is somewhat unusual in mental hospital practice, a brief introduction may not be out of place. These units might be considered as another development in a trend which has been progressing for the past 25 years. Until 1930 certification of all admissions to mental hospitals and a mainly custodial régime ensured the majority of patients being largely the end-results of psychiatric illness. Since 1930 the steadily increasing use of the voluntary system has brought many patients to hospital at a stage when their illness can be favourably influenced by modern therapeutic methods. An associated development was the increased provision of wards or units separate from the chronically disturbed cases, or even, as at this hospital, a complete villa system of detached and semi-detached wards for mainly voluntary adult patients.

This study compares the effects of electromyographic-biofeedback (EMG-BF)-guided isometric quadriceps strengthening with patellar taping and isometric exercise alone in patellofemoral pain syndrome (PFPS) among young adult male athletes. Sixty young adult male athletes with PFPS participated in the study. Participants were randomly divided into two groups: (1) EMG-BF-guided isometric exercise training with patellar taping (experimental group, n = 30), and (2) sham EMG-BF training with an isometric exercise program (control group, n = 30). Participants conducted their respective exercise programs for five days per week across four weeks. Study outcomes were pain (measured by the visual analog scale), functional disability (measured by the Kujala Anterior Knee Pain scale), and quadriceps strength (measured by an ISOMOVE dynamometer). Measurements were taken at baseline, Week 2, Week 4, and during a follow-up at Week 6. The experimental group demonstrated significantly lower VAS score at Weeks 2 and 4 compared to that of the control group (p = 0.008 and 0.0005, respectively). The score remained significantly lower at the Week 6 follow-up compared to the control group (p = 0.0005). There were no differences in knee function at Weeks 2 and 4 between the two groups (p = 0.086 and 0.171, respectively); however, the experimental group showed significantly better knee function at Week 6 compared to the control group (p = 0.002). There were no differences in quadriceps strength at Week 2 between the two groups (p = 0.259); however, the experimental group demonstrated significantly higher quadriceps strength at Weeks 4 and 6 compared to the control group (p = 0.0008). Four weeks of EMG-BF supplementation training with patellar taping demonstrated significant improvements in pain intensity, functional disability, and quadriceps muscle strength in young adult male athletes with PFPS.

Despite the number of sailings canceled in the past few months, as demand has increased, the utilization of ships has become very high, resulting in sudden peaks of activity at the import container terminals. Ship-to-ship operations and yard activity at the container terminals are at their peak and starting to affect land operations on truck arrivals and departures. In response, a Truck Appointment System (TAS) has been developed to mitigate truck congestion that occurs between the gate and the yard of the container terminal. The vehicle booking system is developed and operated in-house at large-scale container terminals, but efficiency is low due to frequent truck schedule changes by the transport companies (forwarders). In this paper, we propose a new form of TAS in which the transport companies and the terminal operator cooperate. Numerical experiments show that the efficiency of the cooperation model is better by comparing the case where the transport company (forwarder) and the terminal operator make their own decision and the case where they cooperate. The cooperation model shows higher efficiency as there are more competing transport companies (forwarders) and more segmented tasks a truck can reserve.
