Association between genotoxic properties of inhalation anesthetics and oxidative stress biomarkers

Exposure to inhalation anesthetics (IAs) has been associated with DNA damage as reflected in the increased frequency of micronuclei (MN) and chromosomal aberrations (CAs). The present study was undertaken to ascertain whether there was any correlation between increased MN and CA and the extent of oxidative stress as well as the antioxidant status of a group of operating room personnel exposed to a mixture of IAs, including nitrous oxide, isoflurane, and sevoflurane. In this cross-sectional study, 60 operating room personnel (exposed group) in whom the frequencies of MN and CA had already been shown to be significantly higher than those of a referent group, as well as 60 unexposed nurses, were studied. Venous blood samples were taken from all participants, and malondialdehyde (MDA) levels as an index of oxidative stress (OS) and the activity of superoxide dismutase (SOD) and levels of total antioxidant capacity (TAC) as indices of antioxidant status were measured. The level of TAC (1.76 ± 0.59 mM vs. 2.13 ± 0.64 mM, p = 0.001) and the activity of SOD (11.22 ± 5.11 U/ml vs. 13.36 ± 4.12 U/ml, p = 0.01) were significantly lower, while the mean value of MDA was significantly higher (2.46 ± 0.66 µM vs. 2.19 ± 0.68 µM, p = 0.03) in the exposed group than in the nonexposed group. After adjusting for potential confounders, there were statistically significant associations between exposure to IAs, gender, SOD, and TAC with MN frequency and between exposure to IAs and SOD with numbers of CA. The findings of the present study indicated that exposure to IAs was associated with OS, and this, in turn, may be causally linked with DNA damage.

Introduction: Incidence of hypertension is increasing in young population. Aggressive control is mandatory to preserve and protect public health in India. Several socio-demographic, anthropometric, environmental, lifestyle, biochemical and genetic factors contribute for the development of hypertension in young adults. Hypertension and its chronic complications are also linked to oxidative stress and DNA damage. Methods: The cross sectional study involves 180 young hypertensives between the age group of 18 – 39 years and 140 apparently healthy age and sex matched controls. After obtaining relevant history, anthropometric measurements were taken. Socio-demographic characters were recorded using proforma. After taking written informed consent, five ( 5 ml) of venous blood was collected after 8-12 hours of fasting. 2 ml of blood was transferred aseptically to a sodium heparinised vacutainer for evaluating somatic DNA damages by Cytokinesis-block Micronuclei assay (CBMN assay). Remaining 3 ml of blood is allowed to clot; serum separated and was used for other biochemical investigations such as fasting blood sugar, lipid profile, urea, creatinine and malondialdehyde (MDA). Statistical analysis done using SPSS software. Results: Fasting blood sugar, Total cholesterol, LDL cholesterol, triglycerides, urea and creatinine were significantly elevated in young hypertensives. Oxidative stress and DNA damage is significantly elevated in young hypertensives compared to control group. BMI and abdominal circumference are also significantly elevated in young hypertensive adults compared to control group. Conclusion: Increased oxidative stress and DNA damage in young hypertensives contribute to the complications associated with hypertension.

Obese individuals without metabolic comorbidities are categorized as metabolically healthy obese (MHO). MicroRNAs (miRNAs) may be implicated in MHO. This cross-sectional study explores the link between circulating miRNAs and the main components of metabolic syndrome (MetS) in the context of obesity. We also examine oxidative stress biomarkers in MHO vs. metabolically unhealthy obesity (MUO). We analysed 3536 serum miRNAs in 20 middle-aged obese individuals: 10 MHO and 10 MUO. A total of 159 miRNAs were differentially expressed, of which, 72 miRNAs (45.2%) were higher and 87 miRNAs (54.7%) were lower in the MUO group. In addition, miRNAs related to insulin signalling and lipid metabolism pathways were upregulated in the MUO group. Among these miRNAs, hsa-miR-6796-5p and hsa-miR-4697-3p, which regulate oxidative stress, showed significant correlations with glucose, triglycerides, HbA1c and HDLc. Our results provide evidence of a pattern of differentially expressed miRNAs in obesity according to MetS, and identify those related to insulin resistance and lipid metabolism pathways.

This study was undertaken to determine whether exposure of operating room personnel to inhalation anesthetics, nitrous oxide, isoflurane, and sevoflurane was associated with any hematological changes. This historical cohort study was performed in 2018 at a large public hospital in Shiraz, where 52 operating room personnel and 52 administrative staff were investigated. The blood sample was taken from all individuals for Complete Blood Count. Furthermore, demographic information was collected through questionnaires. Mean atmospheric concentrations of nitrous oxide, isoflurane, and sevoflurane, to which subjects were exposed, were 850.92, 2.40, and 0.18 ppm, respectively. The hematological parameters were within the normal range in both groups. However, the mean values of hemoglobin, hematocrit, mean corpuscular hemoglobin, mean corpuscular hemoglobin concentration, and red blood cell count in the exposed group were significantly lower than the control group. No significant differences were noted between the two groups as far as other hematological factors were concerned. These findings provide circumstantial evidence to further substantiate the notion that occupational exposure to inhalation anesthetics, under the exposure scenario explained in this study, is associated with subtle, subclinical, prepathologic hematological changes. Long-term consequence and ramifications of these effects require further investigation. The range of exposure levels to anesthetic gases in operating rooms.

Background It has become evident that gadolinium-based contrast agents (GBCA) may have nephrotoxic potential. Oxidative stress is one of the most important pathways in the pathogenesis of iodinated contrast-induced nephropathy. Purpose To investigate the effects of static magnetic fields and gadopentetate dimeglumine (Magnevist®) on oxidant/antioxidant status via measurement of total antioxidant capacity (TAC), total oxidant status (TOS), and serum malondialdehide (MDA). Material and Methods Two age- and sex-matched groups of patients not under oxidative stress conditions that underwent magnetic resonance imaging (MRI) were recruited to this study. While contrast-enhanced (Magnevist®, 0.2 mmol/kg) MRI was performed in group 1, MRI without GBCA was performed in group 2. Fasting blood glucose, C-reactive protein, serum creatinine, liver enzymes, uric acid, and lipid parameters were examined in all patients. Peripheral venous blood samples in order to determine TAC, TOS, and MDA were collected before and 6, 24, and 72 h after the MRI procedures. The TOS:TAC ratio was used as the oxidative stress index (OSI). Patients were followed up to 72 h. Results There were no significant changes in serum TAC, TOS, and MDA levels (Δserum TAC, Δserum TOS, and ΔMDA) in either group 6, 24, or 72 h after the procedures (P > 0.05). Furthermore, OSI did not change after the procedures in either group (P > 0.05). Conclusion Magnetic field and gadopentetate dimeglumine (Magnevist®) do not change the oxidant or antioxidant status at a dose of 0.2 mmol/kg.

Abstract Purpose: Professional communication and professional values are two basic concepts in operating rooms and should be studied more closely in view of the nature of work, the high circulation of patients in operating rooms. Methods: The present work is a descriptive-analytic study with a cross-sectional design. The sample was 603 operating room doctors and personnel selected from the public hospitals of Shiraz. The data collection instruments were the 41-item professional communication questionnaire and the 26-item professional values scale.Result: Results shows the operating room nurses and doctors were found to perceive the status of professional communication and professional values to be satisfactory. About professional communications, the participants’ perception of the domains of mutual respect and trust (p≤0.001), teamwork (p≤0.001), ethical competence (p≤0.017), and workplace conflicts (p≤0.001) was significant. About professional values, only the dimension of care (p≤0.016) was perceived to be significant. Moreover, a significant positive relationship was found to exist between professional communication and professional values (p≤0.001). conclusion: Considering the significance of the concept of professional communication and its connection with professional values, it is recommended that operating room personnel and doctors receive systematic education about professional communication and the harms of destructive attitudes as part of their academic education and afterwards.
