Competence and Compassion: Key Elements of Professional Care at the End of Life From Caregiver’s Perspective

In the act of caring for and helping people in the end-of-life process, the professional who provides care and assistance must know how to maintain a relationship of closeness, empathy, and compassion for the pain and suffering of the person who is going to die. The objective was to understand, elaborate on, and characterize the key elements of end-of-life care of patients from a caregiver’s perspective through a qualitative phenomenological multicenter study. Participants were caregivers who had lost a family member at least 2 months but less than 2 years in the past. The techniques used were 5 discussion groups and 41 in-depth interviews, which included a total of 81 participants. To analyze the information, a protocol developed by Giorgi was followed. Two dimensions or units of meaning, with subdimensions, emerged: (1) Technical competence, with the subdimensions “Control of symptoms” and “Continuity of care,” and (2) Compassion, with the subdimensions “Effective/affective communication,” “Attitudes of kindness and closeness toward the patient and the family,” and “Generosity and personalized flexibility of care.” Assistance at the end of life requires the proper preparation of professionals who care for these patients, in addition to a compassionate attitude on the part of professionals and the people accompanying the dying person, that fosters a more humanized and dignified treatment in the dying process.

Introduction: Watson established the humanized care as a research phenomenon. It remains an ethical responsibility of nurses in the context of a transpersonal relationship that enhance the harmony and individuals integrity. Humanized care now faces a dilemma for a coexistence of diverse institutional, biomedical, administrative-economist and humanist axiological models. Objective: To know the perception of care, according to lived experience of three retired nurses, through authentic stories. Methodology: A qualitative, phenomenological study describes the experiences through authentic stories of three retired nurses obtained by in-depth interviews. Homogeneous sample selection criteria: nurses belonging to 3 generations removed extensive experience in closed care, participation in training students. Results: four categories were found concatenated with each other, which reconfigure the essence of care, emphasizing the integral character of the people and harmonizing the relational dimension with the technical-scientific dimension. Nurse as a caregiver; person as being of care; essence of care and vocational training. Conclusion: Knowing the perception of retired nurses, regarding care is a contribution for reflection. It is necessary to aim for humanized care, as an ethical duty of nurses. The intervention of various institutional axiological models weakens the possibility of guiding care towards humanization. There is a risk of fragmenting people, leaving aside individualized, warmth care, generated through a transpersonal relationship.

The architecture of Towani Tolotang’s traditional house has characteristics that represents the identity, activities, social and cultural values ​​of the community. The Towani Tolotang people strongly maintain the relationship of togetherness and unity shown by the architecture of traditional houses. This study discussed and examined the characteristics of micro spatial patterns, functions, and meanings of the Towani Tolotang’s traditional house. The study was conducted in Amparita Village, Sidrap Regency, South Sulawesi. The discussion on the architecture of the traditional house of Towani Tolotang was carried out by applying the naturalistic phenomenological method. This type of research is qualitative with the process of collecting data through participatory observation, in-depth interviews, and document studies. Data analysis was performed using descriptive qualitative, describing in accordance with the natural situation. The results of the study revealed that there were two spaces in the spatial pattern of Towani Tolotang’s traditional house being the characteristics different from Bugis’s houses in general. They were tamping and special rooms. The function of the space was similar to the Bugis’s house in general, except the two spaces. Tamping was functioned as a place to receive guests with the status of ordinary people, and the special room was used as a place for ritual and taking prayer for the people of Towani Tolotang. Every space in the Towani Tolotang’s traditional house has its own meaning, included tamping which means respect for the traditional leader and special room which means the sacred space.

ABSTRACTDrawing on data from in-depth interviews with 35 men and women aged 73–91, this article examines the ways in which older adults with multiple chronic conditions talk about and prepare for death and dying. While the focus of the original study did not include questions concerning the end-of-life, the majority of our participants made unprompted remarks regarding their own and others' mortality. The participants discussed the prevalence of death in their lives as it related to the passing of significant others, as well as their own eventual demise. Additionally, the men and women expressed hopes and fears about their impending death, in particular with respect to prolonged pain and suffering, institutionalisation, and a loss of mental acuity and independence. Many of our participants also described their end-of-life plans, which included making funeral arrangements, obtaining living wills, and planning their suicides. They further reported a number of barriers to their planning for death, including a lack of willingness on the part of family members to discuss their wishes as well as a scarcity of institutional resources and support. We discuss our findings in relation to the extant research concerning older adults' experiences of death and dying, as well as Glaser and Strauss' (1971) theory of status passage and Marshall's (1986) conceptualisation of authorship and the legitimation of death.

Two distinct spatial metaphors for the passage of time can produce disparate judgments about grieving. Under the object-moving metaphor, time seems to move past stationary people, like objects floating past people along a riverbank. Under the people-moving metaphor, time is stationary; people move through time as though they journey on a one-way street, past stationary objects. The people-moving metaphor should encourage the forecast of shorter grieving periods relative to the object-moving metaphor. In the present study, participants either received an object-moving or people-moving prime, then read a brief vignette about a mother whose young son died. Participants made affective forecasts about the mother’s grief intensity and duration, and provided open-ended inferences regarding a return to relative normalcy. Findings support predictions, and are discussed with respect to interpersonal communication and everyday life.

Basically, Islamic education is a continuous, sustainable, and everlasting process of human life. Educational success cannot be separated from the educators, who are essentially the people having the responsibility to educate, guide, direct and lead their learners to get of success both in this world and in the hereafter. Therefore, qualified educators and professionals should have specific criteria and requirements that must be met in order to achieve the purpose of life, and also the properties that adorn his personal duty and responsibility as educators in the view of Islam. A good educator is a person who pays attention to the duties and responsibilities to the students, based on faith and piety to God, and also able to develop the potentials of either the inner or outer (physical, psychological, and spiritual). Pada hakikatnya, pendidikan Islam adalah suatu proses yang berlangsung secara kontinyu dan berkesinambungan dalam kehidupan manusia dan berlangsung sepanjang hayat. Tugas dan fungsi pendidikan memiliki sasaran pada peserta didik yang senantiasa tumbuh dan berkembang secara dinamis, mulai dari kandungan sampai akhir hayatnya dan keberhasilan pendidikan tidak lepas dari aspek pendidik. Pendidik pada hakekatnya adalah orang yang telah mendapatkan amanat dan mempunyai tanggung jawab dunia akherat dalam mendidik, membimbing, mengarahkan dan mengantarkan peserta didik ke gerbang kesuksesan baik di dunia maupun di akherat. Oleh karena itu untuk menjadi pendidik yang berkualitas dan profesional harus memiliki kriteria dan persyaratan tertentu yang harus dipenuhi dalam rangka pencapaian tujuan hidup dan juga sifat-sifat yang menghiasi pribadinya dalam menjalankan tugas dan tanggungjawab sebagai pendidik dalam pandangan Islam. Pendidik yang baik adalah pendidik yang memperhatikan tugas dan tanggung jawabnya terhadap peserta didik, yang dilandasi iman dan taqwa kepada Allah SWT, dan juga mampu mengembangkan potensi yang ada baik lahir maupun batin (jasmani, psikis, maupun rohani).

Thus can drawing conclusions about the relationship of the school with the community is essentially a very decisive tool in fostering and developing the personal growth of students in schools. If the relationship between the school and the community goes well, the sense of responsibility and participation of the community to advance the school will also be good and high. In order to create relationships and cooperation between schools and the community, the community needs to know and have a clear picture of the school they have obtained.The presence of schools is based on the good will of the country and the people who support it. Therefore people who work in schools inevitably have to work with the community. The community here can be in the form of parents of students, agencies, organizations, both public and private. One reason schools need help from the community where schools are because schools must be funded.

This study provides the synonyms for the terms deliberate self-harm and self-destructive behaviour, together with a psychological portrait of self-harming adolescents, the consequence of self-harm, the purpose of self-harm, and the forms of self-harm. It also presents the results of a survey regarding the prevalence of people with non-suicidal self-harming behaviour, the gender of people with non-suicidal self-harming behaviour, the age of the first non-suicidal self-harming behaviour in these people, the frequency of non-suicidal self-harming behaviour in these people, the association of the non-suicidal self-harming behaviour with substance misuse in these people, the relationships of the people with non-suicidal self-harming behaviour with their fathers, mothers, and siblings, the relationships of the people with non-suicidal self-harming behaviour with their friends, the possible causes of self-harming behaviour in these people, and the relationship of people with non-suicidal self-harming behaviour with religion. Some of the results confirmed literature results, while others shed a new light on other aspects related to people with non-suicidal self-harming behaviour

Introduction: The purpose of this study was to investigate the effect of visual impairment and correctable visual impairment (i.e., uncorrected refractive errors) on being out of the labor force and on unemployment. The effect of health on labor force status was also investigated. Method: National Health and Nutrition Examination Survey (NHANES) data from 1999 to 2008 ( N = 15,650) was used for this study. Participants were classified into three vision status groups: normal, correctable visual impairment, and visual impairment. Statistical analyses utilized were chi-square and logistic regression. Results: Having a visual impairment was significantly associated with being out of the labor force, while having a correctable visual impairment was not. Conversely, having a correctable visual impairment was associated with unemployment, while having a visual impairment was not. Being out of the labor force was not significantly associated with health for those with a visual impairment, although it was for those with correctable visual impairments and normal vision. Discussion: Given previous research, it was surprising to find that health was not associated with being out of the labor force for those with visual impairments. Perhaps other disadvantages for the people with visual impairments identified in this study contributed to their higher out-of-the-labor-force rates regardless of health. Implications for practitioners: Researchers utilizing national data sets that rely on self-reports to identify visual impairments should realize that some of those who self-identify as being visually impaired may actually have correctable visual impairments. Current research is needed to understand why a majority of people with visual impairments are not seeking employment and have removed themselves from the labor force.

Abstract Objective Dying is mostly seen as a dreadful event, never a happy experience. Yet, as palliative care physicians, we have seen so many patients who remained happy despite facing death. Hence, we conducted this qualitative study to explore happiness in palliative care patients at the University of Malaya Medical Centre. Method Twenty terminally ill patients were interviewed with semi-structured questions. The results were thematically analyzed. Results Eight themes were generated: the meaning of happiness, connections, mindset, pleasure, health, faith, wealth, and work. Our results showed that happiness is possible at the end of life. Happiness can coexist with pain and suffering. Social connections were the most important element of happiness at the end of life. Wealth and work were given the least emphasis. From the descriptions of our patients, we recognized a tendency for the degree of importance to shift from the hedonic happiness to eudaimonic happiness as patients experienced a terminal illness. Significance of results To increase the happiness of palliative care patients, it is crucial to assess the meaning of happiness for each patient and the degree of importance for each happiness domain to allow targeted interventions.

Abstract Advance directives (AD) help to ensure patients’ wishes are honored and contribute to improved end-of-life care. According to normative life course theory, retirement is a significant role change that signals a transition into the third age and its socially prescribed activities. To the extent that ACP is viewed as something to do when one reaches a more advanced stage in life, retirement may spark recognition that planning for incapacity and the end of life is now personally relevant and appropriate. This study tested whether transitioning from work to retirement prompted AD completion. The sample included Health and Retirement Study participants 65 and older who, in 2012, had no ADs and were not completely retired (N = 919). Retirement was operationalized as both a categorical status and as a multistep process. Three waves of data were analyzed using multinomial logistic regression to test associations between retirement transition and advance directive completion. By 2014, 21% had completed ADs and another 17% completed them by 2016. Those who completely retired between 2012 and 2014 were almost twice as likely to complete ADs between 2014 and 2016. Graduated increase in level of retirement between 2012 and 2014 was associated with higher odds of new AD possession in 2016, but did not reach statistical significance at p < .05. These results suggest the period following retirement may be an optimal time to encourage patients and clients who have not already done so to complete advance directives.
