Background: Gestational weight gain (GWG) is a modifiable risk factor for obesity in women. Black women have the greatest prevalence of high body mass, which predisposes them to excessive GWG. Increased understanding of genetic influences on GWG has implications for the health of women. The purpose of this study was to explore the associations of GNB3 and FTO risk alleles in pregnant women with prepregnancy body mass index (BMI), GWG, and postpartum and infant birth weights. Research design and Methods: This was an observational, prospective candidate gene association study. Pregnant, low-income Black women ( N = 97) were enrolled in early pregnancy and followed until 6 months postpartum. Results: GWG differed depending on number of FTO risk alleles. The mean 6-month postpartum BMI differed, although not significantly, by 4 kg/m2 between homozygous women. There was an interaction between the FTO risk allele and prepregnancy BMI ( p = .022), with obese homozygote AA women having significantly higher mean GWG than obese TT women. When controlling for age and smoking, the FTO gene and physical activity predicted GWG ( p = .032). Although not statistically significant, women who carried the GNB3 T risk allele gained 6 pounds more than noncarriers, and mean 6-month postpartum BMI differed by 2.2 kg/m2 between homozygous women. Neither the GNB3 nor FTO gene predicted prepregnancy BMI, infant birth weight, or postpartum weight. Conclusion: Obese women homozygous for the FTO risk allele were at greater risk of excessive GWG compared to nonrisk allele homozygous obese women or nonobese women. This study provides evidence of the FTO gene’s effect on GWG in Black women.

The aim of diabetes care of pregnant women with gestational diabetes mellitus (GDM) is to attain pregnancy outcomes including rates of large-for-gestational-age (LGA) newborns, pre-eclampsia, C-sections (CS) and other neonatal outcomes similar to those of the non-GDM pregnant population. Obesity and excessive weight gain during pregnancy have been shown to also impact perinatal outcome. Since GDM is frequently associated with elevated body mass index (BMI), we evaluated the impact of maternal prepregnancy BMI, development of GDM and gestational weight gain (GWG) during pregnancy on perinatal outcome. We compared 614 GDM patients with 5175 non-diabetic term deliveries who gave birth between 2012 and 2016. Multivariate regression analysis was used to evaluate the independent contribution of each factor on selected perinatal outcome variables. Additionally, subgroup analysis for obese (BMI ≥ 30 kg/m2) and non-obese women (BMI < 30 kg/m2) was performed. LGA was significantly influenced by BMI, GWG and GDM, while Neonatal Intensive Care Unit (NICU) admission was solely impacted by GDM. Maternal outcomes were not dependent on GDM but on GWG and prepregnancy BMI. These results remained significant in the non-obese subgroup only. Thus, GDM still affects perinatal outcomes and requires further improvement in diabetic care and patient counseling.

IntroductionBreathlessness is common in the population, especially in women and associated with adverse health outcomes. Obesity (body mass index (BMI) >30 kg/m2) is rapidly increasing globally and its impact on breathlessness is unclear.MethodsThis population-based study aimed primarily to evaluate the association of current BMI and self-reported change in BMI since age 20 with breathlessness (modified Research Council score ≥1) in the middle-aged population. Secondary aims were to evaluate factors that contribute to breathlessness in obesity, including the interaction with spirometric lung volume and sex.ResultsWe included 13 437 individuals; mean age 57.5 years; 52.5% women; mean BMI 26.8 (SD 4.3); mean BMI increase since age 20 was 5.0 kg/m2; and 1283 (9.6%) reported breathlessness. Obesity was strongly associated with increased breathlessness, OR 3.54 (95% CI, 3.03 to 4.13) independent of age, sex, smoking, airflow obstruction, exercise level and the presence of comorbidities. The association between BMI and breathlessness was modified by lung volume; the increase in breathlessness prevalence with higher BMI was steeper for individuals with lower forced vital capacity (FVC). The higher breathlessness prevalence in obese women than men (27.4% vs 12.5%; p<0.001) was related to their lower FVC. Irrespective of current BMI and confounders, individuals who had increased in BMI since age 20 had more breathlessness.ConclusionBreathlessness is independently associated with obesity and with weight gain in adult life, and the association is stronger for individuals with lower lung volumes.

Abstract Background Identifying the risk factors for preeclampsia (PE) is essential for the implementation of preventive actions. In the present study, we aimed at exploring the association between total gestational weight gain (GWG) and PE. Methods We performed a population-based cohort survey of 98,820 women with singleton pregnancies who delivered in Slovenia from 2013 to 2017. Aggregated data were obtained from the National Perinatal Information System (NPIS). The main outcome measure was the incidence of PE. The main exposure variable was total GWG standardized for the gestational duration by calculating the z-scores. The associations between total GWG and PE stratified by pre-pregnancy body mass index (BMI) categories adjusted for a variety of covariates were determined using multivariable logistic regression. We calculated the crude odds ratio (OR) and adjusted odds ratio (aOR) with a 95% confidence interval using a two-way test. Results Excessive GWG was associated with increased odds of PE in all pre-pregnancy BMI categories. The increase in the odds of PE by 445% was the highest in underweight women and by 122% was the lowest in obese women. Low GWG was associated with decreased odds of PE in all pre-pregnancy BMI categories except in normal-weight women with a GWG below −2 standard deviation (SD) and underweight women. The decrease in the odds of PE by 67% was the highest in obese women and by 41% was the lowest in normal-weight women. Conclusion Excessive GWG is a significant risk factor for PE, especially in underweight women, while low GWG is an important protective factor against PE, especially in obese women.

Background: The aim of our study was to assess the influence of prepregnancy Body Mass Index (BMI) changes as well as excessive gestational weight gain (GWG) on maternal and fetal perinatal parameters. Furthermore, we aimed to analyze the influence of increased prepregnancy BMI values and excessive GWG on neonatal early motoric development. Methods: The 203 eligible female participants were evaluated. Prepregnancy Body Mass Index (BMI) and excessive gestational weight gain (GWG) defined according to Institute of Medicine 2009 guidelines in the USA were assessed with tested maternal and fetal perinatal parameters and infants early motoric development (Alberta Infant Motor Scale—AIMS). Results: Significant predictors of increased prepregnancy BMI in perinatal period include: weight at delivery (p = 0.001), GWG (p = 0.002) and BMI at delivery (p < 0.001), while significant predictors of excessive GWG in perinatal period are: prepregnancy BMI (p = 0.029) and BMI at delivery (p < 0.001). In the group of participants with both increased prepregnancy BMI and excessive GWG versus others, significant predictors were: hypertension (HTA) (p = 0.019), amniotic fluid index (AFI) (p = 0.047), Pronation (AIMS) (p = 0.028) and Supination (AIMS) (p = 0.029). Conclusion: Increased prepregnancy BMI and excessive GWG are significantly associated with numerous perinatal factors that could alter the pregnancy course, pregnancy outcome and early motoric development of newborn. Moreover, increased prepregnancy BMI is shown to be a significant predictor of excessive GWG; thus, early selection of pregnant women for close monitoring of weight gain during pregnancy will have positive effects on reducing the risk of less favorable pregnancy course and early motoric development of newborn.

Objective To investigate the obesogenic influence of maternal prepregnancy body mass index (BMI) and gestational weight gain (GWG) on infant weight at birth and 12 months postpartum in an Australian general population sample. Methods Data on 1,305 pregnant women were collected on prepregnancy BMI and GWG through maternal interview, on infant weight at birth through hospital records, and on infant weight 12 months postbirth through direct measurement. Relationships between prepregnancy, gestational weight exposures, and infant weight outcomes were assessed with and without adjustment for potential confounding. Results We observed a 14 to 24 g increase in infant birth weight for every 1 kg increase in maternal weight (infant birth weight: β(BMI) = 0.014, p < 0.000; β(GWG) = 0.012, p < 0.000; and 12 months: β(BMI) = 0.018, p < 0.000; β(GWG) = 0.024, p < 0.000). Effects remained after adjustment for potential confounders (infant birth weight: β(BMI) = 0.014, p < 0.000; β(GWG) = 0.012, p < 0.001; and 12 months: β(BMI)= 0.017, p ≤ 0.033; β(GWG) = 0.023, p = 0.001). However, the effects observed were small, and there was no evidence that GWG mediated relationships between preconception BMI and infant weight. Conclusion In a general population sample, there is a significant but not substantial observed relationship between maternal prepregnancy BMI and GWG and infant weight outcomes, suggesting a minor role for these factors at a population level.

Objectives: The study was undertaken to explore the effects of prepregnancy body mass index (BMI) and excess weight gain on maternal and neonatal outcomes different maternal and neonatal outcomes. Methods: Obstetrics records of 496 singleton pregnant women delivered between 2007 and 2009 in IBN SINA Medical College Hospital were reviewed. On the basis of BMI on their first visit the patients were divided into 3 groups; Mat BMI Gr 1, normal (BMI 20–24.9 Kg/m<sup>2</sup>, n=366), Mat BMI Gr 2, overweight (BMI 25-29.9 Kg/m<sup>2</sup>, n=102), Mat BMI Gr 3, obese (BMI >30 Kg/m<sup>2</sup>, n=28). On the basis of gestational weight gain, the subject divided into 2 categories, Gets WtGain Gr A, gestational weight gain 8–15.9 Kg (n=315), Gest WtGain Gr B, gestational weight gain >16 Kg weight gain (n=181). Data were expressed as number (percentage). Proportion test was performed for comparison between two groups. P value <0.05 was taken as level of significance. Results: of the total 496 pregnant women 74.59% were between 19-34 years of age. Among all the women 64.11% had high school education of different grade. Of all the pregnancies 23.18% were nulliparous. Of the total 496 women 366 (79.79%) were normal weight, 102 (20.56%) overweight and 28 (5.64%) obese. Obese women group had significantly higher proportion of hypertensive cases compared to the normal weight (p<0.001) and overweight (p<0.01) group. Relatively higher proportion of macrosomia, birth trauma, shoulder dystocia and NICU admission among babies of obese women (p=ns). One hundred and eighty one (36.49%) of study subjects had gestational weight gain above the cut-off (>16 kg) value (p<0.001). Women with weight gain bout the cut-off level had relatively higher proportion of macrosomic babies (p=ns). Conclusions: The data reconfirmed that obesity is associated with hypertension. Significant proportion of women had weight gain more than cut-off value which needs to be addressed to ensure sound maternal and fetal wellbeing. However, a multicentre large scale study is warranted which may help the researchers to conclusively comment on the issue and thus plan future strategies for health care during pregnancy. Purpose: To investigate weight gain during pregnancy and its association with birth weight and weight retention postpartum (PP) among women of different ethnic backgrounds. Methods: Cross-sectional study of 5,863 women participating in the Hawai`i Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) in 1997 and 1998. Postpartum weight and height were measured by WIC staff, weight gain during pregnancy and infant birth weight were self-reported. Ethnicity was also selfreported. The Institute of Medicine guidelines for pregnancy weight gain in relation to levels of prepregnancy body mass index (BMI) were followed. The analysis included bivariate tests of association between prepregnancy BMI, pregnancy weight gain, PP weight retention, infant birth weight and ethnicity. Multivariable analysis was conducted to estimate the independent association of the study variables with PP weight retention, birth weight, and ethnicity. SAS was used to analyze the data. Results: The largest ethnic groups were Hawaiian (29.9%), Caucasian (27.2%), and Filipino (19.8%). Asians, Blacks, Samoans, and Other represented smaller percentages. Thirty percent of women were overweight or obese before pregnancy. There were significant ethnic differences in prepregnancy BMI. Samoans had the highest mean prepregnancy BMI (29.7). Filipinos had the lowest mean prepregnancy BMI (22.5). There were significant differences in pregnancy weight gain. Samoans had the highest mean weight gain (37.3 lbs). Asians had the lowest mean weight gain (29.1bs). Almost one half of women (48.1%) gained more than the IOM recommended pregnancy weight. The average weight retention was 9.4 lbs at five to six months postpartum, and 7.4 lbs after eight months postpartum. There were significant ethnic differences in weight retention with Samoans having the largest average weight retention (17.5 lbs) and Asians the lowest (9.6 lbs). Infant birth weight differed significantly by ethnicity. Birth weight was significantly associated with prepregnancy BMI and pregnancy weight gain. Conclusions: In spite of large racial/ethnic differences, on the average there was almost a two-thirds pound adjusted PP weight retention for each pound of weight gained during pregnancy. Our study suggests a need for integrated women’s health services to include nutritional support during the pre and post conceptual period. Findings of this study will provide guidance in the development of culturally sensitive interventions and counseling.
