The association of body mass index, weight gain and central obesity with activity-related breathlessness: the Swedish Cardiopulmonary Bioimage Study

IntroductionBreathlessness is common in the population, especially in women and associated with adverse health outcomes. Obesity (body mass index (BMI) >30 kg/m2) is rapidly increasing globally and its impact on breathlessness is unclear.MethodsThis population-based study aimed primarily to evaluate the association of current BMI and self-reported change in BMI since age 20 with breathlessness (modified Research Council score ≥1) in the middle-aged population. Secondary aims were to evaluate factors that contribute to breathlessness in obesity, including the interaction with spirometric lung volume and sex.ResultsWe included 13 437 individuals; mean age 57.5 years; 52.5% women; mean BMI 26.8 (SD 4.3); mean BMI increase since age 20 was 5.0 kg/m2; and 1283 (9.6%) reported breathlessness. Obesity was strongly associated with increased breathlessness, OR 3.54 (95% CI, 3.03 to 4.13) independent of age, sex, smoking, airflow obstruction, exercise level and the presence of comorbidities. The association between BMI and breathlessness was modified by lung volume; the increase in breathlessness prevalence with higher BMI was steeper for individuals with lower forced vital capacity (FVC). The higher breathlessness prevalence in obese women than men (27.4% vs 12.5%; p<0.001) was related to their lower FVC. Irrespective of current BMI and confounders, individuals who had increased in BMI since age 20 had more breathlessness.ConclusionBreathlessness is independently associated with obesity and with weight gain in adult life, and the association is stronger for individuals with lower lung volumes.

Abstract Background Identifying the risk factors for preeclampsia (PE) is essential for the implementation of preventive actions. In the present study, we aimed at exploring the association between total gestational weight gain (GWG) and PE. Methods We performed a population-based cohort survey of 98,820 women with singleton pregnancies who delivered in Slovenia from 2013 to 2017. Aggregated data were obtained from the National Perinatal Information System (NPIS). The main outcome measure was the incidence of PE. The main exposure variable was total GWG standardized for the gestational duration by calculating the z-scores. The associations between total GWG and PE stratified by pre-pregnancy body mass index (BMI) categories adjusted for a variety of covariates were determined using multivariable logistic regression. We calculated the crude odds ratio (OR) and adjusted odds ratio (aOR) with a 95% confidence interval using a two-way test. Results Excessive GWG was associated with increased odds of PE in all pre-pregnancy BMI categories. The increase in the odds of PE by 445% was the highest in underweight women and by 122% was the lowest in obese women. Low GWG was associated with decreased odds of PE in all pre-pregnancy BMI categories except in normal-weight women with a GWG below −2 standard deviation (SD) and underweight women. The decrease in the odds of PE by 67% was the highest in obese women and by 41% was the lowest in normal-weight women. Conclusion Excessive GWG is a significant risk factor for PE, especially in underweight women, while low GWG is an important protective factor against PE, especially in obese women.

Objectives: The study was undertaken to explore the effects of prepregnancy body mass index (BMI) and excess weight gain on maternal and neonatal outcomes different maternal and neonatal outcomes. Methods: Obstetrics records of 496 singleton pregnant women delivered between 2007 and 2009 in IBN SINA Medical College Hospital were reviewed. On the basis of BMI on their first visit the patients were divided into 3 groups; Mat BMI Gr 1, normal (BMI 20–24.9 Kg/m<sup>2</sup>, n=366), Mat BMI Gr 2, overweight (BMI 25-29.9 Kg/m<sup>2</sup>, n=102), Mat BMI Gr 3, obese (BMI >30 Kg/m<sup>2</sup>, n=28). On the basis of gestational weight gain, the subject divided into 2 categories, Gets WtGain Gr A, gestational weight gain 8–15.9 Kg (n=315), Gest WtGain Gr B, gestational weight gain >16 Kg weight gain (n=181). Data were expressed as number (percentage). Proportion test was performed for comparison between two groups. P value <0.05 was taken as level of significance. Results: of the total 496 pregnant women 74.59% were between 19-34 years of age. Among all the women 64.11% had high school education of different grade. Of all the pregnancies 23.18% were nulliparous. Of the total 496 women 366 (79.79%) were normal weight, 102 (20.56%) overweight and 28 (5.64%) obese. Obese women group had significantly higher proportion of hypertensive cases compared to the normal weight (p<0.001) and overweight (p<0.01) group. Relatively higher proportion of macrosomia, birth trauma, shoulder dystocia and NICU admission among babies of obese women (p=ns). One hundred and eighty one (36.49%) of study subjects had gestational weight gain above the cut-off (>16 kg) value (p<0.001). Women with weight gain bout the cut-off level had relatively higher proportion of macrosomic babies (p=ns). Conclusions: The data reconfirmed that obesity is associated with hypertension. Significant proportion of women had weight gain more than cut-off value which needs to be addressed to ensure sound maternal and fetal wellbeing. However, a multicentre large scale study is warranted which may help the researchers to conclusively comment on the issue and thus plan future strategies for health care during pregnancy. Introduction: There is a well-established inverse relationship between body mass index and frequency of endometriosis. However, these population-based studies have relied mostly on self-reported cases of endometriosis, rather than surgically confirmed endometriosis where disease severity has been objectively assessed. The aim of the current retrospective study was to establish whether the established relationship between endometriosis and low body mass index was independent of disease severity. Methods: Women with menstrual and/or pelvic pain undergoing laparoscopy for suspected endometriosis were recruited for this retrospective study (n = 509). Women were grouped by body mass index (kg/m2) according to World Health Organization criteria: underweight (<18.5), normal (18.5–24.99), pre-obese (25–29.99) or obese (≥30). Endometriosis was scored according to the revised American Fertility Society system. Data were analysed based on body mass index and endometriosis status to identify any relationship between body mass index and disease. Results: The average body mass index of women with endometriosis was 25.0 kg/m2. The body mass index distribution of women with endometriosis differed relative to women in the general population. As expected, fewer obese women had endometriosis than in the lower body mass index categories. However, the obese women who did have endometriosis had significantly higher revised American Fertility Society scores compared to women with normal and pre-obese body mass indices. Discussion: Our results are consistent with the established finding of an inverse relationship between body mass index and endometriosis. The novel finding from this study is that obesity is associated with increased disease severity and reduced frequency of stage I endometriosis. It remains unclear what role body mass index has in the cause or effect of endometriosis; we speculate that body mass index may be useful for sub-classifying the disease.

Gestational weight gain (GWG) may interfere in perinatal outcomes and also cause future problems throughout woman’s life. The aim of this population-based study is to evaluate the GWG in Campinas city, southeast of Brazil. A total of 1052 women, who delivered in the three major maternity hospitals in Campinas, were interviewed during postpartum period. The general average of GWG was 13.08±6.08. Of total women, 13.6% were obese and 24.6% were overweight and, in these groups, 55.9% and 53.7%, respectively, exceeded GWG according to the Institute of Medicine recommendations. 6.2% of total women had low body mass index (BMI) and 35.5% in this group had insufficient GWG. Overweight and obese women had a higher risk of excessive GWG and delivery by c-section. The c-section rate was 58.9% and increased according to GWG. Prematurity was more prevalent first in obese and then in low BMI women. Considering the high BMI in women in reproductive age, it is necessary to take effective guidelines about lifestyle and nutritional orientation in order to help women reach adequate GWG. All of them could improve prenatal outcomes and women’s heath as a whole.

The purpose of this prospective, population-based cohort study was to evaluate the roles of polycystic ovary syndrome (PCOS), obesity, weight gain, and hyperandrogenemia in the development of hypertensive disorders of pregnancy (HDP) through fertile age both in PCOS and in non-PCOS women. The study population—NFBC1966 (Northern Finland Birth Cohort 1966)—allowed a long-term follow-up of women from age 14 until 46 years who developed HDP (n=408) or did not (n=3373). HDP diagnosis was confirmed by combining hospital discharge records, data from Finnish Medical Birth Registers, and the questionnaire data at age 46. Women with self-reported PCOS (srPCOS; n=279), defined by both oligo-amenorrhea and hirsutism at age 31 or with PCOS diagnosis by age 46, were compared with women without reported PCOS (n=1577). Women with srPCOS had an increased HDP risk (odds ratio, 1.56 [95% CI, 1.03–2.37]), but the association disappeared after adjustment for body mass index. In women with srPCOS and HDP, body mass index increased from age 14 to 46 significantly more than in srPCOS women without HDP (median [interquartile range], 9.82 [6.23–14.6] and 7.21 [4.16–10.5] kg/m 2 , respectively; P <0.001). Also, in non-PCOS women, the increase was higher in women with (7.54 [5.32–11.62] kg/m 2 ; P <0.001) than without HDP (6.33 [3.90–9.33] kg/m 2 ; P <0.001). Increase in waist circumference between ages 31 and 46 years was associated with HDP but not with PCOS. Hyperandrogenemia at 31 or 46 years did not associate with HDP (1.44 [0.98–2.11]). In conclusion, obesity, especially abdominal obesity, and weight gain from adolescence to age 46, but not srPCOS or hyperandrogenemia, were associated with an increased risk of HDP.

Aim: To evaluate the effects of gestational weight gain on perinatal outcomes in woman with excessive pre-gravid body mass index. Material and Methods: The study group consisted of 197 overweight or obese women who delivered full-term newborns at the 1st Department of Obstetrics and Gynecology, Medical University of Warsaw. Group A included 89 (45.2%) women with inadequate or adequate weight gain. Group B included 108 patients (54.8%) with excessive weight gain during pregnancy. Results: No significant differences in the occurrence of gestational diabetes, hypertension or intrahepatic cholestasis between the analyzed groups were observed. Excessive weight gain was positively correlated with the incidence of cesarean delivery (67.6% vs 32.4%, p=0.009), newborns’ birthweight (3627g±519 vs 3406g±521, p=0.009) and the risk of low for gestational age infants (26.9% vs 12.4%, p=0.01). The highest risk of low for gestational age new born was related to gestational weight gain of about 15 kg. Appropriate preconception counselling for overweight and obese women may reduce the risk of low for gestational age infants.

Background: Yoga may provide a strategy for healthy weight management in young adults. This study examined prevalence and characteristics of young adults’ yoga practice and associations with changes in body mass index. Methods: Surveys were completed by 1830 young adults (31.1 ± 1.6 y) participating in Project EAT-IV. Cross-sectional and 5-year longitudinal analyses were conducted stratified by initial weight status. Results: Two-thirds (66.5%) of nonoverweight women and 48.9% of overweight women reported ever doing yoga, while 27.2% of nonoverweight women and 16.4% of overweight women practiced regularly (≥30 min/wk). Fewer men practiced yoga. Among young adults practicing regularly (n = 294), differences were identified in intensity, type, and location of yoga practice across weight status. Young adults who were overweight and practiced yoga regularly showed a nonsignificant 5-year decrease in their body mass index (−0.60 kg/m2; P = .49), whereas those not practicing regularly had significant increases in their body mass index (+1.37 kg/m2; P < .01). Frequency of yoga was inversely associated with weight gain among both overweight and nonoverweight young adults practicing yoga regularly. Conclusions: Young adults of different body sizes practice yoga. Yoga was associated with less weight gain over time, particularly in overweight young adults. Practicing yoga on a regular basis may help with weight gain prevention.
