A narrow view: The conceptualization of sexual problems in human sexuality textbooks

This study examined the ways in which the meaning of ‘sexual problems’ is constructed and defined in undergraduate human sexuality textbooks. Drawing on feminist and critical discourse frameworks, the dominant as well as the absent/marginalized discourses were identified using critical discourse analysis. Sexual difficulties were largely framed by the American Psychiatric Association’s Diagnostic and Statistical Manual of Mental Disorders. Thus, medical discourse was privileged. Alternative conceptualizations and frameworks, such as the New View of Women’s Sexual Problems, were included marginally and peripherally. We argue that current constructions of sexuality knowledge reinforce, rather than challenge, existing hegemonic discourses of sexuality.

This paper addresses the challenges of downtoning the speaker’s probability in discourse and revealing its means of expression. A study of objective statements in various discourse registers using the linguistic category of modality has potency of investigating the correlation of hedging and the type of modality which is considered to be an aspect of interpersonal metafunction in language competence (Aumuller, 2014). The term "modality" shares a range of concepts within the fields ofphilosophy, morphology, syntax, semantics, and discourse-analysis. Studies on modality as an interpersonal metafunction in Systemic Functional Linguistics have in the literature, mainly focused on variants of genres and/or discourse namely; conservative, legal, media, literary, academic, political and medical discourse as well as the contemporary English usage (Jespersen, 1924). The present paper continues the author’s] series on hedging in discourse (Mykhaylenko, 2017). The discourse strategy of hedging/downtoning seems to play a paramount role in discourse: the speaker gives the hearer a possibility to objectively interpret his/her intentional meaning, on the other hand, the hearer expects a definite deontic constituent on the part of the speaker. Based on quantitative and qualitative methods, the article argues that the use of modal expressions can be better explained as reflecting the strategies of hedging used by writers for dealing with the social conditions. Within critical discourse analysis, modality is understood as encompassing much more than simply the occurrence of overt modal auxiliaries such as may, might, can, could, will, would, shall, should, must, and ought (Fowler, 1985). Rather, modality concerns the speaker's attitude toward and/or confidence in the proposition being presented.

Zusammenfassung. Hintergrund: Nach 12 Jahren Entwicklung wird die 11. Version der International Classification of Diseases (ICD-11) von der Weltgesundheitsorganisation (WHO) im Januar 2022 in Kraft treten. Methodik: Im Rahmen eines selektiven Übersichtsartikels werden die Veränderungen im Hinblick auf die Klassifikation von Angststörungen von der ICD-10 zur ICD-11 zusammenfassend dargestellt. Ergebnis: Die diagnostischen Kriterien der generalisierten Angststörung, Agoraphobie und spezifischen Phobien werden angepasst. Die ICD-11 wird auf Basis einer Lebenszeitachse neu organisiert, sodass die kindesaltersspezifischen Kategorien der ICD-10 aufgelöst werden. Die Trennungsangststörung und der selektive Mutismus werden damit den „regulären“ Angststörungen zugeordnet und können zukünftig auch im Erwachsenenalter diagnostiziert werden. Neu ist ebenso, dass verschiedene Symptomdimensionen der Angst ohne kategoriale Diagnose verschlüsselt werden können. Diskussion: Die Veränderungen im Bereich der Angsterkrankungen umfassen verschiedene Aspekte und sind in der Gesamtschau nicht unerheblich. Positiv zu bewerten ist die Einführung einer Lebenszeitachse und Parallelisierung mit dem Diagnostic and Statistical Manual of Mental Disorders (DSM-5). Schlussfolgerungen: Die entwicklungsbezogene Neuorganisation in der ICD-11 wird auch eine verstärkte längsschnittliche Betrachtung von Angststörungen in der Klinik sowie Forschung zur Folge haben. Damit rückt insbesondere die Präventionsforschung weiter in den Fokus.

Introducción.- Los síntomas somáticos causan malestar y afectan la calidad de  vida de los pacientes, incrementando la frecuencia del uso de los servicios de salud. En Ecuador no existe un instrumento validado que evalúe somatización. La escala Somatic Symtom Scale-8 (SSS-8), es un instrumento usado como medida de referencia en the Diagnostic and Statistical Manual of Mental Disorders(DSM-V), para evaluar la carga de síntomas somáticos, validada en inglés y adaptada culturalmente a los idiomas alemán y japonés. Objetivos.- En esta investigación el propósito fue validar la escala “Somatic Symptom Scale-8” y determinar sus propiedades métricas. Métodos.- Investigación descriptiva y transversal en la que se realizó la validación de la herramienta clínica SSS-8 en 401 pacientes en la consulta externa del hospital Pedro Vicente Maldonado, desde mayo a julio de 2017. Para el análisis de datos se utilizaron los programas estadísticos SPSS versión 23, Latent gold y EpiDat 3.1. Resultados.- El grupo mayoritario fue de 30 a 47 años, con ligero predominio del sexo masculino (con 52,6%) sobre el femenino (47,4%). El SSS-8 mostró adecuadas propiedades métricas (alfa de Cronbach de 0,73). Mediante este análisis se obtuvo que, los pacientes que respondieron: algo, bastante o muchísimo en las dimensiones sentirse cansado, dolor de cabeza, dolor de brazos y dolor de espalda; tenían un 99% de probabilidades de presentar somatización. Conclusiones.- En este estudio, el SSS-8 demostró ser una herramienta útil para evaluar los síntomas somáticos en pacientes que acuden a consulta externa, ya que presentó buenas propiedades métricas: consistencia interna elevada, buena validez y una apropiada capacidad discriminativa.

Objek penelitian ini adalah The Daily Mail, salahsatu tabloid harian terbesar di Inggris. Metode yang digunakan dalam penelitian ini adalahmetode kualitatif, analisis deskriptif. Penulis menggunakan teori analisis wacana kritisFairclough (1995), disertai dengan beberapa teori pendukung lainnya, seperti teori klausasebagai representasi Halliday (2004), teori konspirasi Feaster (2008), Birchall (2006),Barkun (2003), Hodapp dan von Kannun (2008). Hasil dari penelitian ini menunjukkanbahwa representasi kematian Putri Diana dalam dimensi tekstual didapat melalui prosesmaterial, proses mental, proses relasional, proses verbal, dan proses eksistensial yangterdapat dalam artikel-artikel The Daily Mail. This thesis is aimed to describe therepresentation of Princess Diana’s death and The Daily Mail’s perspective as a mediaconspiracy actor through textual and sociocultural dimension. The object of this thesis isThe Daily Mail, one of the widest national daily newspapers in England. The method that isused in this thesis is a qualitative method, a descriptive analytic method. The writer uses thecritical discourse analysis theory of Fairclough (1995) and some other supported theories,such as clause as representation theory by Halliday (2004), the conspiracy theory by Feaster(2008), Birchall (2006), Barkun (2003), Hodapp and von Kannun (2008). The result of thisresearch shows that the representation in textual dimension that appear on Princess Diana’sdeath is derived from material process, mental process, relational process, verbal process, andexistential process. While The Daily Mail’s perspective on Princess Diana’s death is derivedfrom situational level, institutional level and social level of sociocultural practice dimension.Keywords: Transitivity, Critical Discourse Analysis, Media Conspiracy.

Abstrak: Artikel ini bertujuan menganalisis secara kritis visi dan misi 10 perguruan tinggi terbaik di Indonesia versi 4 International Colleges & Universities. Metode yang digunakan dalam penelitian ini yaitu deskriptif kualitatif. Penelitian ini merupakan analisis wacana kritis yang mengacu pada teori Michel Foucault. Teori analisis wacana Foucault mengacu pada wacana sebagai alat bagi kepentingan kekuasaan, hegemoni, dominasi budaya, dan ilmu pengetahuan. Berdasarkan hasil analisis ditemukan bahwa visi dan misi 10 perguruan tinggi tersebut mengandung unsur-unsur yang sejalan dengan fungsi wacana menurut Foucault. Hal tersebut didasari pada visi dan misi setiap perguruan tinggi yang memiliki tujuan mengembangkan institusi berkelas internasional, namun tetap dengan berdasar pada pengembangan ilmu pengetahuan yang berakar pada budaya lokal. Kata kunci: wacana, visi, misi, perguruan tinggi Abstract: This article aims to critically analyze the vision and mission of the 10 best universities in Indonesia version of 4 International Colleges & Universities. The method used in this study is descriptive qualitative. This research is a critical discourse analysis that refers to Michel Foucault's theory. Foucault's theory of discourse analysis refers to discourse as a tool for the interests of power, hegemony, cultural domination, and science. Based on the results of the analysis it was found that the vision and mission of the 10 universities contained elements that were in line with the discourse function according to Foucault. This is based on the vision and mission of each college that has the aim of developing international-class institutions, but still based on the development of science rooted in local culture. Keywords: discourse, vision, mission, college
