Narrative reconstruction and post-traumatic growth among trauma survivors: The importance of narrative in social work research and practice

A substantial body of theoretical work on meaning-making processes postulates that assisting clients in reconstructing their personal narratives in the aftermath of trauma helps survivors to integrate the traumatic experience into their identities and life stories. However, the relationship between trauma survivors’ (re-)construction of a coherent life narrative and their development of post-traumatic growth (PTG) has rarely been explored. In this study, I conducted life story interviews with 46 university students with trauma histories to examine: (1) How, and to what degree, trauma survivors (re-)construct a coherent life narrative; and (2) If and how this process is connected to the development of PTG. I found that survivors who were able to articulate a coherent story about their lives experienced more PTG, and I identified key characteristics of three stages of post-trauma change. I also found that trauma-related therapy, writing, informal conversations, and self-reflection played important roles in the narrative reconstruction process. I argue that some narratives are easier to reconstruct than others because not all narratives are equally valued in society. The presence or absence of narratives in the discursive environment, the reception these stories receive within society, and the access that individuals have to these narratives are influenced by the historical moment, social norms, politics, power, privilege, and individuals’ social locations. To promote empowerment and social justice, social workers should help trauma survivors to reconstruct their life stories, create spaces for the less-welcomed narratives, and engage in mezzo- and macro-level efforts to address social problems and inequalities.

IntroductionEach individual experience cancer in a different way. While some perceive cancer as a complex and traumatic experience by developing some psychosocial and additional physical problems, others overcome cancer-related difficulties by gaining benefits such as post-traumatic growth (PTG) owing to their resilience. Resilience and PTG that are very valuable concepts in human life to adapt positively to cancer process have relations which need to be better understood.ObjectivesWe aimed to provide a better understanding of relations between resilience and PTG and relations of these two concepts with cancer experience.MethodsLiterature review.ResultsSuccessful adjustment to life-threatening illnesses such as cancer, require resilience. On the other hand, resilience provides a barrier toward stressors by helping improvement of PTG and so, is an antecedent factor of PTG. PTG ensures a deeper perspective and strength to people after traumatic events. Hence, individuals having higher levels of PTG feel powerful enough to handle the problems in their life and can easily adapt to cancer process by focusing on the positive outcomes of trauma, having improved coping mechanisms and an improved psychological well-being.ConclusionsResilience and PTG have strong mutual relations and this phenomenon should be considered for a qualified cancer care.Disclosure of interestThe authors have not supplied their declaration of competing interest.

The concept that a traumatic experience, such as a cancer, can lead to a positive change and transformation of self, life and relationships was named as post-traumatic growth (PTG). A large amount of research measured PTG in cancer survivors arguing an interpretation of the construct as an outcome. Recently, qualitative research shows different types of narrative of PTG, but the narrative markers and their functions of transformation remain still unclear. Within a mixed-method, we aim to highlight the narrative markers and their transformative functions, underlying the PTG, within 12 cancer survivors’ narratives with medium/high and medium/low level of PTG. A redemptive sequence analysis was carried out. In the narratives with high/medium PTG we find a specific transformative function on-thinking focused transformation founded on the change/expansion of the own internal criteria to interpret the relationship with the world centralizing the self in the present and future; in the narratives with medium/low PTG we find an on-acting focused transformation, founded on the change of the operational procedures aimed to live centered on the present and on its moments.

AbstractExpressive writing can enhance cognitive processing and improve stress regulation. Particularly, the use of cognitive words (i.e., insightful and causal words) in writing may be associated with the process of meaning making and promotion of post-traumatic growth (PTG). The aim of the present study was to determine how expressive writing and the use of causal and insightful cognitive words influenced meaning making and PTG during writing. In total, 52 traumatized university students were recruited and randomly assigned to one of two writing conditions involving either an expressive writing task or a neutral writing task. The results showed that participants who engaged in expressive (vs. neutral) writing showed higher scores on the presence of meaning and PTG in the post-writing, self-report questionnaires. Moreover, writing task (expressive or neutral) and frequency of causal and insightful cognitive words were both significant predictors of meaning, which in turn led to high levels of PTG. In conclusion, the use of causal and insightful words might be a fundamental cognitive process for developing meaning in writing, which is essential for our further understanding of meaning making and PTG.

Self-compassion and post traumatic growth have previously been associated with positive mental health and functioning. They have recently garnered increased research interest in psychotherapeutic milieu as they have also been found to promote adaptive responses to trauma. On the other hand, psychological flexibility represents a variable known to have an impact on many human abilities including the capacity to shift mindsets and behavioral responses and is increasingly being understood as a crucial trait to develop for therapeutic change. The present study sought to examine whether self-compassion would lead to post-traumatic growth as well as the mediating role of psychological flexibility in this relationship. Data was obtained from 208 participants (females=143, males=65), age range=18 to 50 years (M =27 years, S.D = 6.89) who reported being exposed to at least one traumatic experience in the last 5 years. The results found positive correlations between all the three variables used in the study. It was found that self-compassion does lead to post-traumatic growth (c’= 4.9303, CI = 0.4947 to 9.3659, p = 0.0295). Psychological flexibility proved as the mediator between self-compassion and post-traumatic growth, with indirect effect IE = 5.9091 at 95% CI = (3.2340, 8.9695). The findings add to the host of literature on positive functions served by self-compassion, specifically in its contribution towards post traumatic growth. Further, this study explicates the mediating mechanism through which self-compassion exerts its potential effects by pointing out to the role of psychological flexibility.

In Euro-American societies, “resilience” and “post-traumatic growth” are commonly used metaphorical terms for positive responses to extreme adversity. However, research on illness narratives has demonstrated that other cultures may have different metaphorical concepts that act as vehicles for shared beliefs about how to overcome extreme adversity or traumatic experiences. The purpose of this article is to identify metaphors used in various cultures to describe positive responses to extreme adversity and to discuss the cultural ontologies and other socio-cultural factors that shape them. Metaphors of this nature were extracted from psychological, anthropological and ethnographic studies and were organized into categories: battle, path, container, balance, weight, object and network metaphors. Findings support the notion that metaphorical expressions related to positive responses to extreme adversity vary widely across cultures. We argue that an understanding of cultural differences in metaphors for mental health-related concepts is crucial to assisting trauma survivors from different cultural backgrounds.

Present study investigated the relationship between bereavement and post-traumatic growth and mediating role of cognitive processes. Sample of 260 bereaved parents and spouses was drawn from the province of Baluchistan, Pakistan. Urdu versions of Core Bereavement Items scale (Burnett, Middleton, Raphael, & Martinek, 1997), Integration of Stressful Life Events Scale (Holland, Currier, Coleman, & Neimeyer, 2010), and Post-traumatic Growth Inventory-SF (Cann et al., 2010) were completed by the bereaved parents and spouses individually. Pearson correlation was checked for direction and strength of relationship between the study variables. Results indicated high alpha reliability and validity for the Urdu version scales. The findings showed an inverse relationship between bereavement and post-traumatic growth. Results also revealed that cognitive processes significantly mediate between bereavement and post-traumatic growth relationship. Results showed significant group differences in bereavement experience intensity and post-traumatic experience based on gender of the bereaved and the gender of the deceased. Limitations of the study and suggestions for future researches are also presented in the discussion.
