Defining the Process of Medical Care to Include Dual Situation Awareness between Patient and Clinician

Directing design of information & communication system in healthcare according to the workflow in the doctor patient relationship might follow established concepts from the field of Human Factors. The Endsley model of Situation Awareness is modified to accommodate both physician and patient, and recalls the Problem Oriented Medical Record of Lawrence Weed. Although the original purpose of computer health applications was documentation, the goal of shared Dual Situation Awareness and improved shared decisions is offered as an alternative to record-keeping. Also, design with these concepts may provide a pathway to enhanced patient engagement in their health care.

E-health has rapidly gained attention as a framework for understanding the relationship between using information and communication technologies (ICTs) to promote individual and community health, and using ICTs for improving the management of health care delivery systems. The use of e-collaborative tools is implicit to the delivery and access of e-health. Development of the capacity to transmit and receive digital diagnostic images, use video telecommunications for supporting the remote delivery of specialized care and surgical procedures, and the use of e-communication technologies to support logistical elements of medical care (such as scheduling appointments, filling prescriptions, and responding to patient questions) are just a few ways in which e-communications are transforming how medical care is embedded within institutional, organizational, family, and community settings. The emerging field of e-collaboration focuses attention on the need for society to critically examine how electronic communication technologies facilitate, shape, and transform the ways in which organizations, groups, and communities interact. There are many works that explain how to (a) develop e-health systems, (b) assess the use of such systems, and (c) analyze the health outcomes that can be achieved with effective e-health applications (Brodie et al., 2000; Eder, 2000; Spil & Schuring, 2006). Less attention has been paid to how advances in e-collaboration research might inform e-health applications development and scholarly discourse. Because of this gap in the literature, few discussions pertain to understanding patient perspectives about the advantages and disadvantages that may result from rapidly emerging interconnections among access to health care, health information, health support systems, and ICTs (Berland et al., 2001; Hesse et al., 2005; Gibbons, 2005; Gilbert & Masucci, 2006).

ABSTRAK Rekam medis adalah berkas yang berisi catatan dan dokumen tentang identitas pasien, pemeriksaan, pengobatan, tindakan dan pelayanan lain kepada pasien pada sarana pelayanan kesehatan. Tujuan penelitian ini adalah untuk mengetahui bagaimana pelaksanan nilai guna rekam medis bagi pasien. Jenis penelitian ini adalah deskriptif bertujuan menggambarkan secara sistematis fakta dan karakteristik objek dan subjek secara tepat. Waktu penelitian ini dilakukan pada bulan Juli di Rumah Sakit Umum Pekerja Indonesia Medan (RSU IPI) Tahun 2018. Populasi dalam penelitian adalah 440 berkas rekam medis. Dalam melakukan penelitian, peneliti mengambil sampel sebanyak 81 berkas rekam medis. Berdasarkan hasil penelitian yang dilakukan di RSU IPI pelaksanaan nilai guna rekam medis sudah terlaksana dengan baik, dilihat dari tersedianya ringkasan masuk dan keluar, resume, lembar operasi, identifikasi bayi, lembar persetujuan tindakan, lembar kematian pada setiapberkas pasien pulang meninggal, asuhan keperawatan didalam berkas rekam medis. Tetapi dalam pengisian berkas rekam medis petugas rekam medis belum mengimplementasikan nilai guna rekam medis dengan baik. Kesimpulannya pelaksanaan nilai guna rekam medis sudah baik namun dalam pengisian berkas rekam medis lebih di perhatikan sesuai Permenkes 269 Tahun 2008 Tentang rekam Medis sehingga pelaksaaan nilai guna rekam medis dan pengisisan berkas rekam medis berjalan lebih baik.   Kata Kunci: Rekam Medis, Nilai Guna Rekam Medis, Berkas Rekam Medis                                             ABSTRACT   Medical record is a document that contains records and documents about patient identity, examination, treatment, care and other services for patients in health care facilities. The purpose of this study was to study how the implementation of the use of medical records for patients. This type of research is descriptive which addresses the systematic problem and the appropriate characteristics of objects and subjects. When this study was conducted in July at the Medan Indonesian Workers General Hospital (RSU IPI) in 2018. The population in this study was 440 medical record documents. In conducting research, researchers took 81 samples of medical records. Based on the results of research conducted at the IPI General Hospital, the implementation of the use value of medical records has been carried out well, seen from the availability of incoming and outgoing assessments, proceeding, surgery sheets, accessing infants, action approval sheets, consent sheets on each patient's return documents, medical care care. However, in applying medical records, medical record officers have not applied the use value of medical records properly. Conclusion the reclamation of the value of the medical record has been better in the reclamation of the medical record is better with the approval in accordance with Minister of Health Regulation 269 of 2008 About the Medical Record requires the implementation of the value of the medical record and the filling of the medical record better.

The incidence of the fatigue caused by medical care brings to attention the emotional reactions to suffering and their possible effects on caregivers. In this study, we shall discuss empathy and compassion. Linguistic analyses and psychological evaluations fail to differentiate between empathy and compassion. We shall therefore make an inventory of the contribution of neuroscientific studies that we consider important. We shall present some research and clinical studies that support the discrimination between compassion and empathy, at the psycho-behavioral level, in terms of vagal and cerebral patterns and in terms of the effects that these emotional states have at the psycho-emotional level. Unlike the interventions aimed at empathic training, cultivating compassion among caregivers produces beneficial effects, decreasing fatigue and increasing resilience. We believe that the differences found between compassion and empathy support the replacement of the phrase “compassion fatigue”, widely used today, with “empathic distress”. We consider the prophylactic and therapeutic capitalization of compassion in health care, by developing training programs to cultivate compassion for specialized staff for patients, to avoid fatigue (empathic distress) and to improve the emotional, humanistic dimension of the doctor-patient relationship, both urgent and necessary.

SummaryObjectives: Despite significant awareness on the value of leveraging patient relationships across the healthcare continuum, there is no research on the potential of using Electronic Health Record (EHR) systems to store structured patient relationship data, or its impact on enabling better health-care. We sought to identify which EHR systems supported effective patient relationship data collection, and for systems that do, what types of relationship data is collected, how this data is used, and the perceived value of doing so.Materials and methods: We performed a literature search to identify EHR systems that supported patient relationship data collection. Based on our results, we defined attributes of an effective patient relationship model. The Open Medical Record System (OpenMRS), an open source medical record platform for underserved settings met our eligibility criteria for effective patient relationship collection. We performed a survey to understand how the OpenMRS patient relationship model was used, and how it brought value to implementers.Results: The OpenMRS patient relationship model has won widespread adoption across many implementations and is perceived to be valuable in enabling better health care delivery. Patient relationship information is widely used for community health programs and enabling chronic care. Additionally, many OpenMRS implementers were using this feature to collect custom relationship types for implementation specific needs.Conclusions: We believe that flexible patient relationship data collection is critical for better healthcare, and can inform community care and chronic care initiatives across the world. Additionally, patient relationship data could also be leveraged for many other initiatives such as patient centric care and in the field of precision medicine.

SummaryThis paper describes the necessity of adapting the major classifications of mental disorders exemplified by the ICD-11 and the DSM-5 for the special needs of primary medical care. An earlier version of the classification – the ICD-10-PHC – is described, and the process of adapting it is described in detail. The new 28 item version of the classification is described, and the procedures to be adopted in the Field Trials to be held during 2013 are set out, together with the specific problems these field trials will address.
