Hypochlorous Acid 0.01% vs Povidone-Iodine 5% for Ocular Antisepsis

Purpose: An alternative ocular antiseptic is needed for patients who do not tolerate povidone-iodine (PI). The purpose of this study is to compare the antimicrobial effect of hypochlorous acid (HA) 0.01% with PI 5% applied topically to the ocular surface. Methods: Swabs of the inferior conjunctiva and posterior lower eyelid margin of 40 patients were taken from both eyes and plated onto blood agar plates. One eye was treated with HA and the other with PI, and swabs were taken after 1-minute exposure. The eye treated with PI was rinsed with sterile saline and another swab was taken. Colony-forming units (CFUs) were recorded after 2 days. Patients rated the level of irritation after treatment in each eye. Results: HA and PI both gave significant reduction in CFUs from baseline ( P < .001 for HA and P = .002 for PI). The mean reduction in logCFU ± 95% CI was 0.850 ± 0.387 or greater for HA and 0.749 ± 0.385 or greater for PI; this was equivalent to a mean reduction of 7.1-fold or greater or 86% or greater (95% CI, 66%-94%) for HA and 5.6-fold or greater or 82% or greater (95% CI, 57%-93%) for PI. CFUs increased in 17 eyes after saline rinse. PI caused substantial irritation in 31 of the 40 participants, whereas no individuals had any irritation from topical HA. Conclusions: Both HA and PI were effective in reducing ocular bacterial load. Unlike PI, HA was not irritating to the eye. Saline rinse after topical PI may increase bacterial counts in some individuals.

Purpose Povidone-iodine (PI) is widely used to reduce the preoperative conjunctival bacterial load. This study aimed to evaluate the employment of PI 10% in an attempt to sterilize the ocular surface prior to cataract surgery, and to show that PI could be left in contact for 3 minutes. The viability of this exposure time in clinical practice, associated adverse events, and visual outcomes were documented. Methods In this prospective cohort study, phacoemulsification cataract surgery was performed in 604 patients by a single surgeon. Preoperative preparation was undertaken with PI 10%, applied to the cornea, conjunctival sac, eyelids, and periorbital skin with sterile cotton gauze. Povidone-iodine was then flushed onto the ocular surface. Operating room staff timed the precise duration of exposure. After the 3-minute preparation, the lids were thoroughly dried with fresh dry gauze. Results The median PI exposure time was 3.17 minutes, with an interquartile range of 0.25. All cases were followed up postoperatively at 1 day, 1 week, and 1 month. There were no complications attributable to PI. Visual outcomes were satisfactory. Conclusions Implementation of a preoperative prophylaxis protocol that used PI 10% with a 3-minute exposure time can be performed in clinical practice. The 3-minute exposure time had no adverse sequelae.

ABSTRACT Objectives: The aim was to evaluate the effect of adding povidone (PVP) iodine powder with different concentrations to irreversible hydrocolloid on both microbiological and dimensional stability. Materials and Methods: Regular set of (alginate) irreversible hydrocolloid was selected as control group. PVP-iodine powder was mixed with the alginate powder at concentrations of 1, 5, 10, 15, and 20% by weight (test groups). All specimens were tested for their antimicrobial effect against Streptococcus mutans and Staphylococcus aureus as well as dimensional stability. Results: The results of test groups showed that concentrations 1, 5, and 10, weight % had little effect against S. mutans and S. aureus microorganisms. While concentrations 15 and 20 weight % had demonstrated greater effect on microbial growth. The mean of dimensional stability in mm of modified alginate with PVP-iodine at 15 and 20 weight % was –0.119 ± 0.255 and –0.035 ± 0.074, respectively. While the mean dimensional stability in mm of unmodified alginate was –0.112 ± 0.176 . The results of dimensional stability showed that 15 and 20 concentrations of test groups adversely affect the dimensional stability. The adverse effect was noticed to be significant in concentration 20%, where as it was nonsignificant in 15% concentration. Conclusion: Modified alginate impression material with 15 weight % PVP-iodine powered give the material, a self-disinfected properties with less deteriorating effect on dimensional stability.

Background Socks are an important element in running and can help to maintain the optimal conditions of warmth and moisture for the foot. Although socks with bioceramic fiber could be capable of having an antiperspirant and antimicrobial effect, there is little scientific evidence for such potential. Therefore, the aim of the present study was to evaluate the antimicrobial effect and the response, with regards to the foot’s perspiration, of a sock with bioceramic fibers, comparing the results with those of a cotton-made control sock. Methods A group of 33 male runners who were about to run a half-marathon race were asked to wear the bioceramic sock (Action®) on their left foot and the control sock (Kalenji Eliofeel Warm®) on their right foot. Before the race, a microbiological culture was taken on the skin under the fifth metatarsal head, followed by a measurement of the skin moisture on five anatomical zones of the foot. These analyses were repeated at the end of the race. Results After the race, there were significantly fewer bacteria on bioceramic socks than on the control (0.5 ± 1.2 × 104 versus 1.6 ± 2.3 × 104 colony-forming units/cm2, respectively, p = 0.022). At midfoot, dorsum, and the fifth metatarsal base, there was significantly less moisture after the race on the bioceramic sock than in the control ( p = 0.011, p = 0.040, p = 0.023, respectively). Conclusion Socks made of bioceramic fibers showed antiperspirant and bacteriostatic characteristics that could help to maintain the normal physiology of the skin of the foot, which could contribute to preventing dermatological diseases.

Background: Hypochlorous Acid Solution (HAS) is a non-irritating, odorless and transparent, pH-neutral substance having antimicrobial activity. Objective: The study aimed to compare the efficacy of HAS with a 10% povidone-iodine (PVI) solution for antisepsis in Phacoemulsification Cataract Surgery (PCS). Methods: Consecutive patients undergoing PCS in the Elbląg City Hospital, Poland, were enrolled in this prospective trial. In the morning just before surgery a swab was taken from the inferior conjunctival fornix of the eye that was to be operated. Patients were assigned to receive conjunctival irrigation with PVI or HAS in the operating room. Three minutes after lavage with PVI or HAS, conjunctival swabs were taken. During surgery the cornea and conjunctival sac were irrigated with Ringer’s lactate. The last swab was taken before removing the eye speculum. Results: Overall, 110 patients completed the study; there were 59 patients in the PVI group and 51 patients in the HAS group. Conjunctival lavage with 10% PVI resulted in a decrease in bacterial load, while HAS application did not. In the HAS group a reduction in bacterial load was found after surgery. Patients after HAS irrigation reported significantly less discomfort associated with conjunctival lavage than with PVI. None of the patients developed postoperative endophthalmitis or any type of eye inflammation within the follow-up period. Conclusions: This study confirms the excellent antibacterial activity of a 10% povidone-iodine solution used for three minutes before cataract surgery. Conjunctival irrigation with Ringer lactate during PCS decreased the bacterial load of the conjunctival sac.

Detergent drawer and door seal represent important sites for microbial life in domestic washing machines. Interestingly, quantitative data on the microbial contamination of these sites is scarce. Here, 10 domestic washing machines were swab-sampled for subsequent bacterial cultivation at four different sampling sites: detergent drawer and detergent drawer chamber, as well as the top and bottom part of the rubber door seal. The average bacterial load over all washing machines and sites was 2.1 ± 1.0 × 104 CFU cm−2 (average number of colony forming units ± standard error of the mean (SEM)). The top part of the door seal showed the lowest contamination (11.1 ± 9.2 × 101 CFU cm−2), probably due to less humidity. Out of 212 isolates, 178 (84%) were identified on the genus level, and 118 (56%) on the species level using matrix-assisted laser desorption/ionization (MALDI) Biotyping, resulting in 29 genera and 40 identified species across all machines. The predominant bacterial genera were Staphylococcus and Micrococcus, which were found at all sites. 22 out of 40 species were classified as opportunistic pathogens, emphasizing the need for regular cleaning of the investigated sites.

The aim was to study the bacterial load and isolate potential pathogens and food spoilage bacteria from kitchen tables, including preparation tables and dining tables.Methods. A total of 53 households gave their consent for participation. The samples were collected by swabbing over an area of 5 cm by 5 cm of the tables and processed for bacterial count which was read as colony forming units (CFU), followed by isolation and identification of potential pathogens and food spoilage bacteria.Result. Knowledge about hygiene was not always put into practice. Coliforms,Enterococcusspp.,Pseudomonasspp.,Proteusspp., andS. aureuswere detected from both dining and preparation tables. The mean CFU and presence of potential pathogens were significantly affected by the hygienic practices of the main food handler of the house, materials of kitchen tables, use of plastic covers, time of sample collection, use of multipurpose sponges/towels for cleaning, and the use of preparation tables as chopping boards (p<0.05).Conclusion. Kitchen tables could be very important source of potential pathogens and food spoilage bacteria causing foodborne diseases. Lack of hygiene was confirmed by presence of coliforms,S. aureus,andEnterococcusspp. The use of plastic covers, multipurpose sponges, and towels should be discouraged.

The bacterial load on the feathers (breast, thigh, and drum), skin (breast, thigh, and drum), and feet of a total of 40 broiler chickens from four different production units was characterized after the chickens were exsanguinated but before the carcasses were scalded. Each broiler carcass contained a mean of aerobic bacteria at or near 8log10/g, a level at which the carcasses would be considered spoiled. Escherichia coli counts ranged from 6.3 to 8.0 log10/g, with the highest presence on the feathers and breast skin and lower levels on the thigh and drum skin, and feet. Contamination of the carcasses followed a similar pattern for Salmonella spp., which ranged between 5.8 and 7.2 log10/g, and Campylobacter jejuni/coli which ranged between 6.1 and 7.5 log10/g. The incidence of potential pathogens was higher on the feathers, breast skin, and feet than on the thigh and drum skin. The incidence of E. coli ranged from 42.5 to 100%, Salmonella spp. ranged from 27.5 to 75%, and C. jejuni/coli ranged from 45 to 82.5%. The mean counts of microorganisms on broilers from the four different grow-out farms were significantly different (P &gt; 0.05) from one another immediately after killing, although the magnitude of the differences was small in most cases. The means ranged from 7.3 to 8.0 log10/g for total colony-forming units (CFU), 6.7 to 7.6 log10/g for E. coli, 5.4 to 6.9 for Salmonella spp., and 5.7 to 7.9 for C. jejuni/coli. All birds were contaminated with E. coli, 60 to 100% with Salmonella spp., and 80 to 100% with C. jejuni/coli, depending on grow-out farm. C. jejuni/coli counts and incidences were higher than those of Salmonella spp., and usually lower than those of E. coli. This research demonstrates that broilers entering the processing plant are highly contaminated. Although processing seems to decrease the number and incidence of microorganisms on the carcasses, additional modifications in production, transportation, and processing are warranted to reduce the microbial population on the birds before they are slaughtered.

The study use intralesional triamcinolone acetonide injection proceduce for chalazion treatment.1. Objectives: To evaluate results of intralesional triamcinolone acetonide injection for chalazion treatment. 2. Method: This noncomparative prospective interventional trial included 72 chalazions of 61 patients. 3. Results: 61 patients (72 chalazions) with 19 males (31.1%) và 42 females (68.9%), the mean age was 24 ± 9,78 years. 31.1% patients was the first time chalazion and 68.9% patients was more than one times chalazion including 78.6% patients was recurrent at the first position and 21.4% patients occur at new position. 72 chalazions with 16 (22.2%) chalazions was treated before and 56 (77.8%) chalazions wasn’t done that. 72 chalazions with 49 chalazions (68.1%) are local in upper eyelid and 23 chalazions (31.9%) are local in lower eyelid. The mean of chalazion diameter is 6.99 ± 3.03mm. Intralesional triamcinolone acetonide is injected to treat 72 chalazions with 16 (22.2%) chalazions are injected through the route of skin and 56 (77.8%) chalazions are injected through the route of conjunctiva. After 2 weeks follow-up, the success rate was 93.1% and 6.9% failed. 4. Conclusion: intralesional triamcinolone acetonide injection for chalazion treatment is really effective. Acute wounds may require cleansing to reduce the risk of infection. Stabilized hypochlorous acid in acetic buffer (HOCl + buffer) is a novel wound irrigation solution with antimicrobial properties. We performed a first-in-man, prospective, open-label pilot study to document preliminary safety and performance in the treatment of acute wounds. The study enrolled 12 subjects scheduled for a split-skin graft transplantation, where the donor site was used as a model of an acute wound. The treatment time was 75 s, given on 6 occasions. A total of 7 adverse events were regarded as related to the treatment; all registered as pain during the procedure for 2 subjects. One subject had a wound infection at the donor site. The mean colony-forming unit (CFU) decreased by 41% after the treatment, and the mean epithelialization was 96% on both days 14 (standard deviation [SD] 8%) and 21 (SD 10%). The study provides preliminary support for the safety, well-tolerance, and efficacy of HOCl + buffer for acute wounds. The pain was frequent although resolved quickly. Excellent wound healing and satisfying antimicrobial properties were observed. A subsequent in vitro biofilm study also indicated good antimicrobial activity against Pseudomonas aeruginosa with a 96% mean reduction of CFU, when used for a treatment duration of 15 min ( P < .0001), and a 50% decrease for Staphylococcus aureus ( P = .1010). Future larger studies are needed to evaluate the safety and performance of HOCl + buffer in acute wounds, including the promising antimicrobial effect by prolonged treatment on bacterial biofilms.

A multicenter, nonrandomized, prospective, controlled study was conducted to evaluate, as perioperative prophylactic treatment, the anti-infective effectiveness of 0.66% povidone-iodine eye drops (IODIM®) against the bacterial flora of the conjunctival surface of patients who undergo cataract surgery. Eye drops containing 0.66% povidone-iodine were applied to the eye undergoing cataract surgery; the untreated contralateral eye was used as control. One hundred and twenty patients set to receive unilateral cataract surgery were enrolled in 5 Italian Ophthalmology Centers and pretreated for three days with 0.66% povidone-iodine eye drops. The contralateral eye, used as control, was left untreated. Conjunctival swabs of both eyes were collected at the baseline visit and after three days of treatment, just before the cataract surgery. A qualitative and quantitative microbiological analysis of bacterial presence was evaluated by means of bacterial culture, followed by identification. Methicillin resistance determination was also performed on staphylococci isolates. Bacterial load before and after treatment of the eye candidate for cataract surgery was evaluated and compared to the untreated eye. A reduction or no regrowth on the culture media of the bacterial load was observed in 100% of the study subjects. A great heterogenicity of bacterial species was found. The 0.66% povidone-iodine eye drops, used for three days prior to cataract surgery, were effective in reducing the conjunctival bacterial load. The 0.66% povidone-iodine eye drops (IODIM®) might represent a valid perioperative prophylactic antiseptic adjuvant treatment to protect the ocular surface from microbial contamination in preparation of the surgical procedure.
