In collaboration with some of the leading headache centres in Germany, Switzerland and Austria, we have established new guidelines for the treatment of migraine attacks and the prevention of migraine. A thorough literature research of the last 10 years has been the basis of the current recommendations. At the beginning, we present therapeutic novelties, followed by a summary of all recommendations. After an introduction, we cover topics like drug therapy and practical experience, non-effective medication, migraine prevention, interventional methods, non-medicational and psychological methods for prevention and therapies without proof of efficacy.

The aim of the work was to justify the algorithm of outpatient drug therapy in patients with COVID-19, based on the principle of «Multi-hit» Approach. The algorithm is based on the published results of clinical studies and observations, authors’ own practical experience in the use and management of more than 4 thousand patients diagnosed with COVID-19 of varying severity during the 2020 pandemic. The article substantiates a complex algorithm for the treatment of outpatients with COVID-19, which includes etiotropic, pathogenetic, and symptomatic components of therapy with different mechanisms of action. The described approach is the 1st stage (outpatient) of a complex algorithm for managing patients with COVID-19. It has been successfully implemented in the system of outpatient care for patients with novel coronavirus infections in several leading medical institutions in Russia. The authors believe that the developed algorithm for providing outpatient drug therapy for COVID-19, based on the principle of multiple exposure, may be useful in real clinical practice of managing patients with coronavirus infection.

This article presents the results of a major project in The Netherlands with the objective to give guidelines for hydrodynamic calculation in urban drainage. The guidelines are based on rainseries simulation using a hydrodynamic calculational technique. The new guidelines do not prescribe a certain software package, but merely define defaults and procedures. They also put constraints on input and output data. The new guidelines are introduced in The Netherlands by the end of 1995 and will be evaluated in 1998 using the practical experience acquired by municipalities, waterboards and consulting engineers.

Diagnosis of glomerulopathy requires correlation between: (a) clinicopathological syndromes, (b) kidney histology for the abnormalities which are useful for the severity of disease, stage of activity and extent of chronicity.  Subsequently, management dictates the choice of effective medications to avoid renal loss and long-term side effects.  In this review article; we provide our practical experience with drug-therapy in common idiopathic and secondary glomerular diseases. Keywords: glomerulopathy, kidney biopsy, nephrotic syndrome, Rituximab, treatment.

Worldwide, the most important reuse of wastewater, in volume, is agricultural irrigation. Therefore, there is a need to properly treat wastewater for such purpose, considering the removal of pathogens while leaving suitable amounts of nutrients and other compounds to increase productivity. Helminth ova are one of the main targeted pathogens in the new guidelines for water reuse in agriculture and aquaculture issued in 2006 by the World Health Organization. However, relatively little research has been done recently on how to remove and inactivate helminth ova from wastewater and sludge and recommendations given several decades ago are still used, but when put into practice, particularly in developing countries, produce unsatisfactory results. One problem is that these criteria were developed using inaccurate analytical techniques and the other is the large number and variety of helminth ova species found in wastewater and sludge from the developing world. In fact, the few technological options to remove and inactivate helminth ova come from research performed using wastewater and sludge with low helminth ova content, and refer almost only to Ascaris (one type of helminth). This paper summarises recent research work and results from practical experience concerning helminth ova control for advanced and conventional sanitation.

The Sovietization of East Central Europe between 1945 and 1948 led to a complete reformulation of the “nationalities question” on the basis of Marxist-Leninist theory and the practical experience of the USSR. The changed political and ideological context provided the expanded camp of peoples' democracies with new guidelines for the treatment of their minorities. From this time onward, the ethnic/national minorities of these states were guaranteed an existence which was “national in form,” but “socialist in content.”

The article demonstrates the practical experience of the effective use of inhibitor of sodium-glucose co-transporter 2 emplagliflozin as a part of optimal drug therapy in patients with decompensation of chronic heart failure with systolic dysfunction of ischemic genesis and type 2 diabetes mellitus. The place of this group of drugs in improving the quality of life and prognosis in patients with severe comorbidity has been indicated according to the data of evidence-based studies.

AbstractThe Homœopathic Pharmacopœia of the United States (HPUS) is one of the three officially recognized compendia of medical drugs in the United States. A well-conducted homeopathic proving is one pathway to entry into the HPUS. In 2013, guidelines for homeopathic drug provings were updated to better assess the methodology of provings and improve the confidence in the outcomes generated. In addition, the team who developed the revised guidelines included a pragmatic approach to assess the quality of proving outcomes in terms of their usefulness in determining the therapeutic profile. The monograph review team, composed of homeopathic clinicians and researchers, has evaluated 25 new homeopathic medicines for monograph purposes. The team has noted that the guidelines have helped frame and standardize the monograph review process. The revised structure for monograph reporting has enhanced the reviewer's ability to ascertain the therapeutic profile for a new substance. Sponsors have stated that the new guidelines have helped increase their conformance with Good Clinical Practices, helped improve the reporting and transparency of their provings, and may help safeguard provings as a valid research method. Similar harmonized guidelines have been adopted by the European Committee for Homeopathy and the Liga Medicorum Homœopathica Internationalis.

The Clinical Fellowship Experience is described by the American Speech-Hearing-Language Association (ASHA) as the transition period from constant supervision to independent practitioner. It is typically the first paid professional experience for the new graduate, and may be in a setting with which the new clinician has little or even no significant practical experience. The mentor of a clinical fellow (CF) plays an important role in supporting the growth and development of this new professional in areas that extend beyond application of clinical skills and knowledge. This article discusses how the mentor may provide this support within a framework that facilitates the path to clinical independence.
