Abstract Background Routine screening for frailty at admission by nurses may be useful to detect geriatric risks and problems at an early stage. However, the added value of this screening is not clear yet. Information about the opinions and attitudes of nurses towards this screening is also lacking. As they have a crucial role in conducting this screening, an exploratory study was performed to examine hospital nurses’ opinions and perspectives about this screening and how it influences their daily work. Methods A qualitative, exploratory approach was employed, using semi-structured interviews with 13 nurses working on different general medical wards (surgical and internal medicine) in three Dutch hospitals. Frailty screening had been implemented for several years in these hospitals. Results The participating nurses reported that frailty screening can be useful to structure their work, create more awareness of frail older patients and as starting point for pro-active nursing care. At the same time, they assess their clinical view as more important than the results of a standard screening tool. The nurses hardly used the overall screening scores, but were particularly interested in information regarding specific items, such as delirium or fall risk. Screening results are partly embedded systematically and in daily nursing care, e.g., in team briefings or during transfer of patients to other wards. The majority of the nurses had received little training about the background of frailty screening and the use of screening tools. Conclusions Most nurses stated that frailty screening tools are helpful in daily practice. However, nurses did not use the frailty screening tools in the referred way; tools were particularly used to evaluate patients on separate items of the tool instead of the summative score of the tool. When frailty screening tools are implemented in daily practice, training needs to be focused on. Additional research in this field is necessary to gain more insight into nurses’ opinions on frailty screening.

Purpose – The purpose of this paper is to investigate the information content of the variables that can help detecting external and internal imbalances in an early stage. The starting point is the Scoreboard, where nine indicators are chosen in order to increase macroeconomic surveillance of all member states. Design/methodology/approach – This paper provides an overview of the variables that could be informative for imbalances by focusing on EU-27 countries over the period 1960-2010. The number of chosen variables is 28, and they are aggregated in six macro-areas. Therefore, once an imbalance is observed in any of those areas, it is possible to detect in a simple way which specific variable is determining such outcome. Findings – In general, this approach provides reliable signal to the policy-makers about the indicators that can drive imbalances within the area, shedding light on the relationship among the variables included in the analysis, too. Research limitations/implications – In fact, the empirical results underline some well-known critical issue for several countries, and is largely in line with results obtained in a variety of EC and OECD studies. Originality/value – The main added value of the approach adopted in this paper is the introduction of more variables than those initially proposed by the European Commission in the construction of the Scoreboard. This provides more information about the macroeconomic situation in each country, preserving, however, the simplicity of the analysis as the variables are aggregated by homogeneous areas.

ABSTRACT Objective: to analyze the knowledge and practices of nursing professionals in caring for people with colostomy bag. Method: a qualitative and descriptive study conducted with 21 nursing professionals from a General Surgery Unit. Data collection used triangulation of techniques, based on non-participant observation, analysis of nursing records, and semi-structured interviews. Analysis followed the spiral technique assumptions. Results: it was possible to comprehend the theoretical and scientific knowledge that subsidize practice; identify contradictions related to statement and care in the profession’s daily work and the intervening factors, which can facilitate and/or make the nursing care process difficult. Final Considerations: knowledge and practices of professionals in caring for ostomates occur within work experiences, in which socialization of knowledge makes it possible to expand the perspectives of care.

Objective: to understand the meaning of nursing care for the family of patients in palliative care. Methods: field, descriptive, and exploratory study with qualitative approach conducted in secondary general hospital in the State of Ceará, Brazil. The participants comprised eight family members of patients in palliative care, with the conduction of semi-structured interviews and direct observation. For data analysis, we used the thematic category analysis. Results: data revealed the construction of three thematic categories. The meaning of nursing care for the family of patients in palliative care is anchored in the physical presence of these professionals, the performance of technical procedures, and teamwork. Conclusions: we verified that the nursing care highlighted by families aims at a humanistic care, established in the presence and based on human interaction.

Abstract This study aims to investigate ten hotels on the island of Gotland and their daily, practical work towards sustainability. An exploratory study was conducted at ten hotel establishments on the island of Gotland in Sweden. Ten hotel managers were interviewed about their daily practice and work towards sustainable solutions at their hotels. The results show that there are big differences between these hotels regarding the way in which knowledge about sustainability issues are implemented and shared to the personnel as well to the guests. In addition, there is a clear difference in the way in which sustainability is expressed in practice: the accountability is largely attributable to most of the hotels, but a few convey responsibility. Some of the managers seem to take a more active role in mediation of sustainability issues than is the case with the majority of the hotel managers. Also, anonymity between hotel guests and staff, large number of seasonal workers and insufficiently trained staff can be seen as a hindrance for knowledge exchange over the sustainability issues. Working with sustainability issues can also be seen as a way to win market shares in the hotel business. The study makes contribution to the knowledge of processes of co-creation of sustainable values by exploring how some hotel managers are working towards sustainability on a daily basis. The study can help to share knowledge of possibilities and hindrances in daily work towards sustainable hotels.

RESUMO Objetivo: conhecer os cuidados de Enfermagem relacionados à administração segura de medicamentos em Neonatologia e Pediatria. Método: trata-se de estudo qualitativo, descritivo e exploratório com 23 trabalhadores das equipes de Enfermagem por meio de entrevistas semiestruturadas, submetendo-as à técnica de Análise de Conteúdo na modalidade Análise Temática. Resultados: referiram-se, como cuidados, à supervisão pelo enfermeiro da equipe de Enfermagem, à identificação dos cinco certos preconizados (criança, leito, medicação, dose e horário), ao registro da medicação administrada em uma planilha de cuidados, à realização do cálculo milimétrico das doses, à checagem da validade da medicação, à não aplicação, no mesmo horário, de medicamentos incompatíveis quimicamente, à garantia da assepsia do local da administração, ao cuidado com a temperatura adequada para o armazenamento, à utilização dos medicamentos e ao esclarecimento das dúvidas existentes antes da administração dos fármacos. Conclusão: conclui-se que a segurança do paciente em relação à administração de medicamentos nas Unidades de Neonatologia e Pediatria é complexa exigindo, dos profissionais da equipe de Enfermagem, habilidades e competências para o seu enfrentamento. Results: they referred to the supervision by the Nursing staff of the Nursing team, the identification of the five recommended ones (child, bed, medication, dose and schedule), the registration of the medication administered in a care plan, the millimetric calculation of doses, checking the validity of the medication, non-application of chemically incompatible drugs at the same time, ensuring the asepsis of the administration site, taking care of the temperature appropriate for storage, use of medicines and doubts prior to the administration of the drugs. Conclusion: it is concluded that patient safety in relation to medication administration in the Neonatology and Pediatrics Units is complex, requiring the professionals of the Nursing team, skills and competencies to cope with them. ABSTRACTPurposeWe expect physicians to be lifelong learners. Learning from clinical practice is an important potential source for that learning. To support physicians in this process, a better understanding of how they learn in clinical practice is necessary. This study investigates how physicians use informal feedback as learning cues to adjust their communication from interactions with patients in the outpatient setting.MethodsTo understand physicians’ use of informal feedback, we combined non-participant ethnographic observations with semi-structured interviews. We enrolled 10 respiratory physicians and observed 100 physician-patient interactions at one academic and one non-academic hospital in the Netherlands. Data collection and analysis were performed iteratively according to the principles of constructivist grounded theory. Our conceptual model describes how physicians use cues to reflect on and adjust their communication as well as to further develop their adaptive expertise.ResultsIn addition to vast variations within and across patient encounters, we observed recurring patterns in physicians’ communications in reaction to specific learning cues. Physicians had learnt to recognise and use different cues to adjust their communication in patient encounters. They established a ‘communication repertoire’ based on multiple patient interactions, which many saw as learning opportunities, contributing to the development of adaptive expertise. Our findings show differences in physicians’ sensitivity to recognising learning opportunities in daily practice which was further influenced by contextual, personal and interpersonal factors. Whereas some reported to have little inclination to change, others used critical incidents to fine-tune their communication repertoire, while others constantly reshaped it, seeking learning opportunities in their daily work.ConclusionsThere is a large variation in how physicians use learning cues from daily practice. Learning from daily practice is a collaborative effort and requires a culture that promotes lifelong learning. Raising physicians’ awareness of experiences as potential learning opportunities might enhance their development of adaptive expertise.

RESUMO Objetivo: conhecer a percepção do familiar acerca do cuidado de enfermagem numa unidade pediátrica. Método: trata-se de estudo qualitativo, descritivo, exploratório, com 21 familiares. Coletaram-se os dados por meio de entrevistas semiestruturadas, analisadas pela técnica de Análise de Conteúdo na modalidade Análise Temática. Resultados: considera-se, pela família, a equipe atenciosa e sempre disposta a dedicar auxílio e informações necessárias. Referiu-se, também, que é cuidadosa no momento de lidar com a família. Citou-se, nas reinternações, principalmente de crianças com doenças crônicas, a criação do vínculo como fator positivo. Relataram-se, também, o carinho e o bom acolhimento como demonstrações dos demais profissionais. Conclusão: entende-se que o enfermeiro se deve fazer presente e fornecer um cuidado humanizado à criança e à sua família, visto que se encontram em um período de fragilidade. Results: the family considers the staff attentive and always ready to dedicate the necessary help and information. She also mentioned that she is careful when dealing with the family. It was mentioned, in the readmissions, mainly of children with chronic diseases, the creation of the bond as a positive factor. The affection and the welcome were also reported as demonstrations of other professionals. ABSTRACTObjective: to verify how nursing care, related to nutrition and hydration in labor, is provided to laboring women. Method: this is about an exploratory study from qualitative approach developed at the obstetric ward of a school hospital in Curitiba city, from August to November 2007, with 10 low risk primiparous, semi-structured interviews were conducted with puerperal women, and analyzed according to Bardin content analysis technique, and five categories emerged. Results: nutrition and hydration during labor is still a controversial topic. Fasting during labor has been considered necessary for many years, due to the fear of gastric contents aspiration during anesthesia, but this is being reassessed by the humanization movement. Conclusion: The humanization of labor care is still a challenge in many hospitals, and nutrition during labor is a caution that should be implemented aiming the improvement of the care for women during childbirth as part of humanized care. Método: foi realizada uma pesquisa qualitativa exploratória, desenvolvida no centro obstétrico de uma maternidade escola no município de Curitiba, no período de agosto a novembro de 2007, com 10 primíparas de baixo risco, e as entrevistas semi-estruturadas, foram realizadas no puerpério e analisadas segundo Bardin, emergindo 5 categorias. Resultados: a alimentação e hidratação no trabalho de parto é ainda um tema controverso. O jejum durante o trabalho de parto foi considerado durante muito tempo, devido ao receio da aspiração do conteúdo gástrico na anestesia, porém com o movimento de humanização, esta prática está sendo reavaliada.  Conclusão: A humanização no trabalho de parto ainda é um desafio em muitas instituições hospitalares e a prática da alimentação no trabalho de parto é um cuidado que deve ser implementado buscando a melhoria da assistência à parturiente como parte do cuidado humanizado. Resultados: la alimentación y la hidratación en el trabajo del parto sigue siendo un tema controvertido. El ayuno durante el trabajo de parto fue considerado por mucho tiempo, debido al miedo de la aspiracion del contenido gastrico de la anestesia, pero con el movimiento de la humanización, esta pratica sigue en revalorización. Conclusión: la humanización en el trabajo del parto sigue siendo un reto en muchos hospitales y la práctica de la alimentacion en el trabajo del parto es todavia una precaución que se deben implementar con el objetivo de mejorar la assistência y la atención a la parturienta como parte del cuidado humanizado. In recent history, different design approaches have been entering fields like management and strategy to improve product development and service delivery. Specifically, entrepreneurship has adopted a user-centric mindset in methodologies like the business canvas model and the value proposition canvas which increases the awareness of the users’ needs when developing solutions. What happens when a service design approach is used to understand the entrepreneurs’ experience through the creation of their startups? Recent literature suggests that entrepreneurial activity and success is conditioned by their local entrepreneurship ecosystem. This study investigates the Entrepreneurship Ecosystem of Medellín, Colombia - an ecosystem in constant growth but that lacks qualitative analysis. The sample consists of 12 entrepreneurs in early-stage phase. The data was gathered with two design research methods: Cultural Probes and Semi-structured interviews. The analysis of the information collected facilitated the development of 4 insights about the entrepreneurs and an experience map to visualise and interpret their journey to create a startup. The results of this study reflected the implications of the ecosystem, the explanation of the users’ perceptions and awareness and propose a set of ideas to the local government to improve the experience of undertaking a startup in Medellín.
