A review of physician preparedness in response to a systemic crisis: The case of Eastern Regional Health Authority, Newfoundland, Canada, estrogen receptor (ER) breast cancer (BC) errors

e17551 Background: From March until October 2008, a Commission of Inquiry was held in St. John's, Canada, investigating systemic errors leading to incorrect ER results in BC patients. 386 ER negative BC samples tested between 1997 and 2005 were incorrect. Of 91 witnesses, 26 physicians testified about their roles in this “crisis.” Physicians have traditionally been trained to respond to individual clinical crises and acute traumatic situations. This study summarizes the physicians’ training and preparedness in response to a systemic organizational crisis. Methods: Inquiry exhibits, submissions, and transcripts of physician testimony were reviewed for evidence of education and experience in crisis management (CM) as applied to a complex organization. Education and experience with organizational health care crisis were tabulated and prevalent themes relevant for physicians summarized. Results: Evidence was reviewed from 5 oncologists, 17 pathologists, 1 health information analyst, and 3 senior physician leaders. 20 physicians held departmental or divisional leadership roles, one had an advanced degree in Public Health and one had direct experience in a complex private organization. 5 out of province expert consultants had direct experience with continuous quality control applied to large organizations. No physicians were formally trained in CM in terms of acute crisis response. Two prevailing concerns were expressed: 1) delayed and incomplete disclosure of information to patients and public, and 2) numerous preceding systems problems identified by physicians that did not translate into necessary systemic changes to prevent the crisis. Conclusions: The practice of CM recognizes the need to eliminate technological and human systems failure and to develop formal communication systems to avoid or to manage crisis situations. Large-scale organizational crisis management is an important concept for physicians to understand and actively participate in their professional roles. At present most physicians do not receive formal training in CM. No significant financial relationships to disclose.

Little is known about how the idea of ‘resilience’ translates into practice. It has nonetheless emerged as a dominant theme in the governance of crises, such as political instability, armed conflict, terrorism, and large-scale refugee movements. This study draws on interviews with humanitarian and development practitioners in Turkey, Jordan, and Lebanon working under the Regional Refugee and Resilience Plan to explore how resilience is interpreted and translated on the ground. Results suggest that resilience is translated as the economic self-reliance of refugees, and the capacity for crisis management of refugee-hosting states, enacted through ‘localization’ and strengthening the ‘humanitarian-development nexus.’ The prominence of the political and economic context and the power relations between crisis response actors that it generates reveals the limits of what a buzzword like resilience can achieve on the ground. The findings highlight the need for researchers, policymakers, and practitioners to engage in continuous critical reflection on whether the ways in which resilience policies and programmes are implemented actually improve the ability of systems and vulnerable populations to recover from crisis, as well as on the validity of the assumptions and interpretations on which such policies and programmes are built.

Purpose The purpose of this paper is to explore the relationship of organizational culture, leadership and crisis management through exploration of these three constructs with respect to crisis management. Design/methodology/approach In this paper, a conceptual framework has been proposed that is based on the literature findings of organizational culture, leadership and crisis management. Two types of cultural elements are used; internal versus external focus and low versus high flexibility. Organizational crisis management process is explained through the five-stage life cycle, including signal detection, prevention, damage containment, recovery and learning. Four types of leadership are included; directive, transactional, cognitive and transformational that are critical during crisis management. Five research propositions have been proposed for each stage of crisis management. Findings Five research propositions have been proposed based on the stages of crisis management. Research limitations/implications The conceptual framework needs to be tested for validity. More research is needed on how changing demographics and technology affect these constructs. Organizations need to develop through reflective practices that focus on leadership competencies and crisis-prone culture to tackle any crisis event. Practical implications Organizations need to develop leadership competencies and crisis-prone culture. Organizations needs to be reflective on their practices. Originality/value The proposed conceptual framework is an expanded version of the crisis response leadership matrix (CRLM) model of Bowers et al. (2017). In this paper, an unique concept is presented by aligning leadership, culture and crisis management with respect to each stage of crisis management and types of crisis.

Effective responses to severe strains and large-scale disasters demand efficient crisis management, i.e., capabilities that depend on inter-organizational collaborations. Inter-organizational exercises are performed to maintain and develop the inter-organizational crisis management (ICM) capability. In a previous review of scientific literature, we identified nine themes (i.e., aspects) of ICM capability (ICMC): interaction, coordination/C2, decision making, relationships, situation awareness (SA), resilience, preparedness, system performance, and information infrastructure. This paper presents empirical testing of the nine ICMC aspects, by observational studies in two ICM exercises. The ICMC aspects were implemented in a structured observation protocol, which allowed observations based on classification of single utterances and actions. All ICMC aspects except system performance were observed. Actions related to SA, interaction, and coordination/C2 were frequently observed, while relations, resilience, and preparedness were observed to a lesser extent. In addition, exercise evaluators rated the relevance of all nine ICMC aspects as high.

Organizations should prepare for crises, through identifying crisis concerns, having written crisis communication plans, and designating teams for crisis planning and response, for example. Nonprofit organizations, which represent an important sector of U.S. society, are no different in needing to prepare, but to date, a review of their crisis communication preparedness is lacking. Therefore, a national online survey of 2,005 U.S. charitable organizations was administered to determine nonprofit organizations’ adoption of an anticipatory perspective of crisis management. The anticipatory perspective shifts the organization’s focus from reaction to crises to anticipation of them. According to the survey, 75% of organizations reported at least one organizational crisis in the 24 months prior to taking the survey (circa 2017–2019). Loss of a major stakeholder was the most common organizational crisis that had occurred and the greatest future concern. Most nonprofits (97.5%) reported implementing some crisis communication preparedness tactics. Importantly, charitable organizations can enact communication preparedness tactics without significantly detracting from program delivery. Moreover, given the general concerns within the sector, nonprofit organizations should prepare specifically for loss of a major stakeholder and technologically created crises such as data breaches and negative word of mouth on social media.

AbstractThe tremendous growth of data traffic has spurred a rapid evolution of optical communications for a higher data transmission capacity. Next-generation fiber-optic communication systems will require dramatically increased complexity that cannot be obtained using discrete components. In this context, silicon photonics is quickly maturing. Capable of manipulating electrons and photons on the same platform, this disruptive technology promises to cram more complexity on a single chip, leading to orders-of-magnitude reduction of integrated photonic systems in size, energy, and cost. This paper provides a system perspective and reviews recent progress in silicon photonics probing all dimensions of light to scale the capacity of fiber-optic networks toward terabits-per-second per optical interface and petabits-per-second per transmission link. Firstly, we overview fundamentals and the evolving trends of silicon photonic fabrication process. Then, we focus on recent progress in silicon coherent optical transceivers. Further scaling the system capacity requires multiplexing techniques in all the dimensions of light: wavelength, polarization, and space, for which we have seen impressive demonstrations of on-chip functionalities such as polarization diversity circuits and wavelength- and space-division multiplexers. Despite these advances, large-scale silicon photonic integrated circuits incorporating a variety of active and passive functionalities still face considerable challenges, many of which will eventually be addressed as the technology continues evolving with the entire ecosystem at a fast pace.

The Iowa caucuses are the inaugural event of the American presidential nomination process. When the state Democratic Party failed to report the 2020 caucus results in a timely manner and manage the consequences, the crisis situation threatened the legitimacy of the party and the integrity of the results. This research presents an in-depth case of the Iowa Democratic Party’s public communication response regarding an event described by the Des Moines Register as “hell” and a “results catastrophe.” Specifically, we were interested in how the Iowa Democratic Party responded to the crisis event and the extent to which the party organization was successful in disseminating favorable messaging about the caucus process to the local press. Drawing on organizational crisis management and echoing press perspectives, this analysis uses network and qualitative analytic approaches to assess message development, dissemination, and ultimately adoption. A local event with national implications presents a critical case in investigating how a political party, due to its institutional role in American elections and unique organizational structure, struggled to respond to the crisis.

Abstract Unmanned aerial vehicles (UAVs) have been widely used in communication systems due to excellent maneuverability and mobility. The ultra-high speed, ultra-low latency, and ultra-high reliability of 5th generation wireless systems (5G) have further promoted vigorous development of UAVs. Compared with traditional means of communication, UAV can provide services for ground terminal without time and space constraints, so it is often used as air base station (BS). Especially in emergency communications and rescue, it provides temporary communication signal coverage service for disaster areas. In the face of large-scale and scattered user coverage tasks, UAV's trajectory is an important factor affecting its energy consumption and communication performance. In this paper, we consider a UAV emergency communication network where UAV aims to achieve complete coverage of potential underlying D2D users (DUs). The trajectory planning problem is transformed into the deployment and connection problem of stop points (SPs). Aiming at trajectory length and sum throughput, two trajectory planning algorithms based on K-means are proposed. Due to the non-convexity of sum throughput optimization, we present a sub-optimal solution by using the successive convex approximation (SCA) method. In order to balance the relationship between trajectory length and sum throughput, we propose a joint evaluation index which is used as an objective function to further optimize trajectory. Simulation results show the validity of the proposed algorithms which have advantages over the well-known benchmark scheme in terms of trajectory length and sum throughput.

Killer whales (Orcinus orca) can produce 3 types of signals: clicks, whistles and vocalizations. This study focuses on Orca vocalizations from northern Vancouver Island (Hanson Island) where the NGO Orcalab developed a multi-hydrophone recording station to study Orcas. The acoustic station is composed of 5 hydrophones and extends over 50 km 2 of ocean. Since 2015 we are continuously streaming the hydrophone signals to our laboratory in Toulon, France, yielding nearly 50 TB of synchronous multichannel recordings. In previous work, we trained a Convolutional Neural Network (CNN) to detect Orca vocalizations, using transfer learning from a bird activity dataset. Here, for each detected vocalization, we estimate the pitch contour (fundamental frequency). Finally, we cluster vocalizations by features describing the pitch contour. While preliminary, our results demonstrate a possible route towards automatic Orca call type classification. Furthermore, they can be linked to the presence of particular Orca pods in the area according to the classification of their call types. A large-scale call type classification would allow new insights on phonotactics and ethoacoustics of endangered Orca populations in the face of increasing anthropic pressure.
