Effects of a Short-Term Vitamin D3 and Calcium Supplementation on Blood Pressure and Parathyroid Hormone Levels in Elderly Women

Calcium supplementation is effective in reducing blood pressure in various states of hypertension, including pregnancy-induced hypertension and preeclampsia. In addition, calcitropic hormones are associated with blood pressure. The hypothesis is that short-term therapy with calcium and vitamin D3 may improve blood pressure as well as secondary hyperparathyroidism more effectively than calcium monotherapy. The effects of 8 weeks of supplementation with vitamin D3 (cholecalciferol) and calcium on blood pressure and biochemical measures of bone metabolism were studied. The sample consisted of 148 women (mean ± sd age, 74 ± 1 yr) with a 25-hydroxycholecalciferol (25OHD3) level below 50 nmol/L. They received either 1200 mg calcium plus 800 IU vitamin D3 or 1200 mg calcium/day. We measured intact PTH, 25OHD3, 1,25-dihydroxyvitamin D3, blood pressure, and heart rate before and after treatment. Compared with calcium, supplementation with vitamin D3 and calcium resulted in an increase in serum 25OHD3 of 72% (P < 0.01), a decrease in serum PTH of 17% (P = 0.04), a decrease in systolic blood pressure (SBP) of 9.3% (P = 0.02), and a decrease in heart rate of 5.4% (P = 0.02). Sixty subjects (81%) in the vitamin D3 and calcium group compared with 35 (47%) subjects in the calcium group showed a decrease in SBP of 5 mm Hg or more (P = 0.04). No statistically significant difference was observed in the diastolic blood pressures of the calcium-treated and calcium- plus vitamin D3-treated groups (P = 0.10). Pearson coefficients of correlation between the change in PTH and the change in SBP were 0.49 (P < 0.01) for the vitamin D3 plus calcium group and 0.23 (P < 0.01) for the calcium group. A short-term supplementation with vitamin D3 and calcium is more effective in reducing SBP than calcium alone. Inadequate vitamin D3 and calcium intake could play a contributory role in the pathogenesis and progression of hypertension and cardiovascular disease in elderly women.

This study designed to examine association between-174G/C polymorphism of interleukin-6 gene and phosphate, calcium, vitamin D3, and parathyroid hormone levels in Iraqi patient with chronic kidney disease on maintenance hemodialysis. Seventy chronic renal failure patients (patients group) and 20 healthy subjects (control group) were genotyped for interleukin-6 polymorphism and genotyping was performed by conventional polymerase chain reaction-restriction fragment length polymorphism. No significant differences in phosphate levels were observed in patients and control with different interleukin-6 genotypes. Control had non-significant differences in calcium levels, while patients with GG and CG genotypes displayed significant elevation with time. Conversely, control and patients with GG and CC genotypes had significant elevation in vitamin D3 levels with time. Regarding parathyroid hormone, control had non-significant differences, while patients with GG and CC genotypes displayed significant elevation with time. Patients with GG genotype displayed significant changes in calcium, vitamin D3 and parathyroid hormone levels with time.

Berbagai penelitian menunjukkan bahwa defisiensi vitamin D dan hiperpara-tiroidisme sekunder menimbulkan dampak serius pada kesehatan, antara lain meliputi osteoporosis, osteomalasia, kelemahan otot, jatuh dan fraktur osteoporotik. Tujuan penelitian ini adalah (1) mengetahui pengaruh pajanan UVB sinar matahari pada konsentrasi 25(OH)D dan hormon paratiroid (PTH) perempuan usia lanjut Indonesia. (2) mendapatkan saat dan lama pemajanan yang optimal. Penelitian uji klinik acak terbuka ini melibatkan 74 perempuan berusia 60 - 90 tahun yang tinggal di 4 panti werda di Jakarta dan Bekasi. Randomisasi dilakukan untuk memisahkan kelompok studi dan kontrol. Kelompok kontrol hanya mendapat kalsium 1000 mg/hari, sedang kelompok intervensi dipajankan dengan matahari selama 6 minggu. Hasil yang diukur sebelum dan sesudah 6 minggu pemajanan adalah konsentrasi 25(OH)D, PTH, dan ion kalsium. Ditemukan bahwa, waktu pemajanan yang optimal adalah 1 jam sebelum dan sesudah tengah hari. Prevalensi defisiensi vitamin D pada wanita usia lanjut adalah 35,1%. Pada kelompok terpajan, konsentrasi 25(OH)D meningkat lebih tinggi daripada yang tidak dipajan (51,8% vs 12,5%). This study was conducted to compare the effect of UVB from sunlight exposure in combination with calcium supplementation, and control (calcium only) on the vitamin D status and parathyroid hormone (PTH) concentration in Indonesian elderly women. This study was a randomized clinical trial in institutionalized care unit. Subjects included 74 elderly women with a mean age 71 years. Intervention was random allocation of UVB from sunlight exposure at 0.6 MED/hour noted in the UV meter on the face and both arms and calcium 1000 mg, three times per week for 6 weeks, and without treatment (calcium 1000 mg only). Main outcome measured were fasting serum levels of 25(OH) D, PTH, and calcium ion at 0 and 6 weeks in both treatment and control groups. The incidence of vitamin D deficiency in this population study was 35.1 %. In the treatment group, 25(OH) D increased from 59.1 nmol/L to 84.3 nmol/L (mean value after 6 weeks of sunlight exposure) with only a slight increase of 25(OH) D in the control group (51.8% vs 12.5%). 25(OH)D deficient levels in 15 out 16 subjects became normal after 6 weeks of sun exposure. There was no change of PTH levels in both groups. Additional results of this study are mean calcium intake of 248 mg/day and vitamin D intake of 28 IU/day.Keywords: Vitamin D deficiency, elderly women, parathyroid hormone

Objective: Moderate-to-severe subacute thyroiditis is clinically managed with 6 to 8 weeks of glucocorticoid therapy. However, no studies have evaluated short-term prednisone treatment for subacute thyroiditis. Methods: This 24-week, prospective, single-blind, randomized controlled study enrolled patients (aged 18 to 70 years) with subacute thyroiditis who were hospitalized between August, 2013, and December, 2014. Patients with moderate-to-severe symptoms were randomly assigned to receive either 30 mg/day prednisone for 1 week, followed by 1 week of nonsteroidal anti-inflammatory drugs, or the conventional 6-week prednisone therapy. The primary endpoint was intergroup differences in treatment efficacy at the end of the treatment course. Secondary endpoints included between-group differences in post-withdrawal adverse effect parameters and thyroid function at weeks 6, 12, and 24. Results: We screened 96 patients, randomized 52 participants, and 50 participants completed the study. Efficacy and recurrence rates were not significantly different at withdrawal in both groups ( P = .65). At treatment discontinuation, parathyroid hormone (28.8 versus 38.9 pg/mL; P = .011) and systolic blood pressure (113.9 versus 122.4 mm Hg; P = .023) were significantly lower in the experimental group than in the control group. There were no significant intergroup differences in other secondary endpoints at withdrawal and in thyroid function at weeks 6, 12, and 24. Conclusion: Fewer side effects of glucocorticoids and similar efficacy and recurrence rates were observed with short-term prednisone compared with the 6-week treatment for subacute thyroiditis. Short-term prednisone, with a better safety profile, may be an alternative strategy for ameliorating moderate-to-severe symptoms of subacute thyroiditis.
