Sexy till I die: What applied theatre can offer in approaches to sexual health

Abstract For older adults in Canada, too often shame and silence describe their experiences of sexual health. With more citizens over the age of 65 than ever before and increasing rates of Sexually Transmitted Infections (STIs) in older adults, we are facing a serious issue. Applied theatre offers an innovative approach to deconstructing social stigma in sexuality across the life course, whereby new understandings and perceptions may emerge for people of all ages. The usefulness, gaps and application of three different approaches to sexual health issues are examined to highlight innovations in addressing sexual health and critique ageist, sexist and heteronormative assumptions through a feminist, critical pedagogy lens. The analysis culminates in a proposed outline for an intergenerational, community-based theatre project to address the social stigma of sexuality across the life course entitled You're Doing What?! At Your Age?!

Abstract Sexual activity contributes to quality of life throughout the lifespan. However, stigma about sex in late life influences older adults’ perceptions and healthcare professionals’ perceptions of older adults’ sexual health/behaviors. Using a multi-methods approach, we examined attitudes and knowledge about sexual health/behaviors in late life. Using longitudinal data from the Midlife in the US Study (Wave 1-3; N=7049), we ran age-based growth curve models to analyze changes in levels of optimism about sex in their future. We also piloted a survey with healthcare professionals assessing attitudes, knowledge, and awareness of policy about sexual health/behaviors among older adults. Adults’ expectations became less optimistic with increased age (β = -0.1, SE = 0.003, p < .0001). Men were more optimistic than women at age 20 (p = 0.016), but men’s optimism decreased over the life course at a faster rate than did women’s (p < .0001), so that from ages 40-93, men were less optimistic than women. Among healthcare professionals (N=21), the majority indicated never or rarely asking their clients about sexual history or health/behaviors; however, they indicated some knowledge about issues relevant to older adults (e.g., safe-sex practices, sexual dysfunction). Few indicated awareness about policies related to sexual behavior among residents (i.e., issues of consent, STIs). Among adults, there is a need to address declining optimism for expectations about sex in late life. Health professionals are well-situated to raise awareness and normalize discussions about sexual health, thus countering negative stigma and contributing to increasing optimism for expectations to remain sexually active.

People may seek to embody cultural ideals of the life course through their use or rejection of medical interventions, including but not limited to anti-aging treatments. Here, I analyze this phenomenon via interviews with men engaging with two different forms of sexual health medicine in urban Mexico: erectile-dysfunction treatment and testing for sexually transmitted infections (STIs). I argue that, in contrast to the biomedical understanding of patients as individuals who change during their lives, my interlocutors understood themselves as components in broader “collective biologies” that change on a longer timeline. These are culturally-defined groups that people understand to be comprised of interrelated members whose behaviors concretely affect the group’s physical and social well-being over time. In both medical arenas discussed here, men used or rejected sexual health interventions in response to local narratives about the nature of the Mexican population as a collective biology, including ideas about how it should change over time away from its roots in the colonial past. They characterized predispositions to machismo and disinterest in preventative health care as embodied inheritances that the Mexican population should reject in order to achieve health-promoting modernity in the future. My analysis describes how these interlocutors sought to live out desirably modern forms of race and gender through their medical decisions in a way that they hoped would contribute to positive, embodied change in the Mexican social body over time. These findings show that, despite the assumptions of individualism generally naturalized in anti-aging treatment and biomedicine, people may make medical decisions in an effort to aid collective change over population-level timescales.

Background: The Lifetime of Experiences Questionnaire (LEQ) assesses complex mental activity across the life-course and has been associated with brain and cognitive health. The different education systems and occupation classifications across countries represent a challenge for international comparisons. The objectives of this study were four-fold: to adapt and harmonise the LEQ across four European countries, assess its validity across countries, explore its association with brain and cognition and begin to investigate between-country differences in life-course mental activities.Method: The LEQ was administered to 359 cognitively unimpaired older adults (mean age and education: 71.2, 13.2 years) from IMAP and EU-funded Medit-Ageing projects. Education systems, classification of occupations and scoring guidelines were adapted to allow comparisons between France, Germany, Spain and United Kingdom. We assessed the LEQ's (i) concurrent validity with a similar instrument (cognitive activities questionnaire - CAQ) and its structural validity by testing the factors' structure across countries, (ii) we investigated its association with cognition and neuroimaging, and (iii) compared its scores between countries.Results: The LEQ showed moderate to strong positive associations with the CAQ and revealed a stable multidimensional structure across countries that was similar to the original LEQ. The LEQ was positively associated with global cognition. Between-country differences were observed in leisure activities across the life-course.Conclusions: The LEQ is a promising tool for assessing the multidimensional construct of cognitive reserve and can be used to measure socio-behavioural determinants of cognitive reserve in older adults across countries. Longitudinal studies are warranted to test further its clinical utility.

A model of singleness in later life was developed to show how the social context may influence the personal and social resources of older, unmarried persons. The unmarried (especially the divorced) will be an increasing proportion of the aged population in the future, and they will require more services than will the married. Role transitions of the unmarried over the life course, finances, health, and social relationships of older singles are discussed with implications for practice and future research.

Although originally considered to be a disorder of childhood, attention deficit/hyperactivity disorder (ADHD) is increasingly being diagnosed for the first time in adulthood. Yet we know little about the social characteristics (race, gender, and social class) of those first labeled in adulthood, how these differ from those first labeled in childhood/adolescence, and whether the ADHD label is applied proportionately across social groups given ADHD symptomology. Using data from the National Longitudinal Study of Adolescent to Adult Health, the current research considers how typifications of ADHD affect application of the ADHD label in childhood/adolescence and in adulthood. Results indicate that even after controlling for ADHD symptoms, social characteristics are important predictors of the ADHD label in childhood/adolescence but are less influential in predicting ADHD labeling in adulthood. Additionally, results indicate the importance of race in moderating the association between childhood ADHD symptoms and application of the ADHD label throughout the life course.

Analyzing emigration, immigration, and re-migration concurrently, under the framework of contemporaneous migration, directs us toward evaluating what it means to stake claims to different components of citizenship in more than one political community across a migrant’s life course. This chapter examines the way the Mainland Chinese migrants negotiate social reproduction concerns that extend across international borders, their multiple national affiliations, and aspirations for recognition and rights as they journey between China and Canada across the life course. Patterns of re-migration are transforming the social relations of citizenship, re-spatializing rights, obligations, and belonging. Source and destination countries are also reversed during repeated re-migration or transnational sojourning. Transnational sojourning forges citizenship constellations that interlink how migrants understand and experience citizenship across different migration sites.

The inclusion of research impact in the 2014 Research Excellence Framework in the UK (REF2014) was greeted with scepticism by the academic community, not least due to the challenges of defining and measuring the nature and significance of impact. A new analytical framework of the nature of impact is developed in this chapter and it distinguishes between policy creation, direction, discourse and practice. This framework is then applied to the top-ranked impact case studies in the REF2014 from the Social Work and Social Policy sub-panel and the ESRC Early Career Impact Prize Winners in order to assess impact across the life-course of academics.
