Background Polycystic ovarian syndrome (PCOS) is increasingly being diagnosed and treated with sometimes variable lifestyle advice and pharmacological interventions. Obesity is considered as the sole culprit and variable definitions in clinics compound the understanding of pathogenic heterogeneity of this syndrome. We evaluated the differences between various simple to calculate anthropometric indices along with some anthropometric-biochemical equations in subjects with or without PCOS. Objective To compare traditional measures like waist to hip and height ratio (WHpR and WHtR), BMI, newer markers depicting central obesity like Abdominal Volume index(AVI), Body roundness index (BRI), A Body Shape index (ABSI), Conicity index (C-index) along with biochemical-anthropometric equations like lipid Accumulation Products (LAP), Visceral Adiposity Index (VAI) and Chinese Visceral Adiposity Index (CVAI) for diagnosing PCOS as per the Rotterdam criteria Design Cross-sectional analysis Place & Study Duration Naval hospital, Islamabad from Jan- 2018 to July- 2019 Subjects and Methods From our finally evaluated 333 female subjects we initially compared the differences for the presence of hirsutism as per modified Ferrimen Gallwey scores and biochemical hyperandrogenism by measuring free androgen index (Total testosterone/SHBG x 1000. We evaluated waist circumference, BMI, WHpR, WHtR,AVI, BRI, ABSI, C-index along with biochemical-anthropometric equations like LAP, VAI and CVAI for differences in subjects diagnosed to have PCOS by Rotterdam criteria or ultrasonography alone. Results Differences in hirsutism as defined by modified FG score between subjects defined to have PCOS or otherwise as per Rotterdam defined criteria were as [(PCOS=169, Mean=17.33 + 9.05) (No PCOS=164, Mean=8.21 + 5.74), p< 0.001] and ultrasound [(PCOS=87, Mean=16.95 + 9.57) (No PCOS=246, Mean=11.38 + 8.51), p< 0.001]. Similarly, the differences in FAI between subjects defined to have PCOS or otherwise as per Rotterdam criteria and ultrasound were as [(PCOS=169, Mean=6.41 + 4.88) (No PCOS=164, Mean=2.77 + 1.79), p< 0.001] and [(PCOS=87, Mean=5.75 + 5.01) (No PCOS=246, Mean=4.22 + 3.68), p= 0.011]. Anthropometric measures and anthropometric-mathematical equations were raised in non-PCOS subjects than PCOS subjects. Lean-PCOS demonstrated lower degree of hirsutism and biochemical hyperandrogenism in comparison to obese-PCOS. Conclusion Hirsutism and free androgen indices were raised in PCOS females. Anthropometric based measurements were not different in PCOS cases and non-PCOS females. Lean-PCOS demonstrated lower degree of hirsutism and biochemical hyperandrogenism in comparison to obese-PCOS.

Abstract Background: Visceral Adiposity index (VAI) and atherogenic index of plasma (AIP) are relatively new indicators for predicting Non-Communicable disease (NCDs). The aim of this study was to assess the association AIP and VAI with risk of developing cardiovascular diseases (CVDs) in adults.Materials: This was a cross-sectional analysis conducted on 7362 individuals aged 35 to65 years participated in Ravansar Non-Communicable Diseases (RaNCD) cohort study. AIP was calculated based on the value of TG and HDL-C. VAI was calculated using BMI (Body mass index), waist circumference (WC), serum triglyceride, and High density lipoprotein cholesterol (HDL-C).All participants were stratified into three groups based on AIP and VAI tertiles. Simple and multiple logistic regression models were applied to assess the effect of these indices on CVDs.Results: The mean of AIP and VAI was significantly higher in CVDs patients than in non-CVDs (P < 0.001). After adjusting for age, sex hypertension, hyperlipidemia and smoking the risk of CVDs in the second and third tertile of AIP were 1.27 (95% CI: 1.06, 1.52) and 1.63 (95% CI: 1.31, 2.03) times higher comparing to the first tertile, respectively. Risk of CVDs in the second and third tertile of VAI were1.41 (95% CI: 1.18, 1.70) and 1.98 (95% CI: 1.60, 2.46) times higher than the first tertile, respectively; while adjusting for age, sex, hypertension and dyslipidemia.Conclusion: According to the findings, AIP and VAI were positively associated with CVDs in adults. Therefore, AIP and VAI can be useful in identifying high-risk subgroups of CVDs in general population.

Abstract Objective: We aimed to investigate the possible relationship between visceral adiposity index (VAI) and carotid atherosclerosis(CAS) in different genders and age groups in China. Methods: This study was an observational cross-sectional study and included 1996 participants who were health examination population .From January 2018 to June 2019, people over 18 years old who had physical examinations were included in the study. Each participant completed a standard questionnaire, anthropometric measurements, ultrasonic examination ,and provided blood samples for biochemical measurements. Regression models were utilized to evaluate the correlation between VAI and the CAS risk. An ROC curve was utilized to predict VAI diagnostic efficacy for carotid atherosclerosis. Result: After adjusting for potential risks, high-VAI subjects had an increased OR of having CAS in women aged>44 years[OR= 3.09,95% Confdence interval (95%CI)=1.64⑶5.82, p <0.001]. In females, the AUC and sensitivity specificity were [(0.595, 76.68% and 39.85%, all females), (0.575, 78.51% and 30.37%, age ≤44), (0.609, 84.71% and 40.46%, age >44). When compared with traditional obesity indices(BMI, WC, and HC) , the predictive ability of VAI was stronger in females >44.The same relationships in males were not signifcant. Conclusions: The VAI were associated with an increased risk of CAS,and it could be selected as new and simple predictors of CAS for middle-aged and elderly women(age >44)in China .

Risk of CVDs in the second and third tertile of VAI were 1.28 (95% CI: 1.06, 1.53) and 1.52 (95% CI: 1.25, 1.83) times higher than the first tertile, respectively; while adjusting for age, sex, hypertension and dyslipidemia and smoking. Conclusion According to the findings, AIP and VAI were positively associated with CVDs. Therefore, AIP and VAI can be useful in identifying high-risk subgroups of CVDs in general population.

The increased visceral adiposity index associated with metabolic syndrome in obese adolescentsBackground: Metabolic syndrome is a complex risk factor of cardiovascular disease and diabetes mellitus type II. Metabolic syndrome scores are calculated to describe the severity of the risk of metabolic syndrome. Adipose tissue is an independent risk factor for metabolic changes related to metabolic syndrome. One indicator to calculate the distribution and fat function is through the calculation of the visceral adiposity index (VAI).Objective: Analyzing the correlation of VAI with metabolic syndrome score in adolescents in the City of Semarang.Methods: A cross-sectional study of 95 obese adolescents in Semarang City aged 12-17 years who were selected by consecutive sampling method. The inclusion criteria were students aged 12-17 years; subjects were obese based on BMI for age (BMI/U > 95th percentile CDC 2000 curves); not in chronic pain or the care of a doctor; and not on a certain diet. Data collected were waist circumference (WC), blood pressure, fasting blood sugar levels, triglyceride levels, and high-density lipoprotein (HDL) levels. Metabolic syndrome scores are calculated using metabolic syndrome risk score (cMets). VAI is calculated to measure fat distribution and dysfunction. Correlation of VAI with metabolic syndrome and the component was analyzed with the Spearman test.Results: VAI score has a range of 1.8-14.9 with a greater mean in female subjects. The metabolic syndrome score has a range of -5.9 to 6.3 with a greater mean in male subjects. The Spearman correlation test showed a significant relationship between VAI and HDL levels (r=-0.427), triglyceride levels (r=0.914), and metabolic syndrome scores (r=0.439) in male, and HDL levels (r=-0.427), triglyceride levels (r=0.955), WC (r=0.346), and metabolic syndrome scores (r=0.691) in female.Conclusions: VAI has a significant relationship with the metabolic syndrome score which indicates the body's metabolic profile which is getting worse, so the body's metabolic profile can be described from the magnitude of VAI.
