The purpose of this article is to illustrate how mixed methods research can be applied as a rigorous method for developing a quantitative instrument responsive to an indigenous culture in Malaysia. In a sequential research design, a three-round Delphi study was conducted to integrate open-ended and free-listing data, and to generate a list of items. The items were subjected to a field test to ensure reliability. A correspondence analysis of scale was employed to examine the validity of the instrument. This research contributes to mixed methods by illustrating the value of Delphi groups as part of a mixed methods research design and the utility of mixed methods research in advancing the validity and reliability of an indigenous cultural values instrument.

BACKGROUND The development of an integrated care pathway with multidisciplinary input to standardize and streamline care for pregnant women experiencing breech presentation at 36 or more weeks of gestation poses several challenges because of the divisive and contentious nature of the phenomenon. Although many clinicians are interested in obtaining the skills required to safely support women desiring a vaginal breech birth, the primary trend in most health care facilities is to recommend a cesarean section. OBJECTIVE This paper aims to discuss the mixed methods approach used in a doctoral study conducted to generate new knowledge regarding women’s experiences of breech birth in Western Australia and professional recommendations regarding the care of women experiencing breech presentation close to or at term. This study was designed to inform the development of an integrated care pathway for women experiencing a breech presentation. This mixed methods approach situated within the pragmatic paradigm was determined to be the optimal way for incorporating multidisciplinary recommendations with current clinical practice guidelines and consumer feedback. METHODS A mixed methods study utilizing semistructured interviews, an electronic Delphi (e-Delphi) study, and clinical practice guideline appraisal was conducted to generate new data. The interviews were designed to provide insights and understanding of the experiences of women in Western Australia who are diagnosed with a breech presentation. The e-Delphi study explored childbirth professionals’ knowledge, opinions, and recommendations for the care of women experiencing breech presentation close to or at term. The clinical practice guideline appraisal will examine the current national and professional breech management and care guidelines. This study has the potential to highlight areas in practice that may need improvement and enable clinicians to better support women through what can be a difficult time. RESULTS Data collection for this study began in November 2018 and concluded in March 2020. Data analysis is currently taking place, and the results will be disseminated through publication when the analysis is complete. CONCLUSIONS The results of this study will guide the development of an integrated care pathway for women experiencing a breech presentation close to or at term, with the hope of moving toward standardized breech care for women in Western Australia. IntroductionNursing home residents typically have greater needs for medical care than community-dwelling elderly. However, restricted cognitive abilities and limited mobility may impede their access to general practitioners and medical specialists. The provision of medical care in nursing homes may therefore be inappropriate in some areas of medical care. The purpose of this mixed-methods study is to systematically assess, evaluate and explain met and unmet medical care needs in German nursing homes and to develop solutions where medical care is found to be inappropriate.Methods and analysisFirst, statutory health insurance claims data are analysed to identify differences in the utilisation of medical care between nursing home residents and community-dwelling elderly with and without need for long-term care. Second, the health status and medical care of 500 nursing home residents are assessed and evaluated to quantify met and unmet medical care needs. Third, qualitative expert interviews and case conferences and, fourth, quantitative analyses of linked data are used to provide structural, case-specific and generalisable explanations of inappropriate medical care among nursing home residents. Fifth, a modified Delphi study is employed to develop pilot projects aiming to improve medical care in nursing homes.Ethics and disseminationThis study was approved by the Ethics Committee of the University of Bremen on 23 November 2017. Research findings are disseminated through presentations at national and international conferences and publications in peer-reviewed scientific journals.Trial registration numberDRKS00012383.

While recent research explores the dark side of entrepreneurship there remains little exploration of what, if any, role entrepreneurship education (EE) plays in producing dark side outcomes. Accordingly, we explore dangers and unintended consequences EE poses to students and universities as well as the role of both student and instructor in mitigating dark side concerns. Specifically, we use a Delphi technique to distill from a panel of experienced educators the dangers EE may pose as well as strategies for mitigating dark side consequences. In sum, this work contributes to EE research by providing a unique mixed-methods view of the dark side of EE and raising awareness of the unintended consequences borne by the students and institutions we serve.

Nowadays, technologies play a key role in educational research processes, especially in the context of complex designs or integrative perspectives (qualitative and quantitative). Affordances provided by new tools and technological devices are constantly impacting the way educational research is carried out. Consequently, this growing importance (and dependence on) technology for educational research calls for a deeper reflection, not only about its clear benefits, but also about its potential drawbacks and limitations. This paper explores this tension in a specific mixed-methods research design aimed at understanding the barriers preventing the adoption of Learning Design (LD) tools/methods, a significant problem in the research field of Technology Enhanced Learning (TEL). Different actors (teachers and researchers) were involved in the research design, which included a systematic literature review, a Delphi study and a case study. Such an articulated design required the adoption of technologies to support the process in all its phases. The paper describes the main methodological implications of the employed technologies in the different methods applied and in the overall research. The conclusions of the paper highlight that while technologies definitely support a higher level of complexity and articulation of the research design, they might also lead to superficial or biased results. En la actualidad las tecnologías desempeñan un papel fundamental en los procesos de investigación educativa, especialmente cuando se realizan diseños complejos o desde perspectivas integradoras (Cuantitativas y Cualitativas). Las facilidades ofrecidas por nuevas herramientas y dispositivos tecnológicos influyen constantemente en la forma en que se lleva a cabo la investigación educativa. Por consiguiente, esta creciente importancia de la tecnología para la investigación educativa exige una reflexión más profunda, no sólo sobre sus evidentes beneficios, sino también sobre sus posibles inconvenientes y limitaciones. En este artículo se explora esa tensión en un diseño de investigación desde una estrategia mixta centrada en la identificación y comprensión de las barreras que impiden la adopción de herramientas/métodos de Diseño de Aprendizaje, un problema importante en el campo del aprendizaje mejorado por tecnología (Technology Enhanced Learning). Diferentes actores (profesores e investigadores) han participado en el diseño de la investigación, que incluye una revisión sistemática de la literatura, un estudio Delphi y un estudio de caso, generando un diseño muy articulado con tecnologías para cada fase. En el documento se describen las principales repercusiones metodológicas de las tecnologías empleadas en los diferentes métodos aplicados y en la investigación en general. Las conclusiones del documento ponen de relieve que, si bien las tecnologías apoyan en gran medida un nivel alto de complejidad y ayudan a articular el propio diseño, también pueden dar lugar a resultados superficiales o sesgados.

Background: Specialist palliative care services have various configurations of staff, processes and interventions, which determine how care is delivered. Currently, there is no consistent way to define and distinguish these different models of care. Aim: To identify the core components that characterise and differentiate existing models of specialist palliative care in the United Kingdom. Design: Mixed-methods study: (1) semi-structured interviews to identify criteria, (2) two-round Delphi study to rank/refine criteria, and (3) structured interviews to test/refine criteria. Setting/participants: Specialist palliative care stakeholders from hospice inpatient, hospital advisory, and community settings. Results: (1)  Semi-structured interviews with 14 clinical leads, from eight UK organisations (five hospice inpatient units, two hospital advisory teams, five community teams), provided 34 preliminary criteria. (2)  Delphi study: Round 1 (54 participants): thirty-four criteria presented, seven removed and seven added. Round 2 (30 participants): these 34 criteria were ranked with the 15 highest ranked criteria, including setting, type of care, size of service, diagnoses, disciplines, mode of care, types of interventions, ‘out-of-hours’ components (referrals, times, disciplines, mode of care, type of care), external education, use of measures, bereavement follow-up and complex grief provision. (3)  Structured interviews with 21 UK service leads (six hospice inpatients, four hospital advisory and nine community teams) refined the criteria from (1) and (2), and provided four further contextual criteria (team purpose, funding, self-referral acceptance and discharge). Conclusion: In this innovative study, we derive 20 criteria to characterise and differentiate models of specialist palliative care – a major paradigm shift to enable accurate reporting and comparison in practice and research.
