Abstract The possibility of use of Failure Mode and Effect Analysis methods is connected with continuous quality improvement of production processes in companies. Interdependence of the quality research methods and production process’s requirements have been taken into account. FMEA method is the analysis aimed at the disclosure and elimination of potential weaknesses of the process, that is the variants that may cause defects during its implementation. The example of implementing Failure Mode and Effect Analysis in polish companies have been presented.

This paper intends to present the application of FMEA method on Three-Way Catalytic Converter (TWC) system. Catalytic converter of auto-exhaust emission is one of the most successful applications of heterogeneous catalysis, both in commercial and environmental point of view. FMEA method will be applied to this system to quantitatively determine and evaluate its risk factors. This method is being employed effectively for identifying and addressing what potentially could go wrong with a product or process. It is expected to enhance the lifetime of the TWC by improving its resistance to deactivation. It is widely accepted that FMEA is one of the best quality improvement tool. For the last several decades, FMEA has been widely used in industry especially in automotive sectors. This research will cover mostly on the system and design of the TWC itself as the most important part for controlling the exhaust emission from automobiles. By improving its resistance to deactivation will contribute to longer lifetime of automotive catalytic converter.

Continuous quality improvement is essential to ensure the competitiveness of manufacturing industries. Design of Experiments, Statistical Process Control and Failure Mode and Effect Analysis are common tools for improving quality. This study proposes a new tool that could encompass quality improvement steps comprehensively, namely Quality Improvement Matrix. This tool integrates several techniques which are House of Quality, Failure Mode and Effect Analysis (FMEA) and Root Cause Analysis. Quality Improvement Matrix covers identification of defects and its root causes, measuring the magnitude of defects (source of defects), formulating alternatives of solutions for reducing root cause of defects and selecting effective solutions to be implemented. Quality Improvement Matrix is developed by following a similar approach as House of Risk. Thus, it also consists of two matrixes that represent two major stages. The first stage is the identification and prioritizing root causes of defect. Then, the second stage is formulating and selecting solutions for improvements.

AbstractThe article deals with the research of operational reliability of forest felling machines by FMEA method (Failure Mode and Effect Analysis). It describes collection of operational data and its analysis. It explains the procedure of realization for the method FMEA in the organization. Harvesters John Deere 1070D in the Company Lesy SR B. Bystrica were chosen for this research. The research was held in real operational conditions. Application of the FMEA method allows flexibility in case of unexpected situations and optimization of human potential abilities. FMEA tool is a tool preventing outages operational reliability and preventive tool for ensuring the maintenance of facilities. The method of information analysis mentioned below is simple ale precise enough for implementation in real working conditions.

BACKGROUND: Supporting employment consultants in their work with job seekers is critical for increasing the employment outcomes of people with disabilities. OBJECTIVE: To better understand how to leverage data for supporting employment consultants, including what metrics to track, what to do with the data, and what can be improved. METHODS: A panel of three directors of employment programs addressed these questions as part of the Association of People Supporting Employment First (APSE) 2020 conference. RESULTS: Most employment service providers collect data for billing and compliance reporting. Innovative providers leverage data for quality improvement. CONCLUSIONS: Tracking metrics designed specifically for monitoring the implementation of effective employment supports is key for leveraging data for continuous quality improvement and thus improving job seekers’ employment outcomes.

Healthcare organizations engage in continuous quality improvement to improve performance and value-for-performance, but the pathway to change is often rooted in challenging the way things are “normally” done. In an effort to propel system-wide change to support healthy eating, Nova Scotia Health developed and implemented a healthy eating policy as a benchmark to create a food environment supportive of health. This article describes the healthy eating policy and its role as a benchmark in the quality improvement process. The policy, rooted in health promotion, sets a standard for healthy eating and applies to stakeholders both inside and outside of health. We explain how the policy offers nutrition but also cultural benchmarks around healthy eating, bringing practitioners throughout Nova Scotia Health together and sustaining collaborative efforts to improve upon the status quo.
