AbstractBackgroundSleep disturbances in intensive care unit (ICU) patients have been studied worldwide for over 30 years. Factors contributing to sleep disturbances are still being identified, and therapeutic procedures aimed at the mitigation of such ailments are consequently being developed.ObjectivesThe aim of this study was to review the literature on sleep disturbances in intensive care unit patients.Material and MethodsMEDLINE PubMed, OVID, Web of Science, and EBSCO databases have been searched using adequate keywords.ResultsSleep disorders in ICUs were common among all of the analysed articles. Noise plays a significant role in sleep interruption (11.5 - 17% of awakenings). It was noted that the introduction of “white noise” into the ICU environment proved unsuccessful in reducing the magnitude of changing noise levels. Nursing care activities significantly disturb nocturnal rest, and 42.7 such procedures per every 12-hour night shift were registered. Aggregating nursing care interventions was suggested in order to reduce the number of stimuli experienced by the patient.ConclusionChanges in sleep structure developing during an ICU stay may significantly contribute to sleep disorders once the hospitalisation is over.

Insomnia is one of the most common clinical syndromes of somnological disorders, including any sleep disorders under sufficient time and appropriate sleeping conditions. Significant sleep disturbances are common in children who enter the intensive care unit. Between 18 and 65 per cent of children aged between 2 and 18 hospitalized in the intensive care unit have a clinical condition of acute confusion, better known as delirium. Sleep fragmentation, reduction of total sleep time, lack of slow, deep sleep and rapid sleep, and shift of sleep time are changes that can also act as insomnia triggers. Their presence worsens the course of the disease in the patient, prolongs the hospital stay, and increases the risk of complications and lethality. Circadian sleep disorder is a condition that requires close monitoring. Although insomnia therapy approaches are not yet reliable, early diagnosis of insomnia can reduce the duration of the delirium. Sleep assessment includes questionnaires and diaries. However, it was recognized that, despite the difficulties in implementing and interpreting polysomnography, it was still a gold standard for more accurate detection of changes in sleep phases and the difference between normal and abnormal sleep. In the absence of a polysomnograph, it is permissible to use a prolonged video assisted EEG. The inventive multimodal video-assisted monitoring method makes it possible to diagnose sleep disorders, pain syndrome and differential diagnosis of apnoea with cardiac and conductivity disorders or abnormal brain activity, including the presence of sub-clinical convulsions in patients of high-risk groups using a safe and non-invasive method for analyzing trends in the total bioelectric activity of different biological structures of a child. The validation of questionnaires according to the age of the child is also an important step in the timely diagnosis of sleep disorders in older children.

The neonatal intensive care unit (NICU) is, inherently, a trauma environment for the extremely premature infant. This trauma is often exacerbated by nurse caregiving practices that can be modified and still remain effective. Our study explored how behavior analytics could be used to implement an intervention known as Care by Cues and how the intervention might, ultimately, impact infant physiologic stability.

Objective: The objective of this study was to investigate the nurses’ approach to three sources of sound that contribute to high noise levels; alarms, doors that open and conversation.Methods: Methods used derived from a theoretical perspective based on interpretive phenomenology and caring culture. In the pediatric intensive care, the caregivers of the children work in a high-tech environment as they are surrounded by sound from several sources. How caregivers understand and acknowledge how these sounds negatively affect a child’s well-being depends on their individual knowledge and awareness of how children are affected by sound. In most cases, coming into an intensive care unit is a new experience for a child. This causes greater stress, both from the environment itself as well as from sound levels. The method was built on a phenomenological perspective and an interpretive non-participation, semi-structured observations were conducted in a pediatric intensive care unit (PICU) of one of Sweden’s metropolitan regions in the winter of 2014-2015.Results: The results show that noise is an overlooked phenomenon in the pediatric intensive care environment as it has given way to other priorities in the nurse’s work. It is also apparent that this depends on the department’s caring culture as it prioritizes other things, resulting in normalizing high levels of noise as a part of the pediatric intensive care environment.Conclusions: Noise levels are not a priority in the department’s caring culture. High noise levels are permitted unreflectedly and appears to be a token of potency and an accepted part of the health care environment.

Mapear as evidências científicas sobre alterações oculares em pacientesinternados em terapia intensiva. Trata-se de uma revisão de escopo, com adoção da estratégia PPC nas bases de dados PUBMED, WEB OF SCIENCE e a biblioteca virtual SCIELO. Adotou-se como elegiveis, estudos primarios, empiricos, quantitativos e qualitativos, publicados em três idiomas. Foram encontrados 204 artigos e após análise, selecionou-se 08 artigos para compor o estudo. A lagoftalmia apresenta-se como principal alteração ocular e fator determinante para o desenvolvimento de danos oftálmicos. Evidencia-se que o ambiente, a terapêutica instituída e as próprias comorbidades do paciente possuem associação com as alterações oculares. As doenças oculares são passíveis de prevenção. O diagnóstico precoce de alterações oculares e a rastreabilidade dos fatores de risco para doenças oculares são medidas necessária para evitar ocorrência de dano ocular aos pacientes internados em terapia intensiva.Descritores: Síndrome do Olho Seco, Doenças da Córnea, Unidade de Terapia Intensiva. Eye changes in an intensive care unit: scoping reviewAbstract: To map the scientific evidence on ocular changes in patients in intensive care. Objetivo: Compreender a ótica dos profissionais de enfermagem sobre a hospitalização de adolescentes. Metodologia: Pesquisa qualitativa, descritiva, utilizando entrevista com perguntas semi-estruturadas, realizada em uma enfermaria especializada em saúde do adolescente no Rio de Janeiro, no período de fevereiro a abril de 2016, com 15 profissionais de enfermagem. Resultados: A partir da análise de conteúdo de Bardin, definiram-se três categorias: a inexperiência no cuidado de enfermagem ao adolescente como um desafio, especificidades da adolescência, e sentimentos gerados pela hospitalização do adolescente no profissional de enfermagem. Conclusão: Conclui-se que os profissionais de enfermagem se colocaram inexperientes em cuidar desse público e declararam que o cuidado a essa população é um desafio para a equipe.Descritores: Adolescente, Enfermagem, Hospitalização.THE ADOLESCENT’S HOSPITALIZATION IN THE OPTICS OF NURSING PROFESSIONALSObjective: Analyze the contents of nursing notes in patients’ records in an intensive care unit (ICU) of a public tertiary hospital in Fortaleza, CE. It is a descriptive study with quantitative approach that analyzed 151 medical records of patients admitted to an ICU, from September 2014 to February 2015, whose 48 hour and discharge notes were evaluated. Data revealed notes with poor content, which did not express the patients’ situation, nor the nursing care provided. Data concerning date, time, and patient identification were adequate. However, the COREN number and the professional’s signature raised concern given the high percentage of non-compliance regarding ethical and legal aspects.
