Prevalence and causes of unreported needle stick injuries among health care workers: a systematic review and meta-analysis

Abstract Objectives Health care workers (HCWs) are exposed to needle needles daily. Despite individual studies, there is no statistics on the prevalence of unreported needle stick injuries (NSIs) have been reported. This study was performed to determine the prevalence and causes of unreported NSIs among HCWs. Content In present systematic review and meta-analysis study, three international databases (Web of Science, Scopus, PubMed) were searched from January 1, 2000 to December 31, 2018. The random model was used to determine the prevalence of unreported needle stick among HCWs. Summary and outlook Forty-one studies performed on 19,635 health care workers entered the final stage. Based-on random effect model, pooled prevalence of unreported needle stick injuries was 59.9% (95% CI: 52.0, 67.7; I2=98.9%). The most common cause of unreported NSIs was: They were not worried about NSIs (n=12). The high prevalence of unreported needle sticks injuries indicates the urgency and necessity of paying attention to strategies to improve reporting among health workers.

Background: Needle stick injury among health workers is regarded as an occupational hazard. Health care workers are at risk of having blood-borne diseases in case they are exposed to blood and other biological samples of the patients. Moreover, staff including doctors working in tertiary care hospitals has high work load which results in increased chances of getting these injuries. The aim and objectives of this study were to find out the prevalence of needle stick injury among different categories of health care workers. Authors also aimed to assess the knowledge, attitude and practices associated with it.Methods: A hospital based cross sectional study design to know the prevalence of needle stick injury among various health care workers of Karpagam Faculty of Medical Sciences and Research, a tertiary level care hospital in Coimbatore, Tamilnadu, India. A total of 250 health care workers were selected for the study purpose. A self-designed, semi-structured, pre-tested questionnaire was used to assess the prevalence of needle stick injuries and the factors associated with it.Results: A majority of health care workers (94%) knew about needle stick injury and 92% were aware that HIV can be transmitted through needle stick injury, 78.4% and 69.65% were aware of Hepatitis-B and Hepatitis-C transmission respectively. About 28.4% of subjects had encountered needle stick injury in their past. Furthermore, it was found that type of exposure and place of exposure was significantly associated with different categories of health care workers (p <0.001).Conclusions: Prevention of health workers against needle stick injury is the best possible way to prevent several bloods borne diseases. There should be a prevention programme which special focus on training of health care workers. Further strategies aiming at preventive measures and reporting of the Needle stick injuries accidents should be made compulsory among health care workers.

Background: The objective of study was to determine prevalence of needle stick injuries among health care workers in a tertiary care hospital of Assam and to assess the knowledge, attitude and practices on needle stick injuries among them.Methods: It was a cross-sectional study conducted from June 2016 to August 2016 amongst health care workers of a tertiary care centre of Assam. Sample size was calculated to be 90. A total of 10 departments were selected purposively based on their magnitude of risk exposure and the required sample size was allocated proportionally among these 10 departments. The required numbers of health workers from each of the ten departments were selected by using simple random sampling. A predesigned and pretested proforma was used to collect the data. Data were presented in terms of percentages and significance was tested using chi-square and Fisher’s exact test.Results: Prevalence of needle stick injury among the health workers was found to be 21.1%. Regarding spread of diseases after an accidental needle stick injury, 100% were aware of HIV, 98.9% were aware of Hepatitis B and 67.8% were aware of Hepatitis C. Regarding their attitude to report to in-charge medical officer, only 21.1% had reported about their injury. Around 58.9% of health care workers used gloves regularly for prevention of such injuries. Practice of recapping among the health care workers was 66.7% and out of total needle stick injuries, recapping contributed to 26.3%. While considering sharp waste disposal, 37.8% health care workers did not follow proper waste disposal guidelines. Only 26.3% of health care workers had done screening for HIV/AIDS and Hepatitis B after their injury.Conclusions: There is a need to have a pre-employment training to improve the knowledge, attitude and practice regarding accidental needle stick injuries among the health care workers.

AbstractBackgroundConsidering medical and economic burden of the coronavirus disease 2019 (COVID-19), a high COVID-19 vaccination coverage among health care workers (HCWs) is an urgent need.ObjectiveTo estimate the intention of HCWs to accept COVID-19 vaccination and to find out related factors.DesignWe followed the Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines for this study.Data sourcesWe searched PubMed, Medline, Scopus, Web of Science, ProQuest, CINAHL and medRxiv from January 1, 2020 to December 02, 2020.MethodsThe heterogeneity between results was very high and thus we applied a random effect model to estimate pooled effects. We performed subgroup and meta-regression analysis to identify possible resources of heterogeneity.ResultsEleven studies, including 8847 HCWs met the inclusion criteria. The overall proportion of HCWs that intent to accept COVID-19 vaccination was 55.9% (95% CI: 43.6-67.9%) with a wide range among studies from 27.7% to 81.5%. Intention of HCWs to accept COVID-19 vaccination was higher in studies with moderate quality and in studies that were conducted in Europe. The following factors were associated with increased HCWs’ willingness to get vaccinated against COVID-19: male gender, older age, physician profession, fewer work experience, comorbidity among HCWs, seasonal influenza vaccination, stronger vaccine confidence, positive attitude towards a COVID-19 vaccine, fear about COVID-19, individual perceived risk about COVID-19, and contact with suspected or confirmed COVID-19 patients.ConclusionsHCWs represent a high-risk group for SARS-CoV-2 infection. The reluctance or refusal of HCWs to vaccinate against COVID-19 could diminish the trust of individuals and trigger a ripple effect in the general public. Since vaccination is a complex behavior, understanding the way that HCWs take the decision to accept or not COVID-19 vaccination will give us the opportunity to develop the appropriate interventions to increase COVID-19 vaccination uptake. Knowledge of the factors that affect intention of HWCs to accept COVID-19 vaccination is limited and there is an urgent need for further and more valid studies.

AbstractObjectiveWorldwide countries are experiencing viral load in their population, leading to potential infectivity of asymptomatic COVID-19. Current systematic review and meta-analysis aimed to investigate the role of asymptomatic infection worldwide reported in family-cluster, adults, children, health care workers, and travelers.DesignOnline literature search (PubMed, Google Scholar, medRixv, and BioRixv) was accomplished using standard Boolean operators, studies published till 07th June 2020.SettingStudies were included from case reports, short communication, and retrospective to cover sufficient asymptomatic COVID-19 transmission reported.ParticipantsFamilial-clusters, adults, children, health care workers, and travelers.ResultsWe observed asymptomatic transmission among familial-cluster, adults, children, health care workers, and travelers with a proportion of 32% 37%, 26%, 6%, and 32%, respectively. This study observed an overall proportion of 31% (95%CI: 0.19-0.44) with heterogeneity of I2 (97.28%, p=<0.001) among all asymptomatic populations mentioned in this study. Among children and healthcare workers, this study showed no heterogeneity; to overcome the interpretation from a fixed model, the random effect model was also applied to estimate the average distribution across studies included in the meta-analysis.ConclusionWe found and suggest the rigorous epidemiological history, early isolation, social distancing, and increased quarantine period (at least 28 days) after screening asymptomatic cases as well as their close contacts for chest CT scan even after their negative nucleic acid testing to minimize the spread among the community. This systematic review and meta-analysis support asymptomatic COVID-19 transmission between person to person depending on the variation of virus incubation period among individuals. Children especially, school-going aged <18 years, need to be monitored and prevention strategy, e.g., chest CT and social distancing required to prevent the community transmission of COVID-19 in asymptomatic mode.Strengths and limitations of this studyExamine the possibility of asymptomatic COVID-19 transmission in the community at different levels.Supports contact tracing, social distancing, early isolation, and increased quarantine period to minimize the risk of virus spread.Supports chest CT scan and viral nucleic acid testing to identify the asymptomatic cases in the community.Supports rigorous epidemiological history with multiple detection methods.A higher proportion of asymptomatic incidence was seen, suggests monitoring, and maintaining social distancing.

BACKGROUND: Needle stick injuries are a dangerous occupational hazard that threatens health care workers with serious consequences in many cases. Because of inadequate reporting of these incidents, the true magnitude of the problem is unknown. The study&rsquo;s aim was to investigate the epidemiology of needle stick injuries (NSIs) among health care workers in Al Madinah Al Munawara, Saudi Arabia. METHODS: A representative sample of health care workers (n=268) were randomly selected from emergency departments of three general hospitals in Al Madinah Al Munawara for a cross-sectional study, and 219 responded to a predesigned questionnaire reflecting exposure to NSI, reporting, post-exposure reactions, and knowledge about NSIs. RESULTS: Almost one third of the participating medical professionals 70 (32%) had been exposed to stick injury during work. One half of the needles (52.9%) were blood stained. Nurses are significantly more likely to have stick injury (38.6%) followed by physicians (30.4%) if compared to laboratory technicians (13.9%) at the p-value of less than 0.05. The main purposes during injuries were drawing blood sample 33 (47.1%), injection 31 (44.3%) and suturing 23 (32.9%). Almost all injured personnel (97.1%) cleaned the injury site immediately and thoroughly. Out of the injured personnel, 50 (71.4%) reported the incident to authorities. A significantly higher proportion of physicians (91.3%) achieved above average score if compared to both lab technicians (72.2%) and nurses (76.3%) at the p-value of less than 0.05. CONCLUSION: Occupational needle stick injuries are fairly common among health care workers at Al Madinah Al Munawara&rsquo;s governmental hospitals. In hospitals, deliberate efforts should be made to ensure adherence to safety guidelines governing needle stick injuries.
