Introduction: Melasma is one of the most common skin pigmentation disorders, which mostly affects the facial skin and has a considerable psychological impact on the patients. Melasma management has been one of the controversial issues in dermatology. We aimed to compare the combined treatment of the Er: YAG (erbium: yttrium–aluminum–garnet) laser plus hydroquinone (HQ) 4% with HQ 4% alone in the treatment of melasma. Methods: Twenty-nine patients were treated with the combined Er: YAG laser and HQ 4% on one side of the face with HQ 4% alone on the other side. Three sessions of the laser rat 4-week intervals. The outcome was calculated using the Melasma Area Severity Index (MASI). Results: The side that received the combined treatment (laser + HQ 4%) showed a statistically significant reduction in MASI compared to the side treated with HQ 4% alone. Conclusion: Our study suggests the superiority of the combination of the Er: YAG laser and HQ 4% in the treatment of melasma compared to HQ 4% alone.

Melasma is a common hypermelanotic disorder affecting the facial area which has a considerable psychological impact on the patient. Managing melasma is a difficult challenge that requires long-term treatment with a number of topical agents, such as rucinol and sophora-alpha. Aims. We aim to compare the combined treatment of skin needling and depigmenting serum with that using depigmenting serum alone in the treatment of melasma, in order to evaluate the use of microneedles as a means to enhance the drug’s transdermal penetration. Methods. Twenty patients were treated with combined skin needling and depigmenting serum on one side of the face and with depigmenting serum alone on the other side. The outcome was evaluated periodically for up to two months using the Melasma Area Severity Index score and the Spectrocolorimeter X-Rite 968. Results. The side with combined treatment (skin needling + depigmenting serum) presented a statistically significant reduction in MASI score and luminosity index (L) levels compared to the side treated with depigmenting serum alone, and clinical symptoms were significantly improved. Conclusions. Our study suggests the potential use of combining skin needling with rucinol and sophora-alpha compounds to achieve better results in melasma treatment compared to rucinol and sophora-alpha alone.

AbstractIntroduction. Melasma is a common skin pigmentation disorder affecting a patient’s life psychologically and socially. Topical medications or lasers can have temporary and limited therapeutic effects on melasma. Material and Methods. This study is a prospective clinical trial comparing therapeutic effects of oral Tranexamic acid (TXA) and topical Hydroquinone (HQ) cream. A total number of 69 patients were examined. During the study, 10 patients failed to appear for the follow-up and 59 of them completed the trial. The patients were also divided randomly into two groups. Group A received TXA capsule 250 mg every 12 hours and group B received 4% topical HQ cream day and night. The patients from both groups were treated for 3 months. Melasma Area and Severity Index (MASI) scores were then calculated at the baseline, 4 weeks, and 12 weeks into the treatment and 3 months after the end of intervention. Results. MASI baseline, 4 weeks,12 weeks, and 24 weeks in TXA group were 21.66, 13.69, 9.10, 9.24; respectively. Reduction of MASI between baseline and 4 weeks was statistically significant. Such a decreasing trend in MASI scores between baseline and 12 weeks was also reported as statistically significant (p=0.001). In the HQ group, MASI baseline,4 weeks, 12 weeks, and 24 weeks were 21.46, 13.57, 10.93, 11.20; respectively. Reduction of MASI scores between baseline and 4 weeks was statistically significant. Moreover, a decline in MASI scores was observed between baseline and 12 weeks that was statistically significant (p=0.001). Considering both groups MASI scores were reduced but the difference between two study groups was not statistically significant (p=0.98). Conclusion. The efficacy of TXA and HQ was the same and both could significantly reduce MASI scores.

An acne vagary is a chronic inflammatory condition of the pilo-sebaceous follicles on the face and upper trunk. It is characterized by the development of black heads, papules, pustules and cysts, which may resolve to leave pitted scars. In Ayurvedic classics we get the knows about this disease in the name of Yuvan pidika, the eruptions like Salmali thorn, on the face during adulthood, caused by Kapha, Vata and Rakta are known as 'Yuvan pidika'.

Hidrosadenitis supurativa is a chronic inflammatory disease with great physical and psychological impact. Although conservative treatments may be effective in mild forms of the disease, extensive surgical resection and reconstruction are necessary in more severe forms of the disease. The purpose of this paper is to describe our two-stage reconstructive procedure regarding this kind of disease. We present a clinical case of a patient with severe, bilateral axillary hidrosadenitis. In the first surgical step we excised the lesions and applied the artificial dermis secured with negative pressure wound therapy. In the second step we used a split thickness skin graft to close the wound and again applied negative pressure wound therapy. The graft take was very good, without complications. The cosmetic outcome is acceptable and shoulder mobility was not compromised. No recurrence was detected (nine months follow up).

Background: Laser therapy can be used as an alternative treatment for onychomycosis; however, there are some limitations to its efficacy as a single agent. Objective: To evaluate the effectiveness of combination therapy with 1,064-nm neodymium-doped yttrium aluminum garnet (Nd:YAG) laser and topical efinaconazole in onychomycosis treatment and identify factors influencing the therapeutic outcomes of combination treatment. Methods: Big toenails with onychomycosis were treated by 1,064-nm Nd:YAG laser at 4-week intervals with daily application of topical efinaconazole. Therapeutic response was assessed through onychomycosis severity index (OSI) and percentage of nail infected (PNI), and its association with a variety of factors that may affect treatment outcome was evaluated. Results: One hundred big toenails were included in the study and significant clinical improvements were observed after treatment (OSI improvement score = 76.68 ± 28.83, PNI improvement score = 72.37 ± 30.37). There was no difference in treatment response according to the number of laser treatments, onychomycosis clinical type, or initial severity. However, patient age was negatively correlated with clinical improvement (p = 0.019). Also, female patients had better therapeutic responses than male patients. Conclusion: Combined treatment with Nd:YAG laser and topical efinaconazole has a significant therapeutic effect on onychomycosis. A randomized controlled trial is warranted in the future.

The present phenomenon has clearly brought a change and the influence of the development of traditional music in Palembang society, if this is not carefully addressed, it will experience a shift, alienation and even lose its supporters. The rejung pesirah music group is one of the music groups that still maintains traditional arts in the people of Palembang. This study examines how the adaptation strategy of the rejung pesirah music group in dealing with changes and developments in Palembang society. For this reason, the approach used is cultural anthropology with qualitative case study research methods in Palembang. Data collection is done through observation, interviews and document studies that use triangulation techniques as the validation of the data, while for data analysis through content analysis and interactive models. The results showed that the adaptation strategy undertaken by the rejung pesirah music group was an act and creative ability and had a positive mindset, understanding in responding to changes and needs as an impulse to develop in the face of environmental change and development through learning processes and cultural modification, which resulted a creativity that is the creation of songs, musical arrangements, and musical instruments in the rejung pesirah music group.

: In this letter, the psychological impact of COVID-19 on cancer infected patients is discussed. Cancer is a serious health-related problem in the human body nowadays. The 2019 pandemic of coronavirus disease has developed into an unheard-of pandemic. Given the havoc wreaked by this pathogen worldwide, many countries have implemented a severe, legally enforced method of social distancing, in the form of a lockdown. Unless adequate preventive measures are taken, the cost of the pandemic and subsequent lockdown can prove to be irreparable. The obvious consequences of this lockout, such as the escalating levels of unemployment, imminent economic crisis, and extreme food scarcity faced by the sudden unemployed migrant labour population, have been widely reported. Cancer patients are a highly vulnerable group even during non-pandemic periods, often presenting late in the course of their illness, without the services required to avail recommended care. The incidence of psychological complications and emotional distress is considerably higher than in the general population, and the trauma of both the pandemic and subsequent lockdown contributes significantly to their mental trauma. This analysis is geared at solving the challenges faced by cancer patients in the face of this pandemic and subsequent lockdown, with a look at potential solutions that can be enforced.
