Ayurveda is the most ancient system of health of the world. Skin forms a barrier against microbial invasion and has properties, which can protect against mechanism, chemical, osmotic, thermal and uv radiation damage. Skin is an important site of immune surveillance against the entry of pathogens and the initiation of primary immune response. Skin is the target of a variety of hormones. These activities can affect the appearance and the functional of individual skin component such as sebaceous glands, hairs and pigment producing cells. Acne is the most common skin disease of now a day. So we decided to take the help of this ancient system of science to fight against this acne vulgaris. An acne vagary is a chronic inflammatory condition of the pilo-sebaceous follicles on the face and upper trunk. It is characterized by the development of black heads, papules, pustules and cysts, which may resolve to leave pitted scars. In Ayurvedic classics we get the knows about this disease in the name of Yuvan pidika, the eruptions like Salmali thorn, on the face during adulthood, caused by Kapha, Vata and Rakta are known as 'Yuvan pidika'.

Background: Acne vulgaris is a very common malady of adolescents. It is common enough to be called as a physiological process but is better regarded as a disease due to its inflammatory component and the disfigurement. Acne vulgaris is a common skin disorder of pilosebaceous unit that primarily affects the face which if not treated leads to pigmentation. Mukhadushika is a Kshudra Roga which mainly involves Kapha and Vatadosa with Raktha. This is a common illness affecting 80 % of adolescents. Lepa is one of the external therapies explained in Ayurvedic classics. Objectives: To evaluate the efficacy of Sidharthakadilepa in the management of mukhadushikaw.s.r. to acne vulgaris. Methods: 30 patients fulfilling the inclusion criteria were selected. They were given Sidharthakadi Lepa for external application over face for one month. Patients were observed for changes on 7th, 14th, 21st and 28th day. The response to therapy was evaluated at the end of 28 days by calculating acne lesion score and the efficacy was determined by the percentage reduction in ALS. Results: The analysis revealed that 40% of patients had complete remission, 30% patients showed marked improvement and 30% showed moderate improvement.

Introduction: Melasma is one of the most common skin pigmentation disorders, which mostly affects the facial skin and has a considerable psychological impact on the patients. Melasma management has been one of the controversial issues in dermatology. We aimed to compare the combined treatment of the Er: YAG (erbium: yttrium–aluminum–garnet) laser plus hydroquinone (HQ) 4% with HQ 4% alone in the treatment of melasma. Methods: Twenty-nine patients were treated with the combined Er: YAG laser and HQ 4% on one side of the face with HQ 4% alone on the other side. Three sessions of the laser rat 4-week intervals. The outcome was calculated using the Melasma Area Severity Index (MASI). Results: The side that received the combined treatment (laser + HQ 4%) showed a statistically significant reduction in MASI compared to the side treated with HQ 4% alone. Conclusion: Our study suggests the superiority of the combination of the Er: YAG laser and HQ 4% in the treatment of melasma compared to HQ 4% alone.

Though Ayurveda is India's ancient traditional medicine written Thousands of years back, its concepts and principles are useful in today's era also. My small research work can be one example of it. Mukhdushika also was known as Youvanpeetika is a common health problem found in Adolescence which disturbs physical psychological and social health. Mukhdushika known as acne vulgaris is the classical stigma of adolescence and inflammation of sebaceous glands which produces physiological and psychological scarring. Dashang lepa is commonly used for shotha in clinical practice, but when it was applied as lepa on the face, its effectiveness was observed in mukhadushika. So after considering effective experience in mukhadushika I decided to go for its systematic study through this small research work, to study the effect of dashang lepa in mukhadushika.

In modern era, Acne is the most common skin disease facing mankind, it afflicts 80% of adolescent in population. In modern dermatology, a similar type of skin disorder named Acne vulgaris or Acne is found to have parity with the disease Mukhdushika or Yuvanpidika mentioned in Ayurvedic text. This problem is aggravated are increasing day by day due to excessive stress, hormonal imbalance, salty food, junk food and modern lifestyle. So, purpose of the study is to evaluate the application of Jalouka and herbo-mineral compound in Mukhdushika w.s.r to acne vulgaris. A 21 years old patient came in OPD (skin care unit) at NIA, Jaipur with the complaints of acne (nodular) for 1 year with pain, tenderness, burning, itching, discharge and discolouration diagnosed as Mukhdushika. Patient was administrated a herbomineral compound along with Jalaukavacharana with the follow up of 7 days. The main drugs of Herbomineral compounds were Manjishtha, Rasmanikya etc. all these drugs have the properties of Kushthaghna, Raktaprasadaka, Varnaprasadaka, Yakrit Uttejaka etc. Jalouka therapy was also given along with every week upto 12 weeks. The result of the combined therapy was encouraging without any side effects. Keyword- Mukhdushika, Jalouka, Herbo mineral compounds, Acne vulgaris

Acne vulgaris is a disease of the pilosebaceous follicle characterized by noninflammatory (open and closed comedowns) and inflammatory lesions (papules, pustules, and nodules). There are four major factors involved in the disease production viz. increased sebum production, cornification of pilosebaceous ducts, microbial involvement and inflammation. This condition is found commonly in puberty. In Ayurveda, it is termed as Mukhadushika In Ayurveda Mukhadushika is described under the heading of Kshudrarogas. The Shalmali thorn like eruption on the face due to vitiation of Kapha, Vata and Rakta which are found on the face of adolescent are called as Mukhadushika or YuvanPidika. Although vata, kapha and rakta are mentioned as dosha-duṣya involved in the pathogenesis of the disease, pittaja symptoms are also found in the disease like daha and paka.  Patient was suffering from Acne over face (Pidika), Burning sensation over face (Daha), Itching over face (Kandu), and Discolouration of skin (Vaivarnya) since 6 months. Patient was treated with jalukavacharan and mahamanjishtadi kwath.

Ardita is a disease-causing Vakrata (deviation) of Mukha Ardha (half of the face). In Modern science, it can be compared to Bell ’s palsy caused by the involvement of the 7th cranial nerve. It is characterized by the deviation of half of the face & associated with the sudden impairment of motor and sensory function of the affected side of the face. In modern science administration of steroids is the treatment of choice for Bell’s palsy. A 22 yr. an old male patient diagnosed as Ardit Vata treated with Ayurvedic shaman Aushadh along with Ksheerbala taila Nasya,Shirodhara, Abhangya & Nadi Sweda. This Ayurvedic treatment gives a significant improvement in this case. No conventional drugs used during treatment. This is evidence to demonstrate the effectiveness of Ayurveda treatment in the case of Ardita Vata.

Acne vulgaris is a common chronic disease involving blockage and /or inflammation of pilosebaceous units. Acne can presents as noninflammatory lesions, inflammatory lesions, or a mixture of both, affecting mostly the face but also the back and chest. Typical features of the condition include blackheads or whiteheads, pimples, oily skin, and possible scarring. It primarily affects skin with a relatively high number of oil glands, including the face, upper part of the chest, and back. The resulting appearance can lead to anxiety, reduced self-esteem, and in extreme cases depression. It is the commonest dilemma associated with complexion and smoothness of skin .sometimes leading to major skin problems,  which is characterized by macula, papulo, pustular eruption on the skin, especially on the face. Both male and female gender of adolescent age group are commonly affected. Genetics is the primary cause of acne in 80%of cases. The role of diet and cigarette smoking in the condition is unclear and neither cleanliness nor exposure to sunlight appears to play a part. In both sexes, hormones called androgens appear to be part of the underlying mechanism, by causing increased production of sebum. In the Ayurvedic perspective Acne Vulgaris can be correlated with Mukhadushika . Mukhadushika is explained by Aacharya Sushruta under kshudra rogas and is considered as swatantra vyadhi in the ayurvedic literature. It is characterized by shalmali kantaka sadrusha pidika on the face. Mukhadushika is also called as Tarunya pitika.

Comedones appear as blackheads and whiteheads and implicate cutaneous zones with dense sebaceous follicles, particularly the face, upper chest or dorsal torso. Acne vulgaris is frequently enunciated with comedones. Hyper-proliferation and aberrant desquamation of ductal keratinocytes is enunciated which impedes sebum exudation within a pilo-sebaceous duct, thus engendering comedones. Also, a prototypicpilo-sebaceous duct unit metamorphoses into a comedone when progenitor component of the sebaceous glands differentiate into epithelial-like cells.“Micro-comedone theory” cogitates acne configuration as “comedogenesis” which is contingent to androgen secretion, hyper-proliferating keratinocytes with accumulation of keratin and sebum within the pilo-sebaceous unit. Hyperkeratinisation is also due to secretion of pro-inflammatory cytokines interleukin -1 alpha (IL-1α) produced as a result of cutaneous colonization of Propionibacterium acnes.

Valmika Mrittika (ant hill soil) is widely used in clinical practice by folklore practitioners in most of the skin ailments. Lepa of Valmika Mrittika has wonderful effects on skin. Many temples in South Canara, give this Valmika Mrittika as Prasadam for the skin disorders. Acne Vulgaris is a common skin disorder of pilosebaceous unit that primarily affects the face, chest and back which if not treated may lead to permanent scarring. Youvana Pidaka (Acne vulgaris) occurs due to vitiation of Kapha, Vata as Doshas, Rakta and Twak as Dooshyas. Valmika Mrittika is very effective in Kapha-Vataja disorder. As disease limited to skin, Lepa Chikitsa is most suitable and convenient treatment suggested for acne. Hence, an in vitro analytical study of Valmika Mrittika w.s.r to Youvana Pidaka (Acne vulgaris) was conducted. Valmika Mrittika collected and analytical study was done including macronutrients and micronutrients. Analytical study revealed the contents such as Protein – 800 μg/g, Phenols-0.03μg/g, Tannins-0.24μg/g in the sample of Valmika Mrittika which are found to have anti acne and Varnya properties.

According to Ayurveda among the 56 Upangas  face is at the top so everyone and mostly youngsters are most cautious and careful about the beauty of face. Unfortunately skin of the face is affected by certain anomaly in adolescence age which is the golden and wonder period of life. If they find any spot or any minor ailment on face, they spend too much money for beauty purpose. In the present scenario of the people are very much conscious about their health as well as beauty and good looking. Face reflects the personality of person. It is saying that “Face is the index of mind”. In Ayurvedic texts Acne is termed as Youvan pidika,was very first described by Acharya Sushruta. Youvan pidika means that the disease almost takes place in young age. A great demand from Ayurveda in the field of cosmetology has been established due to its unique concept about beauty and effective, cheaper and long lasting beauty therapy without any side effect. In the present case study Virechan therapy is given to the patient after Snehan karmn followed by Sanshamn karm. Assessment was done after Virechan karm and after Sanshamn karm. A remarkable changes were found in various symptoms after assessment.
