In recent years much discussion has developed in pediatric circles relative to the pediatrician's role in the broad field of child health. There is a consenses that pediatricians should be broadly educated in the development of later behavioristic problems, even into the adolescent years—in truth a position of eminence in the family constellation. With therapeutics of infectious diseases well documented and immunization procedures well stabilized, his abilities can be directed to the broad area of normal and abnormal sociomedical problems.

In the April 1977 issue of Pediatrics (59:636, 1977), Dr. Cunningham recommends that the patient's medical record be given to the family to keep. He urges that the Committee on Standards of Child Health Care consider this subject. For at least 20 years the American Academy of Pediatrics has published for this very purpose, a "Child Health Record." This is publication HE-4 of the Academy and was last revised in 1968. The central office of the Academy tells me that, at the present time, between 50,000 and 100,000 of these are sold annually to physicians for the distribution that Dr. Cunningham recommends.

It is important to recall the definition of health embodied in the Constitution of the World Health Organization (WHO) over 45 years ago: "Health is a state of complete physical, mental, and social well-being and not merely the absence of disease or infirmity. The enjoyment of the highest attainable standard of health is one of the fundamental rights of every human being without distinction of race, religion, political belief, economic, or social condition." Among the Organization's mandated functions is "to promote maternal and child health and welfare and to foster the ability to live harmoniously in a changing total environment." The challenge of that task is no less today than it was then. Historically, societies have evolved various patterns of family structure for social and economic functions. In preindustrial societies there evolved a great concordance between these functions, with many of the health, developmental, and socialization functions taking place first within the family and then within the immediate community. The rapid social changes of both the industrial and information revolutions have changed drastically the functions of the family, and have shifted many of the health, developmental, and social functions to nonfamily institutions, from which families are often excluded or marginally involved. Much of the international attention to child health in this last decade has been directed at simple interventions to prevent the nearly 13 million deaths each year of children under 5: universal child immunization; the control of diarrheal and acute respiratory diseases; and infant and young child nutrition, particularly breast-feeding.

ABSTRACTThis study explores the degree of cultural diversity in the dinner-table conversation narrative events of eight middle-class Jewish-American and eight Israeli families, matched on family constellation. Conceptualized in terms of a threefold framework of telling, tales, and tellers, the analysis reveals both shared and unshared narrative event properties. Narrative events unfold in both groups in similar patterns with respect to multiple participation in the telling, the prevalence of personal experience tales, and the respect for children's story-telling rights. Yet cultural styles come to the fore in regard to each realm as well as their interrelations. American families locate tales outside the home but close in time, ritualizing recounts of “today”; Israeli families favor tales more distant in time but closer to home. While most narratives foreground individual selves, Israeli families are more likely to recount shared events that center around the family “us” as protagonist. In modes of telling, American families claim access to story ownership through familiarity with the tale, celebrating monologic performances; but in Israeli families, ownership is achievable through polyphonic participation in the telling. This book explains the unbending ideal of the nuclear family and how it has seeped so deeply into American society and consciousness without ever becoming the actual norm for most people in the nation. It presents the rich diversity of family lives in American history from the American Revolution to the twenty-first century and at the same time the persistence and normative power of the nuclear family model. American society—one of the major arguments—is “governed through the family,” and to govern, in this sense, is “to structure the possible field of action.” To make this broad examination of the discourse and practice of the family in American life more accessible, this book focuses on the relations of fathers, families, and society. Throughout American history “the father” has been posed as provider and moral leader of his family, American society, and the nation. At the same time power and difference were established around “the father,” and fatherhood meant many different things for different people. To tell this history of fatherhood, families, and American society, the author presents biographical “close-ups” of twelve iconic characters, embedded in contextual “long shots” so that readers can see the enduring power of the family and father ideals along with the complexity and varieties of everyday life in American history. Each protagonist covers a crucial period or event in American history, presents a different family constellation, and makes a different argument with regard to how American society is governed through the family.
