Genital endometriosis at women a hypothyrosis: pathogenesis, tactics of treatment and prophylaxis of recurrence

The objective: rising of efficiency of treatment of genital endometriosis in combination with hypothyrosis and depression of frequency of recurrence on the basis of improvement and introduction of algorithm diagnostic, medical prophylactical and rehabilitational actions with use of modern endoscopic technologies and medicamental correction. Patients and methods. Аt first was carried out the analysis of frequency of extragenital pathology at 789 patients with an internal genital endometriosis, then we surveyed 110 patients who received conservative and 100 patients – expeditious treatment. Research methods: clinical, laboratory, tool, functional, morphological and statistical. Results. Аt women with various forms of genital endometriosis obligatory inspection of funcshionale condition of thyroid gland, the endocrinologic and immunologic status is necessary; at vibor of a method of expeditious treatment of women with genital endometriosis and hypothyrosis the preference should be given to endoscopic and vaginal techniques, and also organ-preserving volume; for postoperative aftertreatment of women with genital endometriosis and hypothyrosis the preference should be given to hormonal therapy, courses not less 6 months. Conclusion. Introduction of the received results will allow to increase efficiency of treatment of genital endometriosis as is conservative, and operative that increases quality of life of women. Abstract: Normal sexual function is a bio-psycho-social process which is significantly related to the quality of life (QOL). Any disturbance in one of the components can be a disaster in sexual life.  Erectile dysfunction is a disability to get an erection or to maintain the erection well enough for a sexual intercourse, which persists or recurs at least three months conse-cutively due to either psychogenic or organic disturbances, or both. Some psychogenic dis-turbances especially depression, anxiety, and relational distress, play some important roles in ED. Besides that, aging, low testoteron level, physical diseases, and some certain medicines have to be included. Key words: erection, erectile dysfunction, sexual stimulus, depression.     Abstract: Fungsi seksual yang normal merupakan suatu proses biopsikososial yang berkaitan langsung dengan kualitas hidup. Adanya gangguan pada salah satu komponen dapat menjadi malapetaka bagi kehidupan seksual. Disfungsi ereksi dapat dedefinisikan sebagai suatu keti-dakmampuan untuk ereksi atau mempertahankan ereksi yang cukup untuk melakukan hu-bungan seksual yang memuaskan, yang menetap atau berulang paling tidak selama tiga bulan berturut-turut. Gangguan psikogenik khususnya sindroma depresi, ankietas, dan distres relasional berperan penting dalam hal terjadinya disfungsi ereksi. Disamping itu usia lanjut, rendahnya kadar testoteron, penyakit fisik, dan beberapa jenis obat perlu diperhitungkan. Kata kunci: fisiologi ereksi, disfungsi ereksi, stimulus seksual, depresi.

<p><strong>Abstract: </strong>Exclusion Territories are geographical areas under the action of degenerative environmental phenomena of anthropogenic origin, which compromise quality of life in general. One of the greatest examples of such areas is the Guanabara Bay and its surroundings, the scene of some of the worst disastrous incidents and locale of frequent episodes of human misery. This article presents a brief description of the main characteristics of the region, providing some technological suggestions of biogeographic recovery to be adopted by public policies that intend to align themselves with the good practices of ecological economy, sustainability and quality of life. The work falls within the context of macro-engineering <em>cum</em> eco-innovation applied to the preservation and management of water sources and water bodies that serve productive purposes as natural niches and breeding grounds.</p><p><strong>Key words: </strong>Exclusion Territories, Guanabara Bay, waste management, quality of life.</p><p class="-1">===========================================================================</p><p class="-1"><strong>Resumo: </strong>Territórios de Exclusão são áreas geográficas sob ação de fenômenos ambientais degenerativos de origem antropogênica, os quais comprometem a qualidade de vida em geral. Um dos maiores exemplos de zonas desse tipo é a Baía de Guanabara e seu entorno, palco de alguns dos piores incidentes desastrosos e de frequentes episódios da miséria humana. O presente artigo descreve sumariamente as principais características da região, fornecendo algumas sugestões tecnológicas de recuperação biogeográfica a serem adotadas por políticas públicas que pretendam alinhar-se às boas práticas de economia ecológica, sustentabilidade e qualidade de vida. O trabalho se insere no contexto da macroengenharia <em>cum</em> eco-inovação aplicada à preservação e à gestão das fontes hídricas e dos corpos de água que servem a propósitos produtivos como nichos naturais e criadouros.</p><p class="-1"><strong>Palavras-chave: </strong>Territórios de Exclusão, Baía de Guanabara, gestão de resíduos, qualidade de vida.</p>

Summary: Bariatric-metabolic surgery is associated with many health benefi ts and improved quality of life. However, the resulting eff ect largely depends on the patient’s compliance and willingness and ability to adopt a new diet, which is associated with bariatric procedures. A whole team of experts plays an important role in preparing the patient for this change. Appropriately composed diet and adherence to the frequency and size of portions is essential in the prevention of nutritional defi ciencies. 170 Background: While there are different studies investigating the quality of life (QoL) of cancer patients (pts) during active treatment, few studies report QoL of cancer survivors (CS) after at least 5 years (yrs) from achieving complete remission. We have investigated QoL and concerns about physical\psychological symptom of CS enrolled in our survivorship program from April 2015 to December 2016. Methods: We included pts > 18 yrs affected by hematologic or solid tumors after at least 5 yrs from achieving complete remission. A cross-sectional survey was carried out using validated scales: Cancer Survivors Survey of Needs subscale and a single-item measure of global QoL perception. Results: We analyzed data from 178 CS. The median age was 62 yrs (52 yrs at diagnosis), 70% were females. The most frequent histological types were breast (50%), colorectal (11%) and hematologic tumors (16%). Most pts received both chemotherapy and radiation therapy. With a median observation time of 139 months, the 10-yrs cumulative incidence of second neoplasm and cardiac adverse event was 8% and 7% respectively. 133 pts (65%) reported a good QoL (score > 3). The most frequent symptoms reported were weight changes (77%), memory\concentration deficit (65%), dental\mouth problems (61%). The most common physical\psychological symptoms were fear of relapse (83%), genetic counseling (68%), living with uncertainty (66%). A positive statistically significant association (p < 0.05) was observed between previous hormonal therapy and the following concerns (score ≥2): sleep disturbance (42% vs 35%); weight gain (31% vs 24%), osteoporosis (35% vs 18%), living with uncertainty (44% vs 38%). Female reported more fear of relapse (p 0.023) and sense of uncertainty (p 0.006) than males. 92% of pts have fully or partially adhered to the survivorship care plan. Conclusions: Most CS have a good QoL perception but we observed a high percentage of pts with a low score indicating different needs to be addressed. Pts treated with hormonal therapy are at higher risk of having both physical and psychological concerns. So far, the compliance of pts with the program was high.

Thing. A new substantiated method of orthopedic treatment of patients with complete destruction of the crown of the tooth is described, objective results of its use are given, and an economic analysis of its use is carried out in comparison with the traditional method of treatment of such patients. The goal is to create a device for restoring the missing clinical crown of premolars and its clinical, laboratory and economic justification. Methodology. During the study, orthopedic treatment of 20 patients with complete destruction of the crown of the tooth was carried out. A clinical assessment of the effectiveness of treatment using targeted radiography, occlusion and quality of life of patients was conducted using the OHIP-14 RU questionnaire. The compressive strength of ceramic coatings was studied for 10 samples of our device and 10 samples with a traditional ceramic-metal crown. The economic effect of our proposed method is analyzed in comparison with the traditional one. Results. 1 month after the treatment, a favorable outcome was observed in all patients on targeted radiographs; during occlusionography, 10 % of patients showed slight premature contacts in lateral occlusion, which were eliminated by selective grinding. 95 % of patients rated the quality of life as good, 5 % ― unsatisfactory. Safety margin of ceramic coating on samples of our device was 2.4 times higher than on samples of a traditional device. The economic efficiency of the traditional method in comparison with the one we proposed below has a ratio of 1:1.35, respectively. To restore one missing clinical tooth crown, the difference in cost between these methods is 21.2 %, or 1536 rubles. Conclusions. The results of the study allow us to recommend our proposed device for active use in practical dentistry in the treatment of patients with complete destruction of the clinical crown of premolars.
