In our country, complex surgical reconstruction of the distal urinary tract in severe bladder dysfunction in children is performed infrequently, given the number of children who need these operations. These operations were carried out in our country by specialists invited from abroad. Therefore, we want to share our first experience of performing a complex surgical reconstruction of the distal urinary tract in children. Purpose – to іmprove the quality of life of children with severe bladder dysfunction. Materials and methods. The results of reconstructive and plastic operations on the bladder in 7 children with severe bladder dysfunction were analyzed. Four patients had a neurogenic bladder 3 of them had a spinal bladder), and two patients had a microcyst after surgical correction of bladder exstrophy and total epispadias. Augmentation ileocystoplasty was performed in 3 children, two of them also underwent appendicocystostomy, 3 patients underwent only appendicocystostomy. An assessment of the urinary tract condition, the degree of urinary retention, postoperative complications and quality of life of operated children. Results. All operated children recovered. The bladders in three patients after augmentation have sufficient volume and fully provide reservoir function. We did not observe metabolic acidosis or other serious metabolic disorders described by other authors in the children we operated on. Urolithiasis was also not observed after surgery. In 5 patients who underwent appendicocystostomy, the latter is well catheterized, has a good cosmetic effect. In 4 out of 5 patients there is no leakage of urine through the stoma. All operated children were recovered for vesicoureteral reflux and chronic pyelonephritis. According to the results of the survey, the improvement of most parameters of quality of life of operated patients (cessation of urinary tract infection, complete urinary retention with more infrequent and controlled urination, the ability to visit children’s groups and social adaptation of these children in society). Conclusions. Based on the first own experience of augmentation ileocystoplasty and appendicocystostomy according to Mitrofanoff, the good efficiency of these reconstructive-plastic operations is noted. In all operated patients the quality of life significantly improved – controlled urination was achieved, the progression of kidney damage was stopped. All postoperative complications are successfully corrected. Of great importance for achieving a good effect of treatment is the correct choice of the the urinary tract surgical reconstruction type in each case. The research was carried out in accordance with the principles of the Helsinki declaration. The study protocol was approved by the Local ethics committee of all participating institution. The informed consent of the patient was obtained for conducting the studies. No conflict of interest was declared by the authors. Background: Breast plastic surgery is a rapidly evolving field of medicine. The modern view of surgical trends reflects the desire to minimize complications and introduce advanced technologies. These always will be priorities for surgeons. Reconstructive surgery, a branch of plastic surgery focusing on restoration of lost functional and aesthetic component, seeks to enhance psychological rehabilitation and improves the quality of life, as well as aesthetic recovery. Objective: This review addresses the action of fibrin agents and their effect on the quality of surgical hemostasis. Discussion and Conclusion: The fundamental goals for the surgeon are to perform a minimally traumatic intervention and to prevent any form of complication. Achieving complete hemostasis is an intraoperative necessity. Timely prevention of bleeding and hemorrhagic phenomena can affect not only the outcome of the operation, but also the incidence of postoperative complications. Topics include the integrity of microvascular anastomoses, tissue adhesion, and the incidence of seromas and hematomas associated with fibrin glue usage. The literature on fibrin adhesives with respect to prevention of postoperative complications, and the effectiveness with active drainage also are analyzed.

Abstract Background This study aims to assess the severity of lower urinary tract symptoms, and to assess predictors of impaired quality of life among Egyptian adults complain of lower urinary tract symptoms. Methods An observational cross-sectional research was done using an online anonymous poll survey. The survey was implemented through sharing on different social media applications. The survey was posted from June 1, 2020, to June 10, 2020. The overall communities of the Egyptian adults who satisfied the incorporation rules and consented to take an interest in the research were incorporated using convenience and snowball collecting methods (188 adults). A semi-structured questionnaire on socio-demographic characteristics and Arabic Urogenital Distress Inventory (UDI-6) and Incontinence Impact Questionnaire (IIQ-7) short forms were used. Results Approximately 220 Egyptian adults shared in the research but only 188 fulfilled inclusion and exclusion criteria. Approximately 92% of them had at least one symptom of lower urinary tract manifestations. Irritative symptoms presented in 65 (85.5%) of males and 102 (91.1%) of females. Stress symptoms presented in 44 (57.9%) of males and 63 (56.2%) of females with higher statistically significant mean of stress symptoms. Obstruction/discomfort symptoms presented in 51 (67.1%) of males, and 77 (68.8%) of females. Seeking help and duration of the urological problem were statistically significant independent positive predictors of UDI-6-total. The Irritative score, obstruction/discomfort score, and duration of the urological problem were statistically significant independent positive predictors of IIQ-7-total. Conclusions Urological problems are common and have an impact on the quality of life in various domains of physical activity, social relationships, travel, and emotional health.

To explore the influence of human-oriented nursing mode on the self-care ability, unhealthy emotion and quality of life of patients with benign prostatic hyperplasia (BPH), 147 patients with BPH admitted to our hospital from February 2018 to August 2019 were selected and all patients were separated into two groups on the basis of the nursing intervention mode. 77 cases in the research group (RG) were given the human-oriented nursing mode and 70 cases in the control group (CG) were given the conventional nursing mode. The bladder irrigation time (BIT), indwelling catheter time (ICT), postoperative hospitalization time (PHT) and the incidence rate of postoperative complications were observed in the two groups after nursing intervention. Before and after nursing intervention, the self-care ability of patients was evaluated in the two groups by Exercise of Self-Care Agency Scale (ESCA). In the two groups, the anxiety and depression status were evaluated by the Self-rating Anxiety Scale (SAS) and the Self-rating Depression Scale (SDS). The Quality of Life Scale for Benign Prostatic Hyperplasia Patient (BPHQLS) was applied to evaluate the quality of life in both groups before and after nursing intervention. The International Prostate Symptom Score (IPSS) was used to assess the lower urinary tract symptoms (LUTS) of patients in the two groups before and after nursing intervention. The self-made nursing satisfaction questionnaire was used to evaluate the nursing satisfaction in the two groups. The BIT, ICT and PHT in RG after nursing intervention were obviously lower than those in CG, and the incidence of postoperative complications in RG was also obviously lower than that in CG (p < 0.05). The ESCA score of patients in RG after nursing intervention was significantly higher than that in CG (P < 0.05). The SAS and SDS scores of patients in RG after nursing intervention were significantly lower than those in CG (P < 0.05). The BPHQLS score in RG after nursing intervention was obviously higher than that in CG (P < 0.05). The IPSS score of patients in RG after nursing intervention was obviously lower than that in CG (p < 0.05). The nursing satisfaction score in RG after nursing intervention was obviously higher than that in CG (p < 0.05).Condusion: The application effect of the human-oriented nursing mode on patients with BPH is definite, which can improve self-care ability, unhealthy emotion and effectively ameliorate the quality of life.

Bladder dysfunction in multiple sclerosis (MS) can be socially disabling, have negative psychological and economic consequences, and impair patients' quality of life. Knowledge of the functional anatomy and physiology of the urinary tract is essential to understand the symptoms associated with central nervous system lesions and the pharmacotherapies used to treat them. Treatments for neurogenic detrusor overactivity (NDO) have consisted mainly of administration of anticholinergic drugs, which have been shown to provide suboptimal clinical benefits and be poorly tolerated. The US Food and Drug Administration (FDA) approval of intravesicular botulinum toxin therapy provides a second-line option for MS patients with NDO not responsive to anticholinergic drugs. We performed a review of key literature pertaining to the intravesicular application of botulinum toxin. In the management of NDO, administration of intravesicular botulinum toxin using clean intermittent catheterization decreases the incidence of urinary tract infections, promotes urinary continence, and improves quality of life for 9 months after a single injection; moreover, those benefits are maintained with repeated injections over time.

The dysfunction of the lower urinary tract (DTUI) is an abnormal pattern of the urinary control with loss of coordinated capacity storage, stocking and elimination of urine. In order to assess the impact of lower urinary tract dysfunction in their child’s caregivers, this series was performed to evaluate 30 cases of caregivers of children with voiding disorders. Applied HAD scales and SF-36. Of these, 60% (n = 18) had anxiety symptoms and 43.3% (n = 13) depressive symptoms. Despite the scale items of quality of life was above average, the items pain, general health, vitality, emotional limitations and mental health measures were below the average. Thus, the effect of voiding dysfunction in parents of children affects the mood and quality of life in their daily lives.
