Four composite case examples are presented and discussed as they relate to emotional expression, significant moments in the therapeutic process, and communication using a variety of modalities in music therapy with adults diagnosed with intellectual and developmental disabilities. Building on therapeutic awareness through discussing musical elements, body movement and posture, countertransference, and interactional patterns, the implications of deep emotional connection and processing are approached using primarily nonverbal methods. Composite vignettes from the author’s clinical work demonstrate awareness of these factors in the moment as they impacted the session, therapeutic relationship, and other professionals’ understanding of music therapy in this population. Implications for emotional processing in clinical practice are presented as they relate to the concepts presented in this paper.

Abstract This paper describes a conceptual framework in music therapy with adults with severe and profound levels of intellectual disability. Drawing from Carl Rogers’ person-centered work and his notion of the good life as a direction, the author presents four core relational experiences (curiosity, invitation, celebration, and recognition) that underpin the therapeutic process with this client population. Case vignettes from the author’s clinical work illustrate how core experiences are presented through improvisational musical experiences. Considerations are given to the clinical significance of good-life-oriented experiences on the emotional health of individuals with severe and profound intellectual disability, and to its implications beyond the music therapy space.

Many improvisational models of music therapy involve therapist and client improvising spontaneously together. This is widely described as a form of musical relationship. However whilst there is much discussion of the client's musical input in the literature, the therapist's music attracts less attention. The author considers reasons why this may be the case and seeks evidence, from the music therapy literature and beyond, as to whether the therapist's musical input is of significance for the therapeutic process. An example of detailed analysis of the author's own clinical work is presented, in order to establish whether the therapist's musical input has an observable impact on the shared musical experience and might thus be judged to be significant for the therapeutic musical relationship. Concluding that it is indeed significant, the author goes on to consider the implications for the way music therapists consider their work.

IntroductionMost experienced psychotherapists use an eclectic approach in their practice, combining techniques of different schools of psychotherapy to treat their patients. However, there are no good teaching models to train the new generation of psychotherapists in the technique of psychotherapy integration. FEP is a form of psychotherapy that combines techniques from four different psychotherapies: psychodynamics, CBT, IPT and supportive therapy. It also incorporates mindfulness, motivational interviewing, exercise and nutrition. A strong therapeutic alliance is crucial to the success of FEP. Techniques are tailored to the patient's current clinical state. Several techniques from different psychotherapies may be used in the same session. The therapist is empathic, flexible adaptive to the patient's needs and assumes an active role in the therapeutic process.ObjectiveTo present a model of psychotherapy integration that can be used by psychotherapists in their clinical practice.AimsWe aim to present a model of psychotherapy integration that can be taught and implemented by psychotherapists in their clinical practice.ResultsWe present ten case examples were FEP was effective clinically and we compare it to the standard of care received by these patients and show the savings to the healthcare system.ConclusionThe results of our study suggest that FEP could be a cost effective treatment that could be added to the psychotherapists toolbox. In this article, I will focus on highly conflictual couples with extensive emotional deprivation and unresolved trauma, which prevents them from developing healthy romantic relationships and overcoming the challenges entailed in any intimate attachment. I will describe how everyday interactions are experienced as threatening or even lethal movements between the partners. The question which arises in the psychoanalytical therapeutic process is how to help the couple tolerate the sensory reminders of the unresolved trauma as a necessary precursor to any process of symbolisation. In order to provide a safe enough therapeutic attachment bond, extensive time is dedicated to the emotional experience of self and the other in the here-and-now of the session, which validates the emotional experience of the couple as well as contains it. The therapy focuses on the transferential and countertransferential movements inspired by the matrix of the victim, abuser, and uninvolved witness (Davies & Frawley, 1994) to elaborate the intertwining of the unresolved trauma with the couple’s form of attachment. In order to illustrate my argument, I present two examples: one from a fictional narration and another from my clinical work.

Objective: to understand the impact of music on the intensive care for COVID-19 as an instrument to humanize assistance from the perspective of nurses who work on assistance. Methods: qualitative study carried out with seven intensive care nurses working in the COVID-19 Intensive Care Unit of a public state hospital. Sample reached through theoretical saturation. Data were collected using interviews through the on-line application WhatsApp, guided by a semi-structured guide. Results: the following discursive categories emerged: Feelings of health professionals and humanized actions in intensive care; Music therapy to provide integral care for people with COVID-19 in the score of intensive care; Living in the moment; Music therapy as an instrument for spirituality in the intensive care environment. Conclusion: the nursing intensive care did not only carry out a biological treatment, but considered all aspects of the human being, using to do so humanization by music.

EMOTIONAL EXPRESSION IN MUSIC PERFORMANCE includes important cues arising from the body movement of the musician. This movement is related to both the musical score execution and the emotional intention conveyed. In this experiment, a pianist was asked to play the same excerpt with different emotionally expressive intentions. The aim was to verify whether different expressions could be distinguished based on movement by trying to determine which motion cues were most emotion-sensitive. Analyses were performed via an automated system capable of detecting the temporal profiles of two motion cues: the quantity of motion of the upper body and the velocity of head movements. Results showed that both were sensitive to emotional expression, especially the velocity of head movements. Further, some features conveying information about movement temporal dynamics varied among expressive conditions allowing emotion discrimination. These results are in line with recent theories that underlie the dynamic nature of emotional expression.

Abstract Twelve music therapists were observed working clinically in 3 to 5 of their music therapy sessions and subsequently interviewed about their clinical work in order to further examine and define the essential characteristics of therapeutic singing in music therapy clinical practice. Observational and interview data were analyzed separately using procedures consistent with qualitative content analysis and then integrated to provide a comprehensive picture of these singing practices. Analysis of these data revealed 3 interrelated dimensions of therapeutic singing that were integrated into the larger realization of therapeutic singing: (1) foundational vocal skills, (2) vocal engagement, and (3) authenticity. Implications for the education and training of music therapy students, vocal health, and a reevaluation of the American Music Therapy Association’s competencies contextualize these findings for the profession as a whole.

Beginning around Session 5 or 6, emotional processing of the trauma memories can be made more efficient by having the patient focus primarily or exclusively on the most currently distressing parts of the trauma, which the authors term the “hot spots.” The therapist helps the patient identify his hot spots and then select one to begin the imaginal exposure. This should be one of the most distressing parts, if not the most distressing part, of the trauma. Therapy continues with the focus on the patient’s hot spots during the imaginal exposure until each has been sufficiently processed, as reflected by diminished Subjective Units of Distress Scale (SUDS) levels and the patient’s behavior (e.g., body movement, facial expression). This may take several sessions, depending on the number of hot spots, the patient’s pace, and the amount of time spent listening to exposure recordings as homework.

Emotional approach coping (EAC) is a construct encompassing the intentional use of emotional processing and emotional expression in efforts to manage adverse circumstances. The construct was developed in an attempt to reconcile a discrepancy between the empirical coping literature, in which an association between the use of emotion-focused coping and maladjustment often is reported, and literature in other areas describing the adaptive roles of emotional processing and expression. At least two significant limitations in the way emotion-focused coping has been operationalized help explain this discrepancy: widely disparate coping strategies, both approach-oriented and avoidance-oriented, are designated as emotion-focused coping in the literature, and some emotion-focused coping items in published measures are confounded with expressions of distress or self-deprecation. To address these problems in measurement, the EAC scale was developed. The measure includes two correlated but distinct subscales: Emotional Processing (i.e., attempts to acknowledge, explore, and understand emotions) and Emotional Expression (i.e., verbal and/or nonverbal efforts to communicate or symbolize emotional experience). Recent research using this psychometrically sound measure has provided evidence that EAC enhances adjustment to stressors including infertility, sexual assault, and breast cancer. The findings are not uniform, however, and further study of moderators such as the interpersonal context, the nature of the stressor, cognitive appraisals of the stressor, and individual differences is needed, along with additional study of mechanisms for the effects of EAC. Although emotional processing and expression are core components of many clinical approaches, specific measurement of EAC thus far has been limited to only a few clinical intervention trials. An understanding of who benefits from EAC in which contexts and how these benefits accrue will require continued integration of findings from stress and coping research, emotion science, and clinical studies.
