Flexible eclectic psychotherapy (FEP): A model for psychotherapy integration

IntroductionMost experienced psychotherapists use an eclectic approach in their practice, combining techniques of different schools of psychotherapy to treat their patients. However, there are no good teaching models to train the new generation of psychotherapists in the technique of psychotherapy integration. FEP is a form of psychotherapy that combines techniques from four different psychotherapies: psychodynamics, CBT, IPT and supportive therapy. It also incorporates mindfulness, motivational interviewing, exercise and nutrition. A strong therapeutic alliance is crucial to the success of FEP. Techniques are tailored to the patient's current clinical state. Several techniques from different psychotherapies may be used in the same session. The therapist is empathic, flexible adaptive to the patient's needs and assumes an active role in the therapeutic process.ObjectiveTo present a model of psychotherapy integration that can be used by psychotherapists in their clinical practice.AimsWe aim to present a model of psychotherapy integration that can be taught and implemented by psychotherapists in their clinical practice.ResultsWe present ten case examples were FEP was effective clinically and we compare it to the standard of care received by these patients and show the savings to the healthcare system.ConclusionThe results of our study suggest that FEP could be a cost effective treatment that could be added to the psychotherapists toolbox. Aim: New generation anticoagulants rapidly find a wider area of use in the clinic due to the use problems of other oral anticoagulants. Anticoagulants such as Dabigatran, Rivaroxaban, and Apixaban with safer treatment intervals have been accepted in clinical practice guidelines and have taken their place as preferred drugs. In this study, we aimed to retrospectively examine the effects of three new-generation anticoagulant drugs on a group of patients. Material and Methods: In this retrospectively planned study, patients diagnosed with atrial fibrillation (n = 522) were divided into three groups according to the drugs used for treatment (Dabigatran, Rivaroxaban, and Apixaban). Routine blood values of the patients in each group were retrospectively scanned according to age, gender, time of drug initiation and presence of chronic disease. Results: According to the results obtained, it was found that the mean HCT, BUN, AST, ALT, MPV, Iron, and Ferritin were higher in patients using Apixaban than those using Dabigatran and Rivaroxaban drugs, but the age, average values of Hgb1 Hgb2, Hgb1, PLT, CrCl, Gfr and INR of the patients using Apixaban lower than those using Dabigatran and Rivaroxaban. The highest rate (22.5%) was found in the group of patients taking apixaban (n=93) when people taking the drugs were examined in terms of mortality. Conclusion: It has been observed that Rivaroxaban can be used more safely in patients with a history of acute cancer and thrombosis, patients with recurrent venous thromboembolism, and patients with high frailty, three drugs should be preferred instead of oral anticoagulants.

Four composite case examples are presented and discussed as they relate to emotional expression, significant moments in the therapeutic process, and communication using a variety of modalities in music therapy with adults diagnosed with intellectual and developmental disabilities. Building on therapeutic awareness through discussing musical elements, body movement and posture, countertransference, and interactional patterns, the implications of deep emotional connection and processing are approached using primarily nonverbal methods. Composite vignettes from the author’s clinical work demonstrate awareness of these factors in the moment as they impacted the session, therapeutic relationship, and other professionals’ understanding of music therapy in this population. Implications for emotional processing in clinical practice are presented as they relate to the concepts presented in this paper.

Historians of economics rarely consider textbooks as more than passive receptacles of previously validated knowledge. Therefore, their active role in shaping the discipline and its image is seldom addressed. In this paper, I study the making of Paul Samuelson’s successive editions of Economics from 1967 to 1973 as an instance of how textbooks stand at the crossroads between disciplinary knowledge, pedagogy, and larger political and societal concerns. In the mid-1960s, Economics, now at its sixth edition, was at the height of its success. Considered one cornerstone of modern economics, it was also at the center of a number of criticisms dealing with the current state of the economic discipline and its teaching in the universities. While the profession expressed its concern over the lack of relevance of economics to address the pressing issues of the day and pleaded for a new “problem-solving” approach to economic education, the late 1960s witnessed the emergence of a new generation of “radical” economists criticizing the economics orthodoxy. Their contention that mainstream theory had neglected the issues of class struggle and capitalist exploitation found a favorable echo among an increasingly politicized population. Using archival materials, I show how Samuelson, helped by his editorial team at McGraw-Hill, attempted to take into account these changes in order to ensure the continuing success of subsequent editions of his text in an increasingly competitive market. This study emphasizes Samuelson’s ambiguous attitude toward his contenders, revealing, on the one hand, his apparent openness to discussion and outsiders’ suggestions, and, on the other hand, his firm attachment to mildly liberal politics and aversion to Marxism, unchanged through revisions. It also helps refine a notion that is often invoked but never fully expounded in textbook studies: that of the audience.

Purpose Succession in family firms may determine the survival or the failure of the business itself. Management accounting literature has added little to this issue, mainly focusing on the process of succession and change (Songini et al., 2013; Giovannoni and Maraghini, 2013; Giovannoni et al., 2011). This study aims to deal with new management accounting (MA) practices that the junior generation may introduce during the process of succession. The aim of the study is to show that the introduction of new MA practices can contribute to constructing the leadership profile of the junior generation. Design/methodology/approach Drawing on the perspective of actor-reality construction (ARC), the authors conducted a case study at a small-sized family firm producing solar shading systems. The authors examined how the construction of the successor’s leadership derives from the integration of four dimensions of reality: facts, possibilities, values and communication. Such an integration is facilitated by the introduction of a new accounting information system and cost reporting. Findings The case evidence highlights that the construction of the new generation leadership may emerge as a consequence of the introduction of new MA practices. Moreover, the field evidence highlights that the construction of a new generation leadership is a process that integrates the four dimensions of reality. Originality/value From the emergent perspective of ARC, the paper highlights how new MA practices play an active role in constructing the new generation leadership.

The termination phase of clinical practice is an important component of the therapeutic process. The ending of the therapeutic relationship, whether planned or unplanned, can elicit feelings of loss, separation, and guilt, impacting both the client and the practitioner. The reasons for ending service and preparation for termination can affect the client's gains. Systematic research on the termination process and the maintenance of gains is needed to further determine variables for successful termination.

Background: Preclinical teaching of basic sciences provides the basis for the development of clinical reasoning skills and the ability to make management decisions. However, many senior undergraduates, pre-interns and doctors indicate that basic sciences knowledge is poorly recalled and has little relevance to their clinical practice. Objectives: To explore the perceptions of medical students in their clinical years, and pre-interns about the basic sciences courses taught to them in the preclinical years, and to assess how each group rates the applicability of these courses to current clinical training. Methods: A descriptive cross-sectional study was conducted among 118 pre-interns and 146 undergraduate medical students using a self-administered questionnaire, regarding their perceptions on preclinical basic sciences teaching. For statistical analysis chi square test was applied. Results: More than 75% both pre-interns and undergraduates agreed that preclinical teaching was useful and relevant for future clinical work. 50-75% frequently revisited preclinical subjects despite unapparent clinical significance. 55% couldn’t remember most of preclinical content. Physiology was the most retained (76%) and most clinically relevant subject (80%). Majority of (>60%) both the groups suggested more teaching time and >75% suggested concurrent clinical exposure for preclinical teaching. Undergraduates and preinterns differed in that 56% of undergraduates and 37% of pre-interns had studied pre clinical subjects just to pass examinations (p<0.01) Conclusions: Majority of undergraduates and pre-interns felt that preclinical teaching is interesting and relevant for future clinical practice, though recall and clinical significance were suboptimal. Revising teaching methods with interdisciplinary integration, early clinical exposure showing relevance of basic sciences and allocating more teaching time utilizing clinicians should be considered. The wide spreading of the new generation of smartphones, with significant improvements in terms of image acquisition and processing power, is opening up the possibility of new approaches for skin lesion monitoring. Mobile Teledermatology appears nowadays as a promising tool with the potential to empower patients to adopt an active role in managing their own skin health status, while facilitates the early diagnosis of skin cancers. The main objective of this work is to create a mobile-based prototype for skin lesions analysis with patient-oriented features and functionalities. The presented self-monitoring system collects, processes and storages information of skin lesions through the automatic extraction and classification of specific visual features. The algorithms used to extract and classify these features are briefly described, as well as the overall system architecture and functionalities.

Purpose – The purpose of this paper is to introduce applied improvisation (AI) as a tool for training humanitarian aid workers. AI incorporates principles and practices from improvisational theatre into facilitation and training. It is an excellent modality for training aid workers to deal with crisis and disaster scenarios where decision-making and collaboration under pressure are critical. Design/methodology/approach – This paper provides a theoretical base for understanding skills needed in disaster response and provides a case for innovative training that goes beyond the current standard. AI principles, activities and case examples are provided. Interviews with development experts who have participated in AI training are excerpted to reveal the impact and promise of this methodology. Findings – Different from typical training and games, which simulate potential crisis scenarios, AI works with participants in developing the skills necessary for success in disaster situations. The benefit is that workers are better prepared for the unexpected and unknown when they encounter it. Research limitations/implications – The current paper is based on author observation, experience and participant interviews. While AI is consistently transformative and successful, it would benefit from more rigorous and structured research to ground the findings more deeply in larger evidence based processes. Practical implications – The authors offer specific activities, resources for many others and practical application of this modality for training purposes. Social implications – Its application has tremendous benefits in training for specific skills, in creating greater cohesion and satisfaction in work units and breaking down culture and language barriers. Originality/value – This work is original in introducing these training methods to humanitarian aid contexts in general, and disaster preparedness and response in particular.

The problematic field in this article is one of the most vivid, contemporary dispute, i.e. the dispute between “ethics of care” and “ethics of justice”. “Ethics of care” is Carol Gilligan’s concept derived from her controversy with Lawrence Kohlberg’s theory of moral development. “Ethics of justice” is a summary term for different concepts developed within the traditional ethics. This traditional ethics tried to name universal principles of acting, independent from the acting person and her feelings. The central categories were “justice” and “the Good”. For the second kind of ethics the central category is “care” and moral duties are gradable. In this article I analyse the controversy in the field of psychotherapy. I maintain that dynamics of psychotherapeutic relation is such that it “imposes” the “ethics of care”. Referring to some examples from clinical practice I tend to prove how dangerous absence of the “ethics of justice” can be in psychotherapy. I claim that using only the “ethics of care” approach in understanding and approaching the patient’s acting may narrow the therapeutic process and slow down the patient’s moral growth.

It examines the present and potential relevance of Cochrane reviews to clinicians providing hand therapy and gives some pointers for those who wish to take a more active role in evaluating the evidence for their clinical practice.
