ABSTRACT Objective: To understand the management of home care by family caregivers of dependent elderly people after hospital discharge. Methods: Qualitative research guided by hermeneutics-dialectic, anchored in the theory of communicative action. Data collection took place using a semi-structured interview with 11 participants. Results: Two categories were constructed: Management of the many types of care by the caregiver and the relationship between family caregiver and health care network. Care and management actions carried out routinely cause major changes in the family caregiver’s life. He/she does not recognize planning, home care periodicity or support in required procedures. Final Considerations: The management of home care for dependent elderly people after hospital discharge is complex, involving physical and emotional overloads, as well as difficulties in getting support from health services. The planning shared between the health team and the family since the discharge is required, and the better visibility of the role of primary care when the patient is assisted by a home care service.

The aim of this study was to characterize dependent elderly people and their main family caregivers and the association between functional capacity of the elderly and the burden of caregivers. This cross-sectional study was conducted with 112 elderly people and caregivers connected to a primary care service in Porto Alegre, Brazil. The applied instruments related to care and socioeconomic variables were Physical and Instrumental Activities of Daily Living (PADL and IADL) and the Burden Interview. The Spearman correlation coefficient was also used. Average age of the elderly individuals in this study was 81.41 years, while the average score for PADL was 10.36 and 6.25 for IADL. Of the 112 elderly individuals, 71.4% had severe dependence. Among caregivers, 75% were women, 61.6% were the sons or daughters with an average age of 57.98 and an average burden of 29.53, which is equivalent to moderate burden. Results revealed a significant correlation between functional capacity of the elderly and caregiver burden. It was verified that the higher the dependence of elderly people, the greater the burden of caregivers.

Abstract Objective: To know the challenges of family caregivers members for the maintenance of care for children with impaired skin integrity after hospital discharge. Method: This is a qualitative study of action research, developed from March to July of 2017. Participants were ten family/caregivers of the children with impaired skin integrity. The data obtained through interviews were submitted to content analysis. Results: The family caregivers of the child with impaired skin integrity received guidelines for home care at discharge, experienced feelings of relief, getting out of the hospital and, for fear of causing physical and emotional damage, sought help in the services and neighborhood, overprotected the child, to avoid complications and readmission, and faced the lack of resources to care for. Conclusion: Maintenance of care for children with impaired skin integrity after hospital discharge requires application of measures of protection from skin wounds, constant surveillance of the child in order to prevent local traumas and infections, and the search for support in the Health, to obtain material resources and guidelines for care.

Objective: This study clarified requirements for transitioning aging patients with cognitive and physical disorders to home care and developed a new mixed methodology using medical records.Background: Japan has the highest proportion and highest rate of increase of older adults worldwide. Transitioning aging patients from hospital to home care is a key strategy to manage this increase; however, this transition presents challenges.Methods: A mixed method study was conducted, using descriptive data from medical records.Results: Of 41 participants, 61% were amicable settlement cases and 39% were non-amicable. In total, 56.1% went home: 36.6% were amicable and 19.5% were non-amicable. Requirements for transition to home care were [Patient’s mental stability], [Necessary care for the patient within the family’s tolerance level], [Decreasing the family’s burden of care by accessing the public care service], [Preparing the family to bear the heavy burden of caring for multiple family members at home], [Decreasing the risk of worsening condition through patient compliance], and [Setting realistic goals and reconciling them with the family].Conclusions: The transition requirements related to cognitive disorders, gaps in physical functioning before and after hospitalization, and aging society. They were fulfilled through concrete preparation of the family for coping with home care, with the patient’s mental stability as a precondition.Relevance to clinical practice: Fulfillment of these requirements is essential for successful transition to home care for aging persons with multiple disorders. Our original mixed method design promotes a new methodology for qualitative research using descriptive data from medical records.

ABSTRACTThe hospital discharge preparation for the newborn is of fundamental importance to reduce the expectations of relatives on the care when he/she  returns to  home. The orientations seeking to the discharge should begin from the admission. The objective was to analyze the family preparation for hospital discharge of newborn in a Neonatal Intensive Treatment Unit and to verify the family knowledge as for the disease and to the cares with newborn in the home. It is a descriptive study, of a qualitative nature, accomplished in a neonatal intensive treatment unit at Fortaleza, from December/2003 to june/2004. It was used as technique for collecting data, a semi-structured interview and the direct observation. The data were organized through content analysis. We verified that the families were partially prepared to assume cares with Rn without supervision, manifesting difficulties to accomplish some activities. We identified that the family expectation is to have the new integral home free from risks, as parents looking for to live the maternity and paternity fully until then postponed. Parents have revealed some knowledge on the disease and the child's treatment. We verified need to improve the communication process between group and family in the preparation for hospital discharge. Trata-se de um estudo descritivo, de natureza qualitativa, realizado em uma Unidade de Tratamento Intensivo Neonatal, em Fortaleza, no período de dezembro de 2003 a junho de 2004. Utilizou-se como técnica de coleta de dados a entrevista semi-estruturada e a observação direta. Os dados foram organizados por meio de análise de conteúdo. Constatou-se que as famílias estavam parcialmente preparadas para assumir cuidados com o recém-nascido sem supervisão, manifestando, dificuldades para realizar algumas atividades. Identificou-se que a expectativa familiar é ter o novo integrante em casa livre de riscos, enquanto os pais buscam vivenciar plenamente a maternidade e paternidade até então adiados. Os pais revelaram possuir algum conhecimento sobre a doença e o tratamento da criança. Verificou-se a necessidade de aperfeiçoar o processo de comunicação entre equipe e família no preparo para alta. Se identificó que la expectativa familiar es tener al nuevo integrante en casa libre de riesgos, ya que los padres buscan a vivir  plenamente la maternidad y la paternidad hasta entonces pospuestas. algunos padres han puesto de manifiesto algunos conocimientos sobre la enfermedad y el tratamiento del niño. Verificamos la necesidad de mejorar el proceso de comunicación entre equipo y familia en la preparación para el alta. Elderly people are every Indonesian citizen, male and female, who have reached the age of 60 years and over, whether they have the potentially or not (The decree of the Minister of Social affair of RI No.15/HUK/2007). The objectives of this study are to determine the use of da’wah method for elderly and find out the supporting and inhibiting factors in conducting da’wah to the elderly people at Al-Furqon Mosque. This study applies a qualitative method (field research) for data collection, this study employs structured interview, observation and documentation techniques. The findings of the study show that there is no specific recitation for elderly people at Al-Furqon Mosque almost middle age. Routine recitation is held once a week on Sunday night and Wednesday night. It implements lecturing, holding question and answer and applicative methods. And the second finding, supporting factors of conducting da’wah to elderly people it shows adequate facilities and infrastructure, the right time table this activity, is after Maghrib mass prayer, the use of local languages as means of communication to the send the da’wah message from the preacher, The tolerance of other groups or organizations that are contrary to the culture of the Al-Furqon Mosque, lastly the family support of the elderly people who always motivaty them to be “Istiqomah” in joining the recitation at family and the inhibiting factor in delivering the message of da’wah to elderly worshipers in terms of psychological and physical perspectives. Are from the psychological lonely, mourning the loss of a loved one and feel depression due to pressure from family. Are from a physical perspective, they experience decreased hearing ability, decreased vision, and immune system that is getting weaker so they are susceptible to disease. So rarely can take part in recitation.

ResumoObjetivo: Conhecer a percepção do cuidador sobre a hospitalização do idoso com Doença de Alzheimer. Método: Trata-se de um estudo descritivo, exploratório, de natureza qualitativa, realizado com onze cuidadores familiares de idosos com doença de Alzheimer. Os dados foram coletados no período de Abril a Junho de 2013 por meio de entrevista semiestruturada. A análise dos dados foi pautada no Discurso do Sujeito Coletivo. Resultados: Emergiram duas ideias centrais síntese: piora da função cognitiva do idoso com doença de Alzheimer durante o processo de hospitalização e o despreparo da equipe de saúde para cuidar de um idoso com demência.Conclusão: De acordo com os resultados obtidos, urge a necessidade de capacitação e/ou especialização dos profissionais de saúde para atender o idoso com demência e sua família. A família é parte integrante do cuidado a esse idoso, sendo fundamental compreendê-la e assisti-la como uma unidade social complexa.Descritores: Cuidadores, Doença de Alzheimer, Idoso, Hospitalização. AbstractObjective: To investigate the perception of the caregiver about the hospitalization of the elderly with Alzheimer disease. Method: This is a descriptive study, exploratory and qualitative in nature, performed with 11 family caregivers of elderly with Alzheimer disease. Results: Two summary central ideas emerged: worsening of cognitive function in the elderly with Alzheimer disease during the process of hospitalization and the lack of health staff to take care of an elderly with dementia. Conclusion: According to the results obtained, there is urgent need for training and/or specialization of health professionals to meet the elderly with dementia and their families. Resultados: Surgieron dos ideas centrales resumen: empeoramiento de la función cognoscitiva en los ancianos con enfermedad de Alzheimer durante el proceso de hospitalización y la Falta de personal sanitario a cuidar de un anciano con demencia. Conclusión: De acuerdo a los resultados obtenidos, existe la urgente necesidad de formación y/o especialización de los profesionales de la salud para atender a los ancianos con demencia y sus familias. ABSTRACTThis study aims to measure objectively and/or estimate the needs of impaired elderly persons and their families for social care services. The research was carried out in Maebashi, a medium size Japanese city. In our study, ‘impaired elderly persons’ included both physically and/or mentally impaired persons. In total, 693 impaired elderly persons and their families were classified according to 20 specific types of need: these ‘need types’ were cross-tabulated with two sets of factors: (1) Severity of impairments and (2) Difficulty of caregivers in providing the required care. A standard model of the service provision necessary for each ‘need type’ was established through group consultation with social care service professionals. Finally, the total amount of each service required in the city was calculated. Major findings are: (1) Our estimates indicate that 7.1 per cent of all persons 65 and over residing in Maebashi City were physically and/or mentally impaired to a certain degree. (2) Forty per cent of these elderly people were being cared for by caregivers who themselves had serious difficulties. (3) The demand for social care services in local Japanese cities is high. In order for this demand to be met a substantial expansion of both nursing home care and in-home care services is required.
