Abstract Introduction: Functional disability is common in patients with chronic kidney disease, especially in those on hemodialysis. Muscle strength can be evaluated by a dynamometer; however, no study using this technique on the quadriceps of patients undergoing hemodialysis was found in literature. Objective: To assess the effects of cycloergometer training on the quadriceps muscle strength of patients with chronic kidney disease undergoing hemodialysis. Method: This was a clinical trial including 46 patients, both men and women, over 18 years old who had been undergoing hemodialysis for more than six months and signed an informed consent form. Patients were allocated into two groups: intervention group (n = 22) and control group (n = 20). The intervention group underwent intradialytic training in a physical therapy protocol with the cycloergometer for two months, whereas the control group, in turn, was only reevaluated two months after the initial evaluation. All patients were assessed for demographic data at baseline and, two months later for quadriceps muscle strength by standardized dynamometry and with the use of a rigid belt and suction cups. Results: A significant increase was detected in quadriceps muscle strength in the right and left lower limbs in the intervention group when compared with the control group. Conclusion: Quadriceps muscle strength improved after patients with chronic kidney disease on hemodialysis underwent training with the cycloergometer.

Backgrounds: Quadriceps muscle dysfunction in chronic obstructive pulmonary disease (COPD) is caused by systemic or local inflammation, hypoxia, hypercapnia, corticosteroid use, nutritional depletion, anabolic/catabolic hormone imbalances, oxidative stress, genetic susceptibility, and decreased daily activity. Decreased quadriceps muscle strength reduces exercise capacity, physical activity, increases shortness of breath, and decreases quality of life. Exercise trainng of walking increases the quadriceps muscle strength and overcomes deconditioning. The purpose of this study was to analyze effect of pedometer-based exercise on quadriceps muscle strength, quality of life, benefits, and cost on patients with stable COPD. Methods: A clinical study with quasi-experimental pre-post test control group design using consecutive sampling was performed in patients with stable COPD at Dr. Moewardi Hospital from September to November 2018. Subjects were divided into intervention group which were given pedometer based walking exercise for six weeks and control group with standard rehabilitation. Quadriceps muscle strength, SGRQ score, LCADL score, and cost effectiveness were measured before and after exercise. Results: Twenty-seven of stable COPD patients were included in this study. This article studies the effects of arthroscopic imaging treatment and clinical rehabilitation of knee sports injuries. Arthroscopy was used to perform meniscus trimming and resection for 40 patients with knee sports injuries. The ages of the patients ranged from 20 to 60 years old. All patients received routine rehabilitation training such as continuous passive motion of the knee joint, biofeedback of the lower limbs, and air pressure therapy of the lower limbs. In addition, the control group was given muscle strength training, and the training began after the patients received the quadriceps muscle strength test. The removal of the joint cavity and the joint debridement has achieved satisfactory treatment results. In the experiment, the test cases were divided into two groups, and the sensor test platform was used for signal collection. Normal activities can be resumed 2 weeks after the operation. After a follow-up of 6 to 24 months, the knee joint pain disappeared, the joint was free of swelling, and the knee function was normal up to 93%. Arthroscopic reconstruction of the anterior and posterior cruciate ligament joint repair/reconstruction of the medial and posterolateral ligament knots is safe and feasible for the treatment of multiple ligament injuries of the knee joint. It has the advantages of less trauma and quick recovery. Early postoperative systemic and standardized rehabilitation exercises can obtain good knee joint function.

BACKGROUND Despite the growing burden of chronic kidney disease (CKD), disease knowledge and understanding are still lacking, especially in Bangladesh. OBJECTIVE The aim of this study was to evaluate the outcome of a health education intervention in order to enhance knowledge, health-related quality of life (QOL), and motivation regarding healthy lifestyles among rural and periurban adults suffering from CKD. METHODS A parallel-group (1:1) randomized controlled trial is ongoing in the Mirzapur subdistrict, Bangladesh, where two groups of patients with CKD are being compared. Patients aged 18 years and over with CKD (stages 1-3) were enrolled in November 2020. Patients were randomly allocated into either the intervention group (n=63) or the control group (n=63). The control group received usual treatment, while the intervention group received health education through a CKD campaign facilitated by a nephrologist and via mHealth (ie, periodic mobile phone calls) from community health workers. Both groups were followed up for a period of 6 months. The primary endpoint is patients’ increased knowledge measured using the Chronic Kidney Disease Knowledge Questionnaire. The secondary endpoints are improved QOL measured using the standardized EuroQol 5-Dimension 5-Level (EQ-5D-5L) questionnaire as well as improvements in the levels of blood pressure, BMI, serum creatinine, fasting blood sugar, hemoglobin, cholesterol, high-density lipoprotein cholesterol, triglyceride, serum uric acid, blood urea nitrogen, and albumin to creatinine ratio. RESULTS Enrollment of participants began in November 2020; the intervention and follow-up were completed in May 2021. We enrolled 126 patients in the study. Patients’ mean ages were 57.97 (SD 15.03) years in the control group and 57.32 (SD 14.37) years in the intervention group. There were 45 out of 63 (71%) females in the control group and 38 out of 63 (60%) females in the intervention group. In addition, there were 38 out of 63 (60%) literate patients in the control group and 33 out of 63 (52%) literate patients in the intervention group. CONCLUSIONS It is expected that a combined approach, incorporating both a CKD campaign and mHealth, for health education may be an effective tool for increasing knowledge and improving QOL among patients with CKD. Chronic kidney disease is an irreversible and slow-progressing disorder of kidney function so that the kidneys are unable to maintain the body's metabolism, fluid and electrolyte balance and cause uremia which requires dialysis or kidney transplantation. Hemodialysis is management to treat chronic kidney disease that is carried out for life so that the complications that often arise are fatigue, cramps, anxiety, pain. The phenomenon found in the Hemodialysis Room of the Tugurejo Hospital Semarang, there were 34% of the 84 patients who experienced fatigue which was characterized by lethargy, weakness, drowsiness, dizziness. This research is a quantitative study with a quasi-experimental design. The sample size is 4 respondents in the intervention group without a control group. Measurement of the level of fatigue (pre and post-test) using the PFS (Piper Fatigue Scale) which contains 22 subjective statements. Predialytic Exercise intervention is given for 5 sessions with a duration of 10-15 minutes. The results of this study indicate that the fatigue level of hemodialysis patients has decreased after the intervention of predialytic exercise for 5 sessions. The fatigue score of respondents 1 has decreased from a score of 4.8 (moderate fatigue) to 3.2 (mild fatigue), respondent 2 from a score of 6.3 (moderate fatigue) to 4.3 (moderate fatigue), respondent 3 from score 9 (severe fatigue) to 7 (severe fatigue), and respondent 1 from a score of 4.9 (moderate fatigue) to 2.4 (mild fatigue). The results of this study indicate that there is a decrease in each dimension of fatigue levels so that predialytic exercise intervention is effective to reduce fatigue in patients undergoing hemodialysis.

Purpose: Patellofemoral Pain Syndrome (PFPS) is among the most prevalent complaints observed in healthcare clinics. This group of patients encounters Pelvic Drop (PD) and Dynamic Knee Valgus (DNV); its’ causes have been reported as pain and decreased muscle strength. The current study aimed to explore the effects of a 12-week Sensorimotor Exercise (SME) on pain, strength, PD, and DNV in males with PFPS. Methods: This randomized double-blind clinical trial involved 32 patients with PFPS. The study samples were randomly divided into the experimental (n=16) and control (n=16) groups. To assess pain, the Visual Analogue Scale (VAS) was used. Moreover, quadriceps muscle strength was measured by the hip abductor hand dynamometer. To analyze PD and DNV, camera and Kinova software were used during stairs descent. The experimental group performed 12 weeks of SME for 3 one-hour weekly sessions. However, the control group received no therapeutic intervention during this time. The obtained data were analyzed in SPSS using Analysis of Covariance (ANCOVA).  Results: The data analysis results suggested that pain significantly reduced in the experimental group, compared to the control group, after twelve weeks of SME (P<0.001). Besides, quadriceps muscle strength (P<0.002) and hip abductor muscle strength (P<0.001) improved, and PD angle (P<0.002) and DNV (P<0.003) were reduced. Conclusion: SME reduced pain and facilitated the frequency and time of muscle activation. It also increased the strength of the gluteus medius muscle as the main stabilizer of the pelvis chain and led to a reduction in PD and DNV. Thus, SME could be used as a comprehensive protocol treatment to improve various disorders in patients with PFPS.

Fatigue is a major complaint of patients undergoing long-term hemodialysis, which has a high value, so it will affect the quality of life of patients. The purpose of this study was to determine the influence of PMR on the level of fatigue in patients with chronic kidney disease undergoing hemodialysis. This study used a quasi experiment design approach pretest-posttest control group. The number of respondents in the study were 32 patients divided into 2 groups: the 16 intervention group and 16 control group. The research concludes that there are significant differences on the level of fatigue in the intervention group between before and after PMR with p = 0.000. Suggested training PMR can be used as an independent nursing intervention in reducing fatigue in patients with chronic kidney disease undergoing hemodialysis.

Background: Nausea is a common complaint of patients with chronic kidney disease due to an accumulation of urea. Hemodialysis therapy has some side effects that play a role in increasing nausea. The use of pharmacological treatment to deal with nausea may bring a risk of aggravating the work of the kidneys, and thus its administration should be limited. Benson relaxation is a complementary therapy which reduces nausea by increasing the relaxation response that affects the nausea center in the brain. Objectives: This study aims to determine the effects of Benson relaxation technique on decreasing nausea. Methods: This study used a pre-posttest quasiexperimental research design with a control group. The samples were 30 patients recruited using consecutive sampling and were divided into the intervention group (n=15)andthecontrolgroup(n=15).Nauseawasmeasuredusinganumericratingscale. Theresultsshowedthattherewasadecreaseinthemeanofnauseaintheintervention group from 7.13 to 2.26, with p-value = 0.001. The mean value slightly decreased in the control group from 7.20 to 6.40, with p-value = 0.001. Conclusions: Benson relaxation affected the decrease of nausea in patients undergoing hemodialysis. Based on the results of this study, it is expected that nurses apply Benson relaxation technique as a non-pharmacological therapy to decrease nausea in hemodialysis patients.

Background. Research aim was to analyze the quadriceps muscle strength recovery after anterior cruciate ligament reconstruction using electrical stimulation and physical therapy. Methods. There were two randomly selected groups, with eight people in each group. The selection criteria were that the subjects had to have anterior cruciate ligament operation six weeks prior and were very active physically. One group was for research (study group), the other one for reference (control group). The study group had electrical stimulation combined with physical therapy exercises two times a week, for 45 minutes. The control group had exercises to strengthen the quadriceps muscle also two times a week, for 45 minutes. Both groups were tested before and after the research. The things evaluated during the test were – visual pain scale (VAS) scores, quadriceps muscle strength during extension and flexion using (R. Lovett) scoring system and goniometry showing degrees of extension and flexion. Results. Comparing both study and control groups, there was statistically signifcant improvement (p < 0.05), however the study group recovered faster and had statistically greater benefts. Conclusions. After 6 weeks of physiotherapy, the range of motion, quadriceps muscle strength increased and pain decreased in the operated leg. 1. After 6 weeks of physiotherapy and electrical stimulation, the range of motion, quadriceps muscle strength increased and pain decreased in the operated leg. 2. After 6 weeks of physiotherapy and electrical stimulation, the range of motion, quadriceps muscle strength, pain in the operated leg changed more in the study group than in subjects who received only physical therapy.Keywords: anterior cruciate ligament, electrical stimulation, quadriceps muscle, ligament reconstruction, muscle strength.

Introduction: Sarcopenia and osteoporosis increase the risk of fall, resulting in fragility fracture. Intervention programme on sarcopenic hip fracture was set up in our centre. Method: All patients aged ≥65 admitted to Caritas Medical Centre with operatively treated hip fracture and diagnosed with sarcopenia were included. Interventions include inpatient nursing education, dietary advice, therapist assessment and structured 12-week Geriatric Day Hospital (GDH) exercise programme. Those who are not eligible for GDH training were classified as control group. Changes in relative skeletal muscle mass index (RASM), muscle strength and functional scores were measured. Results: There were 11 intervention and 13 control patients. Both groups of patients were noted to have improvement in RASM, knee extension power of lower limbs and functional scores. Between-group analysis showed there was more improvement in injured limb extension power in the intervention group, although it is statistically insignificant (control group: 3.585 kg, intervention group: 5.827 kg, p = 0.147). There was no statistically significant difference in the 3-month change in RASM (control group: 0.581 kg/m2, intervention group: 0.347 kg/m2, p = 0.369) and functional scores. Conclusion: These sarcopenic hip fracture patients may be too frail to benefit from traditional exercise treatment for sarcopenia. However, more improvement in muscle strength was observed in the intervention group although not statistically significant. Intervention programme with exercise prescription in sarcopenic hip fracture patients may have the potential to improve the lower limb muscle strength. A more intensive and longer exercise training programme with incorporation into day rehabilitation model may be needed for these frail hip fracture patients.
