The persistence of agglutinins detected by MAT has created some problems to the interpretation of the results. The aim of this study was to examine the data of serology from 70 patients with serologically confirmed diagnosis of leptospirosis by during 3-13 months after being affected with leptospires in order to elucidate the interpretation of the persistence of agglutinins detected by MAT. Sixty-one patients sera (87.14%) had titers equal or greater than 800. Of these, two individuals maintained titers of 800 thirteen months after the onset. This study showed that only one sample of sera with high titers is not reliable to determine the time at which infection occurred.

A set of 723 diagnostic sera from human patients, submitted for the microscopic agglutination test (MAT) for antibodies to a group of 6 leptospiral serovars, was also tested by MAT for antibodies to the recently-discovered Leptospira fainei serovar hurstbridge. MAT titres of [ges ]128 to serovar hurstbridge were detected in 13·4% of these sera, and titres of [ges ]512 in 7·2%. In contrast, none of 62 sera obtained from a control population of laboratory staff gave titres of [ges ]128. The difference between the number of titres of [ges ]128 given by the two groups of sera was highly significant (P<0·01). The titres observed may have been due to cross-reactions with other leptospiral serovars, but this could not be demonstrated. An alternative explanation is that serovar hurstbridge is present in the human population.

SUMMARYLeptospirosis is a worldwide zoonotic disease. In the present investigation, a total of 89 human sera from a flood prone district of Bangladesh was screened by a one-point microscapsule agglutination test (MCAT). MCAT-positive and-doubtful sera were further tested by microscopic agglutination test (MAT) against 16 reference serovars ofLeptospira interrogans. and the antibody titres determined. In MCAT, 34 sera were positive and 22 were doubtful. Among those positive and doubtful sera. 33 and 20. respectively were tested by MAT. Thirty-four out of 53 MCAT-screened samples were MAT-positive. The titres ranged from 20 to 1600 with antibodies to serovarscopenhageni, australis, cynopteriandicterohaemorrhagiaebeing the most prevalent. Eleven MCAT-positive samples failed to react with any strains used by MAT, suggesting the presence of new or untested serovars. Among the MAT-positive samples, the presence of antibody against two or more serovars was more common than that of a single serovar. The present study suggests that rural people in Bangladesh are at high risk to leptospiral infection.

Serology plays an important role in the diagnosis of leptospirosis. Few laboratories have the resources and expertise to perform the microscopic agglutination test. There is a need for rapid and simple serological tests which facilitate the early diagnosis of leptospirosis, while antibiotic therapy may be most effective. A commercially available indirect hemagglutination assay (IHA; MRL Diagnostics, Cypress, Calif.) was evaluated with multiple serum specimens from 107 patients being investigated for leptospirosis. By using a combination of enzyme-linked immunosorbent assay (ELISA) methods for immunoglobulin M (IgM) and IgG antibodies and the microscopic agglutination test, 54 patients were found to have leptospirosis and 53 were found not to have leptospirosis. The sensitivity of IHA for the detection of acute leptospirosis was 100%, the specificity was 94%, the positive predictive value was 95%, and the negative predictive value was 100%. IHA was negative when 13 antinuclear antibody-positive sera, 24 serum specimens from patients with syphilis, and 16 serum specimens false positive by the Venereal Disease Research Laboratory test were tested. IHA was shown to detect both IgM and IgG classes of antibodies in human sera. Serum specimens from 27 dogs investigated for leptospirosis were studied: 3 samples gave nonspecific hemagglutination, but for all remaining samples, the results of IHA and an IgM ELISA were concordant. Performance of IHA was simple, and IHA requires no specialized equipment. It represents a useful assay for laboratories which require a leptospiral diagnostic capability but lack the expertise to perform specialist investigations.

ABSTRACT The diagnostic utility of immunochromatographic (Leptotek) and enzyme-linked immunosorbent assay (ELISA; Panbio) tests for the detection of Leptospira immunoglobulin M antibodies was assessed in febrile adults admitted in Vientiane, Laos. Both tests demonstrated poor diagnostic accuracy using admission serum (Leptotek sensitivity of 47.3% and specificity of 75.5%: ELISA sensitivity of 60.9% and specificity of 65.6%) compared to the Leptospira “gold standard” microscopic agglutination test.

Leptospirosis is a zoonotic disease, which is caused by Leptospira interrogans. The incidence of leptospirosis in dogs varies according to region and season, and is considered as emerging infectious diseases in humans. Clinical symptoms of leptospirosis in dogs vary greatly, some dogs are asymptomatic, with mild symptoms, and others progress to severe illness until death. The study aims to determine cases of canine leptospirosis in Yogyakarta, and identify the serovar that infect them. A total of 20 dogs without symptoms, with mild symptoms, to severe symptoms of leptospirosis were collected their serum and further tested using Microscopic Agglutination Test (MAT) conducted at the Center for Veterinary Research (BBLitvet) Bogor. History of vaccination is recorded as a consideration in interpreting the MAT results. The results showed that three out of the 20 samples were positive leptospirosis. Of the three positive samples, one was identified as serovar bataviae, another one as serovar bataviae and tarrasovi, and the last as serovar bataviae, tarrasovi, ichterohaemorrhagiae, canicola, celledoni, pyrogenes, cynopteri, and rachmati, respectively. The three dogs with leptospirosis showed similar clinical symptoms i.e. Anorexia, lethargy, and fever. It can be concluded that there are cases of canine leptospirosis in Yogyakarta which is predominantly caused by Leptospira interrogans serovar bataviae. ABSTRAK Leptospirosis adalah penyakit zoonosis, yang disebabkan oleh Leptospira interrogans. Kejadian leptospirosis pada anjing bervariasi menurut wilayah dan musim, dan dianggap sebagai penyakit menular yang muncul pada manusia. Gejala klinis leptospirosis pada anjing sangat bervariasi, beberapa anjing tanpa menunjukkan gejala, dengan gejala ringan, dan yang lain melanjut menjadi penyakit yang parah sampai kematian. Penelitian ini bertujuan untuk mengetahui kasus leptospirosis pada anjing di Propinsi Daerah Istimewa Yogyakarta, dan mengidentifikasi serovar yang menginfeksinya. Sebanyak 20 ekor anjing tanpa gejala, dengan gejala ringan, sampai gejala parah leptospirosis diambil serumnya guna pemeriksaan Microscopic Agglutination Test (MAT) yang dilaksanakan di Balai Besar Penelitian Veteriner (BBLitvet), Bogor. Sejarah vaksinasi dicatat sebagai bahan pertimbangan dalam menginterpretasi hasil pemeriksaan MAT. Hasil penelitian menunjukkan bahwa tiga dari 20 sampel positif leptospirosis. Satu sampel positif terhadap serovar Bataviae, satu sampel positif terhadap serovar Bataviae dan Tarrasovi, dan satu sampel lagi positif terhadap serovar Bataviae, Tarrasovi, Ichterohaemorrhagiae, Canicola, Celledoni, Pyrogenes, Cynopteri, dan Rachmati. Anjing yang positif leptospirosis menunjukkan gejala klinis yang sama berupa anoreksia, kelemahan, dan demam. Dari hasil penelitian ini dapat disimpulkan bahwa terdapat kasus leptospirosis pada anjing di Yogyakarta yang penyebabnya dominan oleh Leptospira interrogans serovar Bataviae.

The efficiency of four Leptospira biflexa strains (Buenos Aires, Patoc 1, Rufino and São Paulo) as single antigen in the serodiagnosis in guinea-pigs experimentally infected with seven Leptospira interrogans serovars (canicola, grippotyphosa, hardjo, icterohaemorrhagiae, pomona, tarassovi and wolffi) was evaluated by the microscopic agglutination test. The four saprophytic strains were not able to reveal antibody titres in sera of guinea-pigs experimentally infected with Leptospira interrogans. Serological cross-reactions were observed between strains Patoc 1 and São Paulo and between serovars wolffi and hardjo.

Abstract: Leptospirosis is a zoonotic disease that usually occurs during the flood and is generally transmitted through rat urine. Indonesia is a country with a moderate risk of transmission of leptospirosis. Leptospirosis has a broad manifestation varying from self-limited to severe disease. The gold standard examination of leptospirosis is microscopic agglutination test. Diagnosis is divided into suspected, probable, and confirmed. Treatment consists of antibiotics and supportive agents. Generally, the prognosis is good, albeit, sequelae can occur. Case-fatality rate in different parts of the world ranging from less than 5% to 30%.Keywords: leptospirosis, diagnosis,Abstrak: Leptospirosis merupakan penyakit zoonosis yang umumnya timbul saat banjir dan umumnya ditularkan melalui kencing tikus. Indonesia merupakan negara dengan risiko sedang penularan leptospirosis. Leptospirosis memiliki manifestasi luas dari self limited hingga sakit berat. Pemeriksaan baku emas leptospirosis ialah dengan microscopic agglutination test. Diagnosis dibagi atas suspek, probable, dan konfirmasi. Terapi diberikan medikamentosa dengan antibiotik dan suportif. Prognosis umumnya baik namun bisa juga terjadi gejala sisa. Tingkat fatalitas kasus di berbagai belahan dunia berkisar <5%-30%.Kata kunci: leptospirosis, diagnosis

Background: Leptospirosis is a zoonotic disease of worldwide significance in human beings and many animals. This zoonotic disease causes infection in dogs and other pet animals and these animals can influence human’s health. Given the unknown prevalence of leptospirosis in dogs in our area, this study was performed to investigate the sero-epidemiology of leptospirosis and evaluate the seroprevalence of Leptospira serovars in both household and stray dogs by microscopic agglutination test (MAT). Methods: Serum samples of 90 dogs in two household and stray groups (45 dogs for each group) were collected. Specimens were examined by MAT (in accordance with the procedure recommended by WHO) against antigens of serovars Canicola, Grippotyphosa, Icterohaemorrhagiae, Pomona, Hardjo, Autumnalis, and Ballum. Results: In total, 6.7% of (n=6) dogs (1 stray and 5 household dogs) had a serum titer of 1:100 or more against Leptospira serovars. Dominant serovars were Canicola (8.9%), Grippotyphosa (2.2%), and Pomona (2.2%), respectively. The antibody titer against more than one serovar was observed only in one dog. The sero-prevalence was 11.1% in household dogs and 2.2% in stray dogs. Since 4 pet dogs had been vaccinated recently, it seems that the actual percentage of infection in household dogs must be just 2.2% and actual infection in dogs including stray dogs is approximately 4.4% in Tabriz. Conclusion: Fortunately, the rate of infection in Tabriz is low and this can be due to special geographical and climatic conditions, regular and annual vaccination of household dogs against this disease, and lack of suitable conditions for the growth and survival of such micro-organisms in the environment

Background: Visceral leishmaniasis (VL) is the most severe form of leishmaniasis in Iran with high mortality rates in the case of inaccurate diagnosis and treatment. This study aimed to prepare and evaluate a new rk39 recombinant antigen from an Iranian strain of Leishmania infantum for diagnosis of VL in humans and dogs. Methods: rK39-based enzyme-linked immunosorbent assay (ELISA) was compared with the direct agglutination test (DAT) for the detection of anti L. infantum antibodies. We screened 84 human sera and 87 dog sera from clinical cases in the endemic area of Meshkin-Shahr, Iran along with 176 sera from healthy controls (collected from 86 humans and 90 dogs) during 2013 -2016. Results: Using the rK39 ELISA, a sensitivity of 85.7% (95% CI, 95-99%) and a specificity of 86.0% (95% CI, 95%-99%) were detected in human sera at a 1:800 (cut-off) titer when DAT-confirmed cases were compared with healthy controls; a sensitivity of 96.6% (95% CI, 95%-99%) and specificity of 94.4% (95% CI, 95%-99%) were found at a 1:80 (cut-off) titer compared with DAT. Kappa analysis indicated agreement between the rK39 ELISA and DAT (0.718) when using human sera at a 1:800 (cut-off) titer as well as (0.910) at a 1:80 (cut-off) titer when using dog sera (P<0.05). Conclusion: New rk39 recombinant antigen from an Iranian strain of Leishmania infantum seems to be used for diagnosis of VL in humans and dogs. Further extended field studies are recommended
