The possibility of pain resultant from tooth bleaching can limit the indication of this treatment in patients with pre-existing dentin hypersensitivity. Purpose: To provide a case series with alternatives for tooth bleaching that might reduce the pos-operative sensibility. Description of cases: Three patients with dentin sensitivity and complaining of tooth discoloration were selected. For each patient, a technique for tooth bleaching was randomly selected; using 15-35% hydrogen peroxide, and application of an desensitizing agent before or after the bleaching (toothpaste or 2% potassium nitrate). Conclusion: Bleaching was successfully performed in the three clinical cases. However, the previous desensitization using the toothpaste resulted in greater comfort for the patient.

ABSTRACT Tooth bleaching is one of the most conservative aesthetic techniques. At-home and in-office bleaching may be performed, as well as the combination of both techniques. As this combination may be done in different manners, distinct orders of combinations were proposed in this report. A 24 years-old man, whose upper central incisor and canine shades were A2 and A3 respectively, was initially treated by chair-side with 40-min application of 40% hydrogen peroxide (HP) for two sessions. Afterwards, an two-week overnight at-home bleaching was performed with 10% carbamide peroxide (CP). The shade of upper central incisors changed to 1M1 and canine was B1. Conversely, a 30 years-old woman was firstly conducted with at-home bleaching (overnight 4-weeks 10% CP) and, subsequently, with a single appointment of in-office bleaching for 45 minutes (35% HP). The shade of her upper central incisor changed from A1 to 0.5M1 and the upper canine from A3 to B1. The VITA classical (A1-D4) shade guide and VITA Bleached guide 3D-MASTER were used to determine the tooth color during the treatments. In both bleaching treatments, patients reported no significant tooth sensitivity and the final outcomes met their expectation.

A significant aspect of cosmetic dentistry is the treatment of tooth discoloration. Discoloration of non-vital anterior teeth can cause significant esthetic concern and requires efficient treatment. Discoloration of teeth can be extrinsic or intrinsic or a combination of both based on etiology, appearance, localization, and severity. Walking bleach involves the use of chemical substances like sodium perborate or hydrogen peroxide which in contact with the tooth release oxidizing agents that diffuse through the enamel and dentin and oxidize the pigments responsible for discoloration. This article aims at presenting a case series on the walking bleach method performed on discolored endodontically treated teeth associated with superior esthetic outcomes.

Nos dias atuais, a estética do sorriso vem crescendo no mercado, visto que muitos consideram como parte do padrão de beleza desejável, o sorriso alinhado e os dentes brancos. Assim sendo, o clareamento dental constitui um dos procedimentos mais procurados e realizados pelos cirurgiões-dentistas, no quesito estética. Dessa forma, o cirurgião-dentista deve ter uma base sólida de conhecimento a respeito, para que faça uma avaliação clínica e o correto diagnóstico da estrutura dental, indicando a melhor técnica em cada caso. Para tanto, foi realizada uma revisão de literatura sobre os tipos de clareamento dental, comparando-os quanto à eficácia, em relação à alteração de cor, e quanto à sensibilidade, com buscas nas bases de dados, Pubmed, Scielo e Scopus, através dos descritores: Tooth bleaching; Carbamide peroxide; Hydrogen Peroxide; Dentin Sensitivity. Foram considerados elegíveis estudos em inglês, português ou espanhol, publicados entre 01/2006 a 05/2021. Assim, notou-se que as técnicas de clareamento são capazes de atingir os mesmos resultados quanto à mudança de cor, mas com variação no tempo e indicação. Quanto à sensibilidade, há uma tendência dessa ser maior, quando usado gel de maior concentração. Porém, já existem atualmente, géis clareadores contendo substâncias dessensibilizantes, e/ou protocolos que podem ser utilizados, para que se tenha uma redução da mesma. Além disso, quando realizada a técnica de clareamento caseiro, sugere-se dar preferência ao gel de Peróxido de Carbamida na menor concentração, uma vez que esse apresenta menor risco de desenvolvimento de sensibilidade dentária e é eficaz, se comparado às maiores concentrações.

ABSTRACT Tooth discoloration commonly occurs after trauma to tooth, which leads to pulpal injury. Trauma to the pulpal blood vessel leads to hemorrhage and releases iron from hemoglobin. The iron which is released from hemoglobin combines with hydrogen sulfide to form iron sulfide, which gives the tooth its characteristic dark appearance. Tooth discoloration can be treated by nonvital tooth bleaching, if the tooth structure is intact. A combination of hydrogen peroxide and sodium perborate reduces the discoloration of the tooth by a process of oxidation. During root canal preparation procedures, the common mishap that occurs is instrument separation inside the root canal. The fractured fragment in the root canal can hinder proper preparation of root canal space. Continuous pain or discomfort may result if it is not removed or bypassed. It is more conservative to bypass the fractured instrument, particularly in cases where access to the fragment is restricted (apical one-third of canal or beyond the canal curvature) and its removal may lead to excessive removal of dentin with associated sequelae. Abstract Dentin hypersensitivity is a common condition of transient tooth pain associated with a variety of exogenous stimuli. There is substantial variation in the response to such stimuli from one person to another. Except for sensitivity associated with tooth bleaching or other tooth pathology, the clinical cause of dentin hypersensitivity is exposed dentinal tubules as a result of gingival recession and subsequent loss of cementum on root surfaces. The most widely accepted theory of how the pain occurs is Brännström's hydrodynamic theory of dentin hypersensitivity. Dentinal hypersensitivity must be differentiated from other conditions that may cause sensitive teeth prior to treatment. Three principal treatment strategies are used. Dentinal tubules can be covered by gingival grafts or dental restorations. The tubules can be plugged using compounds that can precipitate together into a large enough mass to occlude the tubules. The third strategy is to desensitize the nerve tissue within the tubules using potassium nitrate. Several over-the-counter products are available to patients to treat this condition.

In this open-label case series trial, we evaluated the effects of a nitrate-based nutritional formula on oxygen saturation (SpO2) and patient-reported outcomes in individuals with coronavirus disease 2019 (COVID-19). Five adult patients (three men and two women, age 39.6 ± 6.9 years) with a positive COVID-19 test result, breathing difficulties, and SpO2 ≤95%, who were free from other pulmonary and cardiovascular conditions, were recruited for this study. Participants were assigned to receive a multi-component nutritional formula (containing 1200 mg of potassium nitrate, 200 mg of magnesium, 50 mg of zinc, and 1000 mg of citric acid) every 4 hours during the 48-hour monitoring period. In all participants, SpO2 improved immediately after administration of the nutritional formula, from 1 to 7 percentage points (mean increase 3.6 ± 2.7 points; 95% confidence interval 0.3 to 7.0). SpO2 remained above baseline values throughout the monitoring interval, with values persisting over threshold values (>92%) for all patients and at each time point during the 48 hours. No patients reported any side effects of the intervention. These promising and rather unexpected results call for immediate, well-sampled, mechanistic randomized controlled trials to validate our findings.

SUMMARY Objectives: This randomized clinical trial tested the three-month desensitizing effect of two protocols using nano-hydroxyapatite formulations compared with Pro-Argin and fluoride varnish. Methods: Twenty-eight subjects with 137 teeth presenting dentin exposure with a minimal hypersensitivity of four on the visual analog scale (VAS) took part of this study. The subjects were randomly assigned to four groups: Desensibilize Nano-P paste (20% hydroxyapatite [HAP], potassium nitrate, and sodium fluoride [NaF]; 9000 ppm fluoride [F]); Desensibilize Nano-P associated with experimental home-care pastes (10% HA, potassium nitrate, and NaF; 900 ppm F); Pro-Relief professional paste (8% arginine with Pro-Argin technology) associated with home-care toothpaste (8% arginine, sodium monofluorophosphate, 1450 ppm F); and Duraphat professional varnish (NaF varnish, 22,600 ppm F). The professional treatments were performed in weekly appointments over three weeks. The home-care products were used continuously for three months. A VAS was used to assess the tooth sensitivity response after standardized evaporative stimulus at baseline and after one month and three months. The baseline score was deducted from the final score, and the means were analyzed using nested analysis of variance, while the comparison between times was performed by a general linear model (p<0.05). Results: At the first month all treatments were effective, but there were no significant differences among them (p=0.94). At the third month, despite the fact that NaF varnish had the lowest effect in reducing hypersensitivity, no significant difference was found among the treatments (p=0.09). Only Pro-Relief increased its effect over time (p=0.049). Conclusions: Nano-hydroxyapatite formulations (with or without home->care product association) were as effective as the other treatments in reducing dentin hypersensitivity over three months.

Purpose. This study evaluates the effect of bleaching before or after veneer preparation and the depth of preparation on color masking ability of laminate veneers. Methods. Sixty extracted premolars were artificially stained to vita shade A4, verified by digital spectrophotometer (Vita Easy Shade V), and then divided into three groups: NB = nonbleached , BBP = bleaching before preparation, and BAP = bleaching after preparation. Based on the preparation depths, each group was further divided into two subgroups: S 1 = 0.5   mm and S 2 = 1.0   mm . BBP and BAP were subjected to one session of in-office bleaching using 35% hydrogen peroxide. IPS e-max CAD veneers of 0.5 and 1.0 mm thickness (corresponding to the preparation depths) of the same shade and translucency (HT A1) were cemented immediately to the bleached surfaces. Immediately after cementation, the color change Δ E between the baseline (after staining) and the resulted shades was measured using the Vita Easy Shade V digital spectrophotometer and CIELab color system. Results. Bleached groups exhibited a significant Δ E value compared to the nonbleached group ( p < 0.05 ). BAP showed the highest Δ E value. No significant difference was found between BBP and BAP. S2 revealed a significant Δ E value than S1 ( p < 0.05 ). No significant difference was found between S1of BAP and S2 of NB, BBP, and BAP ( p > 0.05 ). Regarding the color coordinates, the difference between the tested groups was highly significant in lightness ( Δ L ∗ ) ( p < 0.001 ), while no significant differences were found in green/red value ( Δ a ∗ ) and yellow/blue value ( Δ b ∗ ) ( p > 0.05 ). Conclusions. In cases of severe tooth discoloration, one session of in-office bleaching before or after veneer preparation and the preparation depth do not influence the color masking ability of laminate veneers.

Recent studies suggested that bleaching agents may whiten teeth by oxidizing the fluorescent materials, which are the proteins located in the organic-inorganic interface. Therefore, we postulated that fluorescence of dentin came from dentin phosphoprotein (DPP) and that bleaching agents might bleach dentin by oxidizing DPP. Fifty-six specimens were randomly divided into 4 groups and exposed to distilled water, hydrogen peroxide (HP), ethylenediamine tetraacetic acid disodium salt (EDTA), and acetic acid for 24 h. After measuring the organic and inorganic components, fluorescence, and color characteristics of dentin before and after exposure, we found that when DPP was removed from dentin by EDTA, fluorescent intensity declined proportionally with the reduction in Raman relative intensity, and dentin was whitened considerably, with an Δ E value 6 times higher than that of the distilled water group. On the contrary, due to the incapability of acetic acid to dissolve DPP during decalcification, fluorescent intensity values and tooth color remained nearly unchanged after exposure to acetic acid. Dentin exposed to neutral HP showed no obvious morphologic and organic/inorganic component changes except for the destruction of DPP. Similarly, dramatically decreased fluorescent intensity and lightened color were found in the HP group. Moreover, DPP solution of the HP group exhibited decreased ultraviolet absorbance, especially between 250 and 300 nm, which arose from aromatic amino acids. The results indicated that DPP was responsible for the fluorescent properties of dentin and that HP might bleach dentin by the oxidization of aromatic amino acids in DPP. These findings are of great significance in promoting our further understanding of the mechanism of tooth bleaching and the fluorescent property of normal dentin.
