Aim:  To  evaluate  the  municipal  program  of  hanseniasis  control  in Canaã dos Carajás, PA. Method: This is a quantitative study, for the evaluation of health services, whose evaluative proposal is the triangulation of methods, through the study of the  structure,  processes  and  results.  Results:  several  weaknesses  were  found  in  the program  structure.  Although  some  attributes  have  not  been  positively  evaluated  in  a general aspect, the scores found allowed a good rating of the municipality's performance in terms of leprosy control actions. The appraised indicators showed fluctuations over the time series. Conclusion: the city has poor structure, high guidance for the development of  leprosy  actions  in  primary  care  and  regular  quality  indicators  according  to  the established parameters.

Knowledge of patients' views can contribute to the strengthening of health services. The aim of this study is to describe the patients' perception of a public oral health service, contributing to evaluations in health services. This is a qualitative study in which a focus group was conducted, with the participation of six patients of the oral health system in the city of Belo Horizonte, MG, Brazil, all with a minimum experience of three years of using the service. A theoretical model with dimensions aimed at assessing integrality and primary care services was used. In conducting the research, a semi-structured script was used. The data were analyzed by content analysis. The most representative categories for evaluating oral health actions in primary care are the health unit; the welcoming and its relation with the creation of the bond; service with a strong emphasis on the humanized relationship between professional and patient and on teamwork and; as a highlight, citizen participation, based on the recognition of a “system” that prevents the proper functioning of services and that must be fought with citizenship. Patients’ perceptions can be used to assess oral health in primary care from the perspective of those who actually use health services, seeking ultimately to constantly improve them. Knowledge of patients' perceptions may enable organizations to know their performance, through assessment methodologies based on the established perceptions.

Abstract Background High rates of antibiotic misprescribing in primary care, with alarming clinical and economic consequences, highlight the urgent need for interventions to improve antibiotic prescribing in this setting. Objectives To assess the effectiveness on antibiotic prescribing quality indicators of a multifaceted intervention targeting health professionals’ and patients’ behaviour regarding antibiotic use. Methods We conducted a pragmatic cluster-randomized controlled trial in the catchment area covered by Portugal’s Central Regional Health Administration. The intervention consisted of a multidisciplinary, multifaceted programme involving physicians, pharmacists and patients, and comprising outreach visits for physicians and pharmacists, and educational materials for health professionals and patients. The following were assessed: relative ratios of prescription of penicillins sensitive to β-lactamase, penicillin combinations including β-lactamase inhibitors, third- and fourth-generation cephalosporins and fluoroquinolones; and the ratio of broad- to narrow-spectrum antibiotics. An interrupted time-series analysis for multiple-group comparisons was performed. The study protocol was registered on Clinical.trials.gov (NCT02173509). Results The participation rate in the educational intervention was 64% (197/309 GPs) in a total of 25 counties. Statistically significant improvements were obtained, not only in the relative prescription of penicillins sensitive to β-lactamase (overall relative change of +896%) and penicillin combinations including β-lactamase inhibitors (−161%), but also in the ratio of broad- to narrow-spectrum antibiotics (−200%). Statistically significant results were also obtained for third- and fourth-generation cephalosporins, though only in the immediate term. Conclusions This study showed that quality indicators of antibiotic prescribing can be improved by tackling influences on behaviour including knowledge and attitudes surrounding physicians’ clinical practice. Accordingly, these determinants must be considered when implementing interventions aimed at improving antibiotic prescribing.

Governador Valadares (GV) is a municipality of recognized leprosy hyperendemicity. From 2001 to 2010, the municipal health management invested in a heterogeneous way in the decentralization of control actions to expand access to diagnosis, highlighting the years 2002 and 2004, when training-campaigns took place in the Family’s Health Program. It is an epidemiological study, of a descriptive nature, of a longitudinal type, developed in the city of GV/Minas Gerais/Brazil. The variables collected were: Age group, categorized as <15 years and> 15 years; Gender; Year of notification; Operational classification categorized as paucibacillary and multibacillary; Number of registered household and outside contacts, classified by whole numbers starting with zero (no communications), and mode of entry, as new or not. The estimates obtained from the analysis of municipal indicators reveal that the coefficients of general detection and children under 15 years old remained at hyperendemic levels during the ten years of study with an apparent decrease in general detection to the period studied. Furthermore, according to national parameters, it was observed maintenance of a consistent number of diagnoses in children under 15 and insufficient contact examination coverage. Our study points to the importance of continuing leprosy control actions over time. Observing the findings related to some epidemiological and operational leprosy indicators in GV from 2001 to 2010 makes us aware of a high disease burden. Keywords: Epidemiological parameters, leprosy, operational indicators

ABSTRACT Objectives: to evaluate the quality of primary care in leprosy control actions, to correlate effects on monitoring indicators. Methods: a cross-sectional study, conducted in Belo Horizonte, from July to September 2014. Interviews with 408 professionals were conducted through the application of a tool that assesses attributes of primary care in leprosy control actions, and monitoring indicators were calculated. Spearman correlation was used, significance level p < 0.05. Results: the overall score correlated with the percentage of cases treated in primary care and of family health staff coverage. The derived score correlated with the percentage of cases treated in primary care, and the essential score correlated with the proportion and rate of cases diagnosed, with grade 2 physical disability. Conclusions: the quality of leprosy control actions performed by primary care professionals produces impacts on health indicators, and developing strategies consistent with the reality of the territory is necessary.

Objective: Evaluate the performance of primary health care in control of leprosy in Teresina, Brazil from the perspective of users and professionals. Methods: This cross-sectional, descriptive study involved 25 healthcare users with leprosy and 34 professionals (physicians and nurses) working in 13 municipal health units within the urban area of Teresina. The data were collected from January to June, 2017, using the Instrument for Assessing the Performance of Primary Care in Leprosy Control Actions, a document based on the Primary Care Assessment Tool/PCATool Brasil which measures the presence and extension of essential and derived attributes from primary health care. Results: The mean scores obtained for the essential, derived and global attributes, through the perception of the users, were lower than 6.6, indicating that the primary health services in Teresina are not sufficiently oriented to carry out leprosy control actions, mainly in terms of access, integrality of services provided, family orientation and community orientation. The professionals showed a contrasting view, with averages exceeding 6.6 for all attributes and scores. The difference in perception between the groups was significant for all attributes except for coordination (Mann-Whitney U test, p = 0.479).The overall evaluation of quality was not influenced by the user’s clinical or socioeconomic variables, but  rather by the organization/provision of services. Conclusions: The municipal health units within the urban area of Teresina must be strongly guided by primary health care attributes in order to reach higher levels of user satisfaction.

Objective Developing an instrument to evaluate the performance of primary health care in the leprosy control actions, from the perspective of users and do the face and content validation. Method This is a methodological study carried out in four stages: development of instrument, face and content validation, pre-test, and analysis of test-retest reliability. Results The initial instrument submitted to the judgment of 15 experts was composed of 157 items. The face and content validation and pre-test of instrument were essential for the exclusion of items and adjustment of instrument to evaluate the object under study. In the analysis of test-retest reliability, the instrument proved to be reliable. Conclusion The instrument is considered adequate, but further studies are needed to test the psychometric properties.
