Topical issues of evaluation, as well as practical application of longitudinal deformities of left ventricular myocardium in the diagnosis of cardiovascular diseases are considered. A study was conducted with an estimation of the segmented reference values for longitudinal deformation of the left ventricular myocardium in healthy middle-aged people using the technique of speckle tracking echocardiography. The essence of the methodic is to track coded areas of the myocardium (speckles) in the gray-scale image with subsequent automatic processing of data by special software. The data obtained are presented in the form of graphs and polar diagrams. This methodic has advantages over the method of tissue dopplerography, because it is more objective, sensitive and requires less time for data processing and analysis. At the same time speckle tracking has a number of limitations. To assess deformation, a good visualization of the image and a regular rhythm are required, and, most importantly, there are no generally accepted standards, since each manufacturer of echocardiographic equipment applies various filters that counteract the scattering of an ultrasonic beam. The effect of these filters on the results is unknown and, therefore, it is impossible to compare the data obtained on different devices. In addition, in the present work, correlation links between the structural and functional state of the myocardium and indices of longitudinal deformation of segments of the left ventricle are revealed. These results reveal the physiological features of the process in myocardial contractility occurring in the heart, both in norm and pathology.

Currently, the methods of early diagnosis and treatment of stable angina are of interest to researchers. One of the methods of diagnosis is echocardiography using a two-dimensional speckle tracking. Technology speckle tracking allows to evaluate systolic and diastolic heart function in various cardiovascular diseases, including coronary heart disease (CHD). By means of this program all types of myocardial deformation: radial, circular, longitudinal, the rate of deformation and rotation of the myocardium can be evaluated. The clinical studies have shown that changes in the indices of deformation are sensitive markers of myocardial ischemia in the early stages [9]. In this study the authors investigated the effect of anti-anginal drug Coronater on the parameters of radial, circular and longitudinal deformation of the left ventricular myocardium in patients with angina pectoris 1-2 functional class (FC). The drug Coronater is phytotherapeutic preparation of anti-anginal action used to treat angina FC 1-2. The main active ingredients are Ligustikum Sichuan and broneol. Most clinical studies confirm the effectiveness of anti-anginal drug Coronater having significant coronarolytic and anti-ischemic effects.

The indicators of global and segmental longitudinal deformation of the left ventricular myocardium are presented and analyzed in patients suffering from coronary heart disease. The latter, as before, remains one of the urgent diseases of the circulatory system. To verify and determine management tactics for patients suffering from coronary heart disease, coronary angiography is performed. Indications for its implementation are based on the results of exercise tests, in particular, stress echocardiography. However, its implementation can be difficult due to poor visualization of the walls of the left ventricle, and the conclusion is not without subjectivity. One of the modern methods for assessing the contractility of the left ventricular myocardium, both global and segmental, is the spatial displacement of the myocardial structure (speckle-tracking- echocardiography). The technique automatically calculates the longitudinal deformation of the myocardium, expressed as a percentage of the initial values. It was found that the global and segmental longitudinal deformation of the left ventricular myocardium in the majority of the evaluated segments in the examined patients did not differ before loading. When conducting a stress test, the exact opposite dynamics of changes in the parameters of the global and segmental longitudinal deformation of the left ventricular myocardium is observed. So, in patients with a high risk of complications of coronary heart disease, a decrease in these indicators was observed, in patients without a risk of complications, on the contrary, their increase was observed, which indicates an increase in myocardial contractility during physical exertion. It was revealed that the indicator of global longitudinal deformation of the left ventricular myocardium is highly informative. The lower value of the decrease in the global longitudinal deformation of the left ventricular myocardium was calculated, which can serve as an additional indication for coronary angiography.

In case of antitumour anthracyclines therapy of patients with non-Hodgkin lymphomas the possibility of using speckle-tracking echocardiography is being considered. It was found that patients with non-Hodgkin lymphomas have early violations of the left ventricular myocardium during the course of antitumor chemotherapy using anthracyclines. They are characterized by reducing of longitudinal deformation of the myocardium with a gradual spread from the apical to the basal segments of the left ventricle. A statistically significant reduction in global and segmental systolic deformation after the 4th course of chemotherapy (cumulative dose of doxorubicin 333.5 88.5 mg/m2) is the earliest marker of left ventricular mechanical dysfunction, appearing before the development of diastolic dysfunction and ejection fraction reduction of the left ventricular. Early violations of myocardial mechanics in the dynamics of the course of antitumour therapy with anthracyclines are identified in the apical area of the left ventricle, it can be connected with circulatory failure in this area. Violations of myocardial mechanics in the apical area of the left ventricle can be an erly predictor of ischemia of this area with possible progression to global contractile muscle dysfunction of the left ventricular. The global longitudinall deformation of the left ventricular myocardium in contrast to ejection fraction of the left ventricular is more sensitive to myocardial damages against the backdrop of chemotherapy using anthracyclines. So, it can be used as an early preclinical marker of myocardial damage, that takes echocardiography closer to the most advanced methods of myocardial visualization.

2-D echocardiography is currently the first-line imaging modality for assessing global and regional function of left ventricle (LV). Using 2-D echocardiography, LV function is most often evaluated visually, as a result of the quality of the research depends directly on the experience and qualifications of the expert. The new technology of two-dimensional speckle tracking echocardiography allows to assessing the contractile function of the left ventricle quantitative. Over the years, the numerous studies have demonstrated the value of speckle tracking echocardiography in the diagnosis and risk stratification of a wide range of cardiac diseases, including coronary heart disease [14]. During the cardiac cycle the speckle tracking echocardiography allows in semi-automatic mode to evaluate the deformation of the myocardium in the three spatial directions: longitudinal, radial, and circular. In addition, speckle tracking estimates the direction of rotation and speed of motion of the left ventricular myocardium. This technology may have important clinical value for quick and accurate assessment of global and segmental myocardial function. The use of estimates of the deformation of the myocardium and the speed of deformation of the myocardium by means of speckle tracking method may be able to increase the sensitivity and precision of stenosing lesions of the coronary arteries [16].

Abstract Background   Management of asymptomatic severe aortic stenosis (AS) patients with preserved left ventricular (LV) ejection fraction (EF) remains controversial. Recent studies using have shown that decreased LV longitudinal deformation assessed by global longitudinal strain analysis can predict adverse cardiac events in AS patients with preserved EF. Tissue-tracking mitral annular displacement (TMAD) by speckle-tracking echocardiography provides rapid and simple assessment of LV longitudinal deformation even when the acoustic window is poor (Fig.1). Purpose  The purpose of this study was to examine the value of TMAD to predict occurrence of the cardiac events in asymptomatic severe AS patients with preserved EF. Methods   We studied 103 patients with severe AS and preserved EF [aortic velocity >4m/s or aortic valve area (AVA) <1.0 cm2, EF >50%]in whom TMAD was measured, and a total of 44 patients were included in the final data setaccording to the exclusion criteria. Using TMAD analysis software, the base-to-apex displacement of automatically defined mid-point of mitral annular line in four-chamber view was quickly assessed, and the percentage of its displacement to LV length at end-diastole (%TMAD) was calculated (Fig.1). We investigated the association between %TMAD and the cardiac events including implementation of hospitalization due to heart failure, decreased EF (< 50%), aortic valve replacement or transcatheter aortic valve implantation due to appearance of symptoms and cardiac death,  Results  In all the final study patients, %TMAD was successfully and quickly (within 10 seconds) evaluated. During a follow-up, the cardiac events developed in 16 (36%) of 44 patients. Tableshows echocardiographic parameters in patients with and without the cardiac events. %TMAD was significantly impaired in patients with the cardiac events compared with those without the cardiac events (9.6 ± 0.6 vs 12.1 ± 0.4%, p= 0.002). The other parameters were not involved in the event occurrence; age, LV mass index, EF, aortic velocity, AVA, tricuspid regurgitation pressure gradient (TR-PG), early diastolic /atrial filling velocity (E/A), early diastolic velocity of the mitral valve annulus (e’) and E/e’. In multiple variable analysis, %TMAD was an independentpredictor of the cardiac events (HR; 12.1, p= 0.001). ROC analysis revealed that the area under the curve of %TMAD was 0.81 for the cardiac events. Kaplan-Meier analysis showed %TMAD (cut-off: 11.9) provides a significant difference in the cardiac event (Fig. 2). Conclusions.  The present results suggests that TMAD easily and rapidly estimated by speckle-tracking echocardiography can be used as a simple method to predict occurrence of the cardiac events in asymptomatic severe AS patients with preserved EF.
