Introduction: From May to December 2019, a literature review of the urinary system iatrogenic injury problem was performed. The most cited, representative articles in PubMed, Scopus, and WoS databases dedicated to this problem were selected. Urinary system iatrogenic injuries include ureter, bladder, urethra, and kidney traumas. It is widely thought that the main causes of such injuries are urological, obstetric, gynecological, and surgical operations on the retroperitoneal space, pelvis, or perineum. Methods: The purpose of the study is to describe all aspects of the iatrogenic injure problem, under the established scheme and for each of the most damaged organs: the urethra, bladder, kidney, and ureter. The treatment of confirmed iatrogenic injuries largely depends on the period of its detection. Modern medical procedures provide conservative or minimally invasive treatment. An untimely diagnosis worsens the treatment prognosis. “Overlooked” urinary system trauma is a serious threat to society and a particular patient. Thus, incorrect or traumatic catheterization can lead to infection (RR 95%) and urethral stricture (RR ≥11–36%), and percutaneous puncture nephrostomy can cause the risk of functional renal parenchyma loss (median 5%), urinary congestion (7%), or sepsis (0.6–1.5%). Results: Lost gain, profits, long-term and expensive, possibly multistage treatment, stress and depression, and the risks of suicide put a heavy financial, moral, and ethical burden on a person and society. Also, iatrogenic injury might have legal consequences. Discussion/Conclusion: Thus, the significant problem of urinary tract iatrogenic injuries is still difficult to solve. There is a need to implement mandatory examining algorithms for patients at risk, as well as the multidisciplinary principle for all pelvic surgery.

THE RELEVANCE is determined by the significant frequency of iatrogenic injuries of the radial nerve during internal osteosynthesis of the humerus, long-lasting functional disorders, a large number of unsatisfactory results, as well as the lack of a unified approach to diagnosis and treatment.MATERIAL AND METHODS. The causes of iatrogenic damage to the radial nerve were studied in 22 patients who underwent internal fixation during fractures of the humeral diaphysis. The ultrasound examination was used to visualize the radial nerve. In 13 patients (59.1%), a pathogenetic treatment was used, including medication, physical and mechanical therapy.RESULTS AND DISCUSSION. In plate osteosynthesis, a greater number of iatrogenic damage to the radial nerve was observed than during osteosynthesis with screws. The ultrasound method had high diagnostic values to determine the continuity of the nerve trunk and identify conflicts with solid structures. The use of pathogenetic therapy of neuropathy led to a more rapid clinical recovery of limb function.CONCLUSION. To prevent iatrogenic injuries and avoid gross manipulations, the access with sufficient visualization of the radial nerve is necessary in plate osteosynthesis and distal blocking outside the projection of the radial nerve is necessary in intramedullary osteosynthesis. Indications for early revision after iatrogenic damage are neurotmesis and a nerve conflict with bone fragments or an implant.

Anesthesiologists consider professional insurance and its medico-legal problems as a remarkable aspect of their job. “Associazione Anestesisti Rianimatori Ospedalieri Italiani—Emergenza ed Area Critica” (AAROI-EMAC) is the Italian professional association of anesthesiologists and intensivists that works to train its subscribers on safety measures. This is a retrospective observational study on an insurance complaints database for anesthetic accidents that result in injuries to patients. The analyzed period runs from 1 January 2014 to 31 December 2016. A total of 1309 complaints related to 873 insurance claims were analyzed. Criminal complaints comprised 805 (64.4%) of the total, and civil complaints were 445 (35.6%). The iatrogenic damage claimed included: death (58% of the cases); peripheral nerve damage (8%); spinal cord injuries (5%); unspecified injuries (7%); dental damage (4%); infections (3%); needing second surgical procedure (2%); and other injuries (13%). There is a statistical significance between the size of the hospital and the number of the claims: small hospital complaints comprised 40.1% of the cases, while complaints against medium-sized and large hospitals constituted 20.6% of the cases (χ2GL = 8 = 39.87, p = 0.00). In Italy, anesthesiologists and intensivists are often involved in litigation even when they are not directly responsible for iatrogenic injuries, and the most frequent claims in ICU are related post-operative complications.

Iatrogenic damage in restorative teeth preparation and management constitute a significant issue that dentists and surgeons might face during and after restoration procedures. Many presentations can be observed for the injured enamel following iatrogenic damage as the presence of fine scratches, 1 mm wide vertical grooves, extensive damage and indentations. Following the occurrence of such complications, subsequent changes to the properties of the affected areas can significantly increase the risk of developing dental caries. It is essential to shedding more light on such phenomena to increase awareness among dentists and physicians and to enhance the potential outcomes. Accordingly, in this literature review, the aim was to discuss the iatrogenic damage in restorative teeth preparation and management as per evidence from the current studies in the literature. At first, the discussion was around the different types of iatrogenic injuries based on the affected regions and the incidence of adjacent teeth injury during restoration was high. Moreover, it had been discussed the potential mechanisms and contributing factors that might flare up the injury and increase the risk of significant damage induction. Hypersensitivity and using rough materials are two common factors that may induce inflammation and induce damage. Further investigations might be needed for the implication of safe practices for surgeons and dentists to enhance the outcomes.

Recent studies indicate there is some disagreement concerning the interpretation and clinical utility of the Type V Bekesy pattern. Bekesy tracings obtained over the past six years from a sample of clinical cases were analyzed and a definition was established for the Type V pattern. This definition was applied to Bekesy tracings obtained from normal listeners, hypoacusics, and pseudohypoacusics. The Type V pattern was found frequently among pseudohypoacusics and only rarely among other individuals.
